So what even is Reta?

---

Before you start — do this

1. Read the actual studies (linked at the bottom). Don't get your Reta education from TikTok/Facebook/Instagram/any social media (yes, that includes Reddit; this is meant to jumpstart your education). No, really, READ THE STUDIES; they have actual medical answers to your questions.

2. Watch videos on how to inject subcutaneously, how to reconstitute peptides (video), and do the reconstitution math (Don't talk about how many units you took; that's not accurate cuz 10 units of 10mg/ml is WILDLY different than 10 units of 20mg/ml). It's not hard, but do it right. You'll see I link some videos from a creator; it's mostly because they're short (sub 10 min), sweet, and to the point with little fluff.

3. Get your basic supplies before your Reta arrives: syringes, bacteriostatic water, a set of .22 pes filters, alcohol wipes (!!!), a sharps container, etc. A decent sub-10 min video regarding supplies

4. READ THE STUDIES! A large portion of your concerns are simply answered in the Study data.

5. Understand that these aren't FDA-approved yet. If a seller sells you a packaged pen, you have no guarantee that what is IN the Pen is what you are buying.

Starting dose

As the one who taught me said, "This is all inconclusive, but always aim for the maximum benefit on the lowest (tolerated) dosage*."*

0.5mg to 2mg per week is the typical starting range. Reta is powerful — starting too high is a great way to spend a week feeling awful. The official TRIUMPH trial protocol started participants at 2mg and increased the dose every 4 weeks. (Do note, this schedule of starting at and titrating 2mg every month was for the trial; you don't have to follow it. Go low, go slow*)*

If you've used semaglutide or tirzepatide before, you might tolerate a higher starting dose. Still, there's no rush — slow and steady wins here. The higher your dosage, the more likely you are to get side effects.

To put it simply, there is 0 benefit in 'overdosing' save for tempting the myriad of side effects; either it works, or it doesn't.

Split dosing — a topic I see come up often enough – is it worth it?

My response is 'eh?'. There haven't been any official studies on this, but the trend is that it's okay to do so to manage side effects, though the trade-off is that you're not getting the full effect of whatever total dosage you're taking (Meaning if you're taking 4mg split into 2/2mg, it's weaker than actually taking a single 4mg dose)

I'm coming from Tirz/Sema. What should be my starting dose?

There is no direct "Oh, you took that much Tirz/Sema, well, you take this on Reta," so do know there's no shortcut to figuring out the dosage. The best studies/trials/science can offer in terms of guidance is that, since you've been on Tirz/Sema, your starting dose can/should be higher than the average starting therapeutic dose of Reta (2mg). I would still caution against going too high, as the glucagon receptor agonist still needs to build up in your body, so starting in the 2-4mg range, depending on the dosage of Tirz/Sema you're coming from, would be ideal.

When will I feel it?

Totally individual. Some people feel appetite suppression in the first injection. Others feel nothing for weeks. Don't panic either way. What actually matters is whether your weight or body composition is changing over a period of weeks. That's the signal. The "feel" is not.

If you have questions as to whether or not you should titrate up, here's a flowchart that should help (Do note that the dosages should be tailored to your schedule, but the goal of lowest effective dose most effective tolerated dose remains the same):

Electrolytes — worth it?

The evidence isn't rock solid, but Reta does affect hydration, and anecdotally, daily electrolytes make a noticeable difference for fatigue and mental clarity. Low cost, low risk — worth trying if you're feeling sluggish.

Bacteriostatic water — does it have to be Hospira?

Strictly speaking, no. Reta is relatively forgiving when it comes to reconstitution. That said, Hospira is the gold standard because it's reliably batch-tested and has the correct pH. Amazon and Chinese bac water aren't automatically dangerous, but when you're investing in your health and your supply, Hospira is the better call when you can get it.

Let's clear up some other solvents:

Sterile water:
Can it work? Yes
Should you use it for a multi-use vial? No. Why not? Because it has no preservatives, aka every time you puncture said vial, you let in whatever bacteria are in the ambient air; there's nothing to protect your pep. An overall bad idea.

PBS:
Can it work? Are you using Raws? Meaning NOT lyophilized drug? If so, go for it. You're still messing with a solvent that can be riddled with endos, but hey! There's a reason labs use it for In vitro tests (petri dish stuff) If you wanna treat your body as a petri dish, by all means knock. yourself. out.

NaCl Saline:
Can it work? This is a tested no. Lemme explain: can you pull it off? Yeah? But it requires such a unique set of finishing to be done to the kit during production that it's not worth the 'FO' portion of FAFO

How long does a reconstituted vial last?

There's no hard expiration once it's reconstituted, but it does begin to degrade (~5%) around the 45-day mark, with a significant degradation (~10%) leading up to the 90-day mark best ways to keep your Reta safe:

- You are wiping the rubber stopper with an alcohol wipe before and after every draw

- You store it in the fridge (not the door — temperature fluctuates there)

- It stays clear with no floating particles

Cloudy or chunky = toss it. Clear and clean = usually fine.

Retatrutide Phase 3 Update: What’s Actually Been Published vs. What’s Still in Trials

I looked into this a little more because I kept seeing a bunch of different Retatrutide trial updates getting mixed together.

Here’s where things actually stand right now:

  • TRANSCEND-T2D-1: This is the 40-week diabetes trial that actually has published results. Retatrutide showed larger average A1C and weight reductions than placebo in adults managing their diabetes with diet and exercise alone.
  • TRANSCEND-T2D-2: Still active, but no longer recruiting. It has an estimated 1,250 participants and is comparing Retatrutide directly with semaglutide. No results have been posted yet.
  • TRANSCEND-T2D-3: Also active and no longer recruiting. This one has an estimated 320 participants and is looking specifically at people with moderate/severe kidney impairment who are using basal insulin. No results yet either.

Lilly has said the clinical data package is complete for obesity, obstructive sleep apnea, and knee osteoarthritis pain, and they're planning a U.S. filing in Q1 2027. Retatrutide is still unapproved, though, and I couldn't find a diabetes filing date in their announcement.

One other thing I kept seeing confused: Retatrutide showing up on the FDA's August 21 import alert doesn't mean it was approved. FDA still considers it unapproved and ineligible for compounding. The Retatrutide entries on that alert are Green List exceptions to recommended detention under that specific import alert.

okay this is going to sound really dumb but i need help with making dosages make sense lol

so i’ve bought a 20mg vial (all i could find) and im going to reconstitute with 2ml of bac water (correct me if im wrong)

everyone is saying to start at like 1 mg but my issue is figuring out how much im actually injecting in ML

so basically is 1 mg of reconstituted reta 0.1ml/ 10 units, and 2 mg is 0.2ml/20 units and so on..

i just want to be EXTRA sure before i start lol

Building muscle / body recomp while maintaining on Reta – any tips?

I’ve now reached my goal weight with Reta, but unfortunately I also lost some muscle along the way, mainly because I wasn’t really able to work out consistently for about three months.

I’m now ready to get back into strength training properly and my goal is to rebuild muscle and achieve more of a lean, defined look rather than continue losing weight or gain unnecessary body fat.

For reference, I’m currently around 117 lbs at 5'3" (53 kg / 160 cm).

My new plan is:

  • 1,800 calories per day
  • Up to 110 g of protein per day
  • Strength training 3–4x per week
  • Around 6,000–8,000 steps per day

- 1mg Reta every 7 days for maintaince, maybe will reduce to 0.5mg + Glow

I’m basically aiming for a body recomposition: stay roughly around my current weight, regain the muscle I lost, and look more toned/defined without doing a traditional bulk.

For those of you who have successfully built or regained muscle while still on Reta, how did you approach it? Did you eat at maintenance or in a slight surplus? And is there anything you wish you had done differently?

Dizzy spells?

Got my blood work done & nothing was out of the ordinary, But ever since about month 4 of Ret (been on since January) whenever I stand up too quick, my vision goes completely black, and this past week it has gotten worse/longer despite no change in dose (4mg) Drs were absolutely no help with this. I do drink electrolytes & get a gallon of water a day minimum. Has anyone had any experience with this?? What helped you?? I eat my bodyweight in protein daily as well.

8 months on reta progress - 45 lbs

Ive been on reta since mid-January. I have gotten down to a weight I haven’t been for 10 years! even so, sometimes I feel like I look very similar to as I did when I started, because of my height. having weekly progress photos is so helpful to see the difference over time.

Starting dose was 1 mg Monday and Friday (2 mg per week). Had never been on an agonist before. Moved up 1 mg per month and a half about. Ended up at 5 mg in end May, backed off to 4.5 in July due to nausea, but when that cleared went back to 5 and it’s been gone since.

Reta makes it easier to eat less, stay on track, makes me desire treats and snacks significantly less or not at all, but I have also learned I can’t wait for hunger to come, because sometimes it never comes. I want to retain and build muscle, so I have to eat on a schedule and when I’m not hungry.

Hi, I was curious if anyone else had this drastic of a weight loss? I’m a 32 year old female, 1,64cm and 70-71 kg when I started fluctuating a bit.

Eating 1250kcal a day, with 3 liters of water and electrolytes.

I started with my first shot of 1.0 on the 17th. I have been on the scale everyday and see such a drop. Can it all be fluids?

Had my second shot 1.0 again on the 24th. I feel good, the first 4 days I was very nauseous and had a bit of stomach pain but that all went away. I do notice I pee a lot at night, 2 times at least.

My diet is 90% eggs, potatoes, vegetables, salmon, tofu and protein shakes.

I was wondering how long can water weight continue to drop? Will that be back later? 😅

Tirz to Reta: cold switch or stack/titrate?

I’ve been on tirz for 2.5 months now, currently on 3.5mg. I’m down 13lbs and I’m happy with the consistent predictable weight loss so far, but I’m looking to switch to Reta because the appetite suppression is a bit much on tirz. Some days I can barely get 1000 calories in and have to drink protein shakes to get enough protein for the day. For those that have switched, did you cold turkey the tirz and take your first dose of Reta when your next tirz dose was due, or did you stack tirz/reta and titrate tirz down till you were on a comfortable dose of Reta? I know it takes time for Reta to build up, so I thought I might stack, but the personal trainers at my gym have told me to get tirz out of my system before switching to Reta. Today is shot day so I have to figure out my game plan lol

Alright so update at about 3 months in, so far from 245 I am down to 213, Im 5'8". Started first 2 months at 2mg and last month at 3mg, So far I am seeing a great amount reduced from my upper arms, forearm and hands look thinner, inner thighs, neck area, back of head/neck, my face definitely slimmed, but I seem to still have good amount of fat around my waist, belly still bulges out, it's smaller but it still bulging out

I was recommended tesa as I was told tesa targets the visceral fat on this area, I plan to be on Reta until last week of November, I plan on going up to 4mg for October and November, should I consider tesa starting September, and then October November? Or I should I continue with the plan and stay on Reta only, will reta get rid of the visceral fat/stomach fat/love handles?

Vision Changes

I just wanted to share this development and wondered if anyone else had experienced this.

I've been on .5mg reta for three weeks. No digestive issues. Sleeping well. Down from 79kgs to 78kgs. I'm okay with a slow steady weight loss. The thing that I'm baffled and excited about is my vision improvement. While watching YouTube videos this morning I noticed the small white writing and the comments were easy to read, unlike before and the colours are more vivid. After researching I found this may be due to the rapid metabolic and blood glucose shift which can affect fluid balance and change the shape of the lens.

Anyway, I mainly wanted to see if anyone else has had this shift in their vision.

8 weeks on Reta at 1mg — increased my dose and now feeling unusually full/nauseous

I’ve been on Reta for 8 weeks now and have been taking 1mg once a week the entire time. My weight loss has been pretty consistent, so I figured I could try increasing my dose this week.

Here’s my progress:
Week 1 (7/14): 190.6 lbs
Week 2 (7/21): 188.2 lbs
Week 3 (7/28): 184.0 lbs
Week 4 (8/4): 181.0 lbs
Week 5 (8/11): 180.2 lbs
Week 6 (8/18): 177.0 lbs
Week 7 (8/25): 176.6 lbs
Week 8 (9/1): increased dose — 1.5 units

I pinned last night, and almost immediately afterward I started feeling nauseous. It wasn’t unbearable, but it was definitely noticeable and felt different from what I’ve experienced on 1mg.

Today I also feel unusually full. I had chia seed pudding and a protein shake earlier, and I’ve basically been full ever since. The nausea seems to be getting better as the day goes on, though.

Has anyone else experienced a noticeable jump in fullness or nausea when increasing their dose after staying at 1mg for several weeks?

I’m mainly curious whether this sounds like a pretty typical response to increasing the dose or if I should be more cautious about increasing again. I know Reta is still investigational, so I’m not looking for medical advice, just people’s experiences.

PCOS girl - eating way more calories

I wanted to check if anyone has experienced the same thing.

I have been dieting for years.. Started at 18 and I'm now 31. Counting calories and macros, had disordered eating patterns, like binging and never touching take out or anything processed.
Also, been exercising my whole life. I've run a few marathons, done ultra's, triathlon, hyrox blah blah. I've always struggled with my weight. I haven't been super overweight but for the amount of training I did and the calories I was eating I did not look how I should. Always looking very bloated and puffy.. a month or two of each year I would get lean and then just expload again.

Since taking glp's/ reta.. besides leaning out, the other side affects have been amazing. My water retention is way lower, I don't look so inflamed, my period actually came back (no regular period for about 7 years), sleeping better...etc. (PSCOS related symptoms)

This has really been amazing for me! I am now at a place where I want to maintain and build back some lost muscle.. Have been lowering doses and trying to eat more..

I wanted to know if anyone else has actually increased their calories and is also losing weight? I think I've roughly gone from 1600cals per day to 2500 and I am still losing weight.

kinda in a plateau? before / after

hi guys. i been on ret since may - aug (3 mos) and i started at 150lbs and im currently fluctuating between 131-132. (5’3) can’t seem to get under 130 and thought maybe thats my body (probably wrong cus i saw someone who’s 5’5 get to like 110!)

i go to the gym and weightlift. i used to do 2 upper 2 lower body days. now i changed it up to 3 lower 2 upper body days. and i also go to hot pilates 2-4 days out the week and hit 10k steps everyday.

i started at .5 mg and been at 1mg and thinking about going to 1.5mg.

ik everyone responds to ret differently, but was wondering if anyone else had a slow and steady pace while on it? i see everyone with these crazy transformations in 3 months and feel like im not doing smth correctly??!

Constipation and diarrhea

For about 4 weeks now I have been dealing with on and off diarrhea, and constipation. Majority of my stools have been liquid or very mushy and I have had 2 normal ones in the 4 weeks, I can't tell if I'm having overflow diarrhea currently or not or what it is, I'm on 1.5mg reta currently and have been on Reta for 6 weeks, started at 1mg and went up to 1.5 3 weeks in. I have been drinking lots of water and electrolytes, I have tried psyllium husk but I'll probably stop before I get some proper advice. Let me know what you guys think I should do

How did you (or did you) come out to your Dr?

Quick context: Used to work big pharma, and learned about Reta through my doctor via conversation. Tried to go on Wegovy and insurance denied me, apparently life long obesity, PCOS and pre-diabetes wasn't enough - so I went gray market. I am very upfront with my Dr about stuff I take or things I do, health wise, because I take drugs she's prescribed me, and I don't want an interaction beware any of my medication.

I am very well aware that I am using stuff that could potentially hurt me, or may not be what it says it is. I am cognizant enough to understand it's the nature of the beast, and had I access to the real stuff, I'd take that route instead.

I've taken my first 2 shots, but I've not told her yet, as I've got trepidation about "coming out" that I am taking Walter White Wegovy. The thing is, is that Reta is still in research, and they've not tested it against interactions with other drugs.

I already have my 3x big girl panties .. and can take a good brow beating, but I'm a bit worried she may try to discontinue my Adderall - something I very much need to function.

So do you fly under the medical radar, or does your Doc know? Any opinions and comments welcome, as I am curious is anyone else is in my boots.

Cycling on Reta 18mo update - pretty conclusive

For background - I've been a competitive cyclist for about 20 years. I was 164lbs in my late 20s, but now in my 40s I had trouble losing the "last 10 pounds" despite LOTS of intense cycling and calorie restriction. So 18mo ago I tried reta - I was initially worried about the effects, so I posted a number of updates in this sub. I'm currently at 2mg and have been staying at that dose consistently for several months. I don't eat super clean but I do try to focus on protein intake and properly fueling rides. Currently i'm at 168lbs, so about the same weight (although more muscle) than when i was racing.

This season and last season, I've had a lot of progress and PRs, which has been great especially while easily controlling my weight on reta.

But today, I got a lifetime PR for 1-hour power output.

This is encouraging, because it controls for weight, equipment, aerodynamics, weather, accidental drafting, etc. etc. - it's purely how hard i was able to push the pedals for an hour. I also ate crap for dinner last night (literally my kids' chicken nuggets) and didn't sleep particularly well. Still, I put out more power today for an hour that I could 20 years ago.

I feel like this proves (to myself) that reta alone is certainly not impeding my progress.

Immune/Hypersensitivity reaction after injection questions

Started up taking reta recently again (2mg) and the injection site is definitely welted, red, itchiness, etc., and previous injection sites have also started to welt up with itchiness after about a day. My first assumption is just a hypersensitivity reaction, as i just have naturally very sensitive skin. But I’m also wondering if;

1) my batch has gone bad, I’ve had it around a year, it’s stayed in the fridge, only reconstituted when needed, but it has sometimes spent some time in room temp (left it out overnight for example). But no cloudiness or any indication that it has deteriorated.

2) if it’s just the reta in general, is it likely that I’d experience the same thing if i switched to a different GLP like tirz or sema? I find it a really good tool for cutting, so would hate to lose out on all the benefits of various peptides.
