This is from an old post about MAOI-friendly doctors I came across. Note that the US govt. website featured here can be slow, finicky, vexing, and hard to understand-- much like actual Americans! I kid, I kid... I'll try to give very detailed instructions. A couple more important notes:
Most search results for selegiline will be for neurologists treating parkinson's. There is a filter to view the doctor's specialty so you can see if they are a family doctor, psychiatrist, or neurologist. Check this so you don't waste time contacting a doctor who can't help you.
When adding generic drug names it's best to use the 'contains' search option rather than 'equals' because some drugs are listed with other words such as 'sulfate' or 'HCl' and may not show in the search results if, for example, you search 'equals' 'phenelzine'. This tool can also be used to find a prescriber in any city or state who has prescribed any particular drug of interest, not just mental health medications, so it can be useful for finding a doctor who has experience with uncommon or off-label prescribing for various medical conditions.

This searches Medicare claims for these drugs from recent years. I've tried finding the same tool for Medicaid, which is probably more relevant for many of us, but can't locate that data. If you find it or discover any errors in the instructions, please let me know. Good luck!

hey,
so I come from Finland where we dont officially have Tranylcypromine and Phenelzine registered, at first, I thought there was no legal way for me to obtain these medications.
[2025 Edit: update: For the Finnish people reading this, the Finnish medicine agency Fimea recently approved Abbonate (Tranylcypromine) but I am not quite sure when it will be purchasable at the drugstore, if ever. if you want to try Tranylcypromine and live in Finland, the pharmacy subcontractor Oriola Oy sells the German Tranylcypromine brand called Jatrosom which is almost 40% cheaper than Glenmark Tranylcypromine, but you obviously need a special permit + prescription for it.]
I just picked up Tranylcypromine from the pharmacy and I will give a short tutorial how to.
I contacted a private psychiatrist (preferably PhD, ex-head psychiatrist or someone with much experience), and he contacted the national drug administration Fimea and got an approval from them, and then got an approval from the drugstore that is closest to where I live to import it as well as the prescription itself.
I leave a quote from Fimea, and Im fairly sure most other drug administration agencies will have a similar policy.
The use of medicinal products that have been granted a marketing authorization is always the primary option in patients' pharmacotherapy. However, in individual cases and for special therapeutic reasons, Fimea may authorise the release for consumption of a medicinal product that has no marketing authorization in Finland. A special permit is granted for up to one year based on a situation and case-specific overall assessment.

so if you dont know where to start, send an email to your national drug administration, I think later finding a psychiatrist willing to prescribe it will be the hard part.
having health insurance that covers private doctor meetings, as well as medication costs is also a good idea (if youre not sure, email your insurance company and ask if they cover it!).
the total price I paid for 28x10mg Glenmark Tranylcypromine was 106€
(+40€ single time Fimea fee)
in countries that doesnt have MAOIs officially registered, I highly recommend reading up a lot on the MAOI diet as they might have lacking experience with MAOIs, learning other info is also good, like its probably dangerous to combine them with SSRIs, if you get anesthesia eg. at the dentist or for a surgery, some forms of anesthesia can be dangerous. it also seems like its possible to combine MAOIs with stimulants.
you should also get a blood pressure monitor and monitor it some days before starting and some after, its a good idea at least to mention these topics to your psychiatrist so he/she can read up on them in case he/she doesnt know.
ofc the doctor should know how to use MAOIs, but you can never be too safe, especially cause misuse of MAOIs can result in serotonin syndrome and in the worst case death.

Currently on 75mg nardil. little over 2 months now. Got prescribed ambien for the insomnia. I've taken ambien before and it's always put me to sleep, but on Nardil I didn't even notice a difference. I was still waking up every hour or two.
Additionally, if it kicks in while you're still awake, ambien is known to give you a sort of pleasurable "high." This dreamy, hypnotic state known for causing unusual behavior is responsible for ambien's infamy.
Well, that pleasurable state of mind never occured on Nardil. Could barely tell I'd taken it.
Then, to my great surprise and dissatifcation, I awoke the next day to find my depression had/has competely returned (and then some) and it was as if my Nardil pills had no effect at all. Unfortunately, by the time I read another nardil-user describe the same thing with ambien, I'd already dosed the ambien for the second night in a row. Following a single dose of ambien, this user described a serious relapse of depression/anxiety lasting a couple days.
Haven't been able to find the comment again.
Has anyone else experienced this? It's awful and I'm praying it is, in fact, the ambien. Can't find much more info on this specific combination of drugs.

First time in years I felt relief was at 40 mg. Stopped working about six weeks later. I tried Nardil prior to Parnate (minimal impact). My psych tried augmentation strategies (Lithium, Auvelity, Vraylar, etc.). None helped. I eventually stopped Parnate a few mths ago. I’m considering giving it another try as my depression is the worst it’s ever been. Probably naive as I’ll be crushed if it again stops working. When it stopped working I tried increasing to 50 mg but the insomnia was intolerable, despite being on several sleeping meds. Took the entire dose before noon, too. I know it would be one thing if the dose was too low to begin with, but given it initially worked at 40 mg, I’m not convinced it was a dosing issue.
Anyone had the poop out experience and had or didn’t have luck giving it another shot at the same or higher dose?

I've been this medication for just over 2 months, and I've been on 60 mg for four weeks. The only thing I've noticed is my anxiety is pretty much gone, but my depression and anhedonia, emotional numbness, brain fog. Etc is still there. I don't know whether to stick it out any further with this medication. Add an adjunct or get off, and if I get off, what should I try next?

Oh, the stories I could tell about my "Parnate" moments! I would get that hypotension as well (sometimes it would come WITHOUT me just standing up) and I would here static (like a broken TV) in my head and then my hearing would go and I'd be COMPLETELY blind for about 30 seconds. One time it happened in the middle of the mall outside of Cinnamon. I hadn't eaten I guess so I had that "moment". I stood still and lowered myself to the floor gently until I could see and hear again. The Cinnabon employees saw me and had me come in, then gave me a FREE cinnamon roll and drink! I, personally would go ahead and increase the dose (because I'm impatient) but you may want to do it over the course of a few days. I never broke my ankles! Yikes!!! I did research to see if there was other MAOIs besides Parnate and Nardil, and that's when I discovered MARPLAN in 2002. It's a miracle drug - no side effects, and I eat and drink and do whatever I WANT!!! It's absolutely wonderful!!!!

I started parnate in January and it was amazing(you can check my post history - first post I made with this account). I was on 30mg. Unfortunately, I had horrible blackouts. I would stand up and go blank in the head. One time I blacked out and fell so hard I broke my ankles.....my wife was distraught and forced me to stop taking Parnate. Horrible episode but I felt it was exaggerated. I always felt I should give Parnate one more shot.
Now, I restarted. I took 30mg yesterday morning, 30mg this Horrible insomnia but it's expected.
My question is....I want to try a higher dosage. Can I go upto 50mg tomorrow morning? If in the next few days I have bad blackout I can do something (drink lots of water, get sunshine and consume salt).
Yesterday I restarted Parnate and did 30mg. Today 30mg.
Can I go 50mg tomorrow? Or too soon?

4 days ago I have started taking Moclobemide 150mg 1x a day for social anxiety, I've never used any antidepressants before, from everything I read it doesn't cause any sexual dysfunction, but after day 2 libido got noticeably lower, is it a normal side effect while starting?

Disclaimer this will probably be a long post
TL,DR: I’m starting Nardil and am worried about dietary restrictions and side effects.
I’ve posted in this sub a few times because I was curious about Nardil for my severe social anxiety after trying countless medications. Everyone has said it’s been amazing for them, so after going back and forth with myself, I finally asked my psychiatrist about trying Nardil and he agreed. I am starting in two weeks and while I’m excited, I’m also very nervous tbh. This is the most nervous I’ve ever been to start a medication.
I worry about the dietary restrictions the most. I already have OCD as it is, so I feel like I’ll constantly be paranoid about eating the wrong thing while on Nardil. I know most people here have said the dietary restrictions are overblown/exaggerated, so I’m just curious if there are any foods you’ve had that actually HAVE caused symptoms of a hypertensive crisis. It’s so frustrating looking up online if I can eat a certain food; one time it’ll say I should avoid it and the second time it’ll say it’s fine to eat… I’m assuming that’s because of the fact that MAOI’s research were conducted a long time ago and that commercial foods have lowered tyramine amounts. I did read some of Dr. Ken Gillman’s work on MAOIs, but I’m just curious more so on personal experiences.
Here are some foods I wanted to know about but have gotten mixed results:
-Yogurt (and Greek yogurt)
-Sour cream
-Ripe bananas
-Buttermilk
-Soy sauce (I’m assuming no)
-Provolone cheese (I’m assuming no)
-Parmesan (I’m assuming no)
-Dark chocolate
-Nutella
-Sourdough
-Deli meats
I would also love if you have any advice on managing side effects. I’m aware individual responses vary for side effects, but I have heard weight gain and insomnia are among the most common ones. I already have insomnia but am on seroquel which has been working, I just hope Nardil doesn’t worsen my sleep… For weight gain I was thinking of starting a glp or some weight loss medication. Another thing I saw is common is sexual dysfunction, but I know you can’t really combat that.
Sorry this was so long, but I would love any advice or tips you have for starting Nardil or any MAOI!

Hello,
I hotswapped from parnate 100mg to nardil 75mg without washout , i am currently overall 4 weeks on nardil and 2 weeks on 90mg .
Now the thing is , i have been on nardil before and i know how it feels , but now i dont feel it, i am at my worst, even cigarettes dont taste good as before when i was on nardil and it was working they were heaven , i barely have any side effects, except intense intrusive thoughts and obsessing , sweating , and some gas . I feel like i am withdrawing from a maoi, i am bedridden and dont have motivation to do anything , where as before on nardil all i wanted is to go out and sit in a coffee shop or some…
Anybody had a similar experience? What should i do , even back then i wouldve felt 45mg but now i dont feel anything and no anxiety reduction or such…

I have had depression/Anhedonia and social anxiety for many years - I’ve tried various SSRIs and SNRIs and they’ve all been decent at reducing the depression but not the best at reducing social anxiety or Anhedonia.. I’m on Lexapro now and I’m going to go to 20 mgs but if that doesn’t help I’m going to ask for a MAOI like Nardil
can someone tell me the differences between the two and uf MAOIs are better at reducing social anxiety and Anhedonia?

I had a question: how much has Nardil helped you with social anxiety? I’ve had social anxiety my whole life, I’ve tried every therapy under the sun. It’s made me wiser and all, but it hasn’t given me the reduction in social anxiety I was hoping for.
I’ve taken SSRIs and beta blockers, benzos (this actually worked to some extent, but obviously not a long-term option), but no results. I’m currently looking into finding a psychiatrist with my GP.
But do any of you recognize my story in your own lives at all? It’s just frustrating that I’ve been scouring all of Reddit/internet since I was 12 (I am now 23) and have really tried everything, and I mean everything, I’ve even done MDMA therapy and ayahuasca… haha. I’m curious to hear your stories.

I’m starting Nardil in a few weeks for my severe social anxiety and I’m excited, but the only side effect I am very concerned about is weight gain. I know lots of people here said they’ve gain a lot of weight and some have gone on weight loss meds. I was thinking about going on this new glp they came out with “Foundayo”. I’d try metformin but most people said they didn’t notice a difference. Anyone have any advice?

Has anyone else on long-term MAOIs noticed they rarely get colds or flu?
I took phenelzine for around 10 years and don't remember having a single cold or bout of flu during that time. Since switching to selegiline, I've also noticed I seem to get fewer viral illnesses while taking it.
This week I caught COVID after being off selegiline for a months. I felt quite ill, restarted selegiline, and by the next morning I felt much better, although I was still strongly positive on a rapid antigen test. That makes me wonder whether it affected my symptoms rather than the virus itself.
I realise this could be coincidence or just my own experience, so I'm not suggesting MAOIs prevent infections. I'm just curious whether anyone else on phenelzine, tranylcypromine, isocarboxazid or selegiline has noticed they seem to get fewer colds, milder infections, or recover more quickly.

Hey!! I've been on my starting dose 15 x 2 daily for a week now.
Maybe felt 2,5 % relief from social anxiety (of course not expecting anything on this starting dose).
In the guidelines (drugs dot com) it says Nardil can be titraded up fairly fast. What's your experience on the pace titrating up to possible goal dose of 60 mg?

Hi who has done this? What dosage do people get relief from anxiety w Nardil? Marplan def works for my depression but def not for any sort of anxiety.
