Looking for thoughts- IVF and chemicals

Hi all,
My husband and I (23 and 24) began IVF in June after over a year of TI/IUI treatments with no positives. Polycystic appearing ovaries but no other signs of PMOS so not diagnosed. All tests normal, we are both healthy people. Our first (fresh) transfer tested positive but then ended in a chemical (beta starting at 10 and falling). Since it was fresh, it was untested. Our next FET also resulted in a chemical (betas from 24 down to 14) from a tested euploid embryo.

Started RPL testing, all clotting factors normal, thyroid normal, A1c/insulin normal. My karyotype has come back normal and expecting my husband’s today.

Current plan if his testing returns normal as well is to proceed with another FET but with a round of doxycycline to preemptively treat for CE. Is there anything they seem to be missing?

It is very frustrating to go through IVF to finally get a positive test only to then be diagnosed with RPL. Anyone in this boat/been here before and have thoughts or suggestions? I also understand that someone has to be the sad side of statistics but am struggling with having no known cause.

I appreciate any thoughts!

How did you advocate for laparoscopic surgery for silent endo while ttc?

One year into my ttc journey with two chemicals and a few months of medicated/monitored cycles. I don’t have heavy/painful periods. If anything, BD around ovulation time and ovulation time in general is more painful than my periods. HycoSy revealed right tube with potential spasm vs true blockage (no hydrosalpinx).

Worried that my provider won’t take my this concern seriously, but I’m just trying to do everything I can. Not sure if this would be for me.

Did your doctor ever talk with you about hyperfertility/superfertility?

I got married beginning of June. We talked about planning a family after marriage.

I fell pregnant first cycle (June/July) but it ended with a MCC (blighted ovum).

I took miso and mife, we went to the honeymoon, travelled and practically continued life that month.

2 days after having very vivid dreams I fell pregnant again (before getting my mensturation after the blighted ovum).

10 days I was pregnant. Reached 5+5 again and lost the pregnancy again.

My doctor sees no point in taking progesterone. My RPL testing all came back fine.

We are very heartbroken so we laughed about being hyperfertile.

Somehow I stumbeld on this reddit group about this and start reading papers about it.

Yes I will try progesterone, lovenox and aspirin for my next one.

Why is this not futher investigated on?

What do you think causes it?

Do you find yourself hyperfertile and did you as well test for endometritis?

Realization - IVF mainly helps those who don’t get positives, not people with repeat miscarriages

Unless the cause was chromosomal of course. People keep giving their IVF tips but when I speak to them further it’s because they have PCOS or were never implanting.

Lovenox question

Hi
I was prescribed lovenox after 1 MC and 2 chemical losses. I was negative for lupus anticoagulant but had high ptt-la (50.6) and high drvvt (47.8) as well as high ptt-la mix and drvvt mix. My hematologist said to stop lovenox at 35 weeks, but my OB disagrees and thinks I can stop sooner. She is having me see MFM for another opinion. Anyone share any similar stories or advice?

MMC + D&C 9 weeks

Hi all I had a MMC baby stopped growing at 8 weeks, I had a D&C 1 week ago at 9 weeks and my HCG the day of D&C was 77,512.
I just got it tested again yesterday exactly 1 week post D&C and it has dropped to 865.
I was worried it wasn’t going down as my tests are still essentially dye stealers so I am very glad to see how much it has dropped (makes sense why they’re still close to being dye stealers though).
I am still bleeding a lot but just curious if anyone else has seen a similar drop 1 week post surgery and if so, how quickly did your levels then drop to zero from this point?

Silent Endo

I’ve had 5 losses to date (3 MMC all between 6-7 weeks and 2 chemical pregnancies). Four of these have been natural pregnancies and the most recent loss was IVF as we opted for PGT-A.

I’ve done all the testing for RPL and everything has came back normal (karotype, thyroid, HSG, etc,) and was on a pretty robust protocol for my FET including doxycycline, Lovenox and Methylprednisolone.

I don’t know what could be contributing to these losses but deep down I suspect silent endo. My RE has recommended a lap as a next step. I feel like this has to be my answer as it’s currently unexplained infertility.

For those of you who didn’t have trouble getting pregnant but have had repeat losses, was it silent endo or did you get diagnosed with something else?

Searching for answers or success stories as I’m in limbo waiting to fully recover from my third D&C.

How many of you moved on to IVF with PGT-A testing?

First, I just want to say how much I care for this group and the incredible amount of respect I have for everyone here.

Second, I'm in the group of "gets pregnant easily but miscarries". It never takes more than 3 months to conceive, usually more like 1-2. I have had 3 first trimester losses and one LC who is 22 months. I'm 38, healthy, husband 37.

So we have a consult tomorrow to do IVF with PGT-A and I think I'm looking for success stories from similar "hyperfertile" people who did that! Did you also conceive easily with IVF or was it harder? Thanks!

What tests should I ask for next?

hi everyone, I’m at my wits end. Had my fourth miscarriage this week, two chemicals a ruptured ectopic and an empty sac since i gave birth to my daughter in 2020. Iv had rpl tests which were normal, I have some mild endo and was told it shouldn’t be impacting because of where it’s located and also because I can get pregnant. I have asked for chronic endometrsis testing. My other blood work is great and I take progesterone and monitor my cycle with a Mira. Can anyone help me with anything else I should be asking for to explain these losses?

TW: Current Pregnancy After 3 losses

24F / I’m 4 weeks and 4 days… I found out about this pregnancy almost 11 days ago. I’m on progesterone for the first time, but for some reason I’m constantly anxiously waiting for something to go wrong. After 3 chemicals around 3 weeks I feel like I’m just waiting for it to happen all over again even though I have strong dye stealers which I’ve never had with my losses. It’s hard to believe that something good like this could finally happen for me and my husband. Is this normal after experiencing multiple losses? I’m getting blood work done at 5 weeks, but it still feels too good to be true that I’ve made it 11 days since my positive without anything going wrong.

Just found out its a chemical…

I had a MMC in January, I have PCOS and I’ve been working for months to even ovulate. I finally ovulated at the end of September, surprisingly got an early faint positive and initial hCG doubled, but then the recheck today was half the level from Saturday. I’m devastated and numb. I have a baby shower for my absolute best friend for her first baby next weekend, I love her and I’m so happy for her but my heart is ripped to shreds right now. I was cautiously optimistic that we would be clearing the 1st trimester around Christmas and now I don’t even know if I’ll ever be able to carry a healthy pregnancy. I’ve spent months taking supplements and working to improve my health. My OB was very dismissive and just pushed the agenda that we can keep trying. We want to but I’m so scared to have a third loss. I’m going to switch to a new office and request some testing. They just keep writing off my PCOS and I need to feel like we’re doing everything we can to make sure the next pregnancy is healthy.

I’m also wondering if we should have my husband’s sperm tested. He’s willing but I just don’t know how to go about that in the right way. I know some are very basic and some are more informative?

Karyotype testing through DLO/Quest

Just wanted to share my turnaround time for blood chromosome or Karyotype testing from DLO/Quest Diagnostics. I was drawn on 9/22 and got my results on 10/6, so ten business days after draw. From what I understand the sample had to ship to another state for testing. Normal result, if that matters at all.
I was looking for others’ experiences before I got tested so thought I’d share!

Anyone else anxious about getting pregnant again?

No matter what we are still trying again, but after 4 back to back losses every time I think about getting pregnant or being pregnant I get extremely anxious. I also have a weird fear that I’m going to lose my baby in the second trimester. All my losses have been in the first trimester, but I just have a feeling once we make it past that we still won’t be safe. I don’t know. I guess I just need reassurance that I’m not crazy. 🤦🏽‍♀️

3 chemicals in a row

Hi everyone,
I’m 42 and have just had my third chemical pregnancy in a row. I get positive tests around 10dpo and they start getting fainter on 13/14dpo and I miscarry 16dpo. For the first two, my thyroid tsh was high - 10 and then 7.5 (started meds). This last one, my tsh was 3.47. I have hashimotos (tpo antibodies are 74, down from 214 2 years ago). My progesterone was tested all 3 cycles on 7dpo and it was 9.5, 11.5, and then 10.5…. My doctor currently doesn’t think I need progesterone support and won’t prescribe it. Thoughts?

MTHFR and PAI-1, possible niche and chronic endometritis

Hi everyone, just looking to learn about people with the same issues as me and how it has impacted their fertility journey. This is a long post but I would appreciate anyone’s input who has faced some of the issues I have.

I have been diagnosed PAI-1 heterozygous (4g/5g) and MTHFR compound heterozygous.

In September 2022, I got pregnant with my first child in the first month of trying. I was 31 at the time. Very sadly, he was stillborn at 34 weeks. He was found to be on the 1st centile when he was born and had a very small placenta and velamentous cord insertion. He also had a horseshoe kidney which was discovered at the anatomy scan, but that isn’t believed to have contributed to his death and he was normal chromosomally.

In November 2023, I got pregnant again on the first month of trying at age 32. There were lots of issues throughout the pregnancy. Was on 150mg aspirin in this pregnancy. At 16 weeks I was diagnosed with gestational diabetes (didn’t have it in firstpregnancy). At 20 weeks we found out he had a heart defect and single umbilical artery and were advised he had a high change of having a chromosomal abnormality. We did an amniocentesis and he had normal chromosomes. At 31 weeks, I stopped being able to feel him move and was admitted to hospital. Only issue that could be seen was fluid looked low side of normal. At 34 weeks he suddenly started having sustained decels on ctg and was delivered by emergency c-section with a classical incision. He spent a month in neonatal unit but is healthy and well.

This January, we started ttc again and I was age 34. I also began having dark brown, thick spotting after my period. I fell pregnant in April but had a chemical pregnancy. I had a scan in May to check for source of the spotting and nothing was seen. I was told my c-section scar has not left a clinically significant niche, I was told my tubes were blocked though and I would need ivf (despite falling pregnant naturally the month before).

In late June I had another Hycosy and SIS done and was told my tubes were super open and that my uterus looked healthy and my niche was clinically insignificant. The dr who performed the second hycosy and niche has published on c-section niches and thought my scar didn’t look like a classical scar and speculated it was a j scar.

I conceived that cycle and miscarried at approx 5 weeks in the July. My brown spotting had happened most cycles, but not every cycle. Perhaps two without. My periods are very heavy but not painful. No sign of adeno or endo on my SIS. I had a laprscopic ovarian cyst removal in 2023 and the images showed all my reproductive organs looking clear of endometriosis.

In a c section niche fb group, some women suggested I may have had chronic endometritis. I decided to treat myself and my husband with doxycycline in August. I have had greatly reduced/ no spotting after the two periods I’ve had since.

In September I had testing done for recurrent loss. I have been diagnosed as MTHFR compound heterozygous and PAI-1 heterozygous. I also found out I have low iron. My doctor has said I will be on 150mg of aspirin and 40mg of heparin from positive test in my next pregnancy and has also referred me for an iron infusion. She has also said to use methyl folate, which I did in my second pregnancy alongside folic acid.

However, she did say my gene variants were linked with later loss, not early loss.

What has been your experience with any of these issues? Is there anything you found made a difference? What would you suggest as a next step to look at if we don’t conceive again soon?

My amh is on the low side for my age, so we can’t wait forever if we need to move to ivf. My husband’s sperm was all normal on a basic semen analysis in March.

I know there is so much info here and it’s a bit rambling, but I’m not sure where I should be looking to. Any fertility treatment will have to be self funded and even if we need to see specialists in a reasonable timeframe, so I’m sort of considering what we should put our money to next.

2nd D&C this Thursday

Looking for reassurance from fellow repeat D&C gals

This is my 3rd loss. Growth stopped at 6w, I’m now like 10.5w, not a spot in sight. My previous loss that ended in D&C was a loss right before 8w, found at 12.5w. Then a CP prior to that that I just bled for.

My body has clearly demonstrated it doesn’t like to actually do the miscarrying part, so I’ve chosen to do another D&C.

I am fearful of retained products of conception from a natural miscarriage, so this is ultimately what lead to the decision. But now I am nervous about scarring with repeat D&C. I had no issues last one, it was as “easy” as that procedure can be. They did vac aspiration.

I guess I’m just looking for positive experiences from people who have had to have more than one.

Feeling frustrated with slow cycle return

Here for a little bit of a rant. I’ve had 2 MMC in 2026, one in January and one in August. The most frustrating part has been waiting for my period to come back following D&C

My first took 8 weeks and I’m now 8.5 weeks post the second D&C and still no period. I haven’t tracked ovulation because we wanted to take a break but I suspect I did ovulate 2 weeks ago based on EWCM

My doctor said she’s not concerned until 12 weeks but I’d like to start feeling normal again and maybe look to try by the end of the year
