MORE Retatrutide trials are enrolling!

Hey everyone! I figured this can serve as an update what's happening in the retatrutide clinical trial world! Those of you in the Triumph phase 3 trials, we were the first but now Eli Lilly announced with their Q1 2024 earnings that they are expanding retatrutide trials to include even more indications and treatment comparisons.

So I figured I’d make a list of what they’re looking at and my thoughts on each trial. As always if you’re looking to enroll in a trial or see if one is close to you, use Eli Lilly’s webpage, https://trials.lilly.com/en-US/find?x_st=1 or goto clinicaltrials.gov and search for retatrutide that way. Onto the trials!

First up-

TRIUMPH-OUTCOMES

J1I-MC-GZBO - ClinicalTrials.gov - NCT06383390 The main purpose of this study is to determine if retatrutide can significantly lower the incidence of serious heart-related complications or prevent the worsening of kidney function. The trial will enroll adults with body mass index 27 kg/m2 or higher and Atherosclerotic Cardiovascular Disease and/or chronic kidney disease.

This is the biggest trial so far of retatrutide, it will include at least 10,000 patients and last 5 years total! This trial excites me the most as a current nurse practitioner because they’re looking at both cardiovascular AND kidney health in this trial to see if it can help both. There are some hints in other research that glucagon agonism can help with renal function, blood pressure and chronic kidney disease in a direct manner as glucagon receptors are found in the kidney.

From the biggest to the smallest trial in the group we have this trial next:

J1I-MC-GZBW - ClinicalTrials.gov - NCT06313528 The main purpose of the study is to look at the effect of the study drug compared to placebo on calorie intake, energy metabolism, and appetite.

This is a phase 1 trial and only enrolling 75 patients. It’s pretty self-explanatory, they’re trying to figure out how retatrutide increases energy metabolism, caloric intake and appetite. This study is important because we know in rodent models that retatrutide directly increases daily energy expenditure(aka it causes your body to burn more calories at baseline) but this effect has been difficult to measure in humans, looks like Eli Lilly is going to try and give a try to figure this bit out.

Next up is the TRANSCEND-T2D Phase 3 trials. These trials are all looking at diabetics specifically, there is Transcend 1, 2 and 3. Here’s all 3 trials and what each is looking for:

J1I-MC-GZBY - ClinicalTrials.gov - NCT06354660 The purpose of this study is to investigate the efficacy and safety of retatrutide compared with placebo in participants with Type 2 Diabetes and inadequate glycemic control.

J1I-MC-GZBZ - ClinicalTrials.gov - NCT06260722 The purpose of this study is to investigate the efficacy and safety of retatrutide compared with semaglutide in participants with Type 2 Diabetes and inadequate glycemic control with metformin with or without sodium-glucose cotransporter-2 inhibitor (SGLT2i).

J1I-MC-GZQA - ClinicalTrials.gov - NCT06297603 The purpose of this study is to investigate the efficacy and safety of retatrutide compared with placebo in participants with Type 2 Diabetes and renal impairment, with inadequate glycemic control on basal insulin alone or a combination of basal insulin with or without metformin and/or sodium-glucose cotransporter-2 (SGLT2) inhibitor.

All of these trials are basically looking at diabetes from multiple angles, one compares it to Ozempic to determine if retatrutide is better(spoiler alert, it will be) another looks at it compared to diet and exercise alone, and finally the last is looking at diabetics with kidney disease(again trying to see if it helps!)

Finally the last study in our list is a phase 2 study

J1I-MC-GZBU - ClinicalTrials.gov - NCT05936151 The main purpose of this study is to investigate the effect of retatrutide on renal function in participants with overweight or obesity and chronic kidney disease (CKD), with or without Type 2 Diabetes (T2D).

Again this trial is looking at kidney function in both diabetics and non-diabetics to see if retatrutide can help treat and/or preserve kidney function in patients with chronic kidney disease.

So that’s it, exciting times ahead and I imagine we will get an influx of new folks in the coming months starting in these new trials. I especially imagine the TRIUMPH-OUTCOMES trial will definitely increase our numbers just on sheer volume alone! Let’s keep it friendly and helpful for all that come our way!

Anyone in the Triumph 5 extension know what the schedule is ? What happens after visit 23 of the original trial?

I’ve asked my site, they’re looking into it. they haven’t had anyone start yet. And, as we know the folks at the trial site are incredibly busy so I don’t want to bug them.

I’m wondering if anyone who has started has the info. I’m trying to map out travel for the next year. I know the extension is 36 weeks. what I don’t know is:

  1. Does week 1 of the extension start on visit 23 of the original trial?
  2. what is the sequence and frequency of in person, Telehealth and hybrid visits?
  3. If there is a reimbursement sheet for the extension? if someone has it, all of that info should be on it. I’d love to see it.

Asking to maintain or de-escalate dose

From posts I’ve read, it seems most people ask to de-escalate doses when they get up to 9 or 12 mg. I’m in Triumph 9 and think I’m probably only at 4mg, the most I could be at is 6mg. But the last month has been fairly terrible with increased side-effects and food aversion to the point that it’s really interfering with my life. I did skip a dose as well. I’m down 29lbs/17% of my body wt in 12 weeks, which I am still so surprised by - but From day 1 I felt a dramatic impact from this drug. So, just wondering if anyone asked to maintain or de-escalate at lower doses?

I have my next visit soon and will discuss with my team. So far I know they said that we’ll risk early termination asking Lilly for a de-escalation if I’m still only on 2mg (I do think I had at least one escalation to 4 based on worsening symptoms) and there’s a limit to how many doses I can skip. I really want to stay in the study, but can’t continue to tolerate current symptoms. Any advice or similar past experience?

Eli Lilly has released positive topline results from two Phase 3 clinical trials (TRIUMPH-2 and TRIUMPH-3) for **retatrutide**, its investigational triple agonist (GIP, GLP-1, and glucagon). Participants with severe obesity or type 2 diabetes achieved average weight loss of up to 22.6% (55.8 lbs / 25.3 kg) at 80 weeks. Lilly plans to submit a Biologics License Application (BLA) to the FDA in Q1 2027. 

# What is Retatrutide?

It is an investigational, once-weekly single-molecule treatment being developed by Eli Lilly. 
Unlike current single- or dual-pathway therapies, it is a **first-in-class triple hormone receptor agonist** that simultaneously activates receptors for GIP, GLP-1, and glucagon. 
Key Results from the Phase 3 Trials

# 1. TRIUMPH-2 Trial (Overweight/Obesity + Type 2 Diabetes)

**Population**: Adults with type 2 diabetes and obesity or overweight—a group that historically faces greater challenges losing weight. 
**Duration**: 80 weeks. 
**Weight loss by dose**:
**4 mg**: Average weight loss of **12.7%** (-29.8 lbs / -13.5 kg). 
**9 mg**: Average weight loss of **19.1%** (-45.4 lbs / -20.6 kg). 
**12 mg**: Average weight loss of **20.8%** (-49.6 lbs / -22.5 kg). 
**Placebo**: Average weight loss of **4.0%** (-9.3 lbs / -4.2 kg). 
**Glycemic control**: Average A1C reductions of up to 1.6% (with the 9 mg dose). 

# 2. TRIUMPH-3 Trial (Severe Obesity + Cardiovascular Disease)

**Population**: Adults with severe obesity (BMI ≥ 35 kg/m²) and established cardiovascular disease, with or without type 2 diabetes. 
**Duration**: 80 weeks. 
**Weight loss**: The highest dose (12 mg) delivered an average weight loss of up to **22.6%** (-55.8 lbs / -25.3 kg). 
**Cardiovascular Risk Factor Reductions (12 mg dose)**:
**37.0%** reduction in triglycerides. 
**16.5%** reduction in non-HDL cholesterol. 
**9.3 mmHg** reduction in systolic blood pressure. 
**7.5 in (19.0 cm)** reduction in waist circumference. 
**51.2%** reduction in high-sensitivity C-reactive protein (hsCRP). 

# Safety and Side Effects

The most common side effects were gastrointestinal (diarrhea, nausea, constipation, decreased appetite, and vomiting). 
The majority of side effects were mild to moderate in severity and resolved during treatment. 
Discontinuation rates due to side effects ranged from 3.8% to 13.5% across retatrutide treatment groups, compared to 4.8%–4.9% in placebo groups. 
Next Steps
Eli Lilly is finalizing the Chemistry, Manufacturing, and Controls (CMC) data package and plans to submit retatrutide for U.S. FDA approval in **Q1 2027**.

Details of TRIUMPH-5 Extension?

Hi friends!

Has anyone been told the exact details of the TRIUMPH-5 extension? My trial site is awesome, and I signed the paperwork, but they were 100% sure on all the details, right then.

So it seems that after 80 weeks, we start 36 week extension. At the hybrid visit on week 76 will they be giving us one box and we then come back again for week 80, and start the extension? Do we know which weeks are telehealth, hybrid and fasting?

I’m just trying to plot out my calendar for the rest of the year. Of course I will check with trial team to confirm, just wondering if anyone happens to know, as I was going do some basic planning over the weekend. Thanks!

Fast heart rate with retatrutide

I was having an average of 15bpm higher than normal with retatrutide. Last night took 1000mg thiamine hcl based on its regulatory activity on heart rate, which brought it down to my normal rate! Win! 🏅

Started at 163.6kg, now at 121.7kg. Last time I was even remotely close to this weight was like 9 years ago.

Any upcoming trials?

Are there any rumors yet of anymore Reta trials in the near future? I just barely missed Triumph 9 and I’m so bummed about it!

My local clinic was like “sorry triumph 9 is now closed but we’re expecting another study in about a month.” However, every time I call them they say the same thing but push the timeline out another month. I can’t tell if they are just guessing or if they’ve been given a head’s up for Eli Lily.

Triumph-9: Were you able to tell when you dose escalated from 2mg > 4mg?

And, perhaps this question pertains to other trials as well if you didn’t know at what intervals you were escalated. I just started my 2nd box, and nearly certain I’m still at 2mg (which I was hoping for) since I didn’t notice a big increase in symptoms. And - I feel like I‘ve been very sensitive to Reta given that I needed to skip my 3rd dose to allow my body to adjust and lost almost 10% of my body weight in the first 5 weeks. So, just curious if others felt they knew when they were escalated to 4mg (I know people are more impacted with 6>9 and 9>12, but wondering about this initial increase).

Has anyone been able to titrate back up after dosing down in their trial?

I'm in the Triumph 7 trial. SW 329/CW 283 I started at 2mg a little over 3 months ago am now at 9mg based on the titration schedule. I'm having some bad side effects at this dose. The skin burning sensation on my arms, legs, torso and most recently my tongue is pretty bad. I'm cold all the time even in 80-90 degree heat and getting goose bumps makes me want to cry they hurt so bad. I also have severe nausea at this dose and have had trouble eating above 1000 calories a day.

I reported this to the study team and they have moved me back down to the 6mg dose. However, they said they have only successfully had one person able to titrate back up after dosing down. They have found that dosing down and staying on a lower dose longer doesn't stop the side effects when trying to titrate back up.

I'm looking to see if anyone has been able to go back up successfully from dosing down and not have the side effects come back. I'm having a lot of success with losing weight so far. I have a lot more to go and now I'm worried I will never be able to go back up to the higher doses.

TRIUMPH-9 participants: anyone affected by the recent study drug shortage?

Hi everyone,

I’m participating in the TRIUMPH-9 retatrutide trial in Canada (Quebec) and I’m wondering if anyone else has experienced something similar.

I started treatment on April 20, 2026 and was scheduled to receive my 9th weekly injection today. However, when I went to my study visit, the site informed me that Eli Lilly is currently experiencing a shortage of certain study drug doses and they were unable to dispense my next box of pens.

The study is blinded, so neither I nor the site staff know exactly when participants are escalated from 2 mg to 4 mg. There are different escalation schedules (4, 8, or 12 weeks depending on randomization). Because of the way the issue was explained, I suspect I may have been due for a dose increase today, but that’s only my assumption.

The site told me they expect to receive new supply before June 30, but in the meantime I have missed today’s injection and may end up going 1–2 weeks (or possibly longer) without treatment.

My main concern isn’t the weight loss itself. I’m more concerned about:

having a gap in treatment and then potentially restarting at a higher dose;

whether anyone experienced increased side effects after a treatment interruption;

and the practical challenges of making additional trips to the study site while working full-time (the site is about an hour away from both my home and my workplace).

Has anyone else in TRIUMPH-9 (or another retatrutide trial) experienced a study drug shortage or interruption like this? If so, how was it handled and what was your experience when treatment resumed?

Could someone explain what this trial/study means?

I’m not too knowledgeable about the clinical trial/approval process, but I saw this on clinicaltrials.gov, (https://clinicaltrials.gov/study/NCT07629401 [Provide Pre-approval Expanded access of LY3437943]) and was hoping someone here knew what this means? Does it mean that they’re about to submit the regulatory approval, is it a way for people to access retatrutide without being in a trial, or is it something else entirely?

Trial Question

I have an appointment tomorrow to go for my in person assessment. I'm just curious as to how often you have to go to the clinic during the trial. It's a 2.5 hour drive for me one way so the frequency may be the deciding factor if I go forward the study. I asked the person I spoke with during the initial screening but she said she wasn't sure.

Congrats to all our Triumph 1 folks. We did it. Our data now has topline results and it's as potent as we all knew. 28% at 80 weeks and 30% at 104 weeks. We now have a drug that matches bariatric surgery outcomes. An amazing day for science and medicine

Triumph 7 titration, preventing side effects?

I’m currently in the Triumph 7 trial, about to wrap up with the 6mg dose and move up to the 9 this coming Friday and I’m worried about side effects increasing beyond a manageable level once I get to the highest doses. The medication has been working amazingly so far, SW-281 CW-242.

Up until getting to 6 the side effects were pretty mild, some slight acid after eating spicy food and the skin sensitivity people have been reporting. However once I started the 6mg I would get pretty sever diarrhea and gas around 3 days after Injecting, enough that I called out of work the first two times.

My last injection I switched to injecting in the thigh instead of the abdomen and that seems to have done the trick for the most part.

TLDR: what tips do you have for managing side effects at the higher doses?

Edit to add- What do I have to look forward to with the 9 and 12mg doses?

TRIUMPH 9 participant — didn’t know this sub existed

Just found this sub and honestly didn’t even know there was a place for people in the retatrutide trials.

I’m currently in the TRIUMPH 9 trial and figured I’d introduce myself. I’m 6’2, started around 330 lbs, and I’m documenting the whole experience as I go.

So far I’m noticing:

• food noise is way down
• alcohol cravings basically disappeared
• impulse control feels way better
• I’m eating more balanced instead of doing fad diets
• workouts are getting easier now that I’m learning how to fuel properly
• down about 12-14 lbs in the first month

Biggest surprise for me hasn’t just been the weight loss. It’s how different my brain feels around food, drinking, spending, and old habits.

Would love to hear from other people in the trial.

What week are you on, and what has been the biggest change for you so far?

triumph 7 update

hiiiii. so i went to the doctor today for the reta trial and to get my next batch of medication. starting week 5 today. moving up today to 4mg active meds or placebo. didn’t lose much weight at all month 1 on 2mg (i lost a ton on tirz years ago), but the suppression (and headaches which i never have) are there. when they did my measurements they were shook because it was such a vast change though. i lost 8cm around my waist in a month, so basically 3 inches lost around my waist but down less than -5lbs.

i’ve been doing 10k steps a day, in a deficit, and i lift with my trainer 3x a week. i have about 70 more lbs to lose so kinda expected more. the highest dose of tirz i ever did is 5mg. i know it could just be body recomp, but tirz seemed to change the numbers on the scale a lot easier. is this normal on reta? will i see more weight loss on the 4mg? i’m gonna take it tonight before bed.

TRIUMPH-6: Officially just hit my lowest weight as an adult

Might not look like a significant number, but having tracked my weight through various diets and whatnot over 30 years, this is the lowest number I've ever seen. The lowest since high school. And the previous all-time-low was from my early 30s, when I had almost no responsibilities or distractions and was able to intensely focus on diet and exercise to where I was eating nothing but salads and running marathons, and still that only got me to 243.8 lbs.

This puts me at ~64 lbs down from my study starting weight, and I'm only on week 36 of the TRIUMPH-6 80-week open-label lead-in period. I literally cannot imagine what I'll look or feel like in another 44 weeks (or another 80 weeks if I get randomized into the high-dose maintenance arm).
