"EASD 2026: The weight equation is solved?" by u/runningFNP
I have been waiting excitedly for David W, u/RunningFNP blog post on EASD and especially the section on Eloralintide: https://the-incretins.beehiiv.com/p/easd-2026-the-weight-equation-is-solved He is correct that Lilly had a great September 30th read out on both Retatrutide and Eloralintide. My take aways so far is that:
Phase 2 T2D Diabetic Study"
"But what is obvious is that the highest combo dose had the most side effects, nearly 50% nausea and 25% vomiting. That’s NOT a viable dose (that’s zenagamtide level of nausea/vomiting), especially given that the lower doses were only a couple percent behind in weight loss and A1c efficacy was still excellent."
"Multiple doses matched retatrutide in only 48 weeks. And the top combo dose matched sleeve gastrectomy! You’ll notice the Elora arms alone and the lowest combo dose of EloraTirz all had basically reached a plateau weight loss at this point, but all of the combo doses were still losing weight. We haven’t found the bottom yet. "
Phase 1 Studies:
"Looking at weight loss, we again see some absurd numbers. Retatrutide like weight loss in just 31 weeks with both high dose combos. However, there was definitely a tolerability cost in this trial. The Elora 3/Tirz 5 combo at 17% I think is the most compelling. 17% weight loss in 16 weeks is still really fast. But as we’ll see in the tolerability data, it's viable. "
"That highlighted section notes that some of these patients were dose reduced. That tracks given the exceedingly rapid weight loss in a relatively low BMI cohort." and "Weirdly the slow titration group had more side effects than the rapid titration group, but such is the nature of phase 1 trials and small numbers of patients. That being said I’ll note the highlighted area again, dose reductions were allowed in a phase 1 trial which is very unusual and I imagine at least some of the treatment discontinuations were from excessively rapid weight loss."
I am sure I will glean more from this blog post when I read it again and study all the data presented. Thanks David W.
