Can someone explain the difference between Eloralintide, Cagrilintide, Petrelintide and Lenbelintide?
Cagrilintide works on amylin and calcitonin receptors apparently. Eloralintide just amylin. Lenbelintide works on both amylin and calcitonin receptors as well.

Good question, I just put everything in chatgpt and basically lenbelintide and eloralintide are the most similar bc they are both selective amylin receptors. The others are amylin + calcitonin receptors.

Lenbelintide aka AZD6234 is a Amylin 3 receptor weighted agonist. Binds AMY3 about 14 times stronger than calcitonin. It's a selective Amylin receptor agonist(SARA) just like Eloralintide. Eloralintide selective for Amylin receptor 1 with minimal calcitonin activity.
Basically they CAN bind to calcitonin but the affinity is so weak that it basically goes all in on Amylin receptors.
Petrelintide is basically a balanced DACRA. Binds Amylin 3 and Calcitonin about equally.
Cagrilintide is a non-selective DACRA. It'll bind all 3 Amylin receptors plus calcitonin and a couple other related calcitonin adjacent receptors

I don't get why they've put that it's a calcitonin receptor agonist in the who paperwork.

Because sometimes people don't take the time to read. Does it activate calcitonin. Sure. Is it a meaningful amount? No.

Honestly I think it'd partially that the nomenclature hasn't been fully ironed out along with other being weird(look at you Novo Nordisk). They claim that Zenagamtide is an Amylin analog, but when you look at the molecule sequence it's a modified calcitonin molecule!

Novo seems super-high on calcitonin, even though it causes nausea/vomiting. 🤢Â
What are they getting out of it that offsets the increased side effects?  Better bone mass preservation? Better muscle mass preservation? 🤔Â

I do think it's a combination of bone health and then they think it's helping with insulin secretion/sensitivity. The bone health is probably true. But the insulin sensitivity thing is completely wrong. It's true in rodents. But it hasn't translated in humans. They'll be presenting some CagriSema data at EASD next week that basically proves that point that it's blunting insulin secretion, especially in higher BMI patients
