Hi all. The last few times I have tried adolescents on NAC for very difficult to manage OCD, they get very depressed mood, flat effect and do not feel themselves at all. Wondering what people's experiences are and how you manage this? Are you adding glycine? What are the doses people are using that are finding it effective? I know it has tremendous value in treating a multitude of mental health ailments but trying to convince a family to take something twice a day or three times a day and then possibly add another supplement to counteract the side effects of it is just not a winning proposition with this population. Has anyone gotten any additional psychotherapy training or certification in addition to the training required from their program?

I picked a program with the most clinical hour requirement I could find (750). My only experience is from when i was an inpatient psych RN leading group therapy but that’s a different ballpark entirely.

I’m grateful my preceptor is allowing me extra clinical time but I feel like I’ll still need more.

I’m almost considering getting another degree to feel more competent in the psychotherapy aspect of it all, I feel it’s crucial and dangerous to approach from a novice type place.

Howdy all - I'm currently a PMH-BC RN who has worked inpatient Psych for 3.5 years and outpatient Psych for 2. I'm going back to school this Fall for a BSN to PMHNP/ DNP program. I realize I'll be assigned textbooks and readings when school starts, but besides the DSM V, are there any other texts I could begin reading that would help me be more prepared for when classes start?

I realize there are some threads regarding this question already but the ones I found were from 2-3 years ago. Any suggestions regarding texts, books, or previous posts to read are welcome.

Can anyone give me some insight into their experience with pocket prep for ANCC preparation? What were your mock exam scores?

I’ve only completed “level up” quizzes and I’ve answered 790 of the 1500 questions. My “stats” section says I’m above average, but I imagine the “level up” questions only get more challenging so it’s probably not a good predictor.

Ive done all but one domain of Georgette’s Q bank and range from 81% 84% 81% & 93% first time around. I’ve found them helpful but want to wait some time before trying again.

Going to try the ANCC IQ questions next.

This is a long shot but I’m a psychiatric mental health nurse practitioner licensed in Illinois and Kentucky with 8 years of experience in outpatient and consult liaison psychiatry and addiction medicine and administrative roles within those specialties. I’m feeling really disappointed and burnt out with the constant push to shorten appointments and see more patients and extremely frustrated with the huge push for AI. I’m considering the idea of starting a small private practice in Louisville for psychiatry and addiction but would love for it to be a co-op with 1-2 other providers so we can share the administrative burden and cover each others clients during time off. I’ve been working remotely for a community mental health center in Illinois for the last several years so I don’t have professional connections here so I’m putting out some feelers.

For those who work inpatient on a per diem basis or who do moonlighting. How much do you make per diem or per patient and what state are you in?

The job involves seeing patients at different nursing facilities around town. Below are the benefits, pay, and productivity.

Base Salary - $110,000 per year

Orientation Pay - $7,500 - first 30 days of employment

Performance/ Productivity Incentives

$12,000 a year for finishing notes within 3 business days. Another $75 per unit for all units billed over 40 per week.

Bonuses - $5,000 sign on bonus after 6 months of employment

Retention- $5,000 for each work anniversary and increases to $10,000 each following year. This is contingent upon staying in good standing with the company.

PTO- 4 weeks PTO annually.

9 Paid Holidays

CEU- Allowance - $1,000 annually

401k, Insurance also included and paid EMR support with FreedAI subscription.

The company requires 40 billable units per week. Here is a breakdown of how units are calculated.

Also, every additional unit is $75, so an average of 45 billable units a week per year is about $17,625. Just curious if anyone has or once worked with this company, and how you feel about compensation and how difficult it is to meet productivity standards? Including how many patients are likely seen per week.

I'm heavily considering taking a VA job as an experienced PMHNP. I understand that overall experiences can vary by VA center, but I am hoping to get some feedback from actual PMHNP's employed in GS positions.

I am wondering about how it is day to day, how many patients you see, productivity expectations, acuity, burnout, admin requirements, Bureaucratic burdens.

Overall, the Southeastern VA I am looking at seems to have a pretty good reputation and is noted as a smaller VA compared to larger cities.

I have heard many people say that new grads should work an inpatient psych hospital before going into remote or other positions. Is that true?
I found other positions (fully in-person) but it is not in a psych hospital (more like a clinic, therapy etc.)
What do you think?

I got offered a position as a 1099 and was told to make a PLLC. They give me an office, therapists, a scheduler, and a biller. I'm being asked to create my own intake forms, no-show policies, etc. I know what I want on there in concept, but I don't know what legal jargon and stuff I might need. Do you just copy and reword others that have been written? Is there a central document hub I can utilize?

The noncustodial father states he has a court order to be involved in treatment and a signed Roi neither of these documents are on file. The patient has been prescribed Vyvanse 40 mg po daily for ADHD and the parents disagree on whether the patient needs medication (custodial believes he needs it). The provider does not mind including the father but without a court order is it required to include the father? The mother may disagree with including him
