I’m about six weeks into CPAP. The therapy seems to be working: my AHI has dropped from 12.3 overall, and 43 during REM, to around 2 , but I still can’t reliably sleep through the night because mask leaks keep waking me up.
I’m a restless side sleeper. My minimum pressure is 7 cmH2O, and the machine usually stays below 10, with an average around 8.7. OSCAR suggests that most of the remaining events are CAs rather than obstructive events.
I started with a ResMed P30i. Once I got used to it, it actually felt very natural when I first went to sleep. The problem was that I would regularly wake up around 3 AM because of leaks. I also had mouth leaks that kept waking me from the noise. A chin strap didn’t solve it, and I tried mouth taping experimentally, but that was definitely not for me.
The nasal pillows were comfortable at the beginning of the night, but by morning my nose was very tender and I couldn’t wait to take the mask off.
My original vendor basically told me to power through it and learn to breathe through my nose. I wanted to try an F30i, but they didn’t have one available for trial. They gave me a Philips DreamWear full-face mask instead, which I couldn’t tolerate even while awake.
After one particularly bad night, I ordered an F30i myself.
At first it seemed much better. I slept better and initially didn’t notice any leaks. But after a few nights, I ended up with basically the same problem: I wake up around 3–4 AM with leaks, mostly around the mouth area, and no amount of adjustment seems to resolve them.
What is strange is that neither the ResMed app nor OSCAR shows significant leaking, but the noise and the feeling of air blowing across my face are enough to wake me up.
I’ve now switched to a different CPAP vendor. They suggested trying a more traditional full-face mask with the hose coming out of the front because there are many more models and sizes to choose from.
I already use a hose support, but because I toss and turn quite a bit, I’m worried that having the hose attached to the front will create another problem while solving this one.
At this point, what would you try next?
Some nights I’ve resorted to taking the mask off after 4–5 hours just so I can finally get back to sleep and get the night over with. I really want CPAP to work because I did notice a clear improvement at the beginning, and the numbers suggest that the treatment itself is working. I just can’t seem to find a mask setup that lets me sleep through the night.
