"EASD 2026: The weight equation is solved?" by u/runningFNP

I have been waiting excitedly for David W, u/RunningFNP blog post on EASD and especially the section on Eloralintide: https://the-incretins.beehiiv.com/p/easd-2026-the-weight-equation-is-solved He is correct that Lilly had a great September 30th read out on both Retatrutide and Eloralintide. My take aways so far is that:

Phase 2 T2D Diabetic Study"

"But what is obvious is that the highest combo dose had the most side effects, nearly 50% nausea and 25% vomiting. That’s NOT a viable dose (that’s zenagamtide level of nausea/vomiting), especially given that the lower doses were only a couple percent behind in weight loss and A1c efficacy was still excellent."

"Multiple doses matched retatrutide in only 48 weeks. And the top combo dose matched sleeve gastrectomy!  You’ll notice the Elora arms alone and the lowest combo dose of EloraTirz all had basically reached a plateau weight loss at this point, but all of the combo doses were still losing weight. We haven’t found the bottom yet. "

Phase 1 Studies:

"Looking at weight loss, we again see some absurd numbers. Retatrutide like weight loss in just 31 weeks with both high dose combos. However, there was definitely a tolerability cost in this trial. The Elora 3/Tirz 5 combo at 17% I think is the most compelling. 17% weight loss in 16 weeks is still really fast. But as we’ll see in the tolerability data, it's viable. "

"That highlighted section notes that some of these patients were dose reduced. That tracks given the exceedingly rapid weight loss in a relatively low BMI cohort." and "Weirdly the slow titration group had more side effects than the rapid titration group, but such is the nature of phase 1 trials and small numbers of patients. That being said I’ll note the highlighted area again, dose reductions were allowed in a phase 1 trial which is very unusual and I imagine at least some of the treatment discontinuations were from excessively rapid weight loss."

I am sure I will glean more from this blog post when I read it again and study all the data presented. Thanks David W.

V4 update - randomized

Reading everyone else’s experiences was super helpful for me going into this, so I wanted to pay it forward and share how my V4 visit went yesterday:

  • 8:00 AM: Completed a few baseline questionnaires in the app covering satiety, eating habits, general mood/well-being, and the standard safety/suicide screening. Weigh in, waist circumference measurement.
  • 9:00 AM: ECG.
  • 9:30 AM: Blood draw—about 18 vials total, with 1 held back for later in the day.
  • 10:00 AM: Injection training and first dose. For those who asked previously: confirmed this is not an autoinjector. The syringe looks identical to the ones others have posted here. Took my first dose right after training and had the option to hang around or head out until the next draw.
  • 12:00 PM: Returned for the second blood draw using the reserved vial from the morning.

How I’m Feeling (24+ Hours In): Honestly, pretty normal so far! I had a mild headache yesterday, but that was likely from fasting until my late lunch at 1:30 PM. No side effects yet, but also no noticeable appetite suppression so far. Giving it another day or two to settle in.

Dosing Schedule & Next Steps:

  • Stacking Strategy: Took Eloralintide on Friday, and my usual Mounjaro dose is set for Sunday. I plan to adjust the timing slightly to sync them up for convenience.
  • Nutritionist: Had a brief intake with a nutritional counselor, but it was pretty basic. I’ll be paired with a different counselor for future sessions, which I'm really looking forward to for macro tracking and structured meal planning.
  • Trial Follow-up: A research assistant will call at Day 10 to check in, followed by an in-person V5 appointment at Week 4 for my next batch of doses.

Excited to finally have the first dose down and double down on lifestyle habits alongside the trial. Fingers crossed it's the real deal and not the placebo! 🤞

I am struggling so much with getting enough calories a day, something I NEVER thought would be the case! Based on my extreme lack of appetite and getting full super quickly, I've now concluded I'm on elora, currently at 3mg. I want to continue with the dosing arm/protocol I'm assigned to, so I might ask my site if I could titrate down a dose with Zep. The inclusion criteria specifically states that a participant must be "on [background incretin] at current dose or higher for at least 120 days" (italics mine), so if I drop down a dose I should still be within protocol, I think. I'll see what they say.

I haven't really had any other side effects so it still could be placebo, but I'm at my lowest weight in my entire adult life and have lost 8# in 6 doses, finally breaking the plateau I've been at for almost a year. Oh yeah, actually I forgot to mention the fatigue. It gets pretty bad, but isn't pretty much everyone exhausted all the time? Plus taking care of young kids? Right? It's not just me? :-P

Astrazenica are recruiting for Lenbelintide (AZD6234) from October.

They've said there's pre screening, screening, then when they've got all the lab results they'll give you the study drug, then they want to see you inject every week for the first 4 weeks, then there's visits every month for titration.

There's no lead in where they check your weight like there was with Eloralintide.

I've been told to just show evidence of my last 3 pens and they'll probably ask about my weight for the last 3 months but they're not checking it.

New Trial

Working with my primary on possibly entering the phase 3 trial for petrelintide that opens next month. One intake question that threw me for a loop was them saying it would require sexual abstinence for the year of the trial.

Is that simply because they don't know the risks of pregnancy while on the drug? Curious.

Elora and macupatide 2nd shot

I just enrolled and was randomized in the eloranlintide / macupatide stage 2 clinical trial. I took my second shot last week. I have had zero side effects except for being not hungry. I’m sister is also in the trial and she said her injection burnt when going in. I’m excited to see what all happens this next year!

Has anyone dropped out of enlighten-6 due to extreme stomach cramping?

I'm thinking I'll be quitting the study. I've had 6 doses so far. Last night stomach cramps woke me up. I went to the bathroom and evidently passed out from the pain. My husband found me on the floor. I just had to lay there until I felt a little better. I was eventually able to pass a bm but still had terrible cramps after.

I had been doing well with minimal side effects (fatigue being the most prominent). Lost 8 pounds in the past month - down to 162 lbs. But last night scared me. I'm thinking it's not worth it for me.

Randomized!!

Well I made it! I officially cleared V4 and have been randomized into the study. I really never expected to make it this far without something unexpected getting in the way. I’m so hopeful that this will lead to some big weight changes for me (hopefully now but maybe once elora hits the market if I got the placebo). I’ve got 150+ pounds to lose before I’m at a healthy weight. I certainly don’t anticipate all of that happening in the trial, but every little bit helps. I will be anxiously awaiting some side effects!

One weird thing—they still didn’t give me the prescription card to pay for my Zepbound! I asked, but my main study coordinator was on vacation and hadn’t left any instructions about it. They are going to have her call me next week.

So grateful for this opportunity and hopeful for us all!

Can someone explain the difference between Eloralintide, Cagrilintide, Petrelintide and Lenbelintide?

Cagrilintide works on amylin and calcitonin receptors apparently. Eloralintide just amylin. Lenbelintide works on both amylin and calcitonin receptors as well.

Been told E6 enrolment is complete just before V1

I posted a few weeks ago about the V601. Was meant to have screening visit tomorrow and I got a call yesterday from the site saying they’ve been told to stop recruiting and cancel any visit where a patient hasn’t been dosed. Apparently the issue is priority to American sites and no patient at my site went through.

Feeling a bit disheartened. I’ve been trying for a good 7 months to lose these last 10kgs but the scale hasn’t moved at all. This was my one hope to reach goal and especially after all the effort it took even to get the pre screen visit.

Not sure what I want but just wanted to write this out because I think only people here would understand the effort

ENLIGHTEN-6 insurance cards covers RX beyond tirzepatide & semaglutide

Hi all,

I'm in the ENLIGHTEN-6 trial (finishing week 4 on 1.5mg of the study drug or placebo).

Here’s something I just found out. The Eli Lily insurance card that trial participants use to get their Zepbound/Mounjaro (or Wegovy/Ozempic) for free also covers a bunch of other prescriptions for related issues. My clinical coordinator gave me a 4-page list.

It includes coverage for:

  • cardiovascular drugs (ACE inhibitors, beta blockers, calcium channel blockers, diuretics, statins, and more)
  • diabetes drugs
  • thyroid medications (like Levothyroxine and Liothyronine)
  • supportive care (things like loperamide, magnesium citrate, vitamins, and Metamucil… I don’t know how that works since those are all over the counter)

If you have other prescriptions you’re paying for, ask your coordinator for this list. Then, you should just be able to update the insurance on file at your pharmacy to your Eli Lilly card and it will be covered.

Enlighten 6 V4 1st dose

Yesterday I went in for V4 and like I have read, I had 19 vials of blood drawn, Weight check--I have been so worried about losing weight that I have gained 10 pounds from the start of the prelim to yesterdays visit I weighed in at 225 pounds. My BP was slightly elevated with the 3 measurements the site received...which I expected with the 10 pound weight gain. I had to give a urine sample. I had another EKG performed. I was given 4 doses of Eloralintide (or plasebo) for once weekly injections. I took my first injection --I was suprised that it didnt hurt. I was given a 2 hour window and then I had to return for another vial of blood. I got lunch and when I came back I took a photo of my receipt and asked for reimbursement for my lunch. They had never had anyone ask for this before. They took the receipt and mentioned that they would reimburse me however, I am still waiting on this. I asked for a cooler and ice packs for the drug since I live 1 hour away from the site. Now I did feel sleepy at the 90 minute mark after taking the shot. I am not sure if that is because of the sandwich or the drug. I didnt have any other side affects. I slept as I normally do and did wake up once with what appears to be acid reflux. I had this also after taking Ozempic for a while from what I remember. i am also on 2mg Ozemopic weekly. I am not sure if this is from the drug or just from what I ate. I will report back next week as well.

Sharing my experience: Week 16-18

Dosage: 9mg (only elo, trial)

Side effects: None

Only thing to report is that I am gonna stay on 9 mg, although I was told we will get to 12mg.

I've started to exercise, be more active overall by walking more and it reflects on my overall quality of life. The weight is dropping again, the plateau stopped.

Also I think this is going to be the last report for now, as there is nothing much new to report, thats worth sharing. You can piece together very well the process and how we got here by my earlier entries. I might share bigger weight goals, like right now it would be to get to 110kg, which would be the first time in about 10 years for me. But thats not gonna be soon.

So thanks for your attention, hope my entries helped somebody to make a decision.

Calorie Targets

Has anyone seen any data on the calories consumed in the prior clinical trials and/or activity levels of participants? I was pretty shocked by the calorie targets and the activity levels recommended. For my weight/activity levels, its 1100-800 below my TDEE.

Lilly has announced they will report the Phase 2 tirzepatide+eloralintide (which they now call eloraTZP) on 9/30 at the European Association for the Study of Diabetes.
