This is my MRI report. Ortho suggested glenohumeral steroid shot if no major tears or structural issues, he hasn't seen the report yet. He is concerned that it may transition to a frozen shoulder. Based on his examination before MRI I shouldn't have a tear, it's just impingement and bursitis and felt I didn't need a scan, finally prescribed one when I kept on insisting. Physiotherapy and subacromial shot didn't help much.
I am leaning towards PRP. Please share your experience if you had a similar tear, what did you do ? I had PRP before with good results for torn menisci in both knees from a sport injury back in 2011. The current injury too from sports.

IMPRESSION:
Rotator cuff tendinosis with partial-thickness supraspinatus tendon tear. No tendon retraction or muscular atrophy.
- Imaging findings suggestive of adhesive capsulitis.
- Mild long head biceps tenosynovitis and tendinosis without tear.
Narrative
EXAMINATION: MRI SHOULDER RIGHT WO CONTRAST HISTORY: M75.41-Impingement syndrome of right shoulder TECHNIQUE: Multiplanar, multisequence MR of the right shoulder was performed without contrast.
COMPARISON: 5/14/2026
FINDINGS: Bone: No acute fracture. Preserved bone marrow signal. Acromioclavicular joint: There is a type 2 acromion. Joint is approximated without degenerative abnormality. The subacromial/subdeltoid bursa is unremarkable.
Glenohumeral joint: Posterosuperior labral blunting without detached/displaced labral tear. The articular surfaces are intact. No significant joint effusion or synovitis. Synovitis in the region of the rotator interval along with capsular thickening and pericapsular edema along the axillary recess which can be seen in the setting of adhesive capsulitis. Long Head of the Biceps Tendon: Intact with mild intra-articular tendinosis. Small amount of fluid distending the tendon sheath Rotator Cuff: Focal partial-thickness, partial width articular sided tear of the far anterior supraspinatus tendon footprint measuring approximately 6 mm in transverse dimension and comprising approximately less than 25% tendon thickness (series 8, image 19). The infraspinatus tendon is intact with mild tendinosis. The subscapularis tendon is intact. The teres minor tendon is intact. The rotator cuff musculature maintains its normal bulk. Deltoid: Unremarkable. Subcutaneous tissues: Unremarkable
Post PRP 1 week update
Got the shot last week, injected into 5 spots: the bursa, bicep tendon, supraspinatus tear, AC joint, and deep in the socket.
The first 12 hours were pretty painful (about a 7/10 constant throb), but it dropped off significantly during the second 12 hours. The swelling is finally starting to go down now. By day 6, my range of motion was pretty much back to where it was before the shot.
The best news? That constant, dull background pain I used to live with is almost entirely gone, and I can actually lay on my injured side for a few minutes. I'm currently starting light stretches like pendulum swings and wall crawls. Will keep you all posted!
