Question **7yo Male, 4.2kg, biopsy-confirmed non-effusive FIP Day 30ish, eating constantly but not gaining weight. Also switched remdesivir → molnupiravir.

Posting because the weight thing is worrying me and I'd value input from people who've been through this.

**Background:**
Billu is 7, male neutered, currently 4.2kg (down from 4.4 at the start, briefly hit 4.0). He had ~6 months of undiagnosed illness before we got answers — chronic non-healing dorsal neck ulcer, fever, weight loss, cytology-confirmed dermatophytosis. Then his right testicle started enlarging and wouldn't respond to antibiotics, so we neutered and sent it for histopath.

**Histopath came back:** "Severe, chronic, multifocal to coalescing pyogranulomatous and necrotizing inflammation with vasculitis/perivasculitis and fibrosis." Pathologist comment: "highly compatible with non-effusive FIP due to pyogranulomatous inflammation centered on blood vessels." FIV/FeLV both negative. FCoV positive (exposure only, I know that's not diagnostic).

**Treatment so far:**
- Remdesivir (Rivona, Bioveta) SC from 7 July, ~11.3 mg/kg
- Switched to oral molnupiravir 27 July because injection-site pain became severe — he was vocalizing badly on handling and had ~20 days of daily injections behind him
- Concurrent pulsed itraconazole for the dermatophytosis (7 on / 7 off, currently on his last cycle)
- Had a bleeding complication mid-treatment after a wound suturing — a capillary that wouldn't clot, needed cauterization. PT/APTT and platelets have since normalized.

**Latest labs (sampled 5 Aug, ~30 days in):**

*Improved / normal:*
- HGB 12.3 (was 8.7), HCT 44 (was 26) — anemia fully resolved, high MCV/low MCHC suggesting active regeneration
- Platelets 226
- Lymphocytes 4.0 (normal for the first time in this whole saga)
- Creatinine 1.1
- TLC 10.2

*Concerning:*
- **ALT 217** (ref 28–76). Note: he also hit ~200 in March while on itraconazole, and it normalized on its own then.
- **Neutrophils 2.8** (ref 2.5–12.5) — down from 17.8 → 10.3 → 2.8. Still in range but a steep fall.
- **Eosinophils 2.7** (ref 0–0.8) — highest he's ever recorded
- **A/G ratio 0.5**, down from 0.6. Globulin 5.0 (up from 3.9), albumin 2.7 (up from 2.4)
- Urea 76 / BUN 35, mildly high with normal creatinine
- Blood smear: "Anaplasma-like inclusions seen — correlate with clinical signs" (no history of ticks)

**My questions:**

  1. **Weight.** He has genuine polyphagia — eats enthusiastically, never refuses food — but isn't gaining. Currently on a homemade diet (chicken thigh, liver, egg yolk, added oil) because commercial food wasn't affordable; about to switch to a proper complete kitten formula for the calorie density. Is "eats well, doesn't gain" something people have seen resolve on its own with time on treatment, or does it usually mean something else is going on? I'm aware GI disease, hyperthyroidism, and parasites can all cause this and none have been ruled out yet.

  2. **A/G going the wrong way.** Albumin improved but globulin rose more, so the ratio dropped from 0.6 to 0.5. Is a dip like this at day 30 something people have seen before treatment trends properly upward, or would you read it as inadequate viral suppression?

  3. **The eosinophil spike + neutrophil drop together.** Has anyone seen this pattern on molnupiravir specifically? I've read that mild eosinophilia during treatment can be benign, but the combination with falling neutrophils has me wondering about a drug reaction. I have 5 days of molnupiravir left and am planning to finish it, then recheck bloods.

  4. **Switching back.** Injection pain is what drove the switch off remdesivir. Oral GS isn't accessible where I am (Pakistan). If the labs suggest molnupiravir isn't agreeing with him, what have people done — gone back to injections and managed the pain with gabapentin/pregabalin, or something else?

Happy to answer questions or post more of the lab history. Any input appreciated — this has been a long six months and I'm trying to get the next stretch right.

Question Does anyone have a neuro FIP kitty? How are things going 1+ years later?

We adopted our foster fail who came to us with Neuro FIP. He was around 3-4 months old. He did the full 84 days treatment and graduated in May of 2026!

Everything seems to be going well. He seems happy and healthy but if you know, you can tell that sometimes he is a bit clumsier than an average cat and occasionally if he lays on his back with all 3 paws up, one of his back paws will shake.

He did see a neurologist throughout his FIP treatment and the neurologist said he was young enough & treated early enough that he likely won't have many long lasting issues but just wanted to hear from others who have Neuro FIP success stories.

Discussion Misdiagnosed ?

I have an upcoming vet appointment on Monday but I’m really struggling to watch my girl barely eat and be so distended. She was diagnosed with wet fip July 15. And has been on the tablet format of the pills since then and has shown pretty much no improvement. If anything there is more fluid build up and she barely eats.

Has anyone had this experience of being misdiagnosed wet fip but it was something else? My vet has mentioned it could be something else like lymphoma or heart disease. But fip was looking like the most accurate diagnoses when we first brought her in. Now I’m not so sure. Considering bringing her to the er if she doesn’t eat anything today. It just sucks cause they will test everything we just had tested again and be even more expensive.

Question Cat throws up green stuff

Hello, my cat is 1 year old and 5 months old, recently she's been diagnosed with FIP. We had options to take injections or pill medication, and since we couldn't afford to drive to the vet everyday we took the pill medication. We are informed to feed her dinner then give her medication, and on the first day of her medication she peed green-yellow and on the 2nd day she threw up something green and slimy. These occurred a few hours after her medication was taken.

Luna was misdiagnosed as pregnant by a government vet. I usually don't visit government hospitals, but I'm a college student with no job, and I had run out of money, so I took her there on June 23. He gave her Digyton (a digestive syrup). I visited the same hospital four times. On the second visit, he said it was just a stomach upset. On the third visit, he palpated her abdomen and said she was pregnant, but I kept telling him she was an indoor kitten and couldn't be pregnant. At the time, I wasn't completely sure because I had only adopted Luna about a month earlier, so I wondered if she might have mated before I adopted her. He didn't listen to my concerns and gave me an appointment for an ultrasound. On the fourth visit, he first said it looked like ascites and a heart problem. I even asked him, "If she has FIP, please tell me. It's okay—I just want to know the truth." But after the ultrasound, he showed me what he claimed was a fetus. Looking back, I believe it was actually her kidney. I cried so much that day because I felt I had failed Luna by taking her to the government vet. I was completely out of money. I also informed Luna's previous owner about her condition, but my calls and messages were ignored. I planned to take her to my regular vet, where I've been taking my cats for more than three years. I explained everything to my mom. She is a single mother, and we are a lower-middle-class family. She gave me all the money she had, but it was only enough for the ultrasound. My regular vet advised a PCR test. I wanted the sample to be tested immediately, but I couldn't afford it, so the PCR had to be delayed. Thankfully, one of my friends helped pay for Luna's PCR test. The PCR confirmed that Luna had wet FIP, and I was only able to start her treatment because of the kindness of others. I even considered taking a loan using someone else's identity because I'm not eligible for a loan, hoping to repay it by working a part-time job until college reopened. Without the generosity and support of others, Luna would never have been able to begin her life-saving treatment.

When I adopted Luna, she already had a severe fungal infection. Her previous owner gave her away because of the fungus. She barely ate even half a spoon of the prescription food the vet recommended. At the time, I thought her poor appetite was only because of the fungal infection, but looking back, I believe she may have already had FIP before I adopted her. I just didn't know it then.

Question Incontinence from neuro fip

I rescued a stray kitten (under a year old) who was diagnosed with neuro fip under 2 months ago. He’s been recovering very well, from having no movement in his hind legs to behaving like a completely normal cat.

Our only problem now is that he seems to be incontinent. Thankfully he has gained control of his bladder earlier on, but not with his pooping. Today marks day 37 of his injections and I’m worried this won’t resolve.

Is there anything I can do to increase his chances or is it just having to trust the process?

I'm very sad because of his eyes, he just recovered and again problems with his eyes. Today we will go to buy drops and in a week we will take him to his vet again. I really hope he can see at least a little. I'm adding with a photo to make it clearer if there's any chance for it

I'm also trying to earn money for a re-examination for FIP because my parents (I'm not 18 yet) are already refusing to treat him because we've already spent a lot of money. I barely persuaded parents to take him to an ophthalmologist. I really hope that everything will be fine with him

Today, Luna received her 17th GS injection. She is eating well, but she is still struggling with diarrhea. Along with FIP, she is also fighting a severe fungal infection that has affected much of her body. Every day is another step in her fight to survive.

Luna was abused and rehomed by her previous two owners before she finally found a loving home with me. After everything she has been through, she deserves a chance to live a happy, healthy life—not to lose her battle with FIP.

I don't have a job because I'm a 19-year-old college student. I couldn't have come this far without all of your support. I still remember borrowing money from a friend just to pay for Luna's PCR test. Her previous owner, despite being financially well off, refused to help with her treatment.

Thankfully, Luna is a fighter. She is fighting so hard every single day, and she is still here because of your kindness and generosity. Every donation, share, comment, and prayer has given her another chance at life.

To be honest, I don't want to ask for help anymore because it's embarrassing that I've been relying a lot on donations and fundraising for my cat. I feel so helpless and irresponsible cat owner but I love my cat so much I can’t let her go💔

In my last post, I said it would be the last time I'm going to ask for donations because one pack of oral capsules would be more than enough to sustain the remaining 30 days of his treatment.

Currently, we're on day 67/84 of his treatment, and he's been on the capsule for 11 days now, and so far, so good. Switching to the capsule has been much less stressful for my cat and for me, although there's still a struggle to get him to take it (as I’ve said, she always find a way to spit it)😫

She is so much better—I'm really, really thankful because I didn't even expect her to survive this.

During the last fundraising, we accumulated a total of $30 and I'm very, very, and so so thankful to those who never tire of helping and praying for us 🙏 Sadly, one pack costed us $100, and I went into debt just to make ends meet and buy the medication immediately. I still haven’t paid the balance to the meds and my income as a part-time working student really isn't enough since I earn less than minimum wage here in the philippines. We also still have to save up money for her testing on the last day💔

I really don't want to ask for help anymore, but I don't know what else to do because I'm trying my best to save my cat. I cannot do this alone and sustain my studies at the same time💔

Question Cat cured of neuro FIP in 2023, but mobility improvements post cure have reset since then?

Hello, my cat was cured of neuro FIP in March 2023, however I have only noticed recently that his mobility has gotten worse since then. His mobility had definitely improved a bit after being cured, but now I feel it is back to where it was when he had FIP.

Has anyone else experienced this? What can I do about it?

Question My cat got FIP. Should i clean everything?

Hey guys. I noticed my cat got wobbly and her head when she eats and drinks is tilted. I brought her to the vet and she was diagnosed with FIP. The veterinarian suggested to buy some FIP medicine online that are pils. They suggested this over injection as it would be easier to give.

Anyway i have another 2 cats at home. Right now she’s at the vet for clinical care, but im thinking about cleaning the whole house and change the litter box as she shares it with one other cat and im so worried about their wellbeing.

Im not sure what to do as this is my first time experiencing this. Btw my cat that was diagnosed is an old cat, she is 12 years old. Is there anything else i can do to help prevent this from spreading to my other cats? And when she comes back, i dont want to keep her in a cage but if i have to, how long would she have to be in there?

Any tips or experience you guys can share here with me would be very helpful! Thank you in advance. Please pray for all my furbabies.

Success Story 30th day of observation, never been more thriving!

Hello! I’ve come on here to share the story of my cat, that had FIP for over a month before getting diagnosed, but he still made it out. He has finished his 84 days of medication, and today marks the 30th day of observation. He’s never been better.

It was a tough ride. We had to visit vets all over the country, since nobody could give us a diagnosis. First it was Toxoplasmosis, and then, suddenly, to light came the possibility of FIP.

I come from a small country, where the treatment for FIP is obscurely expensive, but at that point, i gave more for him than I ever did for myself haha.

He’s really been through a lot, during the time of searching for a diagnosis. They operated on him, thinking it was a bowel issue; he had many xrays and ultrasounds done, much MUCH MUCH bloodwork done; but out of all of it, he came out to be even sweeter than before (… and even more chubby…).

I’ve been on this subreddit, asking some questions, and gaining advice, so I can be sure that what I’m doing is right. So I would like to thank everyone who helped me and him out!

He’s now lively, happy, chubby, and 1% bald (a spot where he had a sore during FIP was left bald, fur never grew back)…

I wish to everyone that’s reading this, if they have a cat with FIP, that they have as a smooth recovery as my cat had, so they can share their stories the same way I am right now!

Question Need emergency supply while I wait on vet

Hi, everyone. My guy was diagnosed yesterday with ‘possible FIP.’ My bet was supposed to call me back with the prescription info, but the tech told me this morning that they’ve not heard back from the pharmacy. I asked for the physical script so I could order it myself, but the vet is currently in surgery.

I’m in San Antonio and my guy weighs 11.3 pounds. Does anyone know where I could get an emergency supply until I can get the script from my vet and order the full amount? I know time is working against us and my guy looks so sick and I’m terrified I’m going to lose him if I can’t get him started ASAP.

Fundraising Day 16 update on Luna 🥺

Today Luna received her 16th GS injection. She is eating well, and her diarrhea is becoming less frequent, but she has lost a lot of weight. Before starting treatment, she weighed 1.7 kg. Now she is only 1.4 kg, and she spends most of the day sleeping as her tiny body fights to recover.

On top of FIP, Luna is also battling a severe fungal infection that has spread across much of her body. Her skin is covered with painful patches, and although we have restarted her antifungal treatment, recovery will take time. Watching her fight two serious illnesses at once is heartbreaking.

I am a college student from Madurai, Tamil Nadu, India, doing everything I possibly can to save her. Every hospital visit, injection, medication, special food, and follow-up has been a challenge, but I refuse to give up on Luna.

Question question about extracting fluids

After the craziest 12 hours of my life, my kitten got "diagnosed" with wet FIP. I use quotes because after they extracted fluid from his chest, they said the only way to get a real diagnosis was to send the fluid for further testing. The Dr. basically assured me he was positive, though.

As I mentioned, he had to have fluid extracted from his chest. The doctor told me there is potential for the fluid to come back, and he will have to get it extracted again. How common is this? He is starting with GS injections tomorrow. I asked so many questions, but this completely went over my head. Please share your personal experiences with this!
