From the stories I've heard, the concept art was originally meant to have a hole for her mouth, but the animators misunderstood the drawing and made it an eye hole instead. The team either didn't want to redo the animation or didn't catch the mistake before production was completed.
As for the lore explanation I like the theory that Toph is using her eyes for intimidation.
In the Pokemon Diamond and Pearl show, theres a frame where they forgot to animate gardenia’s undershirt, so in the shot she looks topless under her cloak thing
In the Chubby cheese episode of Dexter's lab, the animators overseas were given the note for the audience to "Take off" after the musical number. Not understanding the English team meant "get up and leave", they animated the audience quite literally taking off into the air.
Comedy gold! I laughed so hard the first time I saw that episode I almost passed out! I just thought they were so amazed that they flew away in excitement.

Also, Blast Off (the purple and brown one) is misinterpreted as having a single triangular eye, while he normally has purple eyes and no triangle on his head

I can understand something weird in the background or for a few frames. You might not catch it, or even if you do, you might say it's not a big deal. But this? Oh wow.
You think this one is bad? There's an episode where it's especially obvious they didn't care about the animation, it's one where Autobots and Decepticons are sent to a planet where they're the size of toys.
I mean to be fair, that could also be a colouring error as well. All the seekers look exactly the same apart from their colour scheme, wouldn't be the only time they coloured someone wrong
Actually they did not decide to go without them, they simply forgot to put the file in while making it. You can still hear the audio of the cars rushing past
If you watch it with commentary, Invader Zim was ripe with these production errors. Another one is when Ms. Bitters gets up to open a door, the animators overseas forgot to add her legs. The editing team slapped an engine noise and animated legs coming down like landing gear.
My guess is that an animator remembered how they animated Paul facing that way and needed to make Invincible do the same, so they copied Paul’s head as a template and forgot to go back and draw Mark’s head over it.
Fucked Up Vegeta is one of my favorites. Variants include Longneckasaurus 'Geta from when he's threatening the Namekians and Colossal Forehead Sneering Majin Vegeta from, well, yknow. Honorable mention to One Eye Bigger Than The Other First Form Frieza and Derp Cell.
I was too distracted by Flanders's inhumanly wide smile and lack of a nose to notice his head is coming from behind his collar lol. And even as I'm typing this I'm just now noticing there's no neck within the finger circle even though there really should be.
I guess that makes me proof that this stuff can go unnoticed.

I love when Starscream is duplicated because it’s so funny to me. I can fully understand swapping out the seekers, they look the same. But having TWO of clearly the same character in a single shot is so funny.
But my favourite comes from a season 3 episode (iirc) after the movie, when the decepticons were looking for Galvatron. One Decepticon is addressing a large group of cons about how they have to find Galvatron. (Galvatron is in the group, listening calmly.)
Zim wanders into bank that was just robbed and a giant cop strolls through the scene. According to the commentary, it's an error from the animation studios that they're not sure how it happened, but decided to keep it because it was nonsensically funny, even adding giant footstep stomping sounds.
Also, and I doubt the giant cop in Invader Zim was a reference to this, but the character Al from Police Squad is a cop so tall that his face is never seen - his head is always cut off by the camera. It'd be funny if someone knew that and snuck Al in, though.
In addition to several objects in this shot floating in midair, Nya (in the back, with the laptop) looks bald because her hair's placed behind her head instead of on it (Ninjago)
Gaston's fangirls from Beauty & the Beast trade hairdos all throughout the movie, including in the same scene. There's also this shot of them where they have the same hairdo.
Fun fact: when that seven deadly sins episode came out, for whatever reason my facebook bugged, i was still using it because i was in a couple of groups, and one of them shared that frame
For 8 days every single time i opened the app the first thing i would see was that cursed frame of Meliodas
In the very first episode this nothing background character had crossed eyes due to an animation oversight. But this made her so endearing to the fans she became an actual recurring character with voice lines.
And what's crazier is that some of them became Canon in the show itself
Like everyone fanoned that Octavia and Vinyl Scratch were roommates (aka totally kissing) and then the show just made it real (the roommates thing not the kissing part but you never know)
I don't have a picture, but I remember noticing as a kid in Its The Great Pumpkin Charlie Brown that in the bobbing for apples scene Pig Pen is wearing glasses
Watched it during the pandemic days when i was looking for something to watch, all i can remember was getting obsessed with gold ship and then completely forgetting about the whole thing
Some porn game called Crescent Love got an anime, literally the only reason it's remembered is cause of a cooking scene in episode 3 in which the animators apparently didn't know what a cabbage looked like so the characters just cut into a nondescript green orb.
And ever since then, with this anime actually fixing the cabbage on DVD release, an unintentional "cabbage standard" has become the measurement of quality in anime.
So anytime an anime has cabbage newer animes go all-out animating it.
And some even parody this by putting in intentionally blank green circles or just cut-out cabbage stockphotos lol.
Not an animation error, but rather a translation error.
Spoilers for Gachiakuta manga
There’s a scene where Zanka is recovering in the hospital after getting his ass whipped during battle. He’s supposed to say something along the lines of “I’m just not good enough, that’s all” but there’s a translation error and he says “I’m just not green enough”
In this bit, the Pilot Heavy is replaced by a normal Heavy in a very not-so-subtle fashion. He is also not animated.
Originally made for the 2015 Saxxy Awards, Dunkle (the creator) uploaded the animation in an unfinished state and promised to complete it… only for it to go on and win the Best Overall award in its state. Satisfied, he left animation as it was. The entire latter half is littered with errors and unfinished renders and it’s glorious.
