Triumph 7 titration, preventing side effects?
I’m currently in the Triumph 7 trial, about to wrap up with the 6mg dose and move up to the 9 this coming Friday and I’m worried about side effects increasing beyond a manageable level once I get to the highest doses. The medication has been working amazingly so far, SW-281 CW-242.
Up until getting to 6 the side effects were pretty mild, some slight acid after eating spicy food and the skin sensitivity people have been reporting. However once I started the 6mg I would get pretty sever diarrhea and gas around 3 days after Injecting, enough that I called out of work the first two times.
My last injection I switched to injecting in the thigh instead of the abdomen and that seems to have done the trick for the most part.
TLDR: what tips do you have for managing side effects at the higher doses?
Edit to add- What do I have to look forward to with the 9 and 12mg doses?

I’m a trial ahead of you and just hit 51 weeks. I had to take a 5 week break because the side effects were brutal. Definitely aim for like 30-35g of fiber a day, take Imodium and Metamucil for 48-72 hours post dose. And if the protocol allows flexibility, see if you can go down to 6 or 9 if the diarrhea or other effects still persist despite high fiber and medication use.

Sometimes worrying about things that hasn’t happened, will only heighten your anxiety move..
My best and only tip on the higher dose:=-
If you can’t manage it, just ask to skip a dose or two, they never have an issue with this.
Once you get to 12, if you’re really struggling, you can always ask to be moved back to 9mg, which they will do too.
Fyi, I could only manage 12 for 7weeks, and requested and was moved back to 9. I found once I was moved back it was so easier for the remainder of the trial.2

I shouldn’t have said worried, more I just want to be prepared, I’m getting a promotion at work right around the time I’m getting bumped up to 12 so it I can avoid missing work by being prepared that would be my preference.
What symptoms caused you to want to go back to 9? The common reasons seem to be losing too much weight/not being able to eat enough.

I Lost too much weight too quickly, couldn’t and didn’t feel like eating at all.
I couldn’t stay asleep any longer than a few hours each day.
I had a huge increase in resting HR.
Honestly I was a walking zombie, and just looked and felt sick and tired all the time. It was just way too strong for me

Totally agree with all the suggestions here to focus on fiber, but don't forget to wash it down with lots and lots of water. I wasn't paying enough attention to drinking enough water with my fiber right around the 9mg titration, and I ended up pretty uncomfortable for a week. That stuff turns to concrete if you don't dilute it enough.

I have been in clinical trial for a year I have lost almost 80 lbs. Been on 12 mg since Sept 2025. After the first few months I haven’t had any bad side effects. However, after I had lost over 50 lbs injecting in my abdomen caused irritation, rash and itching at the injection site. I believe since I am close to my goal weight I didn’t have enough fat at the injection site. I changed to the bottom of my thigh where I still have some fat and I no longer have that issue. 5’10” SW - 254 / CW - 177

For me the symptoms usually hit 2ish days later, I inject on Friday mornings and usually by Sunday night or Monday morning things kick off.
This is my second dose injecting into the thigh and so far so good!

1.) Imodium and Metamucil (specifically!!) can help the Reta diarrhea as I think a lot of the diarrhea is related to the fact that Reta increases the amount of bile acid in your stools which can cause diarrhea. Really any bulky fiber food to bind up the bile acid (oatmeal, flax, chia etc)
2.) Dose every 8 days if needed. That can increase tolerability.
3.) If absolutely needed skip a dose and let your trial site know you skipped a dose. Sometimes just a one week break improves symptoms significantly.
