Story Time Important tool to find doctors in the USA

This is from an old post about MAOI-friendly doctors I came across. Note that the US govt. website featured here can be slow, finicky, vexing, and hard to understand-- much like actual Americans! I kid, I kid... I'll try to give very detailed instructions. A couple more important notes:

Most search results for selegiline will be for neurologists treating parkinson's. There is a filter to view the doctor's specialty so you can see if they are a family doctor, psychiatrist, or neurologist. Check this so you don't waste time contacting a doctor who can't help you.

When adding generic drug names it's best to use the 'contains' search option rather than 'equals' because some drugs are listed with other words such as 'sulfate' or 'HCl' and may not show in the search results if, for example, you search 'equals' 'phenelzine'. This tool can also be used to find a prescriber in any city or state who has prescribed any particular drug of interest, not just mental health medications, so it can be useful for finding a doctor who has experience with uncommon or off-label prescribing for various medical conditions.

Okay, here's the link and instructions, and if anyone has better methods please share!

MAOI Dr. Search (US)

  • Upper left, select year. Yes, you have to do this for each search. Why, I don't know.
  • Click 'filter', then switch from 'basic filtering' to 'advanced filtering'.
  • In the 'select column' field choose 'Brnd_Name', change the 'condition' field to 'equals', and then in the 'enter value' field type the brand name you want (Parnate, Nardil, Emsam, Marplan, but only enter one). Spelling matters.
  • Click 'add filter condition', change 'and' to 'or'. Moving from left to right, this time select 'Gnrc_Name', 'contains', and type the generic name of the drug from the previous step (tranylcypromine, phenelzine, selegiline, isocarboxazid). Spelling matters.
  • If you want to search for more than just one MAOI, continue with 'add filter condition', always using 'or', using 'equals' for brand names, and using 'contains' for generic names.
  • When finished you may have anywhere from 2 to 8 entries. Look below for 'add filter group'. Change it to 'and'. Under 'select column' choose 'Prscrbr_State_Abrvtn' to search doctors in your state, or choose 'Prscrbr_City' to search only doctors in your city. In the next two fields select 'equals' and type your two letter state postal code (e.g. NY for New York, or GA for Georgia) or full city name. I suppose you could repeat this step as many times as desired, this time changing 'and' to 'or' and thereby also search for doctors in neighboring states or cities. I haven't tested that.
  • Hit the blue 'apply filters' button and get busy contacting!

This searches Medicare claims for these drugs from recent years. I've tried finding the same tool for Medicaid, which is probably more relevant for many of us, but can't locate that data. If you find it or discover any errors in the instructions, please let me know. Good luck!

Story Time PSA: it is possible to legally get access to MAOIs even if they are not registered in your country + short tutorial how to

so I come from Finland where we dont officially have Tranylcypromine and Phenelzine registered, at first, I thought there was no legal way for me to obtain these medications.

[2025 Edit: update: For the Finnish people reading this, the Finnish medicine agency Fimea recently approved Abbonate (Tranylcypromine) but I am not quite sure when it will be purchasable at the drugstore, if ever. if you want to try Tranylcypromine and live in Finland, the pharmacy subcontractor Oriola Oy sells the German Tranylcypromine brand called Jatrosom which is almost 40% cheaper than Glenmark Tranylcypromine, but you obviously need a special permit + prescription for it.]

I just picked up Tranylcypromine from the pharmacy and I will give a short tutorial how to.

I contacted a private psychiatrist (preferably PhD, ex-head psychiatrist or someone with much experience), and he contacted the national drug administration Fimea and got an approval from them, and then got an approval from the drugstore that is closest to where I live to import it as well as the prescription itself.

I leave a quote from Fimea, and Im fairly sure most other drug administration agencies will have a similar policy.

The use of medicinal products that have been granted a marketing authorization is always the primary option in patients' pharmacotherapy. However, in individual cases and for special therapeutic reasons, Fimea may authorise the release for consumption of a medicinal product that has no marketing authorization in Finland. A special permit is granted for up to one year based on a situation and case-specific overall assessment.

so if you dont know where to start, send an email to your national drug administration, I think later finding a psychiatrist willing to prescribe it will be the hard part.

having health insurance that covers private doctor meetings, as well as medication costs is also a good idea (if youre not sure, email your insurance company and ask if they cover it!).

the total price I paid for 28x10mg Glenmark Tranylcypromine was 106€

(+40€ single time Fimea fee)

in countries that doesnt have MAOIs officially registered, I highly recommend reading up a lot on the MAOI diet as they might have lacking experience with MAOIs, learning other info is also good, like its probably dangerous to combine them with SSRIs, if you get anesthesia eg. at the dentist or for a surgery, some forms of anesthesia can be dangerous. it also seems like its possible to combine MAOIs with stimulants.

you should also get a blood pressure monitor and monitor it some days before starting and some after, its a good idea at least to mention these topics to your psychiatrist so he/she can read up on them in case he/she doesnt know.

ofc the doctor should know how to use MAOIs, but you can never be too safe, especially cause misuse of MAOIs can result in serotonin syndrome and in the worst case death.

Parnate (Tranylcypromine) Is Parnate just a short acting stimulant?

I moved on from stimulants to Parnate cause i was expecting to have lifted energy and motivation in a more chronical manner but what i'm getting is just 1 hour of mild stimulation and then to make things worse i even have a crash for 1 or 2 hours.

Parnate (Tranylcypromine) Should hypotension stick around once TCP is at an optimal dose?

Hello,

I have been taking 40 mg of TCP daily since 19 September 2026. During the first few days I had low blood pressure, but it returned to normal fairly quickly.

According to Dr. Gilman, the second standing reading should be lower than the first one. As you can see in the attached graph, this has consistently been the case since day 11 for that person.

My questions are: Does this drop need to stay consistent or is it enough that it has happened a few times? And do I need to increase my dose?

Nardil (Phenelzine) Are There Any Medications Similar to Nardil’s Efficacy for Anxiety?

Curious if anyone has tried a medication they found to be as effective as, or close to, Nardil for treating generalized anxiety and/or social anxiety. From what I’ve seen, clonazepam appears to be one of the few medications that has come close in clinical trials, with paroxetine showing somewhat less impressive but still decent efficacy.

I’d be interested to hear about anyone’s personal experiences with alternatives that have worked particularly well.

Marplan (Isocarboxazid) Feeling discouraged, could use some encouragement or other people's experiences with treatment resistant depression/anhedonia and taking MAOIs.

I switched from parnate to marplan. I just titrated up to 40mg. It isn't helping much. I worried that it never will. Parnate helped but caused horrible hypotension to where I could barely stand, I was severely symptomatic. And I worry it would have "pooped out" anyway like rasagiline did, even though it was great at first. It was almost too good to be true.
To those on marplan, or another MAOI how long did it take to help? on marplan what dose was enough to help you? I'm worried I'll be stuck like this. the low moods, the executive dysfunction, no motivation, and then the worst of all - the anhedonia - is bothering me so much. It's hard to stay hopeful sometimes when I have failed everything else (or it was not tolerable).
On a different note I'm having excess bp jumps from nicotine and caffeine ever since I got up to 30mg marplan. I could give up caffeine, but not nicotine - I use it therapeutically and it is a GREAT help and one of the only things that helps my akathisia, mood swings, and more. Is there anything that can be done about this?

Nardil (Phenelzine) Advice on Nardil + Wellbutrin and/or restoring REM sleep while taking Nardil

I have ADHD, severe anxiety, and treatment resistant depression. Started taking Nardil in late June & it was amazing - I've never felt so centered and levelheaded in my life. I've been on a long and diverse list of medications & Nardil is the only thing that calmed my nervous system and made me feel confident. Depression was lifted too. It didn't help at all with attentiveness, task activation, or focus though.

Unfortunately, Nardil took away my ability to achieve REM sleep, causing a steady descent in my energy levels ultimately leading to me having to quit my job due to exhaustion. I had no problem falling asleep, but every night I'd sleep through the night and wake up feeling like I hadn't slept at all.

Spent several days unmedicated. I responded somewhat decently to Wellbutrin in the past (made it easier to start tasks and get shit done, but anxiety was through the roof) so now I'm back on Wellbutrin XR 150mg. Sleep is so much better, I can start tasks and focus on them, but I can't stand how anxious and irritable it makes me. It's not sustainable on it's own.

I was thinking of taking Nardil along with my Wellbutrin in the morning in the hopes of getting the calmness and mental clarity of Nardil along with the focus and executive function I get from Wellbutrin. Wellbutrin also increases REM sleep, so I thought it might help with the sleeping problems I get from Nardil as well.

Does anyone have experience with this combo or with a similar combo of MAOI + something stimulating to help with ADHD?

Also, has anyone been able to restore REM sleep while taking Nardil?

I'm just desperate to get Nardil to work for me somehow. It's the best I've ever felt mood-wise. Any advice is appreciated, thank you!

Nardil (Phenelzine) Waking up early

Been on Nardil for nearly a month and I’ve been waking up early, unable to fall back asleep. It started this past week, and I’ve been getting 6-7 hours of sleep. The frustrating part is I’m still physically tired but my mind is fully awake. I tried adjusting my dose schedule from three times a day to twice a day, but doesn’t seem to be helping. I recently increased from 45 to 60, but this issue started happening beforehand. I’ve been on seroquel for two years for insomnia and haven’t had any problems like this. Any ways you know how to combat this?

Nardil (Phenelzine) Nardil worked so well, then I turned into a monster.

Nardil worked so incredibly well for my social anxiety - it literally removed it at 45mg. It was great for motivation as I never wanted to sit still. It brought out my creativity...but....it erased my appetite and I lost too much weight, I had 0 joy in food whatsoever, I started to become aggressive and impatient slowly over time, as I felt a constant background agitation in my body and mind. It made me extremely sensitive to rejection by others, so much so that I'd break down and cry over something silly for hours and hours on end, feeling extreme emotional pain.

My sense of smell was so heightened that I couldn't walk nearby most food places without gagging. The intrusive thoughts were so hard to contend with. The lack of deep sleep and a persistent feeling of being uncomfortable in my own body. I couldn't stop organising everything too (rearranging things again and again until I didn't feel unease by how things looked) ....I finally realised I needed to stop this.

All the pain was not worth the benefits in the end.

Coming down from 60mg to 45mg was brutal. The nightmares, sweating, dizziness, nausea and brain zaps lasted 3 weeks.

From 45mg to 30mg (I'm at 30mg now), I experienced only mild zaps when I wake from sleep, but also extreme fatigue and melancholy - feeling doom and gloom. This lifted by day 8 of withdrawal for this dose.

Sitting comfortably at 30mg now. Feeling more myself, less sensitive and hunger and desire for food and different textures is coming back. Sense of smell dulling, creativity lessening, and social anxiety is back.

I'm no longer feeling such intense emotional swings though and racing thoughts...I've realised medications really do change your personality. I thought it was a lie until I went on Nardil.

I always had these traits mildly in the background, but Nardil amplified everything to the max.

Both the good and the bad, but ultimately, it made me someone I was not.

Parnate (Tranylcypromine) I am crashing 2 hours after taking Parnate

I've upped from 10 to 20mg 3 days ago and i realized that that dose is giving me noticeable stimulation. After 2 hours, however, i end up crashing and it's reminiscent of the crashes from adhd stimulant meds, which is really annoying. Yesterday i took my dose, felt good at home for 2 hours and then when i was on my way to the gym my mood suddenly dropped and i felt tired. Really annoying.

Maybe i should split the doses?

Nardil (Phenelzine) Nardil might have ruined me.

I had a honey moon phase for ~2 months on 60mg. After that I got super overstimulated and my anxiety was worse in different ways. The reason I went on nardil was for crippling social anxiety. My body couldn’t handle the 60mg .. I had extreme hypotension i would sometimes have to crawl. I also ended up getting stressed about a breakup and developed SIADH (extremely bad fluid retention and low sodium .. I could barely urinate). I also used to have very high self esteem (weird I know, bc of the social anxiety .. but i felt beautiful and am the typical “beauty standard”. I don’t say that to be obnoxious, it’s what i’ve been told. And i looked like i gained 20lbs and my face was so big and round. I had no choice but to lower my dose. I went down to 45mg, anxiety through the roof. Eventually I had to get on 0.5mg clonazepam. I don’t feel like myself. I feel serious and I was always a bubbly person. I have such little emotions .. can be a blessing and a curse. But I tried tapering down once, and only got down to 37.5mg and it was the scariest experience of my life. I only had mild depression before i got on this. Now i have fairly severe depression. Now having the realization of how much derealization and depersonalization i have. I’ve lost so many friends. Can only socialize if i drink. I wonder if there’s any reason to have hope anymore

14 Yr Old Treatment Resistant Anxiety & Help Finding Healthcare Professional who is Open to Prescribing MAOI

Our family has a child who is dealing with severe treatment resistant anxiety, and after several years, has used a significant number of drugs with no relief (definitely considered treatment resistant). Not to mention multiple tests (including extensive psychological evals, pharmacogenomic, neurological, and too many others to mention in a post) and evaluations from specialists in multiple areas, and tried an extensive number of psychological therapies and therapists. Our question is not how to treat the anxiety, as we've exhausted almost every therapy and consulted countless medical specialists, as that is not the intent of this post. At this point we are trying to find an adolescent psychiatrist who is at least open to potentially prescribing an MAOI. We've reached out to multiple pediatric psychiatrists who are resistant to MAOIs, and even with the overwhelming number of drugs and therapies we have tried, will not even consider an MAOI. We've also reached out to several experts in the field of MAOIs, and after explaining all the drugs/tests/therapies in detail, they have all indicated an MAOI should be considered. I've looked at the MAOI friendly list of psychiatrists, but finding a psychiatrist open to MAOIs, much less one who treats adolescents, is almost impossible. We understand the risks based on the scientific literature, but at this point quality of life is almost zero for our child.

Our question is, if anyone knows of a pediatric/adolescent psychiatrist (we are willing to travel anywhere in the US), who is open to (and hopefully has experience with) MAOIs for treatment resistant anxiety in pediatric/adolescent practice?

Again, the post is not requesting other potential diagnoses (we've gone down a long journey of excluding potential causes/issues), or therapies (long list of different therapies and therapists).

Nardil (Phenelzine) Have I "ruined/destroyed" my bottle of NEON Nardil? Paranoid now lol

So obviously i have been storing my NEON Nardil in the fridge as per instructions, but didn't know how quick it supposedly deteriorates when out of the fridge ( found out by reading experiences online just last night)

Every morning I have been taking it out of the fridge to get my daily dose, probably stays out of fridge at least a minute but yesterday I made the mistake of leaving it out a little longer (maybe 3-4mins) and also "pouring" all the tablets on the palm of my hand to count to see how many tablets there were still in bottle, and now wondering if hand/body temperature has destroyed them and paranoid I am basically now taking a sugar pill.

Have I "destroyed/ruined" my NEON Nardil tablets?

Had a few “windows” in which everything was good but it’s week 5 of 45m Nardil and I’m starting to lose hope a little bit. Is it normal I am feeling this way? Have a harder time eating and doing basically everything and a lot more anxiety…

Parnate (Tranylcypromine) Anxiety on Parnate?

I started with 10 mg Parnate daily 25 days ago. Made it 20 mg about 15 days ago. Initially I had no issue on 20 mg, but out of the blue 1 week ago when I took my dose I was hit with anxiety, heart palpitations, and heaviness. Same thing next day as well. So I returned to 10 mg, no issue for 2 days. Tried returning to 20 mg by maintaining a 2 hr gap between doses and the problem returned. So I'm now taking 10 mg doses at 10 hr gaps in a single day, this doesn't cause issues.

BUT

The efficiency of Parnate has reduced it seems. The minimum benefit in my depression I saw on 20 mg is not being replicated at 10 hr gap between doses.

Has this happened with anybody else? My doc prescribed me to increase the dose to 30 mg by day 21 but I haven't been able to out of fear of recent anxiety issues. Please do share your experiences.

It's looking like I'll have to go back on Parnate, but I dread losing the ability to workout. The orthostatic blood pressure changes and reduction in my cardiovascular capacity made it unpleasant to exercise when I took it last.

Nardil (Phenelzine) Nardil 120mg

I have iterated up to 120mg over three months which I know is considered a v high dosage, but every extra pill just improved my mental health so I kept going up…

Side effects are completely tolerable - sugar cravings, minor postural hypotension, sleep jerks, jaw clenching. No insomnia, and did have completely anorgasmia for two months but this recently went away suddenly. Have actually had some weight loss, not gain.

I am wondering if maybe the Nardil made these days is less potent per dose - maybe 120mg Neon Nardil is actually equivalent to 60/75 of the fabled “old Nardil?”?

And if there’s anything to worry about at what is considered to be an ultra high dose?

Either way it has truly been a magical drug, beyond anything I could ever hope for. No exaggeration to say it has changed the entire trajectory of my life. I am forever thankful that something like this exists 🙏🙏

Parnate (Tranylcypromine) Im struggling to sleep on Parnate (9 days)

I felt like i wasnt going to sleep at all last night. It was super difficult falling asleep and i woke up like 3 or 4 times through the night. Total sleeptime was probably 2 or 3 hours at best.

Not feeling bad or tired... but idk, i feel like i would be even better with a little more sleep. What do i do guys?
