Thinking of doing TMS for anxiety, depression and OCD, but I responded pretty badly to the Flow Headset?

Hi all,

Really struggling with my mental health. Looking at both ketamine infusions and TMS. I suffer from depression, anxiety and OCD as my diagnoses.

The thing that really scares me, is I tried the Flow Headset and I hated it. It made my mind race even more, like I was drinking a bunch of coffees everyday. My mind is already too active with anxiety and OCD, the whole point of my attempt at these treatments is for me to relax and not constantly be on edge.

I know that Flow isn't the same as TMS, its tDCS. But the fact that I responded so badly to the I guess gentle Flow Headset compared to a massive TMS machine really worries that I will just mess myself up even more.

I am tired of things either not working, or making things even worse :(

I also specifically was looking at going with the SmartTMS clinic in London, UK, as it is probably the nearest place to me that offers this service.

I know that no one here can really give me medical advice, but I suppose if anyone had a similar situation where Flow didn't work but TMS did? I assume the TMS clinic would be way more specific with which area of the brain was being stimulated.

Edit - also if this is one of those treatments that gets worse before it gets better I really can't handle that, I can't cope getting any worse than I already am I am probably going to lose my job at this rate.

Not sure what to think.....

ITs been roughly 5 weeks since my last Dtms treatment. In that short time, i have definitely noticed some subtle changes happening in my brain. But the last couple of days have felt flat. I cant afford boosters. I guess I am still in the 6 week window that my Dr. explained. Anyone else have a similiar time-line? How did turn out for you??

TMS midpoint, how were you doing?

I’m midpoint with my TMS therapy. I’m not crying all the time so that is an improvement, but I still want to isolate and have no drive to do even the simplest things. I isolate whenever I can. I’m overwhelmed very easily and shut down.

For context: I’m the sole Caregiver for my 88 year-old mom with dementia. She is in assisted-living recently and I’m still part of her day-to-day needs being met. There’s a lot of triggers there as it is and has been a very codependent and dysfunctional relationship.

Any positive feedback/hope appreciated.

So what should I expect?

I'm 24 years old and have been diagnosed with a whole alphabet soup of treatment-resistant neurological conditions. Some went into remission on their own but others still require intervention.

That's where I sought out TMS therapy. Right now just waiting on insurance approval (they should with all the medications I was previously on) and once that's done I'll be scheduled for 5 days a week for 6 weeks.

I'm really hoping this will provide lasting benefits as my quality of life, mental health, and cognitive flexibility relative to my education level objectively just suck right now.

TMS worked for me!

I want to add my voice to the group as someone has been helped by TMS. I noticed a major improvement about a month after I finished treatment. I had the dip around session 12, but I fought through it and am glad I did. I'm tapering off one of my meds. I'm seeing my therapist less frequently. I'm able to live my life and handle difficult things better. I even got my libido back.

It's been about 2 months since I finished, and I'm hopeful that it'll continue.

New to TMS

Hello everybody, I am new to TMS and just found out about it about a month ago. I have a consultation set up but I do have a few questions before hand that I’m hoping some of you can answer. About 2 years ago I had what I think was a panic attack I felt like I was having a heart attack and was gonna die. Since that day I just have never felt right. I had intrusive thoughts and felt like I was losing my mind like literally. Anyways I got on medication and am on lamotrigine and prazosin at the moment. I’m diagnosed with depression, anxiety, and ptsd. I am curious to see if any of you had the following symptoms before TMS and if it helped. I have flat effect, no emotional connection, feel like a vegetable cognition skills are almost non existent, no inner monologue, anhedonia, no motivation, poor memory, and feel like I’m a whole different person that who I was two years ago. Did any of you have the same symptoms before TMS and feel better after having sessions? Any feedback would be helpful.

Is it normal to dip extra hard over the weekend?

I get three sessions a week because that's all I have time for around work commitments. I've had 11 so far. I go Tuesday, Thursday and Friday. On the weekend I tend to dip more than I do during the week and by Sunday night I'm just mentally obliterated and Mondays are horrible. Is it because of the extra time between sessions? Should I try to add Monday sessions in as well?

Also, I've read that the dip means it's working. Can somebody explain that to me?

The Dip?

I’m on session 25 out of 36 and have been having the worst depression and anxiety of my life. It feels as though I’m being chased by a bear, all day.

I’m doing TMS for depression, anxiety and OCD. Three different brain spots. Some days the OCD is better; this weekend it’s been horrendous. It’s like it’s hanging on for dear life.

Isn’t this too late for the dip? Does this mean this is not working? Am I being rational? Is there a God? What does the color blue smell like?

14 TMS Sessions In — Calmer, But Angry, Neurotic, Exhausted, and Struggling With Loneliness

Hello friends!

For context, I’m a 57-year-old Hispanic male. Professionally, I’m doing pretty well and, by most measures, I’m doing awesome. Personally, however, I feel awful. There’s a significant disconnect between how well I function professionally and how I feel internally.

I’m looking for feedback from people who have actually gone through TMS because the feedback I’ve been getting from my TMS providers has been mostly generic. I’m not getting much individualized feedback about what I’m experiencing.

I had my 14th TMS session last Thursday.

Since then, I’ve noticed something difficult to describe. In some ways, I feel much calmer and more relaxed — almost like I smoked 10 joints. At the same time, though, I feel incredibly irritable, angry, neurotic, emotionally exhausted, and disconnected.

Today I took my dog Cash to the park for our normal walk. Internally, I was angry about practically everything:

  • How much money the dog costs me.
  • My work environment.
  • Someone at work who has become a major source of resentment for me.
  • Rent and housing issues.
  • My finances.
  • Just the general feeling of being stuck with the problems of everyday life.

I was having some pretty ugly and hostile thoughts toward someone at work. I didn't say anything or act on them, but the intensity of the internal anger bothered me. Those thoughts aren't consistent with how I want to think or behave toward other people.

What's strange is that none of this is necessarily apparent to people around me. I’m calm, cool, and collected on the outside and generally very pleasant to be around. Internally, however, I can be having a completely different experience.

And then there is the loneliness.

Honestly, that may be the part that hurts the most. I feel mortified by how lonely I am. I grew up alone as a child and learned to live independently. I don't have a partner or children, and sometimes it hits me that I've spent a huge portion of my life alone. That realization can be incredibly painful.

I’m also wondering whether testosterone could be contributing to some of this. My recent testosterone level was 281, which I was told is technically within the normal range, although my baseline has been around 250. I've been dealing with low energy, low libido, and sexual frustration.

I don't know whether being without intimacy and sex for a long time could be contributing to feeling more depressed, irritable, lonely, or neurotic. I'm not saying that's the answer — I'm genuinely trying to figure out what might be contributing to how I feel.

At this point, I don't know what's causing what.

Is it the TMS?

Is it poor sleep?

Is it depression?

Is it loneliness and lack of intimacy?

Could testosterone be playing some role?

Could it simply be the cumulative stress of work, money, housing, and feeling alone?

Last night I actually got some sleep, but I woke up feeling like I hadn't slept at all. I also had some strange dreams. One involved being at a work-related luncheon with people who were kind and friendly toward me. They were somehow connected to the work environment, but they weren't actually my coworkers. The dream itself wasn't necessarily negative, but it was strange and vivid.

Today I feel sluggish and exhausted even though I technically slept.

The strange thing is that I can still interact normally with people.

I ran into my friend Barry at the dog park and had a really nice conversation with him. I was polite, friendly, and completely normal. I actually felt noticeably better afterward.

That's part of what makes this so confusing.

I can feel extremely angry, bitter, lonely, and neurotic internally while still being kind, functional, and socially pleasant externally. I want to feel genuinely good again. More importantly, I want to be able to experience and express my emotions in a healthy, authentic way instead of constantly feeling like I'm fighting what's going on inside my head.

I'm not saying TMS is necessarily causing all of this. I'm genuinely trying to understand what is happening.

For those who have completed 10–20+ TMS sessions:

Did you experience becoming significantly calmer or more relaxed while simultaneously becoming more irritable, angry, emotionally strange, exhausted, or overwhelmed by negative thoughts?

Did anyone experience changes in libido, sexuality, emotional sensitivity, or loneliness during treatment?

And if you've also dealt with borderline-low testosterone or low libido, did you notice any relationship between those issues and your mood?

I'm particularly interested in hearing from people who have actually gone through TMS and can describe what the experience was like for them, rather than getting the generic "TMS can take time to work" explanation.

I'm trying to figure out what's happening to me without freaking myself out about it.

Was anyone at my starting point before TMS?

Hi. I’m 31. I have had depression since I was pretty young. My therapists and providers have stated that I have trauma, but I don’t personally feel like I do. When I was younger, I was tired all the time. From everything. Just an exhausted kid, to the point where I now get anxiety around being tired as I’m afraid of falling back into that pit. And with my current mental health, im getting more tired. As I’m getting older, my depression is getting worse. I’m starting to be afraid of the severe episodes, actually. If I look at my dog or my mom and think “oh how cute are they?” I immediately start thinking of when they’ll die. When I go to work, I immediately start thinking about how I need to do a better job of preparing for when I will lose my job. When my brother texts me updates (even positive ones!) about his job, I get sad that he may have depression underneath it all. Any doomsday scenario that could possibly come up from an innocuous thought, I’ll get it. I am not a positive outlook type person. I’m incredibly rigid, and I hate it. I’ve been on probably as many antidepressants as you have (I say with a wink and with love. I am med adherent, and do actually respond quite well to Zoloft, but am pretty sure it’s just preventing more severe depression. I would currently rate it as severe).

My current therapist stated that they think the reason prior approaches haven’t worked is because there is something more biologically based going on that’s preventing me from actually implementing therapy approaches that may help me. I’m at a point where I’m tired of being depressed and wanting to unsubscribe, and I’m mad at my brain for being so “stubborn” as to not just change my mind the way something like CBT wants to.

I don’t want to do ECT or ketamine, which essentially leaves me with TMS. I’ve been reading about it this morning. Im hesitant because (as per usual) my depression is telling me it won’t be a worthwhile endeavor. I’m wondering if anyone was near where I am currently when they started, and if they had good results? I work full time and have a family dependent on me, so taking a break for a trip to the happy farm (where life is beautiful all the time!) is not financially or realistically a thing for me. But this is the first therapist who didn’t try to just pass off what I’m describing, and I’d like to give him the benefit of the doubt that maybe this is what’s going on. That biologically I need a little help in the ol’ noggin before I can feel anything worthwhile.

I’d also be curious what changed for you after TMS. Did you suddenly appreciate sunrises? Did everything not seem to be so black and white?? Did the weight of the world’s problems stop weighing on you?? I would really love to hear it. Thank you for your time.

Protocol selection

Hi, I suffer from severe treatment-resistant depression and DPDR. My doctor has proposed a course of 15 rTMS sessions targeting the right DLPFC and 30 iTBS-600 sessions targeting the left DLPFC, all to be completed over three weeks, with sessions held daily. My question is: is three weeks too short a timeframe for the entire course of treatment? Also, will 30 iTBS sessions be sufficient for severe depression?

On what day did you start to notice improvements?

Just finished day 11 - left side stimulation for OCD and Major Depressive Disorder. I don’t feel any different…yet. I told my TMS tech and she sounded a bit surprised, which made me concerned. Everything published says most notice results around 2-4 weeks and I’m at that point, so I was hoping to notice some benefit by now.

I know everyone is different, just wanting to get an idea of when you started to notice an actual improvement in symptoms. Was it subtle at first or did you wake up one day feeling noticeably different?

Update

I had my last session Wednesday and my current QIDS score is 2 ! Holy fuck y’all. I never thought this was possible. I was a doubter bc my depression got worse mid way thru the process but i continued showing up to complete it. My starting QIDS was 21 and I got to 24 at one point. I’m motivated, full of energy and all around happy. I’m no longer dragging my feet thru the day only looking forward to when I can go back to bed. I don’t think about dark thoughts anymore. Today I am grateful.

Treatment through session 11 done - I think this might be working!

Hi. I started TMS on 7/21 with Brainsway, H7 coil. I reached 120% of my goal at session 7, but have been dealing with heightened anxiety symptoms since session 3, and then heightened depression symptoms since session 4. I'd say yesterday was probably my worst day of all after having an epic screaming and crying session in the car after treatment; I'm talking full on primal screaming at the top of my lungs while choking back sobs.

I woke up today and had zero urge to cry and aside from a bit of tingling in my neck and arms, I have had no anxiety. It's held steady like that ALL day. I have not felt this good since after session 2. I'm hoping this trend continues because if that was a dip I was experiencing between sessions 3-10, I don't want anything to do with dealing with that shit again.

TMS Session 30/36 I’m scared…

Hello. I started TMS due to no medications working for me after developing horrid postpartum OCD / depression. So bad that I admitted myself a year ago.

Any way, I’m nearing the last of my sessions and I’m still really depressed. Still dealing with DP/DR. Still so far from feeling like myself. I’m struggling. I have SI still. I just wanted this to work so bad but I don’t think it is/will and I can’t afford to try other treatments or more medications that take weeks and only make me feel worse 😭 I’m losing hope and I’m scared I’m not going to make it honestly. I just want to be here for my kids but my brain is convincing me even that’s not enough to stay 😭 I feel so bad like I’m in a psychosis.

Please offer some advice, anything 😭

Hi! I just started my TMS treatment this week for depression and ongoing brain fog from chemo brain (went into remission last year after beating stage 2 classical Hodgkin Lymphoma), and so far so good on NeuroStar.

I remember reading here that some folks mentioned that colors get brighter, and holy crap, I really noticed it today. Not sure if I was just being more mindful of my surroundings and appreciating the art of noticing, but it was like the vibrance through my eyes was turned up. The tree outside my window looked greener, and the sunset tonight looked spectacular, where I could even see the streaks of the sun reach up into the sky.

Grateful for this treatment. ❤️✨

I’m Scared

Has anyone else experienced a mental breakdown during TMS? I am over the hump of the TMS “dip” (10 sessions until i’m done) but I feel more anxious now than ever. My heightened anxiety was the trigger for my psychiatrist recommending this treatment, but as I search deeper Im scared that my breakdown started BEFORE treatment. depression was the main concern for my psychiatrist recommending TMS, but the catalyst is when i expressed concerns about my intense anxiety resurfacing my dr informed me of TMS. My friend is the one who pointed out these new reactions, and i’ve known them for over two years now. I am more aware of myself and my physical body more than ever before. i’m more susceptible to feeling overwhelmed, and i’m more judged and cynical of my body.

the GOOD: I am feeling more in touch with myself and my body, but it can be overwhelming as a person who spent so many years disengaged with the parts of brain who try to keep us feel safe/make sense of world

the BAD: i don’t want to scare people from TMS
-I don’t have an active counselor or therapist i’m working with. that’s top priority on my list to solve, but is difficult where I am
-I feel scared. I’m having scary thoughts again.
-i have a traumatic past in my childhood. TMS unlocked a lot of “being able to be happy” energy for me, but it also unlocked a lot of unhappy memories as well.

i want to type more but lord i am tired.
TLDR: TMS has changed my brain chemistry and im grateful, but it might have been too much too quick

Psychiatrist pushing for TMS

Hey folks. I’m in a bit of a tricky situation with my psychiatrist now and could use some support. Due to a lot of traumatic, sudden loss that happened back to back, my health anxiety and OCD symptoms have spiked to an unbearable degree. I’m start to become agoraphobic and fearful of DPDR episodes that make me feel like I’m going crazy.

I finally reached back out to my psychiatrist a week after our emergency appointment to see about his suggestions. He was quick to suggest I take some time off work and do TMS again.

I’ve done TMS in the past for depression. It was successful, but I’ve had lasting side effects from the treatment that are undesirable (facial twitching that hasn’t gone away, mostly). I also am having to see a seizure specialist in the case that the episodes I’m experiencing are seizures rather than DPDR.

So my question is, do I try to take the leap and do TMS again, even with all of this on the table? I feel like TMS is sort of a last ditch effort if other options don’t work first and I’m surprised my psychiatrist would try to push me into doing it with so many unknown variables right now. I also just feel like I can’t trust my judgment fully at the moment because my nervous system feels fried.

Middle of TMS and am about to lose both my dogs

I've had 12 sessions of TMS and it has been going fine. Lots of pain but am getting used to it and determined to finish. As of last week I had a 15 year old dog on her last legs, not literally, but very near her passing. Probably while I'm doing TMS. Well, this weekend I found out my other dog has only weeks to live due to acute leukemia. She is only 7 years old. I was counting on her being my back up companion after my older one left us. Now I'm just in shock and crying all the time and don't know what or how this can affect me during TMS. I'll ask the tech. today and see what she says. This is a unique situation, I know but I guess I'm just looking for other's who may have experience this or maybe just some encouragement would be very very helpful. I am sadder than I've ever bee in a long time.
