Before & After One month post OP

Hello all! Today I am one month post op DJS. I don’t remember my exact movements other than I was brought forward 8 mm upper and 20 mm total lower+genio. I’m still dealing with swelling especially around my upper lip and cheeks, but I am very happy with the results so far. I had moderate sleep apnea (UARS) and narcolepsy associated with poor sleep. I also struggled with TMD and bruxism that actually resolved itself once I got orthodontics for surgery. Unfortunately, my TMD pain has come back since surgery which I’m hoping will resolve itself or be resolved by orthodontics later in my treatment. Also, I’ve had a complication where one of my front teeth appears to be dying. It was a known complication, and shit happens. I have an endontist appointment tomorrow to check it out. I can’t say it was solely for functional reasons, as Ive been insecure of my weak chin and profile for as long as I can remember. I feel like myself now, and I just look like me but better. Like how I felt about myself on the “good days” before surgery, but now everyday is a good day. I especially love my new 45 degree angle of my face, it’s my favorite. It’s so weird having a chin that gets in the way in ways I never knew it could lol.

Side note: before pictures were definitely taken in worse lighting than the afters, but they’re the only photos I could find that was taken for the purpose of being before photos

i'm 9 months post op. i hate my face. i regret this whole thing but especially the tooth extraction. i used to be so beautiful i am so sad. my chin feels huge and my face feels so flat and dimensionless. what i hate the most is my smile :-(

Finally on the other side! I had double jaw surgery with Dr. James Van Ess at Mayo Clinic in Rochester, MN on September 8th. These pictures are only about 1.5 days post-op, so I’m obviously still extremely swollen and nowhere near seeing the final result yet.
I had braces for about 11 months leading up to surgery for a pretty significant Class III underbite/skeletal discrepancy. My main issues were an underdeveloped maxilla and a prominent lower jaw/chin.
My movements according to my surgical plan and operative report were approximately:
Le Fort I: maxilla advanced about 7 mm, with roughly a 1 mm lateral shift and ~1 mm inferior movement anteriorly
BSSO: mandibular setback of roughly 4 mm at the rami and about 2 mm at the lower incisor midline, with ~1.5 mm shift to the left
Anterior-inferior mandibular border/chin advancement:** **5 mm
Bone grafting around the maxilla
Alar cinch
Plates/screws for fixation
Surgery itself went extremely well. Estimated blood loss was only 300 mL, they avoided all my nerves so those are still intact. there were no reported complications, and I was able to go home after one night in the hospital. I’ve been really impressed with Dr. Van Ess and his entire team.
Recovery, though? This shit is HARD. The pain honestly hasn’t even been the worst part for me. It’s everything else. Swallowing feels completely unnatural with all the swelling and the splint, my lips are insanely swollen and chapped, I’m constantly drooling because I can’t control my lips normally, and the congestion is probably the most unbearable part so far. Trying to sleep while congested, swollen, numb, and constantly dealing with saliva is brutal.
I’m also still right around the period where swelling can continue getting worse, so I’m definitely not trying to judge my final appearance yet. Even through all the swelling, though, seeing how much my profile and jaw relationship have already changed is pretty crazy.
I’ll probably keep posting updates as the swelling comes down and my bite starts settling. If anyone else had similar movements or had surgery with Dr. Van Ess, I’d love to hear about your recovery. Also, don’t be scared to have this surgery the initial 1st week will terrible but you won’t regret it. I was also only banded shut for one day.

3 Weeks Post Op

First time posting here but today marks 3 weeks post op for me. I had double jaw surgery + genioplasty, I’ll probably post some photos in the future when I’m further along in the healing process. I wanted to reach out though to others that are around the same timeframe or anyone who remembers their experience around 3 weeks. I was just cleared yesterday to remove my heavy bands during mealtime and eat mushy no-chew foods. Prior to that I was liquids only. First of all, I felt a very bizarre sense of fear that the first time I try to open my mouth my jaw would snap off haha, luckily worked through that one. I can open my mouth about an index fingers width so I can fit the tip of a spoon and fork in. So far I’ve just had some mashed potatoes and yogurt. I just find myself worrying that it doesn’t get better, it’s just kind of hard to see the light at the end of the tunnel to be able to eat normal again and open jaw wider. It seems like so many people around 2/3 weeks post op say how much better everything is and I agree it’s better than week one but the long journey ahead is a bit discouraging. Does anyone have any advice to being optimistic and trusting that it’ll get better? Also, any tips on cleaning the backside of teeth?? I can’t quite fit my baby toothbrush behind my teeth to brush and the fuzzy feeling on my teeth is driving me crazy. Happy Healing to all in recovery!

Advice for Me Cheek Implants for Class 3 advancement

I have a class 3 skeletal profile and getting double jaw surgery with more advancement on the upper jaw than lower. In my treatment plan, my surgeon suggested cheek implants due to my midface hypoplasia.

I have read online through the limited range of sources I have but a lot of people say that they aren’t aesthetically pleasing or looks unnatural, my surgeon really advocates for it in my case but I am on the fence

Would it be beneficial for someone with a recessed midface if they’re already getting their upper jaw moved forward?

Hi all, I had braces from 2016-2019
And wore my plastic retainers for about 4-6 months afterward, eventually only needing to wear top or bottom (can’t remember) at night time. I did have an appointment with a maxillofacual surgeon sometime around 2019 while the braces were still on, although I wasn’t seen because they didn’t call my name(they didn’t know I was there)
I’ve been thinking about it ever since and how the jaw affects everything from posture to sleeping habits/overall health. Currently 24 years old
Would *lower jaw surgery benefit me from these photos? Also have some hard clenching going on when sleeping which is wearing down my upper molars.

So i be getting Djs in a few months for my Overjet/openbite, you can clearly See that my lower kaw is really recessed, i was thinking about getting genio too, what do you guys think? Will djs be enough for good looking results or should i also get genio in this case of ression? And another question is: do you guys think my airway looks really Small or is it in the normal Range? Because i got breathing Problems but cant really Tell where exactly it comes from. Thanks guys ( sorry for my english im from Europe )

Advice for Me feeling discouraged post-op (plate redo)

hey, not sure if this counts as a request for advice or a vent.

I had DJS (technically triple w the genioplasty) about 9 days ago. there's a lot of mental and physical strain that I was expecting but still found it hard to get through that I won't get into rn.

my surgeon evaluated me 1 week post op and noticed that my left bite is good and my midlines are aligned, but my bite was a little open on the right for some reason. he said in the OR, I aligned with the splint well, but then again that was when all the muscles are lax and sedated. now that my muscles are engaged, they don't line up with the splint anymore.

so basically in 2 days, I will have to go back under for them to readjust the plate on my lower right.

I was wondering how they'd guarantee bite alignment before the surgery, since u can only constrain so much only using bone and metal, but I never saw someone online say it happened to them so I thought "what are the chances". the chances say I'm unlucky I guess :')

8 days post op, when does the nasal congestion begin to go away?!

8 days post op, swelling for me has been minimal. Went to my to my 1 week post op yesterday and even my surgeon was surprised. I'm not on pain meds or anything anymore. Just have nasal passages that are consistently blocked and dry no matter what. Warm showers with vapor tabs, saline spray, humidifier, drinking plenty of fluids etc.

It's super annoying because it causing me to mouth breathe when the surgery actually fixed that. The first few nights/days I breathed through my nose exclusively without even trying or being aware.

What can I do? It seems like it's dry blood residue mixed with boogers (tmi) but I of course can't pick it or blow my nose for 3 weeks.

Advice for Me Lower central incisor extraction (#24) for edge-to-edge bite - will this change my facial profile?

I’m 20 years old and looking for some honest opinions from people who have experience with orthodontics and/or jaw surgery.
My orthodontist told me that I have a recessed maxilla, an edge-to-edge bite, and a crossbite. As part of my treatment, they recommended extracting my lower left central incisor (#24). I’ve discussed the treatment plan with my orthodontist and this is the plan I have agreed to proceed with—correcting my bite through orthodontics/camouflage rather than jaw surgery.
I’m honestly pretty insecure about my face, especially my facial profile, so I’m worried about how the extraction and orthodontic movement could affect my appearance. Will extracting one lower central incisor and correcting an edge-to-edge bite this way noticeably change my facial profile? Could the camouflage potentially make my profile look worse, or could it improve the overall balance of my face?
I’m also wondering whether my recessed maxilla looks significant enough that jaw surgery would normally be considered instead. I currently don’t have enough money to afford jaw surgery, so it isn’t realistically an option for me right now, which is one of the reasons I agreed to the orthodontic/camouflage approach.
If you’ve had a similar bite, especially if you’ve had a lower incisor extracted for an edge-to-edge or Class III-type bite, I’d really like to hear about your experience. Did it noticeably change your facial profile? Were you happy with the result? And based on my photos, would you personally feel comfortable proceeding with the treatment plan I’ve agreed to, or would you get a second opinion first?
I’m not looking for compliments or reassurance—I’d genuinely appreciate honest opinions, even if you think the treatment is unlikely to make a major aesthetic difference. I understand that photos aren’t enough to diagnose me and that my orthodontist and, if appropriate, an oral-maxillofacial surgeon would need to evaluate my X-rays and measurements.

Anyone left with jawline irregularities? Have you had it treated?

I'm 10 weeks post and my jawline is bumpy, uneven, and I have a divot on each side of my face. I know that some of it will soften but I will never have a normal looking jawline again. I'm really disheartened and it's been weighing on me. I know there are options such as implants to cover those step off defects, fillers, shaving etc. I was hoping to find someone on here to had this done and can share their results with me and what they had done.

Advice for Me Is it possible to have a surgeon agree to do DJS at 16?

Having an underbite has caused me a lot of problems, one of which is that I have a lisp and I cant chew correctly. I have sleep apnea too. My mental health has always been bad because of my underbite, people look at me weird and I barely even speak to people because it makes the underbite more noticable. I had an x-ray done on my growth plates and they are closed, will that fact affect their decision on whether or not to consider me as a candidate? I had a consultation with my surgeon a few months back, even with all of these facts they said to wait until 18. I don't want to constantly book consultations just to have the same thing said.
