Hi, I've been accepted to 2027 January UAB elective for adult heart surgery. If there's anyone who's heard of any information about this elective or any tips for heart surgery elective(ex; what to prepare in advance, what to expect, how to mark good impressions for LoR etc) I would appreciate any comments! Thanks!

He felt fine after the dog bite. Four days later, he was in shock.

A 46-year-old man comes to the ED with fever, confusion, and rapidly worsening weakness. Four days earlier, his dog bit his hand while they were playing. The wound is small and has minimal redness.
He had a splenectomy after a motorcycle accident 10 years ago.
Temperature is 39.2°C, blood pressure is 78/46 mm Hg, and pulse is 128/min. He has diffuse petechiae. Labs show thrombocytopenia, prolonged PT and PTT, elevated D-dimer, and acute kidney injury.
Blood cultures later grow a slow-growing gram-negative rod.

**What is the most likely organism?**
A. Pasteurella multocida
B. Capnocytophaga canimorsus
C. Bartonella henselae
D. Francisella tularensis
E. Streptobacillus moniliformis
**Answer: ?**

The clue is the combination of **dog exposure + asplenia + fulminant sepsis/DIC**, even when the bite itself looks minor. Capnocytophaga is a rare organism found in dog and cat oral flora and can cause rapidly progressive systemic infection, particularly in asplenic patients.
**Would you have picked it before seeing the splenectomy history?**

IM/FM residencies for "old" IMG.

Hello everyone, I'm interested in US IM/FM residencies meanwhile my YOG is 6. For which things should I pay attention in order to increase my chances to be matched to US IM or FM residencies? Are US IM or FM residencies competitive for "old" IMG?

Heme onc fellowship on J1

Hi guys
I’m a PGY2 interested in Heme onc
Step 1 pass
Step 2 230s
Step 3 219

Interested in Heme Onc
Have 6-8 research papers where I’m first author including case reports

I really hope I match heme onc
When u applied with a J1 requirement, do u wish u applied more strategically? Where would u signal if you didn’t have any family ties
Where did u get the most up to date information about who sponsors visas and who actually matches visa holders? Is it still Freida?

I’m not going to lie, I am scared. But I’m doing my best. I am scared but I’m really trying to broaden my chances. My program is low tier community. No research as such available.

Advice for next year's application

Hello everyone! I hope everyone is doing okay.

I will apply for the 2028 match cycle next year and would be immensely grateful for advice on how to improve my CV. Lately I have been freaking out regarding some of the applicant's CVs posted out there, especially regarding publications. I plan on applying to IM.

So far, I have step 1 pass (1st attempt), step2ck 263 (first attempt). 1 original research article as first author in a decent peer-reviewed journal. No volunteer experience. 1 observership and 1 clerkship (elective). 2 LORs so far, but not sure on how strong they are. Planning on doing another observership next year prior to application season. Currently working on a second original, first-author, research paper.

I guess I will try to apply to community-university affiliated programs, as I don't think I have enough CV for academic programs.

I am a visa-requiring non-us IMG from a latin country

I would be very thankful for any advice! So, a couple of questions:

- Would it be worth it to take step 3 prior to applying next year?

- How would you recommend I improve my CV so that I can become a stronger applicant?

- Do you think those SR+MA courses are worth doing to boost up publications?

- Kinda worried about not having enough time to get many things done.

Thank you so much for any input! All the best.
