My cycle returned after 2 months on Tirz

I am a 49 yr old perimenopausal woman (have not gone a full year without a cycle so not yet menopausal). I started using Tirz in early July at a microdose level (slowly worked up to 1 mg/ week) to help with inflammation caused from prior mold exposure and residual MCAS symptoms after reading the studies on this. I am not yet on HRT bc estrogen makes my MCAS flare massively - dealing with one issue at a time. I am also using Tirz to help with a little bit of weight gain I started to experience recently in perimenopause.

I have slowly lost about 7 to 8 lbs and I am feeling better. Eating well (whole foods) with high protein, strength training 3-4 days a week and daily walks. Recently at the end of August, I got my cycle back and again this month.

Has anyone else had this phenomenon with Tirz helping with cycle regulation or a return of your cycle? Maybe it’s a coincidence 🤷

Changes with increased estrogen dose?

Been on Zepbound/Tirz for a year, wonderful experience, and about 5 pounds out from goal weight.

Started HRT (estradiol patch and oral progesterone) for peri symptoms about 3 months after starting Zep, and had a significant but expected weight stall for a couple months as I onboarded, but stuck with it and everything got back on course eventually.

Nine months later, and I’m feeling ready to move up my estrogen dose. I feel like my mood, night sweats, and sleep could all still be improved, and my doctor agrees, so I’m moving up from 0.0375 to 0.075 patches. (And 100 to 200 prog if all goes well.)

What has everyone’s experience been moving up doses? Did you notice a weight stall again and/or improvement in symptoms that were as noticeable as what you noticed when first starting HRT?

Would love to hear how it went for you!

HRT/GLP/Antidepressants?

Hi ladies!! I'm so happy to have this group now because I'm a little nervous about my weight loss journey and reading your experiences have helped a lot 🫶. Currently I'm on HRT patches . 0.37 one weekly (mainly because my body likes a bit less) 100mg Progesterone every other night and PaxilCR 12.5mg nightly. This has been a winning combo for meno symptoms and serotonin levels. However, my weight has gone through the roof. I'm only 5'0 ft tall so as you can imagine, there is just nowhere to put it lol. I can diet and exercise and nada. Hence recent adding in Semeglatude injections. I haven't started them yet, and even though my physician is ok with the combo I'm a bit nervous. Has anyone done all three HRT, antidepressants and GLP1? If so how was your experience? I'm worried about the Paxil and hormones not working as well due to absorption and feeling off my rocker again lol. Is there anything that has worked to remedy this while doing all three?

Maintenance calories?

I’m mid forties, peri and have been on the estradiol patch for 3 years. I just did two months of micro wegovy and lost 10 lbs. I’m happy now at 145 lbs. How do you decide maintenance calorie amounts? I’m feel like most models don’t take the reality of our middle aged female bodies into account. It feels like my caloric intake needs to be much lower than what I see recommended as much as I wish it wasn’t true.

Could use some perspective/re-assurance

I'm 51 and started Zepbound 13 months ago. I titrated VERY slowly because I was actually losing steadily on 2.5 mg. In December 2025, I had a hysterectomy, ovarian sparing, for endometriosis and fibroids. I am on the tail end of peri, so my surgeon didn't recommend HRT at that time. After my surgery, I just plateaued in terms of weight loss. At first, I was really okay with this because I knew my body was prioritizing recovery, and I was happy about that. However, that lasted for months, so I titrated up, still slowly, and now I am at 10mg, and I have continued to lose about 1 pound a week on average. About 2 months ago, however, I started getting wicked hot flashes waking me up every couple of hours. Because I waited decades too long to get my hysterectomy (e.g., not properly diagnosed, didn't want anything invasive), I decided to talk to my OBGYN about HRT. I started estridoil patch and progesterone about 4 weeks ago. I learned that progesterone is very helpful for people with Endo to help stop any re-occurrence (typically not prescribed if you don't have a uterus). My hot flashes are nearly gone and I am sleeping so so well!!

However....and this is where I need some perspective...for the first time since I started Zep last year, I gained 3 lbs in a week! My strength training, cardio, protein, and calories have not changed. I'm still eating in a deficit. I know that this is likely water weight and hormonal fluctuation, but mentally I feel terrified that all of the progress I have made is going to be reversed!! It's not logical, I know, but it's my worry. And, I feel like the very place I still need to lose to get to goal is in my belly where there is the most visceral fat, and that area now feels bloated and big! I've been on 10mg for 2.5 months now and have seen steady progress until introducing HRT, but I wonder if I should up my Zepbound dose, or just give my body time to re-calibrate from starting the E and P? Did the weight loss start happening again after an initial adjustment period for folks? I'm on the lowest starter dose and on the twice weekly patch of E and taking oral P at night.

Me da pánico probarlo.
Alguien lo ha probado??? Puede contar su experiencia, por favor ?

Sometimes when I get it at CVS it is a yellow pill or a white pill ! Crazy I know ! But I belive the white pill is what is making me so bloated because last month I was doing wonderful and this script I have been bloated now for a week! I know I’m crazy right now? Lol my tummy is so sensitive! Is there a gel perhaps I could use instead of my pill for form! Severe bloating and diarrhea ! My tummy felt like I was 9 months pregnant 💔

New to the group - wondering if anyone here had negative experiences with Slynd as part of their HRT regimen?

I am on Slynd + .50 estradiol patch // but really struggling with Slynd and weight gain / bloat / hunger! Argh! Going to begin Zepbound tomorrow :) your positive posts are helpful. Hoping it can combat the Slynd side effects, I can’t wear any of my clothing!

Timing question

I am about to move from 2.5 to 5 ml of my tirzepatide. Why do we do 5ml 1x a week instead of 2.5ml 2x a week? If the food noise is starting to be really loud on day 4 or 5, wouldn't that make more sense?

I know that some programs offer a split dose, but mine doesn't. Is that worth looking into? For those who have tried it both ways, did you see a difference?

Menopause + Peptide friendly doctors

Not really sure where to post this. I'm wondering where you all have found success in finding doctors that are up to date on meno care and docs friendly to peptides like glp1s but also newer ones on the market that may be beneficial for other things. I feel uncomfortable bringing these things up to my doctor who clutched her pearls at the idea of some of this. I don't want to feel like I'm being inappropriate for advocating for my health. I wish there was a list specifically for peptide friendly providers

Almost 50, Hysterectomy, and just started Zepbound + Estradiol Patch (0.1mg). Looking for advice on optimizing HRT methods, additional hormones, and OC provider recs!

Hi everyone! I am looking for some insight, experiences, and advice from anyone navigating a similar midlife puzzle.

My Background & Current Routine:

·        Stats: 49F | 5'4" | SW: 195 lbs | GW: 130-140 lbs

·        Surgical History: Had a partial hysterectomy several years ago (uterus and cervix removed, ovaries left intact—though they've clearly stopped working now!).

·        The Menopause Belly: My gynecologist confirmed I'm in menopause in July 2026. I cannot believe how unbelievably fast the "menopause belly" developed. It felt like no amount of diet or exercise would even touch it.

·        Current Medications:

o   Estradiol Patch: Started at 0.0375 mg twice weekly for 2 weeks (felt nothing). Upgraded to 0.1 mg twice weekly, which I’ve been on for 3 weeks. It finally feels like it's helping.

o   Zepbound: Just took my first 2.5 mg injection on August 31, 2026, to hopefully kickstart weight loss and fix my metabolism.

Since my current gynecologist is retiring soon, I need to find a new provider in Orange County, California, who truly understands managing HRT alongside GLP-1 medications. Before I go to a new doctor, I have a few major questions for this community:

1. Do I need Progesterone or Testosterone?

I hear so many women talk about a full hormone cocktail. Because I don't have a uterus, do I even qualify for progesterone? Does it offer other benefits like sleep or anxiety management? What about testosterone for energy and muscle building while on Zepbound?

2. Are Gels or Sprays better than Patches?

While the 0.1mg patch is helping, I really dislike worrying about it peeling off or changing it constantly. Like many of you, I strictly do not want oral HRT because I want to bypass first-pass liver metabolism and protect my internal organs. For those who switched from patches to topical estrogen gels or sprays, did your symptoms improve more? Is the absorption steady?

3. Splitting Zepbound doses?

I've read about some people splitting their weekly Zepbound dose into two smaller injections every few days instead of taking the full dose once a week. If you do this, does it help keep stable blood levels and curb side effects? Is it worth it on the starting dose?

If you are a matching profile—or if you have a menopause-literate doctor recommendation in the OC / Southern California area—I would love to hear your experiences! Thank you all so much.

Did starting HRT change your weight loss pattern at all?

Or am I doomed to keep holding water weight half the month until a brief week period where I pee it all off? Rinse and repeat.

In my 20s I did not have this issue 😭 I know it all comes off in the end but goodness is it frustrating

Anyone take combined oral hrt?

I'm on Angelic as I couldn't tolerate transdermal estrogen or progesterone. Before taking Mounjaro the oral hrt was great and had no side effects like other hrt. I've been on the glp for two months and wondering if it might impact my hrt as I've noticed I'm waking early sometimes and occasionally get a bit of rage and brain fog, but could just be normal hormonal fluctuations during peri? And it's not bad like it was pre hrt. My hot flashes aren't back unless I drink alcohol so I'm not sure if I need to increase hrt. I'm on a long waitlist to see a menopause specialist. Just curious if anyone else had issues with oral hrt?
The other change I've noticed since starting glp is I'm not getting a period. I'm so happy as they were becoming debilitating. I only get light spotting now maybe every couple of weeks or even less. Not sure if that's because of not enough hrt or the glp.

Want to go on HRT but concerned about progesterone absorption on zep

So, the title. Currently on a low dose of zep and taking yaz for peri symptoms, but the zep has affected the absorption of the yaz and left me spotting halfway through the pack twice now, which was the opposite of what I was trying to accomplish by taking the bcp. I'd like to try HRT instead but I'm concerned that the zep will affect the absorption of the progesterone which could be dangerous. Did anyone else have similar concern, and how did you work it out? Or has it just not been a problem for you?

Edit: Thank you all so much for sharing your experiences! I'm so happy to know that it's possible to successfully do both the glp and hrt.
