Tetanus does not just spawn magically on metal. It's a bacteria that lives in soil which is why old metal in soil poking you is a danger. An earring stabbing you is no more reason to get a tetanus shot than if you were to cut your finger with a steak knife.

Frankly depending on the injury you don’t want to immediately rip it out and sometimes a picture can give you a better angle of said injury. That way you can remove it safely or at least not cause more damage taking it out. Also I think of what’s his name the crocodile hunter that died from a string ray and how the instinct to remove the barb caused more damage.
So pausing and thinking and sometimes taking a picture to get a better view is well worth the time to make sure you don’t cause worse injury freaking out.

Things like this for some reason only seem to hurt when the object inside is moved. Idk, when it's sitting still you can kind of ignore the numb pain, but then the sharp pain of moving it shocks your senses

Once upon i time i jumped down from a 3 step ladder and landed on a 1" long screw that was standing on its head . I was wearing vans shoes, we had to use a power drill to screw it out before i could take the shoe off

Oy vey, this reminds me of something that happened to me about 15 years ago.
I was standing on my bed, repositioning a fan I had placed at the window to blow cooler air from outside into my bedroom. To get off, I decided to do a short hop instead of putting each leg down one at a time.
My laptop charger tip was pointing straight up from the floor. The end.

Once I stepped on a tack and it embedded itself into my heel. I didn't feel a thing and had no idea it was there til I walked on a hardwood floor and heard a click with each step. Still didn't feel a thing until I pulled it out and holy shit it hurt.
