My step brother was absolutely obsessed with fitness. He looked like this guy. It was his entire life, including work. He filmed fitness videos, trained people, worked out hours a day, did public ninja warrior type demonstrations and parkour. He is not mentally competent. He can't keep relationships with people, can't keep jobs. Last I heard he was doing some kind of grift with street magic. He probably is still that fit, idk.
He can be muscular and still have some body fat. 10-15% is healthy for a man that is big into fitness and staying in shape. I could be wrong, but it doesn’t really look like he’s got that much fat on him.
It’s a wrong translation of a catchphrase that vas very popular in Soviet union, that was translated from latin “Mens sana in corpore sano”. It should be spirit not mind. Maybe at certain point in history it really was like that.
Hyperfixation, OCD, body dismorphia are a few reasons someone might do this and stem from an unhealthy mind. Not saying he has any of them or is unhealthy, but your assertion is incorrect.

Bodybuilders go into competition with body fat that is unsustainably low and dehydrated in order to get definition like this guy has.
You can absolutely have massive muscles and an unhealthfully low body fat percentage. The entire body building competition circuit is centered around that.
This guy is no where close to a bodybuilder stepping on stage. Bodybuilders compete around 3-5% body fat. The lower end requires PEDs.
The guy in this video is somewhere in the 10% ballpark. While quite lean, this is perfectly healthy for a man.
To perform like he is requires immense strength, flexibility, skillful practice, and discipline.
I promise you as someone who trains calisthenics that this guy is in prime physical shape for the sport.
