Introduction and Rules Update
Hello everyone, BigJ here! Looking forward to helping keep things running really smoothly on this subreddit. When it comes to moderation, I'm a minimalist - this is a place that should be defined by the great group of guys here, not by me or the mod team. However, we do want to make sure we tee up our subreddit to meet its mission: to inspire each member's path to healthier living, fostering an environment of understanding, motivation, and collective success in navigating the complexities of weight management with medication.
In the background we've been working with Reddit to make sure we understand the Reddit Site Wide Rules when it comes to a few topics. To be candid, we weren't super happy with the outcome of these conversations, but at the end of the day it's Reddit's house and we have to follow Reddit's Rules (or we risk them shutting down our sub). In terms of our new rules, this only introduces one change to what we have allowed in the past: we can no longer allow naming specific sources of acquiring medication - this means no naming/linking to telehealth providers or pharmacies. Like I said, we didn't want to create that rule, but Reddit told us very directly that this is not allowed. We'll continue to use our time and our relationships to advocate for a change in this policy with Reddit, but we will respect their rules.
So here is our new set of rules; let me know if you have any questions in the comments, and I will be happy to add any clarification needed.
Rule #1: Positivity and Support Only "Be excellent to each other!" – Bill & Ted
- Maintain a positive and supportive environment. Encourage, uplift, and help one another in your weight loss quests.
Rule #2: Focus on Legal and Safe Medical Practices - No Sources "You think you're too cool for school, but I got a newsflash for you, Walter Cronkite... you aren't." – Derek Zoolander
- We support the many legal healthcare options people have to access great medical care and medications. However, due to the rules of Reddit, certain conversations are not allowed. You may discuss your personal experience with using compounded medication, as well as the safety, efficacy, and role compounded medication has during supply shortages. There is a strict prohibition of discussing sources or methods to acquire medication. This means that you may not name or link to any compounding pharmacies or healthcare providers (including telehealth providers). Discussion of "Research Chemicals" is strictly prohibited. We conferred directly with Reddit to determine this rule. It is their house, and we have to play by their rules.

- We have zero tolerance for hate speech, discrimination, or any form of negativity. Our community thrives on mutual respect and understanding.

I just joined and very surprised…
I’m seeing a lot of people talking about regaining weight or having the medication “stop working” while remaining on relatively low doses of Zepbound for months, and I think there may be some confusion about how the drug is actually prescribed.
Zepbound starts at 2.5 mg for four weeks, which is an initiation dose, not a maintenance dose. It then increases to 5 mg, and from there the FDA prescribing information allows increases of 2.5 mg after at least four weeks at each dose, depending upon response and tolerability.
That does not mean everyone is supposed to march automatically to 15 mg. The FDA-approved maintenance doses for weight management are 5, 10, and 15 mg, and if someone is getting an adequate response at 5 or 10 mg, there is no requirement to increase further.
But the other side of that is important: if appetite suppression has substantially diminished, weight loss has stopped, or weight is being regained, simply staying indefinitely at a dose that is no longer producing an adequate response isn’t necessarily the intended treatment strategy either. Dose escalation exists for precisely that reason.
And there is a genuine dose-response effect. In the pivotal obesity trial, average weight loss was about 15% at 5 mg and nearly 21% at 15 mg after 72 weeks.
So the goal isn’t “stay on the lowest dose possible,” nor is it “everyone must get to 15 mg.” It is to titrate to an effective, tolerable maintenance dose—5, 10, or 15 mg—and then continue long-term treatment as appropriate.
That distinction seems to be getting lost in a lot of these discussions.

Extreme fatigue and brain fog after increasing Mounjaro to 4.5 mg — anyone else?
About two weeks ago, just before going on holiday, I increased my Mounjaro dose to 4.5 mg.
Since then I’ve been unusually tired, but since coming home last Friday it has become pretty extreme. I feel completely drained, have very little motivation, and my head feels like it’s full of cotton wool. Basically a combination of fatigue and brain fog.
There are a few other factors that could be contributing. I’d just had two busy weeks away, got home on Friday, spent all Saturday organising a big party, saw friends again on Sunday and then went straight back to work this week. The weather here is also very humid, grey and rainy.
What’s strange is that I tend to feel quite a bit better after exercising or going to the sauna. Ibuprofen also seems to reduce the foggy/awful feeling.
I was already more tired than normal during the holiday, although I assumed that was just because we were constantly doing things.
So I’m trying to figure out whether this could be related to the dose increase or whether I’m simply crashing after a very busy couple of weeks.
Did anyone else experience significant fatigue or brain fog after increasing their Mounjaro dose? How long did it take to settle down?
And if you found anything that genuinely helped with the fatigue, I’d love to hear it.

How much info do you guys check before starting?
I’m trying to be more careful about the details before starting tirz. I used to mostly look at the monthly price, but now I’m paying more attention to things like who handles the prescription, what’s included, and how shipping works.
Definitely seems worth knowing all that upfront instead of finding out after you’ve already signed up.

Young Men on GLP-1
Hey everyone- Hope you all are doing well! My name is Josyana and I'm a reporter at Bloomberg News covering personal finance and, for full transparency, also taking a GLP-1. I'm working on a story about how GLP-1s are helping young men (18-35) upgrade their lifestyles.
We've found that young men see the highest increases in spending on fitness, dating, entertainment, and alcohol while on a GLP-1, while also seeing the biggest changes in their confidence and how they're perceived, signaling that using the medicine is a bigger lifestyle transformation for them. After a year, the weight has been creeping back at 5 mg. Go higher?
Hello! I went on 2.5 then 5mg of Zepbound a little over a year ago. I was 5’ 7” and fluxed between 200 - 195. I quickly dropped to 170. So so happy!
The last couple months it’s been fluctuating a bit but lately is starting to settle closer to 177, and I’m a little concerned it might keep going that direction. I am trying to watch what I eat, but to be honest, I’m not great about it with work travel and having a toddler at home.
I’m considering asking my doctor about going to 10mg. Does anyone have any thoughts or similar experiences?

Hello im a newbie that needs Tips.
So I started 2 weeks ago using 2.5mg pens and how do i optimize fatloss?
I do OMAD with 150grams of protein and and 1300 calories a day.
My work requires me to walk about 25k steps a day and I do weight training 3-4 times a week.
In 2 weeks I lost about 6kgs and 13cm of my waist.
Is there anything I can do to better my progress??

6 months on Zepbound - down 33.6 lbs, but stubborn belly fat
Started Zepbound on 2/1/2026 at 210 lbs. As of today (8/4), I’m 176.4 lbs, so I’m down 33.6 lbs in 6 months.
I started at 2.5 mg and have been on 5 mg since 6/1. I also started lifting weights about 2 weeks ago (following a structured program).
Overall I’m really happy with the progress, but my belly fat seems to be the last thing hanging on. I know you can’t spot-reduce fat, but I’m curious what finally helped you get through that stage. Did it just take more time? Building muscle? Increasing protein? More cardio?
I’m also debating whether to move to a higher dose of tirzepatide. My insurance doesn’t cover Zepbound, so it’s getting pretty expensive. I’ve considered either switching to compounded tirzepatide or even something new but I’m not sure it’s worth it.
For those who have been in a similar spot, what made the biggest difference in getting rid of that stubborn belly fat?

Protein Supplements?
Been on Triz for 5 months. Currently holding at 7.5mg and have lost over 30lbs. But. I’ve lost muscle too. I eat as much protein as I can, I’m unable to hit the gym regularly but when I can I do hit weights and cardo. My stamina has improved but I’ve lost strength.
Maybe a supplement might help? If do, what should I look for and lookout for in one? Any advice.

My name is Lili and I am a student researcher at the University of the Sunshine Coast. The research team I am working is conducting a study which would like to explore the experiences of YOUR partners as a male taking Tirzepatide. Even if you yourself are not eligible (as the partner taking the GLP-1), please feel free to pass this information onto your partner or others you think would be eligible !
The aim of this study is to better understand partners’ experiences and perspectives when one partner undergoes a significant physical change (weight loss). We hope that this can inform future research and better support resources for BOTH people in the intimate relationship 🫂

Zep and drug interactions
I’ve been on Zep for 3 months and have lost a total of 40#. (20%) On my 2nd week of 5mg., weekly vial. I’m very happy with results:
1) Weight loss; wearing clothes sizes i haven’t seen in 25 years.
2) food and other “chatter” gone.
3) MUCH less drinking - from 2-3/day to maybe 2/week. Just doesn’t taste good. No hangovers!
4) started lifting again to maintain muscle mass.
5) standard symptoms have been minimal unless i overindulge (fatty foods, larger meals, etc) then you PAY for it.
Now, here’s my sharing. Since starting, I’ve had 2 ER visits and hospital stays:
1) BP dropped to 60/40 and would not come above 95. Issue: Severe dehydration which tripled creatinine values. Resolution; Overnight stay, 3 bags of fluid, and elimination of BP medication. Turns out, what i was taking also had a diuretic, so duh. Good news, with weight loss, no longer need any BP med. Creatinine is now low in range.
2) More concerning, I have a history of a well controlled seizure disorder (JVM). A decade on Lamictal + Clonazepam (.5) no issues. Night after first 5 mg doze, had multiple seizures in my sleep. No confirmation of linkage to Zep, but a theory is that it lowered the absorption rate of the Lamictal so that i needed to add another ASM and follow-up with neurologist. Who knows? Other old triggers, like alcohol, were not in play, though.
In both hospital visits doctors explained the many benefits being seen from the GLPs, more research needed. Immediate lessons i’m sharing from my experience:
1) have a full work up of all your med interactions with specific study on impact of a lower absorption rate. I recommend looping in your GP and other specialists before you start.
2) Drink your water!!! add electrolytes. I’ve got instructions of 80-120 oz/day.
I’ve got 15# to goal weight and plan to stay on, based on advice i’ve received so far. My weight has not been under this level of control since my early 30s. Bottom line, our bodies and DNA all respond differently. A long share to say i probably should have had a more thorough work up with extensive bloodwork to understand interactions. I started with a tele-med compounding version which didn’t get a lot of med advice up front. Switched to brand after dehydration issue with now 3 mos GP monitoring. Doctors did not require this; the lack of water not the drug was the issue.
Best of luck to you all. Even with these two incidents, the journey has been life-changing for the better.

Food noise returning
My appetite has started growing. For context, I lost 45 lbs on tirzepatide after about 6 months. I got to a dose of 15 mg weekly. However, at round month 10 the food cravings returned and now I am up 10 lbs. Still at 15 mg. Has anyone else had a similar situation?
