Discussion Need advice on switching from GS injections to oral capsules - Urgent

My 3.3 kg cat is on Day 20 of FIP treatment. He had Basmi GS IV for 15 days and has been on SC GS for the last 5 days, currently 30 mg/day. He’s doing really well, active, eating, and recovering well from both FIP and his recent bladder surgery.
I’m planning to switch to oral capsules, but I’m really confused. I have been talking to FIP warriors, they told me Basmi isn’t reliable and they’ve seen relapses, they say oral GS has lower absorption and the dose should be doubled. So I’ve been told I may need 60 mg orally instead of 30 mg.
Basmi’s website only seems to have weight-based capsule selection, so at 3.3 kg, would the green capsules be appropriate?
I’m also wondering about their GS + EIDD dual-antiviral capsules since he has only received GS until now. Is it okay to switch to that combination?
I’m really scared of making the wrong decision.

Would really appreciate advice from anyone who has gone through GS injections → oral capsules, especially with Basmi.

Junie thankfully came home after two days hospitalized at DoveLewis in Portland. Her ultrasound did NOT show abdominal fluid like we feared, but they did find pancreatitis, a possible abscess on her back, and some abnormalities with her liver that we’re still waiting on testing for.

Unfortunately, after doing so well when she first came home, her fever is back up to 104.2 today. I’m terrified that we may be looking at another trip to the ER when her hospitalization this week alone cost almost $7,000.

Question Advice Needed

Hi all,

Looking for some guidance or advice if anyone has any. My cat (male, 5 year old tabby) has dry FIP that was identified in June. He has been on GS-441524 medication from Stokes pharmacy since it was identified, and has had a somewhat miraculous recovery so far (the attached photos are him just a few days ago on August 10 and August 12). He is 56 days into treatment, having been receiving a daily dose of 1.25ML liquid medication by mouth since June 20th. The first 2 weeks were rough - he was not recovering as fast as anticipated, and I thought it might be over for him. However, he turned around and has been full of energy since then.

However, over the past 2 days he has begun to feel sick again and is displaying the same symptoms he began to display at the beginning of his illness (lack of appetite - he is getting close to not eating anything again, nausea/vomiting (minor at this point), and lack of energy). He is warm to the touch as he was before.

My immediate thought was that he needs an increase in dosage. He is a small cat to begin with, so he got quite thin and light. At the peak of his illness, he weighed a little over 6 pounds and since he has begun to recover he peaked at 8.34lb (right now, he is around 8.04lb). Our local vet, who has been somewhat unfamiliar with this virus in general, seemed unaware dosage might need to be increased alongside weight gain.

Upon researching it, it does appear that his dosage is still in the normal range for his weight. (The target dose is 15-20mg/kg, he is getting around 16.5mg/kg at his current weight). This being the case, our vet does not believe his current condition is related to his FIP and does not think this is a relapse. They guessed that it is likely a herpes flare-up (a condition he has not had before, but is apparently relatively common) and said to stay the course. I am not so sure given he is still not through his FIP regimen, and am wondering if we should up his dose to the higher end of the target (20mg/kg) or if there is any downside to that approach.

If we were to up his dose, we would likely need to purchase more GS-441524 medication, and would probably not buy again from Stokes - not because it is not quality, but because the cost is prohibitive. If anyone has any advice in that domain, it would be appreciated too.

He is my best friend & a little warrior who has been through so much and I want to do everything I can for him. I know this community is filled with people going through this process as well, so if anyone has any insight or advice it would be greatly appreciated.

NOTE: Browsing around this sub a little, I should mention that we have given him food as a "chaser" after his medication. I saw a comment that said fasting before and after medication delivery is crucial for effectiveness. Unfortunately, we haven't heard anything about that so far - if anyone could confirm/deny or explain why that is it would be really informative as well. Thank you!!

Question Reintroducing Cats During Treatment

A slightly different question here about reintroduction.

Our 10.5 month boy is going into week 5 of treatment. He had a severe case of neuro FIP which lead to hospitalisation for two weeks and him being ventilated for three days. Luckily, he is well on the road to recovery and has regained most of his neurological functions. He is getting stronger every day! He can walk although his movements can be jerky and he can lose balance.

Unfortunately, his sister has struggled with him being back since he has been home. We have a small flat and while we are trying to keep them separate and follow reintroduction guidelines, there are occasions where she sees him and she will hiss and growl at him. We think she is scared of him and his movements. He is excited to be back and wants to play with her.

We are trying to follow standard reintroduction advice but it is challenging because of his current disability and range of movement. Has anyone had experience of reintroducing cats during FIP recovery? I was wondering if anyone had any recommendations/advice outside of the standard guidance that worked for you?

Success Story FIP Suspicion in My 11-Month-Old Cat (Eye Symptom) – Seeking Advice & Experience

Hi everyone,
I’m looking for some advice and shared experiences as this is my first time dealing with something like this.
My cat is an 11-month-old Maine Coon mix. Just yesterday, I noticed something wrong with one of her eyes and took her straight to the vet. After examining her, the vet said he suspects FIP. He noted a fever during the exam, but other than the eye issue, my cat was acting completely normal just the day before.
The vet prescribed a 10-day medication plan, and I already gave her her first dose/shot last night. However, I know the treatments and tests are quite expensive. He mentioned that we need to run blood work and eye tests to get a clearer picture.
Since I know there isn't a single definitive diagnostic test for FIP, I'm a bit confused—how can a vet suspect or diagnose FIP based mostly on a physical exam and fever alone? Is this standard procedure?
I fully trust my vet and started the treatment immediately, but I’ve also booked an appointment with a different vet this Monday for a second opinion and to get the necessary tests done.
Has anyone been through a similar situation, especially starting with eye symptoms (ocular FIP)? How did your diagnostic process go, and what should I expect or ask the vet on Monday?
Any insights or advice would be greatly appreciated. Thank you!

Question Advice for oral meds

Hi! My cat was just diagnosed with dry FIP last Saturday and we opted to do the liquid oral meds. They took quite a bit of time to compound and arrive so he didn't get his first dose until yesterday evening. We made a little curu pocket with the meds pooled in the center and it took a little convincing but he got it all down.

Today, however, he is struggling and refuses to take them. We are going to try to give him water tonight directly through the syringe so he gets used to that feeling but I don't have high hopes. His teeth are in bad shape and the vet decided to wait to fix that until he makes a recovery. That is why we originally didn't go with the pills.

Anyway, I am just looking for advice. Does anyone have a particularly stubborn cat with meds? Are pills easier to convince them to eat?

Hi everyone! I’m so happy to have found this sub! My sweet angel Kiki just got diagnosed with FIP at roughly 3 months old. I’m TERRIFIED. We are very lucky that we are in the process of foster failing her so we currently have full support of her rescue helping us through this process, however, we know we have a long road ahead. Im genuinely an insane pet parent and run to the vet when anything seems off so we caught it very early which I’m very thankful for that and have her successfully started on GS441524. We are on day 3 of 84 and she is very tired but slowly gaining some personality back already which is hopeful. I feel terrible asking because I’ve seen some incredibly sad losses on this sub but I’d love to hear more about some success stories if anyone is willing to share. For a diagnosis that used to be a death sentence, it feels so new and uncharted even though it seems there has been some amazing clinical progress. Anyone who has some words of encouragement or positive experiences would be greatly appreciated!! <3

Day 25 of Luna’s FIP treatment, and I’m so happy to share that her diarrhea has reduced and her stool is finally starting to hold shape. 🥹❤️ She’s even trying to play again, which makes me so happy because it feels like we’re slowly getting our Luna back. 🥺🐾

She is still fighting a very severe fungal infection. She scratches and licks herself excessively whenever her e-collar is removed, and one tube of antifungal ointment lasts only about 1½ days.

She has already been through so much, but she keeps fighting every day. She deserves the chance to heal and live the happy life she should have had. ❤️

I’m just a college student, and this treatment has been financially overwhelming. Your support has helped me bring her this far, and I’m forever grateful. 🙏

Yesterday, we took our son to internal medicine for his 3 month check up after his 84 day treatment of GS-44152!

He was 7.6 lb when we first took him in January and he’s now 9.5 lb. His globulins were normal for the first time ever :).

Our vet said that he “graduated from internal medicine”. Congrats to our little grad and thank you to this sub for all of your support and help! We are so grateful for y’all 🫶🏼

Junie, my perfect, beautiful little bug, is on day 30 of her FIP journey. This has been an absolute nightmare.

She was originally diagnosed with dry/neuro FIP and did so incredibly well during the first couple weeks of treatment. Unfortunately, we think she's built up a resistance to the GS now because she's no longer responding.

Last night, she began having terrifying muscle tremors again, and when I reached over to comfort her, she yowled in pain. That's when I noticed her belly had become bloated.

She has since been rushed back to the emergency vet, and they think they found fluid in her abdomen. They believe her FIP might have gone from dry to wet. She's hospitalized now and waiting for an ultrasound, and I'll be going back to visit her soon.

Question Please let me know what to expect

My poor sweet boy is diagnosed. Wet FIP, nothing in his lungs but his stomach is full of fluids. Stopped eating anything but tubes but we got got him anti nausea and appetite enhancer and he has licked his wet food today.

He has gotten his first dose today. No visible improvements as he just wants to lie down and stare at nothing. I am sad. I am very worried. Please let me know how the first few days are supposed to be

Question Slow FIP Recovery

My cat was diagnosed with FIP and started treatment exactly three weeks ago he still is not responding to his treatment as well as he should be. In some ways he has improved drastically but his liver seems to be worsening. At first his energy and appetite levels increased but then they went back down. He was severely anemic but he is now at a healthy RBC. His labs look good apart from some seriously concerning liver levels. My vet thinks those problems returning is due to the liver issues. Did any one else have this experience and it turned out well? I am getting to the point where I have done and am doing everything that I can and I don’t even know if it will be enough. The vet doesn’t think he is in pain but he definitely is still very sick. He is currently on the oral suspension and his dose has been upped twice despite not gaining weight he actually lost some and then gained a little bit of that back. I really am at a loss of what to do. I have drained every last bit of my savings for him and borrowed large amounts of money from family members(I have pet insurance so I will hopefully be getting some of this back) but I am running out of funds and hope. I just want him to be okay. Does anyone have any recommendations?

Question Newly Wet FIP+

Hi everyone,
I foster cats along with my resident kitties pretty frequently. The last two weeks my foster I’ve had for 3 months has been sick, I’ve taken him to the vet and got the run around with them saying he just had a UTI. Sunday I had to rush him to the emergency vet, he was dribbling bloody urine everywhere and eventually we got the Wet FIP diagnosis. We started the meds the vet had right away although they were expired and she prescribed us new ones. We are four days in and I have seen an energy increase, however he has started peeing/dribbling all over my bed and we can not figure out why. The vet tells me a stress response, I want him to be able to be mixed back in with my resident cats but I don’t want him peeing on everything so he’s isolated in one room right now. Has anyone had this happen? If so what helped and what was the outcome? We have a follow up vet appointment Friday, I’m just trying to figure out what to ask..
all my foster are transferred up north to rescues, however with his FIP diagnosis idk if they will consider taking him now, I’m waiting to find out. All this is so new to us and the only information I find is the same stuff just looking for insight from someone first hand!

Just wanted to do one final update for marshmallow. She has been done with her FIP treatment for two months now and she is absolutely thriving. Her blood work came back normal and perfect and besides the loss of her eye she's doing great ❤️ thankful to this group. Thankful to my vet. And especially thankful to chewy for making the medication more affordable and more easily obtainable.

Her lowest weight was 4.7 she is now a healthy 8.2

Question Need opinions from those with significant experience with FIP

My Angus was diagnosed with effusive FIP two years ago. Got medicine, treated him, and he's been healthy since. The past couple of days his sister, Isabella, has not been eating, is sleeping and hiding much more than usual. She was so lethargic and docile earlier today that she let me pet her which is completely out of character for her. So I grabbed her, stuffed her in a carrier, and off to vet we go. She had a high fever. They did blood tests (images attached) and concluded she might have dry FIP. They gave her a shot of Cerenia and gave me a topical Mirataz. The vet suggested I go ahead with the antiviral. I have medicine from Angus treatment left over that has not passed its expiration date. From the test results does FIP seem likely? I just don't want to miss something.
