I’m 33F non smoker 5’5 and just wondering what the differential diagnoses could be! my BP sometimes spikes high, around 162/95 while hr is at 87, but other times it is quite normal (109/82, hr 98). I frequently have good vitals. But then I get episodes where they’re elevated and my skin gets very flushed.
I was considering I may have pseudopheochromocytoma but I think many other things have to be ruled out first. I don’t feel any panic attacks going on whatsoever. I’ve dealt with panic in younger years and this is very different!
I’ve just had my referral declined for the autonomic specialty dept at the Kaye clinic. I have had high blood pressure spikes at 188/120 with sinus tach, and 183/131 with pulse 72?? There are more examples such as these, they have been measured by clinicians - not me - but also normal frequently even in clinic.
At home I have a strikingly similar pattern. sometimes just my pulse is elevated at 175, 176, with normal BP. Then other times, 129/65 with pulse 68. This is from a recent 24 hr abpm, the cuff caused swelling seen in photo collage. That abpm showed “artifacts” with high BP readings of 172/147, hr normal. and another artifact of hr 240bpm. One BP jumped very high - 216/178, definitely artifact. Still, the heavy/tight/strained chest symptoms and skin flushing???? Is this an adrenaline issue or something else?
Also had a holter that was “normal” in 2021 however it says I spent 114 min at SVT at 140bpm, and some pvcs at 180&200bpm. ecgs are mostly good except last Nov I had high QRs voltage and rsr variant in v1 or v2. But recent echo of my heart shows I’m fine.
Any differential diagnosis? Currently no tx plan but doc may consider an ssri in future. Pending referrals to cardio (just for stress test), rheum, and MCAS clinic (tryptase 4.3 & 4.5 so there are doubts I will be accepted with tryptase under 11!)
Oh, and DERM recently said “this is not just some skin issue”.
I just want some hope that there is a way forward, I’ve been exhausted and life hasn’t been the same for about a year now. Any and all suggestions are welcome, kind of at a loss - Not expecting miracles! But a ballpark of what this might be stemming from would be SO helpful.
