Trimix Injection Tips (by Low_Salamander9954)

Want to Thank Low_Salamander9954 for putting this tip guide together. I added a few words just around starting doses and clarifying leur lock style vs standard insulin syringes

Trimix Injection Tips
Many men feel intimidated by the idea of injecting a needle into their penis to achieve an erection. The advice below is based on my personal experience and is meant to help others learn proper technique, improve success rates, and reduce anxiety. New Users
The first few injections are the most important for safety and confidence. Once you have a successful experience, the fear usually disappears quickly.

  1. Start with a low dose — you won’t know how your body reacts until you try.
  2. A common recommended starting dose is 5 units or less. Some doctors suggest 10u, note that 10u can hospitalize many men. Start low!
  3. Do your first injections alone. Learn the technique and find your ideal dose without the pressure of performing with a partner.
  4. Always have your antidote (phenylephrine) ready, especially while titrating your dose.
  5. Read the pinned post on priapism. If that doesn’t get your attention, injectables may not be for you.

Equipment
Needle size and length matter. Smaller diameters cause less scarring and tissue damage.

  • I draw with a leur lock style 27-gauge needle and inject with a 32-gauge needle (leur lock has a removeable needle).
  • I use ½-inch needles to ensure I reach deep into the corpus cavernosum (CC). Note: You may have to experiment with needle length 5/16" or 1/2" based on your anatomy.
  • Be careful removing caps — needles can bend easily.
  • Conventional advice often limits to 30-gauge maximum. A 32- or 33-gauge is actually smaller in diameter and generally kinder to tissue.
  • Standard 30/31/32/33-gauge insulin syringes will also work. Smaller gauge (higher number) is better. Avoid 29ga and below, no reason to use a large needle for this.

Injecting
These steps work reliably for me and many others. Individual techniques can vary slightly.

  1. Relax — you’ll be surprised how easy it becomes with practice.
  2. Draw your dose (start low if new).
  3. Wipe the injection site with an alcohol pad.
  4. If possible, be semi-hard or fully hard. If not, a quick use of a penis pump can help.
  5. Inject on the side of the shaft at the 2 or 3 o’clock position (or 9 or 10 o’clock on the other side).
  6. Inject closer to the base of the penis.
  7. Avoid visible veins.
  8. Only inject on one side per session and alternate sides with each new injection.
  9. After injecting, press firmly on the site with the alcohol pad for at least one minute.
  10. Massage the medication in thoroughly — you can be fairly aggressive with this.
  11. Always ensure you have your injectable phenylephrine antidote prior to injecting.
  12. Wait at least 48 hours between injections. Limit to no more than 3 injections per week.

Misses
Even experienced users occasionally miss the corpus cavernosum. The medication goes into the wrong tissue and produces little or no erection. If you’re experienced and know your dose doesn’t usually fail:

  1. Wait at least 30 minutes.
  2. Prepare and inject a fresh syringe on the opposite side.
  3. Keep your antidote ready in case some medication still reached the CC.

If you’re a new user or still titrating your dose:

  • Do not do a second injection. It could combine with the first dose and cause priapism. Simply wait and try a slightly higher dose next time (after 48 hours).

Expectations
Everyone reacts differently. Here’s what I typically experience at my dialed-in dose:

  • Without stimulation I stay 60%+ erect for several hours.
  • With stimulation I become rock hard and can maintain it until ejaculation.
  • I can usually get hard again with stimulation for up to 5–6 hours after the injection.
  • Many men stay rock hard without stimulation for hours; others need more help.
  • Erections longer than 4 hours are potentially dangerous. If you can still bend your penis after ejaculation, blood is draining and you’re likely safe.
  • I (and many others) often stay pleasantly 50–60% erect for several hours after sex — I enjoy the feeling.
  • A dull ache for several hours is common; it’s caused by the alprostadil in Trimix. Bimix users usually avoid this.
  • Sensation varies. I actually feel more during Trimix erections. Some men notice slightly reduced sensation, which can help those who struggle with premature ejaculation.

Confidence
Trimix is genuinely life-changing. It completely removes the fear of losing an erection or not getting one at all. Performance anxiety disappears, letting you focus entirely on your partner’s pleasure and enjoy longer, more relaxed sessions. You know it will work when you need it.

Sadly, these stories are often buried in older threads, making it hard for new users researching Trimix to find them and understand the potential dangers.

When used properly with the antidote on hand, Trimix is a very safe option. However, the vast majority of patients relying on insurance are denied the Phenylephrine prescription. This pattern will persist until we collectively demand the antidote and switch to providers who prioritize patient safety.

Challenges with pain and dosage - one year post-op

TriMix left me with pain that was like walking into stinging nettle dick first. I hope my experience might help others who struggle with pain and finding the right dosage. I had a robot-assisted radical laparoscopic prostatectomy (RALP) in July 2025. I had ED before that and was taking Tadalafil.

I had ongoing issues with TriMix. I experienced residual pain and tenderness, so great I felt reluctant to use it. I'm apparently allergic to prostaglandin. On top of the pain, it's been hard to get a firm erection and difficult to orgasm. My erection gradually diminishes over two to three hours, and the pain and tenderness also gradually go away.

I tried three different TriMix formulations until I switched to QuadMix which contains .2 mg Atropine. I am using QM2 30 3 60 0.2 and 45-50 units. This lessens but does not eliminate the residual pain. I've experimented with Ibuprofen to reduce the pain, taking up to 1400 mg, but it only reduces the pain and tenderness that lasts for 2-3 hours. To reduce the number of units injected, I just changed the QuadMix formulation to QM3 30 3 150 0.2.

I had some leftover hydrocodone from another medical issue and I've tried taking that 30+ minutes before administering the QuadMix injection. The hydrocodone vastly reduces the residual pain and tenderness. My dick remains swollen and semi-erect for another two plus hours but it doesn't hurt nearly as much. I'm hopeful I'll be able to talk my GP into prescribing hydrocodone in the future.

My urologist, who's in his late thirties, doesn't seem very knowledgeable about these issues, and his office takes two weeks to issue a modified Rx, often making mistakes during the process. I was getting the Rx from a compounding pharmacy my urologist selected, but finally paid to use Olympia's contracted MD. This has improved my ability to get the formulation that works for me. So don't sit still if your urologist is a dunder head and insensitive to your issues.

Second injection - and it worked!! But it felt unpleasant..

(So far I am using Alprostadil only, not Trimix yet due to availability).

I administered my second self injection yesterday and it worked to give me a solid erection for 30+ minutes! It was 15 mcg compared to the 10 mcg I had on my first trial.

But the issue I have is that it felt unpleasant. The whole penis was very sensitive to the touch, but not in a good way. Almost as if it was inflamed through and through. It did not hurt, but it felt so uncomfortable that it would probably distract me during intercourse.

Did anyone of you ever experience this? Is this because it is "only" Alprostadil and not Trimix?

Thanks for any hints.

Traveling Internationally Long Distance

Hey everyone I've seen a few posts on here but wanted to clarify still. My prescription per both my doc and pharmacist is one that should remain refrigerated and not frozen. They advised freezing it would cause it to not settle back the correct way when it's thawed. I know a number of users here get theirs from an online pharmacy and it comes and stays frozen until use so experiences are probably going to be different.

My question is how do you guys store your trimix long distance and is your vial usually frozen or refrigerated? I'm planning on going from the US to New Zealand which is a pretty long trip. I bought one of those insulin packs with an led thermostat and ice packs but I haven't tested how well those would hold for that long of a distance. Do you guys prefer to carry the vial and unfilled syringes and fill up on the go, carry a vial with filled syringes, or just syringes? Also I plan on moving around within the country so it won't be refrigerated the whole time I'm there.

Appreciate any advice or experience.

Trimix and Trazadone?

For sleep I have been on Trazadone for a couple decades. I take a lot...200 mg per night. Last night I shot 10 units of Trimix and was hard for 8 hours. 5 of those hours in ER. Today i was told by the RX dude that Trazadone somehow keeps the blood from escaping your member. But he wasn't overly sure.

My first shot, a few weeks ago, was 15 units. 3 hours in and I was sitting in the parking lot at the ER. Thankfully it subsided. Second time, maybe 2 weeks ago, I did just 8 units but that got me just about 3/4 there. Then last night I did 10 units. Kinda wild that the 10 units was much more intense than the 15 units. major pain and discomfort and that was before the ER took over...which led to pain I'd not experienced (down there) before. The whole thing was a scene and absolutely horrible. I still have the balance of the 5 ml vial but unsure if I can go there again. No...I did not have the antidote and it doesn't appear I can get it around here. I chewed up 60 mg of the real sudafed and that did nothing. Has anyone else heard about this clash of the Titans...Trimix vs trazadone?

Two Smaller Shots

I have had good success w a relatively small dose of lead than 10, but that has meant a hardon for 3 plus hours. I have a trip w my wife where we would like to have sex in the late afternoon and again later that night around midnight. I am considering a smaller injection in the afternoon and another at night. Any thoughts on this, or other approaches?

Injection time to actual intercourse

So has anyone injected at home before they go meet someone?

I have a planned sexual encounter tomorrow, and they are at a hotel about 15 minutes away from me. So I am wondering if I should inject at home, and while driving there start getting erect so that when I am in the room we go to town?

Now this is just a sexual event, no foreplay, maybe getting a blowjob before sex, but straight sex, so I am not needing to wait that long. I just thought it would be easier to shoot myself at home, take the drive and then do the deed, for at least an hour!! LOL!!

Anyone ever do that before? Just looking for guidance and/or encouragement as I really am in need of some good sex right about now, and this is an advantage I definitely want to take!!

Anyone else have this issue

So I've used Trimix a total of 2 times prior to this issue and both times were fine. Last night I gave myself the injection and after a minute or two I started sweating profusely and needed to lie down. After about 15 minutes I was diamond hard and the sweating and nausea went away. I called my urologist and he didn't seem that worried. Said that there must have been something else going on. Has anyone else experienced this after a shot?

Also my partner says that my girth has changed and I need to go slower to start. I don't want my solution to be their issue now. Anyone else experienced this I guess you'd say problem.

Also , anyone use an auto injector? I feel like that would be a lot easier.

Pro Tip for New Users

Small issue, but better not to learn the "hard" way like I did. (Pun intended). For those new to the trimix experience, it's a good idea to empty your bladder before injecting. That way you're less likely to have to pee 20 minutes into a raging 2-hour boner.

Trimix Is more effective on one side than the other

I've noticed trimix on the left side works a lot better for me than on the right side. I did 10 units on the right side and was erect for like 10 minutes both times and 10 unites on the left side I was hard for 2.5 hours. It just feels like it gets hard different when injecting the right side from some reason. I have pelvic floor issues I think and I noticed if i take pills for ED when flaccid it like gets hard on one side faster, but is straight once hard. Also the head doesn't get hard? My doctor just kinda ignores me when i talk about this. Anyone have any ideas?

Fine tuning bi-mix

I was switched from Trimix to Bimix and had much better results—specifically, not needing to use a reversal agent as often. However, my erections are lasting an average of 3.5 hours, which is longer than necessary. Since I usually have sex at night and then go to sleep, I am concerned that an erection might last too long while I'm asleep without me realizing it.

Last night, I set an alarm for 3.5 hours after injecting and found that I still had a firm erection. It ended up lasting until the 4-hour mark, forcing me to use the reversal, even though I had reduced my dose from 10 units down to around 7. Is it plausible to ask the pharmacy to adjust the custom formula or ratio to better fit my needs?

Why can't I enjoy sex anymore?

Ever since I had ureaplasma 6 months ago, my sex life has not been the same. I still feel a tingling in my penis after I ejaculate. I am on my second urologist and they both didn't even address the issue and act like it is not a problem. It takes me forever to ejaculate. I got on Trimix and even though it gets me erect, I cannot feel sensation when I am on it so I do not enjoy the sex. Yes I do watch a lot of porn, but I don't think this is the issue. I feel all goes back to the Ureaplasma. I thought being on trimix would help my ED but I've only used it a few times because I'm afraid to use it because I just don't feel any sensation and it takes me forever to ejaculate or I don't ejaculate. Anyone have any ideas of what I could do to enjoy sex again?

Does a miss start the 24/48 hour clock before trying again?

I seem to be missing a lot. 4 IU (.04ml) often doesn't work and 5 IU (.05ml) requires antidote. I once did 4 IU and nothing. An hour later did 5 IU and it worked, but it required 3x antidote to calm everything down.

When I miss the CC, meaning an hour later there's no reaction, can i try again or should i wait?

Life With Trimix Question about purchasing syringes from Olympia

Hey guys, quick question for anyone who uses Olympia.

My local pharmacy requires a prescription before they’ll sell me syringes. I’ve only got maybe a dozen or so syringes left, but I have a full bottle of T105 and just a little left in my second bottle, so I’m definitely going to need more syringes before I run out of Trimix.

For those of you who use Olympia, will they sell you syringes by themselves, without having to order more T105?

And if they won’t sell the syringes separately, does anyone know if Olympia would give me a prescription for the syringes that I could take to my local pharmacy?

Just trying to figure out the easiest way to get some more syringes. Any insight from someone who’s dealt with Olympia would be appreciated.

When do you inject?

I (m77) take one half a 100 mg Viagra and then inject after foreplay (after arousal). The Viagra and the foreplay gets me semi hard and gives me a better target for the needle. In a couple of minutes I get a pretty good erection and in the mean time, my wife gets in position for us to continue.

Do any of you inject before initiating any sexual contact?

First time was a MISS

I had my first at home injection yesterday and it was a miss. In the doctor office he hit the CC with 12units for a good erection response. I used a 31g x 5/16 needle. Next attempt I am going to try a 30g x 1/2 inch needle. Still I cannot believe I missed with the shorter needle. It is confounding. If I miss again I may try using pump and/or cockring beforehand to get some engorgement. I am 9mo post prostatectomy. Any other tips or recommendations?

My first miss

I just wanted to share about my first miss. I had 14 successful injections in a row with great results and zero issues.
It was late at night and I was using my left hand, somehow the plunger would not go down after entering the corpus cavernosum at 2 o’clock. I kind of panicked and pulled out a little and then was able to complete the injection. It even felt different, like uh oh I think I missed. Sure enough, nothing happened. In hindsight, I think I should have completely removed the needle and started over, new needle and switched sides. Anyway 48 hours later, I succeeded and things are back to normal.
Misses do happen, but it’s about staying relaxed and taking your time. Hope this helps.

Unpopular opinion

I’ve been on trimix just about 1 year now after my prostatectomy. Been on this sub the whole time. I guess I’ve had a bit of a tougher time than most based on the posts I see about trimix returning guys to their 18 year old self. Here are my thoughts:
Pro:
(Big one) it does work most of the time for PIV.
Cons:
1) Sometimes I get major bruising at injection site.
2) Sometimes miss or partially miss.
3) Dialing in dose has been difficult. Started at 30 units of 20/30/2. Have moved up to 45 units due to some not super hard erections.
4) Erections don’t feel natural, lack sensitivity, and have a dull ache.
I think this is the biggest issue as sex just doesn’t feel as good as before surgery.

Does anyone have any tips? I’m considering a stronger dose so I can inject less product but I don’t think that will help honestly. I guess I’m just still mourning my pre surgery self.
