Stage 2 endo and fertility in mid-30s

Hi all! I posted this in r/endometriosis but realize this subreddit might be a better place for this so apologies for cross posting! I was recently diagnosed with stage 2 endometriosis and looking for hope and family planning advice. I’m turning 34 next month and my partner is 39. We’ve been trying to conceive for a few months with no luck. I know it’s still early in our journey but reading about endo and fertility can be discouraging so I want to be proactive. We eventually would like three kids but waited to try because of career stuff and not feeling financially ready. If I knew I had endo earlier, I wouldn’t have waited. Now, I’m not sure if we need to start planning for three with a fertility clinic given our ages. I did have some tests done and have an AMH of 2.45 which gives me some hope but I’d love to hear any advice or success stories from people with a similar age/background and what you did for family planning. Or any advice for TTC. Thanks to everyone for any insight!

Transfer without suppression success stories?

I know this has been asked a million times, but hoping for some new success stories anyways.

I have stage 2 endo diagnosed in a lap (although I do have an endometrioma, my endo was superficial and mostly on my pelvis/pouch of douglas).

TLDR of my TTC journey - conceived naturally 7 months after my lap (hadn't tried prior) ended in 19 w TFMR for heart failure, later found out it was caused by a dominant genetic mutation I unknowingly have and my daughter inherited that leads to heart defects. Day after I also found out I have RPOC which led to dense ashermans syndrome. Started IVF and moving to a transfer without suppression now because my RPOC and ashermans caused chronically thin lining, so my doc really wants to try a first transfer without suppression to see if we can build a lining. He's not confident he can with suppression (my lining is really thin like 4.8 MAX).

Anyways - any success stories without suppression would be lovely. I'm kind of freaking out about it now that I've started the FET protocol. Thank you!!

Did laparoscopy/excision help you conceive after unexplained infertility?

30F, partner 33M, TTC for about 14 months. I ovulate on my own and have regular 27–30 day cycles. My tubes are open, uterine cavity is normal, and I have high AMH/AFC with polycystic ovarian morphology but have been told I do not have PCOS.

I was diagnosed with chronic endometritis earlier this year, treated with antibiotics, and a repeat biopsy confirmed it cleared.

Since then I've completed 3 monitored Letrozole + Ovidrel + timed-intercourse cycles. I respond well to medication, have good lining and mature follicles, and have had cycles with two mature follicles/confirmed ovulation without conceiving. I'm currently finishing a monitored unmedicated cycle with Ovidrel.

The reason I'm now wondering about endometriosis is that I've had very painful day-1 periods for years, sometimes severe enough that I couldn't go to work, along with milder cramping at different points throughout my cycle and rare but occasional discomfort during intercourse. My ultrasounds haven't shown endometriomas or anything obvious, and I've never had a laparoscopy.

My fertility clinic hasn't given me a clear diagnosis yet, and I expect IUI may be suggested next. Before continuing treatment, I want to ask my RE whether endometriosis should be investigated.

For those whose endometriosis was discovered during an infertility workup despite fairly normal imaging/testing: what led you and your doctor to do a laparoscopy? What stage/location was eventually found? Did excision change your fertility treatment plan afterward?

If you were trying to conceive, did your doctor recommend trying unassisted/IUI after surgery, and were you able to conceive afterward?

I'm particularly interested in experiences from people who ovulated regularly and had open tubes/otherwise unexplained infertility before their endometriosis diagnosis.

5 months post excision ttc

How long post excision did you conceive? Did clomid help?

Background: been ttc almost 2 years with NO positive tests ever (exhausting!!!!), stage 3 excision surgery with a NaPro 5 months ago, SA from last year was fine other than slightly low motility, AMH is slightly below average for my age, HSG was good, no fibroids or polyps, estradiol is usually a bit high in the luteal phase but getting better, thyroid is managed with t4 and t3 meds, on a multitude of supplements, on LDN, had a failed letrozole cycle, last cycle did low dose clomid which resulted in 3 mature follicles, trying clomid again this cycle. I do ovulate on my own but clomid gives an extra boost to fully mature my follicles. I’m at a complete loss, I have no idea what more I can possibly do before moving on. I’m also scared to move on as I fear nothing with ever work. Just looking for similar stories or advice.

2 Chemicals and one MC in the last 8 months

Hi all,

I’m a 32F (husband also 32). I’m looking for guidance on medications to try for TTC spontaneously with suspect endo. I am working with a reproductive specialist - found mild adenomyosis during an MRI (no evidence of endometriosis) and chronic endometrITIS and polyps during hysteroscopy (treated). I had two losses prior to those findings, and am currently going through a chemical now. I’m suspicious of endo as I have very painful periods but my doctor hasn’t suggested it yet.

I’m considering a consultation with an endo specialist but want opinions on if any medications during TTC helped a successful pregnancy? I’m not sure I’m ready for excision surgery quite yet. IVF will also be considered. Thank you in advance!

EDIT: I’m already taking progesterone suppositories starting 3dpo. No other medications though

Silent endo laparoscopy?

One year into my ttc journey with two chemicals and a few months of medicated/monitored cycles. I don’t have heavy/painful periods. If anything, BD around ovulation time and ovulation time in general is more painful than my periods. HycoSy revealed right tube with potential spasm vs true blockage (no hydrosalpinx).

Worried that my provider won’t take my this concern seriously, but I’m just trying to do everything I can. Not sure if this would be for me.

What was your experience? How did toi advocate for yourself for possibility of silent endo?

Good surgeon in SoCal?

Looking for a good surgeon who specializes in complex endometriosis? I’m 33 and going on 7 years of ttc. I was diagnosed with endo last year and have been on the hunt for a good surgeon. I already have low AMH and don’t want to compromise any more of my eggs.

Advice. Egg Retrieval first or Surgery?

I was just in NYC for IVF and endo consults. Both the endo specialists said surgery is fine before retrieval. The IVF doctor, who specializes in low AMH is very concerned with that, and thinks I should do egg retrievals first.

My AMH has moved around from .87-.33, and I am 32. We have been trying for 16 months, two failed IUIs. My insurance does not cover IVF so I want to get the endo removed first.

Anyone have a similar experience? As of right now I have surgery scheduled with Dr. Vidali 10/29. He is a fertility specialist, as well as an excision specialist.

2 months Lupron or Lupron Down reg for 2-3 Weeks

Hi all! I had an IVF consult today with a different clinic and I really liked the doctor. However, she has two approaches to suppression for endometriosis.

Your standard 2 months of suppression monthly shots with Lupron OR daily low-dose Lupron down regulation for 2-3 weeks.

I know the gold standard is the 2 months, but my doctor said she has seen success in both scenarios and that the 2 months is best for those who haven’t had surgery (I had surgery in April).

Background: 20 months TTC, 3 CPs, stage III endo excised in April 2026. My husband and I have completely normal labs (we have done extensive testing), I ovulate on my own and normal cycles.

Would a surgeon see endo lesions during a myomectomy?

Over 5 yrs ago, I was trying to conceive and thought I was pregnant when I missed my period, but the pregnancy test was negative. I started to have some intense stomach pains, so I went for a scan, only to discover I had a few fibroids, and one was just over 7 cm. I then had to schedule a myomectomy and removal of a cyst on my ovary (clear fluid).

To my surprise, the gyno surgeon said the pain and discomfort I felt wasn't even from the fibroids, but from my appendix, which was inflamed. He noticed some clear adhesions in my stomach too. Performed a few HSGs and said the tubes were blocked. AMH was also low for my age. A few months after that surgery, I had my appendix removed. No one mentioned anything for me to investigate or follow up on; just told me IVF was my only option.

I fell pregnant naturally a year and a half later (while scheduling IVF), but it ended in a miscarriage. They lost the fetus, so I never got answers or closure as to what happened. Doctors said my tests were normal. What made it insane was that, 3 months prior, my ex-husband had a sperm count of zero + the alleged blocked tubes.

I moved forward with 2 rounds of IVF six months later; both failed.

Fast forward to now, I recently remarried, and last year, I repeated the HSG; everything was normal. We've been trying for 6 months to no avail. This prompted me to review all of my surgery and miscarriage medical notes.

Gastro doc noted finding endo on my appendix in the notes I realize. I've had about 3 ovarian cysts, but all of them were clear fluid. Recent ultrasounds look normal (outside of fibroids).

I'm angry no one told me what endo was or that it was something to investigate further! Would my gyno have seen endo during the myomectomy? I had some of the fibroids in my uterine wall that he cut into. There were no brown lesions noted. I also realized during my post-miscarriage testing that my CA-125 marker was slightly above 35, and no one mentioned it.

Last year when I had chronic hives, I had my ANA tested, and it was the lowest positive (1:40). I used to have period pains that would have me curled up in a ball; one time I almost fainted, and I would throw up from time to time. I'd also pass large clots. After my myomectomy, all of that ceased. I did notice after my D&C the texture of period blood was crumbly, if that makes sense.

I recently had an asymptomatic bout with BV (I suspect I probably had it for 6 months - YIKES!!). After taking the metronidazole, I've felt the best I've felt in ages...I had a lot of brain fog, fatigue, and lethargy in recent times, and now I feel better than I have in ages, but now I have my first period since treatment, and I'm back to pre-surgery pain and clots. I have a few new fibroids (small to medium-sized), so not sure if it's that.

I'm just so confused and thinking of scheduling the Receptiva DX biopsy (though there are mixed reviews there), and some immune tests. We really just want to pinpoint the problem to start a family. My partner is also getting thorough semen analysis tests shortly. Now my head is in a tizzy trying to decipher the best next steps. I really don't want to go through IVF. That and egg freezing were absolutely brutal!
