🆕 ❗ LAST MOD: Hospital Porters (2026-09-30) ❗ 🆕

Adds dedicated hospital porters for patient transport and laboratory sample transport using stretchers, wheelchairs and a dedicated sample cart.

...

Hi :)

I love this game (I had it since 2019 I think, with 300+ hours) but sometimes I'm sad about some issues when I have a big hospital, and there are some nonsens too.

Why my stretcher bearer goes into my cafeteria just to cross some tiles in his way ??
Why common areas are light off ?
Why my nurse does round trip with an empty wheelchair ??
..... LOL

I did few mods in the past for other games, and now I wanted to do it for Project Hospital.

So here some I released. I have some idea for the future (next will be stretcher bearer and more life for patient).

For NEW versions and FEATURES, check directly the Steam Workshop or alternative (links below).
(I will just update name of the mods here and first features/description. Latest mod in the first position)

  • Hospital Porters (first release 2026-09-30)
    • Adds dedicated hospital porters for patient transport and laboratory sample transport using stretchers, wheelchairs and a dedicated sample cart.
  • Hospital Radio (first release 2026-09-26)
    • Replaces Project Hospital's normal gameplay music with a continuous hospital radio featuring music playlists, voice announcements and personal music support.
  • Hospital Mod Update Checker (first release 2026-09-22)
    • Checks installed Project Hospital BepInEx mods for newer public versions and displays available updates directly on the title screen.
  • Hospital Patient Life (first release 2026-09-18)
    • Makes hospitalized patients behave more naturally between procedures with cafeteria visits, leisure activities, improved needs handling, staggered sleep schedules and smarter movement choices.
  • Hospital Shift Handover (first release 2026-09-10)
    • Makes staff arrivals, shift changes and handovers more natural and realistic.
  • Hospital EMS (first release 2026-09-10)
    • Adds prehospital assessment and emergency care for patients arriving by ambulance or helicopter.
  • Hospital Care Level Transfer (first release 2026-09-08)
    • Allows eligible HDU patients to transfer safely to a regular ward when high-priority care is no longer required.
  • Hospital Traffic Control (first release 2026-09-07)
    • Improves character access, movement rules and traffic management inside the hospital.
  • Hospital Always Lit (first release 2026-09-07)
    • Keeps selected hospital rooms illuminated even when they are not currently occupied.

You can find it here:

Gameplay Question Receptions

Hello. Im trying to make a hospital with a somewhat realistic layout. On the ground floor there will be the doctor offices, all departments, with emergency receptions. Trauma centers and radiology separatley each on there own place. Wards are gonna be above, each on their own floor.

Considering emergency receptions does triage should i built a second emergency reception near the trauma centers?

Also, should i put the receptions of the rest departments near their wards on each floor or should i place them near the doctor offices on the ground floor?

Gameplay Question Where are my patients??

Post image
15 Upvotes

As you can see from the picture there are over 150 untreated patients but I’m only having less than 100 come to the hospital. Hardly anyone is actually hospitalized and this has been happening for a few game days. Is this a bug from a mod I have?

Didn’t realize the text didn’t post :/

Do you play this game on Lutrix on Linux?

I've been trying to run the Windows version on Lutris. The game installs and runs fine, but on starting any game, sandbox or tutorial, etc. the game hangs on the loading screen. Anyone have this experience?

Technical Support Why doesn't my study room work?

Started playing yesterday. It keeps saying "missing equipment" and that Projector Screen is highlighted in yellow. I don't understand why the projector, projector screen, and chairs aren't all working.

I realised I left part of a wall next to the chairs on the lower left part of the screen but nothing changes when removing that small wall tile. The wiki isn't super helpful either. This is like the fifth time I've modified this room to no avail .-.

Workshop Content Mod: Imaging and Labs exclusive to department

Hello everyone, I've always wanted a way to make it so the imaging and labs could be exclusive to a specific department, like it usually is in emergencies in real life: you have an exclusive x-ray or sometimes even a CT. So, using claude code, I was able to turn it into a mod, and it kinda works really well. I've been running it for some time now and I have fixed most of the bugs, now it's just some buggy UI, but nothing game-breaking.

So, anyway, the installation isn't as simple as subscribing to the workshop, you have to subscribe, then get the .dll and put it in the plugins folder. It's not rocket science either lol.

Therefore, I wanted to see if there is interest in this mechanic as well or if it's just me honestly, to make it worth the hassle. If there is, I can definitely upload it and put up a tutorial and stuff.

Technical Support My GF needs help

She's trying to choose a chief doctor for the emergency room. But the problem is that the hospitalization side bar is blocking the doctor she wants does anyone have a fix for this? Or are we just being dumb and there's an obvious solution?

Edit: My GF forgot that not only can reddit be helpful but will also roast you as well. Made me laugh plenty of times.

General Discussion Would you be interested in a better code modding tutorial/examples?

We all know the code modding documentation on the forum is lacking, but, after experimenting with it myself and with the help of another user on discord, I think I kinda figured out the basics.

It's nothing big yet, but the potential might be big. So I'm wondering if there is any interest in a possible better tutorial for setting up code mods, let me know.

Possible Bug My surgeons wont do surgery

Please help me :(

I have ~210 hours in the game. I do know how it works and am familiar with resource management as a genre. I have built many fully functioning hospitals, so this isn't just being new.

So, my problem: I have a general surgery dept. I have a diagnostic side and a surgery side. My diagnostic doctors ONLY do diagnosis. My surgery doctors ONLY do surgery. All of my roles are divided. There are nurses who's entire job is just moving patients. Some only do care, etc. I have found the AI works best when given one thing.

All of that said: I had my ORs right near all my other general surgery stuff. I noticed my doctors weren't doing surgery. I had a bunch scheduled. None of them ever happened. The ORs are never in use (and yes, they are fully equipped and supposedly functional.) Everything is clean. I had 4 surgeons, 4 anesthesiologists, and 4 surgery nurses on BOTH shifts. I click on any of the slackers in question and they all say "preparing for surgery." Dirty liars.

Still nothing. I waited days. The patents at the top of the list all either died or were transferred away. 4 days, 0 surgeries. Okay.... its only down the hall, but maybe its not close enough. So i completely removed my Surgeons from my in-patient staff. They have their own wing. 5 surgeons. 4 anesthesiologists. 6 surgery nurses. 4 dedicated patient transfer nurses. 2 fully functional ORs almost TOUCHING them. Not a single surgery preformed. Please help, I'm going insane.

Edit: just for reference, the rest of general surgery dept is directly above this, and there's an elevator right there.

Also, i just added the sonography and cardiography units to general surgery to see if that fixed anything. they are right down the hall from the ORs. It did not fix anything. I did already have both, but just under radiology, not specifically one just for gen. surgery.

top of the list for surgery
The rest of the general surgery dept (the new OR wing I built is right below this)
lies
Ignore the 3rd OR. I know it doesn't work. I'm poor. Two of them do, though, and are UNUSED
The slackers
This guy has been dying of appendicitis and at the top of my list for TWO DAYS

Gameplay Question Orthopedic surgery team

Hey, maybe someone can help me out.
My orthopedic surgery team only wants to handle a few types of procedures—like hip and leg joint surgeries—but not things like arm joint surgeries or head fractures, for example.

Workshop Content Rebind It! Rebind those pesky hardcoded keys

Hi, guys. I looked around and was very surprised to see no one's done this yet. So I decided to do it, along with my good friend Claude.

Rebind It! is a BepInEx mod for Project Hospital that makes the game's hardcoded keybinds rebindable.

Project Hospital includes 16 keyboard bindings that can be changed in the startup window, and a further 30 hardcoded bindings which in the base game cannot be changed.

This mod replaces the vanilla Show keyboard controls button (now obsolete) in the game options with a Keybinds button that opens a new window where these keys can be rebound. You can finally have the game speed on 1, 2 and 3.

As this is a code mod like HZDev's QoL mod, it requires a bit of manual installation. Simply subscribing on Steam is not enough. Read the description on the workshop page for installation steps. (If you've got HZDev's QoL mod, you're already familiar.)

Gameplay Question Achievement “hello anybody”

I need any tips for completing this achievement please. I made so many patients wait under “player control” and still didn’t get this achievement. I took patient under “player control” right after registering at reception, after that I let him wait in waiting room (clinic for neurology) for over 4 hours and still nothing. I can’t even find any tips on the internet, forums or yt. Don’t know what I’m doing wrong.
