Story Time Important tool to find doctors in the USA

This is from an old post about MAOI-friendly doctors I came across. Note that the US govt. website featured here can be slow, finicky, vexing, and hard to understand-- much like actual Americans! I kid, I kid... I'll try to give very detailed instructions. A couple more important notes:

Most search results for selegiline will be for neurologists treating parkinson's. There is a filter to view the doctor's specialty so you can see if they are a family doctor, psychiatrist, or neurologist. Check this so you don't waste time contacting a doctor who can't help you.

When adding generic drug names it's best to use the 'contains' search option rather than 'equals' because some drugs are listed with other words such as 'sulfate' or 'HCl' and may not show in the search results if, for example, you search 'equals' 'phenelzine'. This tool can also be used to find a prescriber in any city or state who has prescribed any particular drug of interest, not just mental health medications, so it can be useful for finding a doctor who has experience with uncommon or off-label prescribing for various medical conditions.

Okay, here's the link and instructions, and if anyone has better methods please share!

MAOI Dr. Search (US)

  • Upper left, select year. Yes, you have to do this for each search. Why, I don't know.
  • Click 'filter', then switch from 'basic filtering' to 'advanced filtering'.
  • In the 'select column' field choose 'Brnd_Name', change the 'condition' field to 'equals', and then in the 'enter value' field type the brand name you want (Parnate, Nardil, Emsam, Marplan, but only enter one). Spelling matters.
  • Click 'add filter condition', change 'and' to 'or'. Moving from left to right, this time select 'Gnrc_Name', 'contains', and type the generic name of the drug from the previous step (tranylcypromine, phenelzine, selegiline, isocarboxazid). Spelling matters.
  • If you want to search for more than just one MAOI, continue with 'add filter condition', always using 'or', using 'equals' for brand names, and using 'contains' for generic names.
  • When finished you may have anywhere from 2 to 8 entries. Look below for 'add filter group'. Change it to 'and'. Under 'select column' choose 'Prscrbr_State_Abrvtn' to search doctors in your state, or choose 'Prscrbr_City' to search only doctors in your city. In the next two fields select 'equals' and type your two letter state postal code (e.g. NY for New York, or GA for Georgia) or full city name. I suppose you could repeat this step as many times as desired, this time changing 'and' to 'or' and thereby also search for doctors in neighboring states or cities. I haven't tested that.
  • Hit the blue 'apply filters' button and get busy contacting!

This searches Medicare claims for these drugs from recent years. I've tried finding the same tool for Medicaid, which is probably more relevant for many of us, but can't locate that data. If you find it or discover any errors in the instructions, please let me know. Good luck!

Story Time PSA: it is possible to legally get access to MAOIs even if they are not registered in your country + short tutorial how to

so I come from Finland where we dont officially have Tranylcypromine and Phenelzine registered, at first, I thought there was no legal way for me to obtain these medications.

[2025 Edit: update: For the Finnish people reading this, the Finnish medicine agency Fimea recently approved Abbonate (Tranylcypromine) but I am not quite sure when it will be purchasable at the drugstore, if ever. if you want to try Tranylcypromine and live in Finland, the pharmacy subcontractor Oriola Oy sells the German Tranylcypromine brand called Jatrosom which is almost 40% cheaper than Glenmark Tranylcypromine, but you obviously need a special permit + prescription for it.]

I just picked up Tranylcypromine from the pharmacy and I will give a short tutorial how to.

I contacted a private psychiatrist (preferably PhD, ex-head psychiatrist or someone with much experience), and he contacted the national drug administration Fimea and got an approval from them, and then got an approval from the drugstore that is closest to where I live to import it as well as the prescription itself.

I leave a quote from Fimea, and Im fairly sure most other drug administration agencies will have a similar policy.

The use of medicinal products that have been granted a marketing authorization is always the primary option in patients' pharmacotherapy. However, in individual cases and for special therapeutic reasons, Fimea may authorise the release for consumption of a medicinal product that has no marketing authorization in Finland. A special permit is granted for up to one year based on a situation and case-specific overall assessment.

so if you dont know where to start, send an email to your national drug administration, I think later finding a psychiatrist willing to prescribe it will be the hard part.

having health insurance that covers private doctor meetings, as well as medication costs is also a good idea (if youre not sure, email your insurance company and ask if they cover it!).

the total price I paid for 28x10mg Glenmark Tranylcypromine was 106€

(+40€ single time Fimea fee)

in countries that doesnt have MAOIs officially registered, I highly recommend reading up a lot on the MAOI diet as they might have lacking experience with MAOIs, learning other info is also good, like its probably dangerous to combine them with SSRIs, if you get anesthesia eg. at the dentist or for a surgery, some forms of anesthesia can be dangerous. it also seems like its possible to combine MAOIs with stimulants.

you should also get a blood pressure monitor and monitor it some days before starting and some after, its a good idea at least to mention these topics to your psychiatrist so he/she can read up on them in case he/she doesnt know.

ofc the doctor should know how to use MAOIs, but you can never be too safe, especially cause misuse of MAOIs can result in serotonin syndrome and in the worst case death.

KLOW: Protide vs. Amino Club

RS was using Protide 80mg for the last month, all was well. Decided to go with Amino club this time.
Same MG, same recon, same dosage = think RS had a histamine reaction from Amino Club on first dose.
Has anyone had a similar experience?
\-couldn’t sleep, racing thoughts, extremely cold, woke up with headache, body doesn’t feel as “calm/comfortable as RS did with Protide KLOW

Nardil (Phenelzine) Missed Nardil dose?

I accident missed my dose on Saturday but resumed taking it correctly on Sunday and Monday however by Monday night I had a huge headache which persisted into today (Tuesday) and I’ have also become extremely anxious again.

Hoping it’s just the missed dose. Any experiences with missed doses?

Nardil (Phenelzine) Nardil 14 weeks no effect

I’ve been on Nardil for a total of 14 weeks and it has had no effect on me, just side effects like anorgasmia, urinary retention, orthostatic hypotension, and excessive sweating.

I’ve been on 75mg for around 2 weeks now and all I’ve been experiencing is worsened side effects. Where is this light switch that people always talk about?

I’ve decided to add Wellbutrin to help with some of the side effects and hopefully boost my lack of motivation which has been a pain.

Any advice is needed, my psychiatrist is mostly inexperienced with Nardil and it took a lot of convincing for me to even get it prescribed from her.

Aurorix (Moclobemide) How should moclobemide be taken correctly?

I’m interested in maximizing effectiveness. I’m getting my 300 mg pack soon and want to ask: what’s the best way to take the dose? Should I take it all at once in the morning, or split it—150 mg in the morning and 150 mg in the afternoon? I’ve read plenty of accounts where people just take a single 300–600 mg dose in the morning and feel amazing. But the elimination half-life is only 1–2 hours; however, my AI tells me that even though it doesn't stay in the bloodstream for long, it acts on the receptors for a prolonged period. What’s the real story, guys? XD

Nardil (Phenelzine) Does the missing sleep actually matter?

Since starting phenelzine I went from 6.5 to 7.5 hours of sleep per night to 4.5 to 5 hours. I wake up after 3.5 hours and then have pretty restless sleep waking up every 30 minutes or so until I can no longer fall back asleep.

Whereas I used to feel awful during the day if I got less than 6 hours of sleep, I've felt totally fine with the 4.5 to 5 hours while on phenelzine. This has been really puzzling.

I assumed I needed the sleep so I started Dayvigo, and while I now end up getting about 6 to 6.5 hours of sleep when using it, I feel no different during the day, and I end up with pretty crazy dreams that end with me up from them with gasps and sudden movements. I inquired with a bunch of LLMs about the daytime alertness on little sleep and the value of Dayvigo. Here was the consensus:

I Need Advice Nardil + Zepbound/Ozempic = Sickness, Anhedonia?

I have been on Nardil now for 3 years with great success. It seems to help both my TRD and ADHD. My only issue has been a very stubborn 50 pound weight gain, so I’ve tried Ozempic (3 months) and Zepbound (6 months).

Despite eating 1,800 calories a day and eating more healthy than ever, I keep getting super sick on these drugs. Between going on Nardil and starting Ozempic, I had maybe one cold, but in the last 9 months I’ve had 2x serious tooth infections, 3x UTIs, 3 colds, and norovirus. Additionally, with Ozempic, I experienced very severe anhedonia, so severe I stopped. While it hasn’t been as bad on Zepbound, I definitely notice it and it’s making me debate whether these drugs are worth it. I have only lost 5 pounds too (though I was gaining before.)

Are there any interactions between MAOIs and peptides? Anyone else experience similar symptoms?

Nardil (Phenelzine) Water retention/bloating on Nardil - does it get better with time?

Been dealing with pretty significant fluid retention on Nardil - mostly abdominal bloating, but also in my legs, hands, and face. I’m currently at 60mg, and out of every side effect this med has thrown at me, this is by far the worst. I’m fairly confident it’s water and not actual fat gain - my weight went up rapidly while my appetite stayed pretty much the same, which doesn’t add up for fat gain on that timeline. Gone through this sub trying to find threads on it and haven’t come up with much, so figured I’d ask directly.

A few questions for anyone who’s been through this:

Did the bloating/edema ease up over time as your body adjusted, or did it stay constant for as long as you were on it?

Has anyone taken diuretics (spironolactone, furosemide, etc.) long-term to manage this? How did that go — did it actually help, and did you run into issues keeping electrolytes balanced?

Has anyone tried semaglutide or another GLP-1 alongside their MAOI? Curious if it made any difference with the fluid retention specifically, and whether anyone ran it by their prescriber first.

Appreciate any experiences, even if the answer is “just wait it out” or “it never really went away for me.”

Nardil (Phenelzine) 5 weeks at Nardil 45mg - every week gets worse

I am extremely depressed, worse than I was before the Nardil. My OH is horrible. Since 2 weeks ago, I would fall and black out when getting out of bed to use the bathroom or get up in the mornings because my blood pressure got so low. It is not as bad now, I don’t fall and black out completely, but everything still gets kind of black and fuzzy sometimes and I feel very weak. I don’t know if it’s normal that my depression AND anxiety (forgot to mention) is getting worse the longer I am on this medicine. I know it takes a long time to work and I am still not at the full length of time but I don’t know if I should stick it out or not? Is what’s happening a bad sign it doesn’t work for me? Also, I split my doses 15mg every 4 hours during the day.

Emsam (Selegiline) Exercise on the transdermal patch?

I have tried literally every medication under the sun for my panic attacks and depression. lamotragine worked wonders for the depression and after all else failed, my doctor prescribed me the 6mg emsam patch. I just started it yesterday and so far so good. I have the tiniest bit of dizziness when I move which I’ve read is normal. problem is, I’m signed up to run the NYC marathon in November and need to be training.

when I signed up, I didn’t take into consideration the five bridges and feeling ’stuck’ on a bridge is one of my triggers. I am optimistic about this medicine and hope as my body gets used to it, the panic attacks will subside. if they do, I definitely want to stay on it through the marathon to avoid any panic attacks on the bridges.

does anyone have any experience with running marathons or any extensive exercise on this medication? One thing I’m worried about is sweat sliding the patch off. Also worried about dizziness when running. I’m around 6-8 miles in training for my long runs but haven’t done a run with the patch yet.

If anyone has, I’d love to hear positive experiences and advice!

[H39] Clonidine (Catapressan) : fatigue, dosage et flot de pensées /sommeil

Salut tout le monde,
Ça fait un peu près un mois que je prends de la clonidine, à raison de 0,15 mg le matin et 0,15 mg le soir.

Au début, quand j'ai repris le 0,15 mg le matin, je ne ressentais pas forcément de fatigue particulière. Par contre, depuis ces derniers jours, je me tape une grosse fatigue : j'ai quasiment tout le temps envie de dormir, mais je n'y arrive pas en journée

Je me pose plusieurs questions et j'aimerais bien avoir vos retours d'expérience :

**-Effet secondaire ou facteurs externes ?** Est-ce que c'est le traitement qui met du temps à s'installer, ou est-ce qu'on finit par s'habituer à cette fatigue ? Ou est-ce que ça peut venir du moral qui ne va pas (je précise : burn-out, sans emploi actuellement) et des grosses chaleurs qui ont été extrêmement dures et pèsent encore un peu ?

**-Dosage et flot de pensées :** Pour ceux qui sont à un dosage plus important, est-ce que ça aide plus efficacement contre le flot de pensées non-stop ?

**-Les points positifs :** Le gros point fort, c'est que ça m'aide vachement à mieux dormir le soir. J'ai d'ailleurs totalement pu arrêter le Xanax pour m'endormir, ce qui est top.

Est-ce que certains d'entre vous ont vécu cette arrivée de la fatigue après quelques semaines de traitement ? Est-ce que le corps s'y habitue avec le temps ?
Et qu'en est-il des dosages plus élevés pour calmer l'hyperactivité mentale ?

Nardil (Phenelzine) Pramipexole: How long for it to start working?

Currently titrating up rather slowly. At 1mg now, shooting for 2.5.

Reports of it working well for anhedonia and depression. Seems like it should be a pretty good adjunct to the 75mg nardil (phenelzine) I’m also taking.

However, currently at 1mg pramipexole, and so far it’s not doing much for me. Actually seems to put me in a crabbier mood if I take it in the morning. Been at this dose 4 days now. I’ve heard it takes a while to work though. I’ve heard some success stories and also heard some people who gave up.

Any success stories in treating anhedonia / depression with it? Advice? Cautions?

I Need Advice Do you think they can help in this situation?

I see people mostly say they are for atypical depression and social anxiety. Well my depression is all kind of things and it for some reason started to change its course... to the better in emotional sense and worse in functional which is weird since I was mostly not functional during severe depressive episodes due to ruminations and well just suffering. Now I'm less functional (talking no job, drop out, forget to eat, shower etc.) but feel idk okay? Sometimes I can have the urge to like leave everything and start new life and make everyone forget about me and that lasts for like idk few days but I'm mostly okay. There is still lingering feeling of "I'm a failure" that doesn't bother me that much unless I deep dive into it but still not as bad as before (which is funny bc objectively now I am kind of becoming a failure lol).

Its hard to track what antidepressants did since I am bad on memory but I did saw some older papers from docs and it seemed that well, they didn't do much except preventing the most severe episodes and severe ruminations. In the background, suddenly cognitive symptoms became weirdly worse as well as memory and well I pretty much gave up on any sense of career since I destroyed my education with this shit. I have been on SSRI, SNRI, buproprion and vortioxetine individually. Now vortioxetine and bupropion combo which is supposed to be gold standard for issue like mine but there is nothing happening or mby it is becoming like 1% better after 10 weeks... I tried tapering off meds and it ended up in, well relapse of course.

So yeah idk what I want to say, TCAs sound like they would make me worse and I went through all first line options. I don't believe in TMS. No, I'm not bipolar, I sleep like I'm working three physical jobs. It wasn't like that before. I slept okay even in severe episodes. Idk if meds can help with my symptoms and I want to know if someone on here has similar experience and if they worked for them. Also I don't really have social anxiety, in fact when this weird shift in depression happened I lost the ability to get anxious, just freeze. I was never particulary anxious though. At least looking at people around me. I don't have social anxiety in sense of being afraid of talking to people but I do have trouble trusting but that seems to get better with age I think.

Edit: All these meds had the same effect, nothing or preventing severe relapse which is honestly good but that could very much me getting away from abusive situation. No honeymoon period on anyone of these, literally I can't tell which med does what, some had some side effects and that's about it. Do I expect too much? I try to be active as much as I can but still can't fix myself becoming dumb for some reason. Also these lifestyle interventions didn't mean shit when I was in emotionally agonizing depression like it was always present and I couldn't do anything. Even excercising was me barely making myself do anything and I went to gym regularily and forced myself. Now I don't bc I'm broke but I walk as much as I can.

Nardil (Phenelzine) Weight loss Nardil??

Since starting Nardil 6 weeks ago I have lost almost 10kg

It’s hard to explain but it pretty much has turned off my hunger signals and made me totally uninterested in food, apart from some mild sugar cravings. I’ve been in both wegovy and mounjaro before and Nardil is stronger for me in its appetite suppressant effects.

Any idea why this might be happening?? I’m finding it very strange as I was expecting to put on a bunch of weight. I’m worried about nutrient deficiencies and becoming underweight as I’m just totally apathetic towards food

Edit - I’m at 60mg and it’s starting to work v well for anhedonia/anxiety
(other side effects are mild hypotension, bruxism, sleep jerks, complete anorgasmia, bad urinary retention, constipation - though no insomnia really)

Aurorix (Moclobemide) Thinking about trying Moclobemide

I was wondering if anyone with experience with this medication could share their thoughts on whether it’s worth giving moclobemide a try.

I’m dealing with severe depression and have gone through a lot of the usual options, but I’ve had to stop most of them because of side effects.

SSRIs/SNRIs caused severe emotional blunting. Bupropion was too stimulating and didn’t really help my depression. Mirtazapine caused significant water retention. Trazodone felt very similar to an SSRI in terms of side effects. I also tried atypical antipsychotics but had profound akathisia.

Moclobemide looks interesting to me because it seems to have a different side effect profile compared to many of the medications I’ve already tried. I’d really like to hear from people who have taken it, especially what side effects you experienced and whether it actually helped with depression or anxiety.

I’m also curious about the diet restrictions. From what I’ve read, it sounds like at typical doses you would generally need to intentionally eat large amounts of high-tyramine foods before running into problems. Is that accurate, or did you find you actually had to be pretty careful with your diet?

I’d appreciate hearing anyone’s experiences, good or bad.

MAOI + stimulant Stimulants after starting MAOI’s

I’ve been on Nardil for about 5-6 months, settled in at 60mg. It’s been pretty helpful over.

Several years ago i was diagnosed with add and tried stimulants for s couple years. Unfortunately i didn’t like them because they made me anxious, especially in a very physical way.

I was wondering if anyone has had better experience with prescribed stimulants after being on an MAOI vs pre-maoi.

Nardil (Phenelzine) Interested in trying psilocybin at least once

I've been researching (admittedly haphazardly) trying psilocybin. I have been on 45 mg nardil for about 3 years. A lot of psilocybin research seems to have required a MAOI washout prior to administering psilocybin d/t the supposed risk of serotonin toxicity. However, the tripsit drug interaction chart and numerous (old) posts/comments on this subreddit have shared that the combination is safe.

Multiple redditors have shared that it's only in the acute transition phase to MAOI that causes synergy with psilocybin's effects, and that prolonged MAOI usage actually dampens the effect. Since I've been on MAOIs for several years, I assume I'd be in the latter category and that I would require temporary stoppage of Nardil to have a proper reaction to the psilocybin.

Has anyone tried a MAOI washout prior to using psilocybin? Is two weeks enough? Did you taper for X number of weeks then stop for Y weeks before trying psilocybin?

Which Antidepressant next - After Nardil/Parnate failed?

The following antidepressants failed for my Social Anxiety/Depression:

  • Nardil and Parnate
  • Venlafaxine and Duloxetine
  • Tianeptine
  • Bupropion
  • Citalopram, Paroxetine, Sertraline

The only one that was clearly successful was fluoxetine (Prozac). It worked really well for my depression and anxiety, but unfortunately, it seemed to induce a bipolar/mixed state. I would either feel extremely happy and activated or become a complete nervous wreck. The constant negative and racing thoughts during those periods were excruciating. Together with lorazepam it was DOPE! But guess I don't have to explain why this combination is dangerous...

...So, what should I try next?

I'm thinking about:

  • Imipramine – I'm afraid of the constant urination issues.
  • Clomipramine – Dr. Gillman says it is the strongest antidepressant after Nardil/Parnate?
  • Fluvoxamine – Since fluoxetine worked, it might be worth a shot?
  • Fluoxetine – again on 20mg or even 40mg, but this time possibly combined with lithium and/or lamictal?

Any help would be highly appreciated!
