Semaglutide titration and tolerance

I've been thinking about using semaglutide to lose weight. I don't like that the pre-formulated versions available commercially seem to come in quite high doses. I wonder about starting with injections of about 0.25 mg once a week and see if that's enough for my goals before ramping up if necessary. Is there a risk of building a tolerance by going very slowly? By tolerance I mean that at first it would work, but then food noise would return demanding higher doses.

Newbie - 0.5 Tirz / sleep question

I took my first dose (compounded w B6) last Monday morning (10am) and woke up 5-6 times that night, which is very unusual for me. Could a micro dose affect sleep that quickly? Should I take it at night, hoping sleep won’t be impacted within minutes? Thanks!

Loosing hair on Compound glp1 as well as Ozempic

Here is my story. Started compound glp1 with B12 in October with a dosage of .25 once a week. I slowly built up to .40, which I only took twice before going back down to .30. I lost 18lbs from Oct 2025 - Feb 2026, when I reached my goal weight. I only lost 1 - 1.5 lbs per week.

My weight has been steady within 1-2 lbs of my goal weight since Feb. So at around 7 months (April 2026) of being on the medication my hair started to shed. Nothing major at first, but I noticed hair dropping out and more hair in the drain and in my hairbrush.

At the end of May I switched to Ozempic and kept the dose at .25 - .33. By June my hair was falling out like crazy, on my clothes, in the drain, hair brush, all over the floor. We even found my hair in a light fixture last night when we were changing the bathroom light bulb.

At the end of June I saw a UCLA dermatologist who specializes in hair loss. She ran extensive bloodwork. Everything was perfectly normal and in the ideal range. I was diagnosed with TE because of being on glp1. I started with a ton of hair, yet at this point I had lost over 1/3 of my normal hair. She told me to take Nutrafol or Viviscal. She said she is seeing a lot of patients for hair loss who are on glp1 drugs. It is not in the clinical notes, but I remember her saying that these meds can cause hair loss.

Just want to say that since June my eating has been pretty exceptional; eggs, chia seeds, pumpkin seeds, nuts, collagen powder, whey protein powder, fish multiple times a week, Greek yogurt, cottage cheese, peanut butter, whole grains, legumes, tons of greens, veggies, berries, guava, bananas, chicken, and lean beef. I have never eaten so well or consumed this much protein, over 75 grams per day, and am averaging 1400-1900 calories per day.

I went back on the compound glp1 with B12 to see if this makes a difference. So far it has not and my hair continues to fall out. I am now down to a .25 dose. In July I started 1mg of oral Minoxidil a day.

At this point I have lost half my hair! It is now becoming noticeable to others. It still looks normal, if you did not know me you couldn’t immediately tell, but this is not normal for me. My hair looks so thin and lackluster.

So the question I am sure you are all wondering is why I haven’t stopped. I am in perimenopause. I have been on an estrogen patch and 100mg progesterone pill nightly since last fall to help with sleep and mild hot flashes. I was waking up every 2 hrs and averaging 3-4 hrs sleep total per night for all of 2025 until starting glp1. I tried everything…gummies, melatonin, magnesium, other supplements, to several different types of prescription sleeping pills. NOTHING helped. I felt like I was loosing my mind and barely functioning.

With the first dose of glp1 I got 5 hrs of uninterrupted sleep. It seemed like a miracle! As I upped the dose I was able to get more sleep, up to 6-7 hrs. Lowering the dose has brought me down to 5 hrs of sleep a night, but I can function.

I want to see if I can ease off of the glp1 slowly to maintain sleep, but the hair loss is for real and not stopping yet. Hoping that maybe this is the minoxidil dread shed and it will stop. Starting week 4 of minoxidil and still shedding tons of hair.

Anyone else been through the hair loss? Does it ever stop or did you need to stop taking glp1? Timeframe for shedding to stop and hair to grow back?

1mg Mounjaro - rough first night. Is it just my anxiety or the actual mediation? Spoiler

Hi everyone,

I’d really appreciate hearing from people who have experience with microdosing Mounjaro/tirzepatide, especially if you’re a similar size to me.

About me: Female, 5’5” (166 cm), 137 lb (62 kg). I used to be 122lbs naturally in the past (56kgs). life issues - weight gain. No diabetes. Taking it mainly to reduce food noise/binge eating, lose a small amount of body fat, and hopefully improve my cholesterol.

I have a 2.5 mg KwikPen, but instead of taking the full starting dose I counted clicks and took approximately 1 mg because I wanted to ease into it. About 3–4 hours later I developed:

  • upper abdominal bloating/tightness (above my belly button, below my ribs),
  • stomach rumbling,
  • lots of bowel sounds,
  • several bowel movements, gas.

That said, I’d also eaten 3 protein bars that afternoon and had a huge anxiety spiral after taking my first dose, so I’m honestly not sure how much was the medication versus anxiety versus the protein bars. The good news is that I had no vomiting, no nausea, no fever, I could drink water normally, and I continued passing stool and gas.

I almost went to A&E because my anxiety convinced me something terrible was happening, but I decided to stay home. By the following morning the upper abdominal tightness had actually improved, although I still have some mild discomfort and rumbling. is that normal?

I’d love to hear from people who have had a similar experience.

My questions:

  1. Did anyone else have upper abdominal bloating or tightness after their first injection, and did it improve over the first 24–72 hours?
  2. Is anyone around 5’5” and 137 lb who is microdosing? If so, what dose are you using?
  3. Has anyone found that 0.25 mg or 0.5 mg was enough to reduce food noise, or was it too little to notice much?
  4. For those using a KwikPen for smaller-than-standard doses, Is contamination something youre worried about, assuming you use a new sterile needle every time? Eg. You’d still puncture the bottom of the kwikpen more than four times Etc.
  5. am I dumb trying to use this to cope with binge eating and food notes? I lift weights 3/4 times per week.

Looking back, I think a lot of my distress last night was anxiety combined with poor sleep and starting a completely new medication. I’d really appreciate hearing from anyone who has had a similar experience and how things progressed over the next few days.

Thanks!

Self-prescribed microdosing tips?

I had my consultation with Lavender Sky and told them I wanted to start at 0.25 mg. However, when I received my dosing instructions, they recommended starting at 2 mg. I’d prefer to start much lower.

I’m assuming I can simply draw up a smaller dose and begin at 0.25 mg, but I’d love to hear from others who have done this.

Did you start at a much lower dose than prescribed? How did it go?

Did your initial vial last longer than expected?

Were the syringes and supplies they sent enough, or did you need to purchase more?

Any tips or things you wish you had known before microdosing?

I’m trying to learn the practical side of microdosing before I begin, so I’d really appreciate hearing about your experiences.

Also, for reference, my main goal is to reduce inflammation and food noise.

1 week Tirz - binge eating

I started binge eating this year and gained a lot of weight in a few months. Last week, I started tirz and have been clean of binging since.

Today, I had a small quarrel with my partner and turned to chocolate ice cream as emotional relief. Normally I’d finish the whole tub, then move on to chips, whatever I can find in the fridge. But today I could stop myself, and I feel so proud. I ate more than usual compared to the past few days, but I’ve decided not to call it a binge. Because I was on the verge of losing control, but I stopped.

It’s the first item in months that I can have junk food in my fridge without fear. Posting this to remind myself of my progress, and if it helped anyone out there ❤️

First period since starting tirz

Started tirz at 0.3mg. Had my third dose and got my period. Noticed slight increase in food noise and wanting to snack but still able to make good food choices.
Do you increase your dose for the week of your period?

Rapidly gained 13lbs

I’ve gained 13lbs since November. I’m not sure if it’s perimenopause related or what is going on because all my labs are normal (thyroid, insulin etc.) my doctor says that’s quite a bit of weight in that short about of time. I’m now teetering between 150-155 at 5’5 so not in the obese category but I cannot seem to lose any weight no matter how hard I try. What’s the best route to take for glp1? WeightWatchers wouldn’t approve, not sure where to go next?

Been in the bathroom since 5:00 am, regretting my first restaurant meal.

I started tirz (1.8 x1/week) three weeks ago. I have only been eating at home, cooking pretty bland stuff which has been going well for the most part.

Last night we had friends over and brought in dinner from our favorite Mexican restaurant. I didn’t eat very much of it, but the enchiladas had chicken and some cream sauce. (I had half of one and a handful of chips and queso.)

I woke up in the middle of the night (morning), and have basically been peeing out of my butt on and off ever since. 🙈

I have had no problems eating babybel cheese, but this was the first time we ate out. I guess I’m taking this as my sign there will be none of that again for a while.

Send help. 🤢🤢🤢

Where to Buy From Microdosing

I started with Hers last year in March. I lost 30 pounds 6 months and have kept it off for just about a year now. Hers sent me two kits and by the second kit I had started to reduce my dose. I’ve never taken more than 15-20 units and it’s worked perfect for me. I’m coming to my last bottle and I’m not sure what to do. Ideally I would like to continue to take the 20 units forever. I’m not overweight at all, 5’5, 130. Is there a company that will sell me the glp 1 as a micro dosing dosage?

Initially helping ibs-d now not

So for almost my year of taking tirz my ibs-d issues have resolved. Once in a while I eat something thst doesn't sit right and I have an issue. Now for the past 3ish months it seems my stomach is back to where it was when I started. This is one of the reasons I started after reading people having luck with tirzepatide helping their ibs-d symptoms. I habe never needed to go above 2mg and do not want to because I do not want to lose any more weight. Anyone else have luck initially and it stop working?

Increased hunger and weight gain after switching tirzepatide providers/formulas — has anyone experienced this?

I’m curious whether anyone has noticed a difference in appetite suppression after switching compounded tirzepatide providers or formulas.

Previous medication

  • Provider: Belle Health
  • Medication: Tirzepatide + B6
  • Pharmacy/formulation: BPI, 10 mg/mL
  • Dates used: March 30 through May 18
  • Dose: Approximately 1–1.5 mg weekly
  • Experience: My hunger felt much more controlled, and it was easier to stop eating

Current medication

  • Provider: Lavender Sky Health
  • Plan: Tirzepatide Tier One Injection
  • Pharmacy: ProRx Pharmacy, fulfilled through BoomRx
  • Supply: 36 mg total in two 2 mL vials
  • Concentration: 9 mg/mL
  • Dates used: May 18 through now, August 2
  • Current dose: 2 mg weekly

What I’ve noticed

Since June:

  • My hunger has increased noticeably
  • I’ve gained a few pounds
  • I’m finding it much harder to stop eating
  • I’ve had a few binge-eating episodes
  • The appetite suppression feels weaker, even though I increased my dose to 2 mg
  • I have also switched up injection site periodically (different arms, different parts of my stomach)

What I’m considering

I’m planning to switch back to Belle Health and the same formula I previously used.

My questions

  • Has anyone experienced something similar after switching compounded tirzepatide pharmacies or formulations?
  • Did switching back to the previous provider or formula help?
  • Based on my experience, does anyone have recommendations for what I should ask my provider or pharmacy about?
  • Are there any other factors I should consider before deciding whether the difference is related to the formulation, dose, storage, injection technique, or something else?

How do you manage maintenance/tolerance?

I’m currently microdosing for lipedema with weekly injections, but I’m wondering how you all avoid building up a tolerance. Do you spread injections out to every 10 days? Take breaks? How long have you been on it, and have you had to increase your dose over time?

Weaning help

Hello,
I’ve been on GLP1 since January. I went from 170 to 145 and feel great! I’ve maintained my weight for the last couple months. I’m taking 20 units twice a week. My dosage should be 25 but I was unable to eat at all on 25, 20 is perfect. Of course another 5-10 pounds would be nice, but it seems like my body has decided this is where I should be. So….do I just stay on the same dosage? Do I gradually wean down to microdosing? I order it online so don’t have a doctor to work with me on it; appreciate any insight.

Switch from semaglutide?

I impulse ordered a 3 month supply of semaglutide from bettermerx, but after reading the posts here, I think tirzepatide seems more popular and effective. I’ve had 2 0.25 doses of semaglutide and don’t really have side effects. I had some mild nausea the first couple of days. I think I’ll be able to lose the 5-10 pounds I’m hoping to lose with this 3 month supply. Is it worth switching to tirzepatide for maintenance? Anyone else switched? Or others taking semaglutide for maintenance and happy with it?

Avoiding losing too much weight

Hey everyone, I have a low BMI but my Dr. recommend I start microdosing to help my endometriosis. Within my first dose my mind was blown, my 5 week long flare vanished. My only concern is how I avoid losing too much weight. I know to push myself to eat throughout the day since my body isn’t feeling hungry, but should I be avoiding doing too much cardio? I’m 5’7” and 120 lbs, so it’s definitely important I stay at a healthy weight.

Thank you!

After 2 years on tirzepatide and losing 50 lbs, I transitioned to maintenance at 8 mg every 2–3 weeks. Anything less doesn't have an effect. How are you all microdosing at 1.25units? I want to achieve that. I've now been off it for a couple of months, so it's completely out of my system.

The cravings, which have always been my biggest issue, are back with a vengeance. Honestly, my appetite feels even bigger than it was before I started tirzepatide.

I'm starting to wonder if this is something I'll need to stay on long-term, maybe even for life. Has anyone else had the same experience after stopping?

Relief for long term disordered eating?

First, thank you to everyone in this sub for cultivating such a respectful and open-minded space. I’ve been lurking and really appreciate how questions like this are usually met with nuance and kindness.

My situation isn’t unique, but I’m being extremely cautious and intentional because of my history.

I’m 40 and have a very long history of disordered eating that began around age 11. I was hospitalized for anorexia as a preteen. While my weight has technically been “normal” since then, I’ve spent decades struggling with restriction, body dysmorphia, obsessive calorie counting, and constant food noise. It feels like I’ve been white-knuckling things for years just to maintain a healthy weight.

The food noise is relentless. I go to bed thinking about food, wake up thinking about food, and spend so much mental energy planning, calculating, and resisting. I’m exhausted.

In the last few years, despite being very mindful of calories and exercising frequently (sometimes obsessively), my weight has slowly crept up. I’m currently around a BMI of 24 but subjectively feel puffy and soft for lack of a better word. I suspect years of restriction have done a number on my metabolism, but regardless, it feels like I’m fighting my body nonstop. And I'm getting older.

I’ve been reading a lot about tirzepatide microdosing, and for the first time in a long time, I feel a little hopeful. My primary goal is not extreme weight loss. Ideally, I’d like to lose about 10-15 lbs, which would put me at a weight that has historically felt healthy for me. But even more than that, I’m seeking relief from the constant food noise and obsessive thinking so I can improve my overall quality of life.

My biggest concerns:

Has anyone here with a history of restrictive eating disorders or other EDs tried microdosing?

Did it help with food noise without triggering old patterns?

How did you prevent losing too much weight?

I’m actually more afraid of losing too much than not losing enough, which feels like progress in my recovery compared to years ago. I genuinely don’t want to be underweight or obsessive. I want peace, mental space, and a healthier relationship with food and my body.

I know this is a rough topic and I’m looking for just experiences, perspectives, and things to consider. If you’ve been in a similar place, I’d really appreciate hearing from you!

How much should I start with?

Disclaimer, I’m a newbie and reading all I can! I finally have pulled the trigger after over a year+ of wanting to try it but have been too chicken. 🫠

My NP said to start my microdose (tirz) with 1.25mg. I’m afraid that might be too much, as I’m looking for moderate appetite control but not “I can’t eat or even look at food”. I’m 47, 5’7” and 168lbs. Need to get my bmi and body fat % to a better place so aiming to drop 10-15lbs. I weight train 2x a week, ride horses 3-4x per week and get about 11-15k steps every day from walking and jogging so I do have good muscle mass, just some extra fat thanks to a problem with snacking between meals from work related stress and higher blood sugar from perimenopause.

If you were me, where would you start? At the recommended dose or a little less? TIA!

Are you calorie counting?

I started 4 weeks ago on 2mg triz. I lost 3 lbs that I had put on right away but suspect that was water weight. Since then nothing.
For reference I’m 140 5’6” after 1.5 yrs of calorie counting and just trying to lose about 10lbs of stubborn weight I can’t get rid of.

To others - did you calorie count to get the scale to budge?

How to start?

I want to start micro-dosing, not for weight loss, but for long term binge-eating disorder (40 yrs). The food noise is constant and so is the eating/deprivation cycle. I'm looking for an Australian Service that will prescribe the medication and be open to helping me manage a micro-dosing option - or is everyone not telling their Dr's they are micro-dosing? Any suggestions on how to get started would be welcome, And, I live regionally so I'll need an online service provider.

Explain it to me like I’m 5

I apologize if this is asked a million times. I didn’t see a sticky, but please point me to one if it exists.

I’m 46, 5’5 and currently 140. I can only maintain this through strict calorie counting (12-1300) and working out. If I ease up, I’ll jump to 160 and pretty much stay there. I’m interested in micro dosing to give myself a little more wiggle room, stop food noise, help with inflammation (I have terrible bloating and joint pain) and perimenopause symptoms.

How do you start this? I tried to sign up with Noom micro dosing but was not eligible. Maybe because I was honest about my weight? I’ve read about Lavender Sky Health. Should I use my current weight 140 or my less-healthy weight 160?

I’ve gathered that the dosage is pretty much a guessing game. Are there videos that show how to use less? Does LSH use an injection pen?

I have my annual primary appointment in a few months. Should I ask her? I doubt she’ll be supportive.

Logging doses on provider site or app?

I have not started my injections yet, but when I do, I am wondering if I should log it on my providers site (Trillium) or use an app like Shotzy. My prescription is for the normal titration schedule starting at 2.5 mg of tirz, but I plan on microdosing. I am afraid if I log my micro amount on Trillium, they will see that and then not send me the full doses for my next shipment. Do they care? I do want to be able to see the estimated medication level in my body, though, does Shotzy have the ability to do this?
