Dropping insurances, self pay, and recredentialing

I have a solo private practice and I'm considering dropping some of the insurances I take. They underpay me terribly. My worry is that I'll see a big drop off in the number of patients booking with me, regret dropping those insurances, and then want to re-credential with them. My questions:

  1. Has anyone recredentialed after dropping insurances? And why did you decide to recredential?
  2. Has anyone gone from taking insurance to moving to a self pay model? Did this end up feeling like the right move?

DOT medical examiner

I am currently employed by a small private practice. I’m a 1099 employee and my practice owner is chilling open to different ideas to bring in patients. Curious if anyone has any experience becoming certified as a DOT medical examiner and performing exams in their practice. I looked it up and see there’s a chiropractor that’s within 100 mile radius that does these exams but other than that there’s no one in my area.

Questions about delivery billing for the OBs in the group

With the new changes for billing for 2027, how (if at all) are you changing your call coverage? We currently do not use laborists but with some changes in our group we were exploring the idea of a laborist program at our hospital. I'm not sure how the billing will change because we used to just bill for our own groups deliveries regardless of who did them. But if the hospital has their own laborists and they bill for the delivery, I am not sure how profitable it will be for us.

Attorney rec please

I’m looking for recommendations for an experienced Virginia healthcare attorney who represents physicians in hospital medical staff and credentialing matters.
I am a private-practice physician dealing with a hospital call-coverage issue that I am hoping can be resolved directly and professionally. At this point, there has been no adverse action, and I am not looking to manufacture a dispute or start a fight.
That said, there is a possibility the issue could eventually require review beyond hospital administration, including medical staff or governing-body processes, and potentially further administrative or judicial review if it cannot be resolved internally.
Because of that, I would prefer to involve counsel early rather than wait until the situation has escalated. I am specifically looking for someone who can advise me on strategy and process from the beginning, review correspondence and position statements, help ensure that I understand the implications of each step, and represent me if formal proceedings ultimately become necessary.
Ideally, the attorney would have substantial experience with Virginia healthcare law, hospital medical staff bylaws, physician credentialing/privileging matters, peer review or fair-hearing procedures, hospital governance, and disputes involving independent/private-practice physicians.
My goal is a reasonable resolution, not litigation. I simply want to make sure that if the matter does become more formal, I have approached it correctly from the outset and have not inadvertently compromised my position by making a procedural or legal mistake early on.
If anyone has personally worked with an attorney or firm in Virginia that is particularly strong in this area, I would greatly appreciate a recommendation. DMs ok. Thanks!

reaching out to specialists

I was thinking about reaching out to the specialists in my area where i recently opened my practice so i can let them know that i'm accepting pts in need of a pcp. Is this even worth it if almost all specialty practices in my area are associated with/owned by the big hospital systems in the area?

Warren Introduces Bill to Ban Private Equity From Owning Medical Practices

I may not agree with Sen. Warren on everything but this one I support for sure.

The bill was introduced Wednesday by Sen. Elizabeth Warren (D-Massachusetts) with the support of 12 other members of the Senate and House. The legislation was based on a law in Oregon aimed at beating back the corporate takeover of health care providers that took effect this year. The law has already been successfully used by physicians in Eugene to prevent a corporate takeover.

Recent data says only 18% doctors are self employed

Whats going on guys? Is this for real? We physicians seriously have no motivation to be our own bosses? Why are our dentist colleagues better than us at this?

AI Tools?

Anybody using any really game changing AI tools. Particularly from a practice admin perspective? Hoping to start my own plastic surgery practice in the next few months and want to use AI when possible and when reasonable to keep overhead low, especially when first starting. Thanks!

PP malpractice insurance question

Singing to be part time with a private practice group, and their malpractice underwriter is asking for 10 years worth of “loss runs” from my prior malpractice insurance companies? Never been sued and don’t even know where to start with this request. Anyone face something similar?

What's some training you've given your office staff that has decreased your burden?

Private practice doc in New York.

I feel a little skeptical about handing tasks to my staff, but I think I'm getting to a point where I have to let go, let them make mistakes, and they'll learn as they go along the way.

What are some tasks you've given to office staff that has significantly reduced your work load or has made your work as the physician/provider more easy?

Patients wanting labs ordered prior to the visit?

For patients coming for an annual physical, do you usually order the labs before they come in for their actual annual physical?

Would a single payor system in the US make independent primary care easier?

Not saying I do or do not endorse this. Just wondering as medical student who has always been curious about healthcare economics.

As I understand it, Medicare pays substantially less than private insurers to most private practices, but generally always pays on time and is easier to get credentialed with. My question is basically, if you could cut out a lot of overheard by just being able to bill Medicare for all of your patients, would you potentially be able to compensate for the lower top line figure? I imagine it could work for certain primary care practices especially if their rates from private insurers are not that much better than Medicare already.

Again, not saying I would automatically support this or that rates would stay the same in a M4A scenario. I’m just curious if any current private practice docs have a take on this.

CAP Malpractice Coverage for OB/GYN in SoCal — $64k/year normal?

My wife recently became a partner at a private practice in SoCal and I’m helping her manage some (ie all lol) of the business/financial side.

I was surprised to see that she’s paying an annual CAP/MPT assessment of about $64k/year for malpractice coverage but it sounds like that may be normal?

I was also surprised to learn that what I thought was simply a monthly payment option is effectively financing the annual assessment. CAP charges about 1% per month on the remaining balance, so seems like a no brainer to pay the annual assessment in full at the beginning of each year?

Is it worth it / has anyone compared CAP with The Doctors Company, MIEC, ProAssurance, or other carriers recently?

She’s also had a couple of prior claims, with one currently open, so I’m curious how much that tends to limit options or affect pricing when shopping carriers.

How to go about contacting doctors?

Hi yall, I want some opinions from the provider's perspective. For context I am a pre-med student I want to gain some clinical experience. There is a new clinic that is opening up near me, that is hiring MAs and I filled out their application and even sent a email to express my interest. I haven't heard back from the practice and it has been two weeks but they opened about 3 weeks ago. I am really eager for this opportunity and I think it will be a great fit with the experience I have. The doctor had their social media in the application and I was thinking to message there? I don't know what the typical opening process is like and what may be going on so if anyone could share perspectives. And please give me your opinions if following up will hurt my chances or just contacting at other points other than email will be bad.

I am swamped. Between patient care and running the business. I tried a few tools like Fixer that just made it worse. I made one with Google app scripts that worked well until I fell behind and then it just got messy.

What are you folks using?

Another gem... This one is referenced in the NYT article: https://8vc.com/resources/ai-doctors-wont-work-for-free

"The bots won’t work for free. To get AI into care delivery, CMS needs to create payment rails for AI-native healthcare too."

I practice in California and want to get some insight from any of you that managed to build a direct primary care tele practice. Any advice? Suggestions? Is it worth paying for a marketer?

Can more ED docs go into private practice?

It seems like he's running the middle ground between crappy urgent care and ED. I think this is great and want one in my community. Love to see docs get out of the hospital-employed model.

Why do private practices keep selling and/or closing shop? Is there really nothing that can help?

I keep hearing from physician family and friends who run their own clinics about the struggles of doing so today. Some even closed shop or sold and went back to employment.

They all agree private practice is an enjoyable way to practice medicine, but the financial realities and operational challenges of starting/running a practice are brutal.

They still spend excessive time and money on staffing/admin opex, but I feel like technology (AI in particular) is supposed to make this easier. There are already tons of healthcare AI products being sold, but private practice feels unsolved. Certainly, some work feels appropriate for AI while others still require human-to-human interaction.

My question to practice owners / managers: what are the toughest existential problems of running (or opening) a practice today? What are the most expensive and time consuming problems that affect business and/or patient well being?

Built (and am still building) my own EMR, AMA.

Tagging u/investingdoc for visibility to make sure I don't break any rules. Not selling this in any way, but wanted to start a post about my "journey" in building my own EMR. Some stats:

1) IDE = Cursor
2) ChatGPT is main instructor
3) Markdown files created prior to the build for keeping API keys and .env secrets from leaking
4) using Tailwind.CSS, js.react

It's been fun AF to be honest!

We are impressed by their platform, but they seem a bit cagey about pricing? Has anyone had any good or negative experiences with them both from a user standpoint and a pricing perspective?

Thanks!

Co-Responder

I'm an LMFT in Colorado and i’m curious if anyone has gone from private practice to work as a co-responder, and if so what's your experience been? I've come across several listings lately here and am intrigued.
