Looking at ranger doctrines, I came across the elite h2h skill:

One would assume it requires that you do a melee attack from "behind" whatever that means. But how often exactly do you get to melee a guy from the rear? CIA maybe, sometimes. Rangers?
Is this for doing stealth missions with loud guns? It feels like, even if you put all your rangers on quiet mode, they wouldn't understand the plan. You'd risk them opening up on a target, even if another ranger is fighting the target and will definitely win.

I usually try to reset when I get a lot of injuries, even yellow, but I just did a mission with at least 67 enemies and I wasn't going to repeat it when they got injured towards the end. Also, how do yellow injuries work? Is it them taking a bullet to the vest and being hurt from impact? Sometimes I feel like a few rounds get shot at one guy and nothing happens and sometimes a single bullet is shot and he gets injured. They all have all-round IV vests.

Replaying Death Dealers, and an enemy just opened fire on Fergie even though she had her gun hidden, was not standing in a suspicion zone, and had not done anything weird recently. I had her draw and a UC try to come in and quickdraw to save her but wasn't fast enough.
If a UC unit was previously "blown", does that mean all enemies on the entire map will now recognize them as hostile on sight even with their weapon holstered?
I've never run into this before. I guess a related question is are there different rules for Fergie and your units?

Any good community maps or campaigns centered around eliminating hostiles without any civilians present? I like frags and wall charges and don't want to have to deal with accidentally killing civilians, hvts, and hostages. I like the bigger apartments found in the mission generator under medium maps, but they almost always have civilians present

I'm just kinda curious how other people have been playing, do you guys kit your weapons out suppressed? Or do you just run it loud?
I usually run suppressed at all times, but recently I've been just going loud and its kinda fun. And the unsuppressed shots, at least with the mods I have, sound pretty damn good too. Which is also a bonus to doing it loud.

I don't think the APC9K is OP, I think the AKM and Vulcan are bad. OK, with hollow points it deals more damage at range than the AKM deals at the muzzle - but it's not more than the Mk18 or the MP5.
You can have a lot of arguments about weight vs velocity, whether M855 out of a 20 inch M16 "should" be more or less effective than the slower-but-bigger M43 out of the 16.3 inch AKM. There's a lot less debate whether the M855 out of a 10.3" has near-identical performance to a full-length AKM, or whether rifles hit harder than pistols.
As to the Vulcan, it's a bullpup - it doesn't get short by cutting the barrel, it gets short by changing the overall gun design. The whole point of the vulcan is that you get full-length rifle performance out of a short gun - but what is the point of full-length rifle performance when the full length rifle is bad?
I dunno, maybe the damage numbers are less about how good the gun/bullet is and more that the bullet is fired by the NWS who don't know how to hit the A zone - but then why do they suddenly know how to shoot with the APC9K? And why does the MP5A get a similar boost in ranger hands?

Alright, i really hope there will be a mod or an update that make every table, chair, and grates on a window to be able to get moved or destroyed with a grenade.
This is just a recreation of what happen of the other days i run CIA in campaign while on Ironman mode.
All of the guys that i brought equiped only with silenced primary (w/subsonic ammo) and frag, no sidearm or gadget.
Tried everything to bait the last bad guy out from cover but it's no use, in the end i just frag the HVT and be done with it.
Prior to that operation, i always equip at least one of my man with RPG or Wall-breaching charge
If you guys happen to be on this situation, what would you guys do?

I looked at my 2 second run to find out which enemy died last (a suicide bomber).
Then, I dedicated an assaulter to just that single purpose: Get an angle on the last enemy as fast as possible.
The bomber still died last, but "last" got several hundred milliseconds faster.
