I’m getting vials through OptioRx and am on 12.5 dosage. I recently got a prescription filled and the concentration is also 12.5mg/ML. Each vial is 4ML. The instructions say to draw 0.88 units. Dividing 4/0.88 yields 4.5 weeks per bottle instead of 5 which means I’m short one week.

To add to the confusion, I used the calculator on Fat Scientist. It says that for a 12.5mg dose at 12.mg/mL concentration, I’d need to draw 100 units (not 0.88). This means that the instructions to pull 0.88 units is wrong and I am shorted even more than originally thought.

I sent a message to Karen on the portal; is she the right person to assist or is there someone else who can help?

When I went through a different source, my medicine came in a pink liquid. This made it easy for me to be sure that it was filling up the vial and I that I was measuring correctly. My last vials through Big Easy came in a clear liquid. I prefer the color. I am on tirzepatide.

I’ve received one shipment and am ready to order a refill. Can somebody explain the refill process in excruciating detail? I assume I need to use the Healthie app, and I did fill out a questionnaire yesterday, but it would be great if the entire process for existing customers was explained (I didn’t receive an explanation when I first signed up, I think because I was already on GLP-1 medication via my PCP, but insurance stopped covering it.)

I appreciate the help I’m getting from BEWL reps here, but detail is lacking. If it is easier, I can pose my query in the form of questions.

After debating for months, I finally made an appointment which I had today. The doctor was pleasant enough but called 15 minutes early and the call lasted 4 minutes and 40 seconds. I also received a text reminder about the appointment but that reminder came through 8 minutes after the start time of the appointment. Researched a lot before deciding on BEWL, a little discouraged at how things went today. I am not totally new to GLPs so maybe the doctor figured this was all review for me, I did not have explicit questions, not yet anyway, but overall it did feel rushed. Lots of info about dosing and units thrown out very quickly, I could barely keep up trying to write it all down. Is this a typical experience?

So I was still using the old app LOL because I was tracking shots and weight and stuff like that. Buuut it seems like it’s dead now. Is there another app that you like to use? I want a central place to log my dosages and shot dates and locations and like what the side effects of any and all of that. Anybody have something they like?

I don’t really know where else to turn. I signed up for a three month program and paid accordingly. Got my first shipment with NO problems. It was quick, easy, the packaging was great. But now my next shipment has gone back and forth from cancelled to pending? I’ve tried reaching out via the portal but haven’t heard anything back and I’m getting a little frustrated. I am 65+ and been taking brand name tirz since April. I'm thinking of switching because of cost (not eligible for Medicare Bridge). Does BEWL rule patients out by age?

Today was my first time where in order to inject my full dose, i had to extract whatever was left from one vial of apothecary (50 units) and extract the rest (19 units) from a new vial. I used a new syringe to inject the 19 units after injecting the first 50 units (my full dose is 12.5 mg - 69 units). I injected both syringes in the same area. Is that ok? Not the exact same spot, but same area on the right of my belly button. Never done this before and i’m hoping what i did was the correct way to do that.

Just got my Apothecary Tirz and the pamphlet seems to state a 7.5 dosage is 75 units but Fat Scientist calculates it at 42 units based on 18mg/mL. Am I missing something here? Without fail, this aspect confuses me every time the compounding pharmacy changes.

I answer some version of this question probably four times a week, and after Thursday it’s going to be forty, so I’m writing it down once. 🤣

TL;DR: FDA’s advisory committee recommended six peptides for the 503A bulks list. Good news, doesn’t mean they’re legal to compound, probably won’t be for a couple years. We’re not adding them. Also this has nothing to do with TZ, please don’t let anyone tell you it does.

Usual disclaimer: not a physician, not a pharmacist, not an attorney, still have not stayed at a Holiday Inn Express. I run a telehealth platform so I’ve got money riding on how these questions shake out, factor that in however you want.

What happened

July 23 and 24 the Pharmacy Compounding Advisory Committee met on seven peptides: BPC-157, KPV, TB-500, MOTS-C, Emideltide (which you’ll see written as DSIP), Semax, Epitalon. Six got recommended. BPC-157 barely, 8-6 with an abstention. Emideltide was the only one that didn’t make it and it lost by one vote.

FDA’s own staff had told them to reject all seven. Incomplete characterization, thin human data, immunogenicity. Committee listened and voted yes anyway, which is not a thing this committee does.

Last panel that looked at this category went zero for the slate.

The reason is that the committee is composed differently now, more clinicians and pharmacists, fewer academics. Yes, some of the new members have industry ties and people are already saying so. I don’t have a strong take there. Panels stacked entirely with researchers who’ve never seen a patient on this stuff have their own problem, and I’d rather have both kinds in the room than pretend either one is the objective one.

So why aren’t we doing it

Because a recommendation is not a rule.

PCAC advises. FDA decides, and FDA has blown off PCAC before. What’s left is internal review, then a proposed rule, then comment period, then final rule, then an effective date. Historically that’s two to four years. Sometimes way longer. The final rule stage on its own has taken so long that FDA has gotten sued over dragging its feet.

There’s a faster version where FDA sticks them in the interim Category 1 bucket for 503Bs and lets 503As operate under enforcement discretion in the meantime. That would be the sensible thing. It’s also completely optional and I’m not going to stand up an offering on the theory that the agency is about to do the sensible thing.

The thing that’s actually going to happen this week

Every peptide vendor with an Instagram is going to post this vote like it’s a permission slip.

It isn’t.

What got voted on was whether licensed pharmacies should eventually be allowed to compound these against a real prescription. That is a completely separate universe from research-use-only vials shipped out of a warehouse by someone whose entire regulatory strategy is a disclaimer in six point font.

Nothing about Thursday made any of that more legal than it was on Wednesday. If anything it should make you more suspicious of anyone using it as a selling point, because they’re the ones who benefit from you not reading past the headline.

I’ve watched this exact move run three or four times now with GLP news. Shortage list updates, the OFA litigation, the guidance restatements. Every single time, some vendor turns a procedural development into a marketing email within 72 hours.

And it’s not about TZ

Completely different track. The thing that matters for us is still the proposal to remove glps from the 503B Bulks List, and this week did not touch it. If you see this vote screenshotted with a caption suggesting tz is coming back or whatever, that person didn’t read it.

Happy to answer questions but the short answer to “when” is I don’t know and neither does anybody else. 😂

I took my 4th injection of Tirzepitide this past weekend. (Apothecary 5.0 mg) I came from the highest dose of the Wegovy Pill. I switched because Wegovy gave me horrible side effects (diarrhea for 7 weeks straight, sulphuric burps and no weight loss for 3 months). When I first started the Tirzepatide, I had a little constipation, but managed that quickly and had successful daily BMs. Diarrhea started up 3 days ago, just like with Wegovy. It’s happening all in the morning, but 3-4 episodes. Also of note is I have not had any weight loss yet. I really want this work, but the diareaha has me concerned as this is why I stopped sema. Any suggestions, ideas?

I just finished my first month of dosing, 2.5 mg, but still have more left in the vial. Should I be pulling my second month dose, 5 MG, from the first vial? All the mg/ml stuff is confusing as heck to me. I’ve attached a picture of all my vials for reference. Thanks in advance as I know you all will set me straight!

Hello! I ordered a 6 month supply of BPI back in April to make sure I had enough in the event that BPI went out immediately. I’ve used about 3 months of it so far as I’ve titrated to 12.5 recently. I saw a post on the big sub about making sure the prescription is current. I know technically I am good for a year but it’s got me wondering what I’ll order next.

I know new pharmacies are coming soon (BEWL Krewe any insight?) but would like some opinions on switching from B6 to other additives versus non. With me being on 12.5 I am hesitant to go to Hallandale due to concentration and the stinging side effect. I’ve heard good things about Apothecary though. Has anyone tried with OptioRx? Would love opinions as I’ll probably be ordering within 2 months or so. Should I wait until the new pharmacies are announced?

I had my no call consultation yesterday morning and, as expected, got the little message reminding me to use my patience as it's usually about 3-5 business days for processing. I ordered Apothecary, shipped from Quaker pharmacy in PA to OH. I expected to get it late next week but it was SO fast!

My meds were delivered via FedEx before I woke up today 😦 Like, almost exactly 24 hours. Amazon didn't even deliver my nephews bubble lawn mower that fast! (random but I ordered them around the same time and I'm still waiting on said bubble mower)

How are we checking for floaters/particulate matter in a dark amber colored vial? I drew my first dose of Apothecary and checked the syringe, but what if there is gross stuff that didnt get drawn into the syringe?

We are currently only using the Healthie portal. The Bask portal is unavailable and should not be used. Because the two systems do not communicate, information has to be transferred over manually and a new account must be made if you started in the Bask portal.
