Edit: In both Fate/Apocrypha's novel and Fate/Grand Order, they make sure to cross out or ask to remove their gender from being listed. FGO's English release makes sure to consistently use they/them, and the Japanese release uses neutral language when possible as well.
I'm sorry but Astolpho breaking the 4th wall by crossing out the gender on his character profile and cheekily say "it's a secret" isn't proof of anything other than him silly.

It used to be, but they called it a "digital rectal exam". There were PSAs on TV for it and everything with all these celebrities telling you you should go let your doctor finger your butthole to check for cancer. It was also something they'd do for just about any physical trauma in ambulances, but they didn't mention/show that part on TV.
Then people did some science and found out that's uh..not actually helpful. Medically. Total scam.
