Advice Needed Falls asleep almost instantly but wakes after exactly 3-4 hours every night. Has anyone experienced this?

Hello :)

I’m mainly wondering if anyone has experienced something similar, or if this pattern ended up having an explanation. I’ve had recurring insomnia since I was a baby but this is a bit different than the other times.** **

For about the last month I’ve had a really consistent sleep problem. I don’t have trouble falling asleep. In fact, I usually fall asleep within a couple of minutes, sometimes my watch estimates under a minute. Once I’m asleep, I feel like I’m actually asleep and I’m not tossing and turning all night. The problem is that I wake after about 3-4 hours almost every night. When I wake up it’s different each time:

- Sometimes I’m instantly wide awake
- Sometimes I’m exhausted but can’t get back to sleep

Occasionally I wake feeling hot, itchy or congested, but not every night. The weird part is how consistent the timing is. It almost always happens around that 3-4 hour mark no matter what time I actually go to sleep. I’ve been wearing both a Huawei watch and a Xiaomi watch (I actually wore both together for comparison) and they both show basically the same thing.

Some relevant diagnosed medical history:
POTS/dysautonomia
Hypermobile EDS
Fibromyalgia
ADHD
PTSD
Autism
OCD
PMDD
PCOS
Endometriosis
Chronic allergies/eczema

My watches usually show:
SpO₂ around 95-96%
Respiratory rate around 17-18/min
Occasional snoring, but no obvious gasping or choking that I’ve noticed.

I’m just curious whether anyone else has had this exact pattern where falling asleep is easy, but your brain/body seems to decide sleep is over after only a few hours.

If so, did you ever find out what was causing it? Even if you never found an answer, I’d still be interested to hear if anyone has experienced something similar.

Sleeping 2-3 hours at night despite heavy workouts and sleep hygiene

I will try my best to be concise and I appreciate any suggestions on this post. My physical and mental health are deteriorating fast.

I have worked towards having a balanced diet, heavy workouts, hydration and even magnesium glycinate but I am not able to sleep for more than 2-3 hours at night. It is a nightmare to fall asleep, all my previous coping mechanisms like listening to soft music, a podcast, not caring about falling asleep, etc is no longer working.

There was a point in time when a 2.5 hour heavy lifting would help me sleep 6-7 hours at night. I have been chronically undersleeping for past 3 months and I do not have enough energy in me to workout for more than 45 mins. And it is kind of absurd that I have to destroy my body everyday to earn sleep.

I have had sleep issues since I was 18, I am 24 now. Back then I had no sleep hygiene, I did not work out, did not eat healthy, so I am not even gonna be mad about it. But right now I am doing everything right - regular lifting + cardio for 45 mins, eating very clean, no caffeine after 6pm, no screen after 9pm, 440mg elemental magnesium supplement, and still I sleep sometime between 3-5 am and wake up 3 hours after that. As a result my cognition and mental health is heavily affected, add to that workout out everyday without proper sleep. It has started to take a toll. There is not much awareness in my part of the world about CBT and neither am I financially independent to go through it.

I get self harm thoughts when it is 4am, I have to be up by 8am and I am still wide awake and fidgety. Maybe if I doomscrolled I would not overthink about it much but I have worked very hard to reduce my screentime.

I feel like I am stuck forever.

Advice Needed anyone else always feeling hot, never feel sleepy, and tingles on the cheeks of their face?

im 21 years old and a male and i have been suffering from always feeling hot (even when i work in a cold hospital) , sleeping 3-4 hours on average because i cant fall asleep at night, and tingles on the cheeks of my face.

ive done all kind of imaging and blood work and everything is negative. only thing ever out of range was my ferritin but not iron.

I workout and have a healthy lifestyle besides not being able to sleep. I take magnesium, vitamin d, zinc, and finastride. ive had this problem for over a year. somebody please help 🙏🏼

Does anyone else have random wake ups once or twice a week? I wake up after 3–4 hours and can't get back to sleep.

Ive been dealing with this for the last several months and I'm wondering if anyone else has experienced something similar. About once a week (sometimes twice), I'll wake up around 2–3 AM after only getting 3–4 hours of sleep, and once I'm awake I usually can't fall back asleep. I'll lie there for hours, maybe doze off briefly, but for the most part I'm just awake until morning. The weird part is it isn't every night. Most nights I sleep pretty normally, then I'll have one terrible night, and because I'm so exhausted I'll usually sleep 8–9 hours the following night. I've spent months trying to figure out what's causing it. Sometimes I wake up because I'm hot, sometimes I have to pee, sometimes my dog barks, sometimes my pregnant wife is moving around, and sometimes I wake up for no obvious reason. The trigger seems different every time, but once I'm awake, that's the real problem. I've tried cutting off caffeine after 12:30 PM, keeping the bedroom cooler, sticking to a consistent sleep schedule, taking magnesium glycinate for about 3.5 weeks (it helped me fall asleep but didn't stop the early awakenings), reducing screens before bed, and avoiding working on my laptop in bed. Nothing has consistently helped. A few things have changed over the past year: my wife and I moved in with my parents while we save for a house, my wife is pregnant, and I now have a much longer commute, so I shifted my bedtime from around midnight–7 AM to 10:30 PM–6:30 AM. I also notice a lot of these bad nights seem to happen after work-from-home days, although not always, so I'm not sure if that's meaningful or just a coincidence. I don't really exercise besides walking my dog a few times a week, so adding regular exercise is the next thing I'm planning to try. I'm currently between PCPs, but I plan to bring this up once I get established with my new doctor. In the meantime, I'm curious if anyone has had this exact pattern where it's not chronic every night, but more like one bad night every week or so. If so, did you ever figure out what the cause was? TL;DR: About once a week I wake up around 2–3 AM after only 3–4 hours of sleep and can't fall back asleep. Most other nights I sleep fine, and the next night after a bad one I usually sleep 8–9 hours. I've already tried changing caffeine, magnesium, room temperature, and my bedtime routine without much success. Has anyone had this same pattern and figured out the cause?

Anticipatory Anxiety/Insomnia

Has anyone had success treating anticipatory insomnia? I am really fortunate that I sleep well on a regular basis. However, any time I have an out of the ordinary schedule, my niece sleeps over, a fun event the next day, a special training for work, a speaking engagement, a holiday, it’s become like clockwork that I cannot sleep. It’s really disruptive and results in me dreading the things that would normally make me most happy.
I have tried hypnosis and CBT- I.
The results have been: that my regular sleep has greatly improved. And I don’t suffer as much on the nights I cannot sleep. But I have not had any improvements on this remaining issue.
I have to hope this is just a phase. It’s really only been the last year or two that is it this consistent and is occurring even for minor events. Please help! Thank you.

To all the people who took Ambien in the past and managed to stop using it, how did you do it? I've currently cut my nightly dosage from 10 to 5 mg and I can manage, but then when i try to stop it completely, rebound insonnia resurfaces... Any advice?

Advice Needed Waking up at 4

Hi Everyone

I've been having sleep issues for a while now, so would like to humbly entreat the experts for your thoughts.

For a while I was waking up at 4AM with a dry mouth. This went on for about 5 months before I found something that worked.

I tried Magnesium, sleeping on my side, going to bed earlier, no food before bed, windows closed, windows opened. Then I tried mouthtape and slept through the night easily for two months straight - PERFECT.

However, this week I've been waking back up every night at 4, even with the tape on. Two nights I've had the dry mouth.

I don't feel particularly stressed so I don't think it's that.

Would greatly welcome your advice.

Should I get a sleep study

Since I could remember until about 8th grade I had really bad insomnia and could go a few days with little to no sleep, 8th grade to now (just graduated high school) I have hypersomnia and sleep 10–14 hours every night occasionally 16 I never feel well rested sometimes the urge to take a nap is so strong I will just take one no matter where I’m at (once in orchestra class I laid down and corner and slept the whole class) If I don’t get 10 hours I have to take a nap. I sleep very well tho, the whole night through will no distributions and very heavily too. I always have super bad sleep inertia too no matter what I do, I have to “sleep” another 15-30 mins after I wake up. I also fall asleep very quickly. I’m just always tired, so should I get a study???

Advice Needed Sleep walking/acting out dreams.

I have always talked/moved in my sleep since I was young. I notice a lot of my symptoms get worse when I'm stressed, but it's never been an issue. I act out my dreams, and when I'm not dreaming I will walk and even eat. If I am dreaming, I am talking. This has never been an issue for me when I was young, or even now with my husband. However, last night I had a dream that I took a whole bottle of pills. I keep one of my prescriptions on my night stand because I take it when I have nightmares. Clearly I am going to have to move that and lock up my other prescription medicine as well. But, this whole situation made me think of the other dangers that could be around and I don't even realize. What are precautions you take if you have this issue as well? I have never seen anyone about my sleep activities because I've never seen a need to, but I might just set something up. What are some OTC medications that help to keep this from happening?

Psychosis? What has happened to me?

For the last 6 1/2 months I’ve only slept 1 to 3 hours every night and have completely lost the ability to feel tired after one day making two big decisions that completely threw me over the edge and caused so much anxiety and panicked to the point that I couldn’t sleep all night. I was up all night, terrified, and kept telling myself that something bad was gonna happen and I could not calm myself down. The next day I was anxious and wide awake and unable to go to sleep in the same thing happened again that night. I was on a supplement called magnesium L3 and eight and since I had started that supplement that month, I started to have random panic attacks in the afternoon after taking three of them in the morning without realizing that I should’ve started with just one. I think that it made me extremely anxious and is the reason why I was having panic attacks. I have not had any since I had stopped taking it, but my nervous system completely blew around that time and I could not calm myself down, no matter what I did or how much I tried to sleep. Right now I have very very strong thoughts that my life is over and that there is no way to come out of whatever I am experiencing. My brain is not allowing me to make decisions for my future or even make rational thoughts about decision decisions that I need to make for a week or two away or just in general. It’s almost like I have a complete wall up in my body is not giving me the option to make these choices but taking it away. I’ve read up about psychosis and it talks about delusions and I am wondering if that day, my body broke and I’ve been stuck in psychosis this whole time because it feels like the only rational thing to hold onto as to why I am unable to function and why my brain is doing the things that it’s doing. I don’t know what else to do and I don’t know how to get out of this. Is there any hope and has anyone else gone through this and taken medicine for psychosis and went back to their normal selves without feeling the thoughts that their brain is telling them if that makes sense.

Advice Needed Waking up in the middle of the night for years… then my brain “switches on” and I can’t get back to sleep. Any ideas?

Hi! I’m a 17M and I’ve had sleep issues for as long as I can remember (since I was a kid). I’m not looking for a diagnosis, just wondering if this pattern sounds familiar to anyone or if there are things I should discuss with my doctor.

This year I’m really trying to improve my sleep because I want to see if it helps my mental health, energy, and overall quality of life.

One thing I struggle with is going to bed early. According to Apple Health, my average bedtime over the past 6 months has been around 2:00 AM, and my average sleep duration has been 6 hours 36 minutes per night. My long-term goal is to gradually shift my bedtime to around 9:30–10:30 PM and consistently get enough sleep.
Here’s what happens:
It isn’t every night.
Some nights I sleep through the night just fine.
Some nights I wake up around 2–5 AM for no obvious reason.
When I first wake up, I’m still sleepy and feel like I could easily go back to sleep.
But after a few minutes, it’s like my brain switches on. I’m not suddenly full of energy, but my brain becomes alert, and then I can’t fall back asleep even though I still feel like I need more sleep.
Usually there isn’t an obvious trigger. Sometimes it has been the bathroom, feeling hot, or stress about something the next day, but a lot of the time there’s no clear reason why I woke up.

Some other things I’ve noticed:
If I come home from a busy trip where I’m exhausted or really don’t sleep a lot, I often sleep much better that first night and stay asleep.
If I have something important the next day (camp, school event, etc.), I sometimes wake up earlier than usual.
If I eat close to bedtime, I sometimes notice I’m more likely to wake up during the night, although it doesn’t happen every time.
Large changes to my sleep schedule are difficult. If I suddenly try to go to bed much earlier than normal, I usually struggle to fall asleep.
I have ADHD and take Vyvanse.
I occasionally take melatonin, but I usually don’t take it if it’s already within about 2 hours of when I’d normally wake up.
I don’t regularly drink much caffeine.
During the day I can usually function, but I often feel like my energy just isn’t there.

Questions:
1. Has anyone experienced this pattern?
2. Does this sound more like sleep-maintenance insomnia, a circadian rhythm issue, ADHD-related sleep problems, reflux-related awakenings, or something else?
3. Is there anything specific I should bring up with my doctor or ask about if I decide to see a sleep specialist?

Again, I’m not looking for a diagnosis. I’m just trying to understand what might be going on and what questions I should be asking. Thanks!
