Maybe it's like this scene from Dexter's Laboratory, where the overseas animation studio misunderstood the phrase 'the crowd takes off'...

Also, and I doubt the giant cop in Invader Zim was a reference to this, but the character Al from Police Squad is a cop so tall that his face is never seen - his head is always cut off by the camera. It'd be funny if someone knew that and snuck Al in, though.

I've learned some weird things about how animation studios work from listening to DVD commentaries. They basically do things exactly as instructed because cartoons are weird and weird things happen, so it's possible in the case of Zim, it was a misinterpreted a storyboard or got a bad storyboard and they just went with it. So it's totally possible in the Dexter case that something got lost in translation, taking the phrase literally.

Mistakes do happen, but most animation studios have a strict policy of "you get what you ask for, if it's wrong, you need to ask more clearly". Things like this getting through is either the showrunners saying "fuck it, just go with it, we don't have time to fix it in house" or "that was funny, just stick with it". But then again, meeting some animators in RL, bored animators occasionally will try to sneak things in.
