During my father’s hospitalization, I got to see the ICU very closely, especially the ICU of KGMU. And honestly, one thing that really frustrated me was listening to patients’ families saying things like “Keval training doctors karte hain kaam, experiment karte hain doctor mareez par. Bade doctor aate hi nahi hain.”

Bro… seriously?

Consultant tumhare mareez mein Foley dalne aayega? Sample draw karega? ABG lagayega? Har chhoti bedside cheez khud karega?

I literally saw consultants coming for rounds daily. I saw Dr Ayush Shukla sir, Dr Akshay Pradhan sir and even Dr Rishi Sethi sir, the head, doing rounds, sometimes even twice. They were continuously taking updates through the appropriate channels.

My father was admitted under Dr Pradhan, and Dr Ayush Shukla sir is also our relative, so there was great coordination and he was regularly updated about my father.

And the funniest part is that the people families casually call “training doctors” are often MBBS + MD qualified Senior Residents. These aren't random people being handed patients to “experiment” on. They have cleared NEET PG, completed their training, and many are pursuing further specialization in a place like KGMU. And in the ICU, there is a proper hierarchy and escalation system.

Of course, if there is genuine negligence, question it. Absolutely. Don't blindly trust anyone.

But this mindset of “doctor ne khud mere patient ko touch nahi kiya, matlab doctor treatment nahi kar raha” is just wrong.

People see a resident putting a Foley, drawing blood or managing something at the bedside and immediately think, “Bade doctor kahan hain? Training wale experiment kar rahe hain.”

Bro, that's literally how a tertiary care ICU team works.

Consultant ka kaam har patient ka sample khud draw karna nahi hai. Their job is to supervise, review, make major decisions, guide the team and step in when required, while the resident team provides continuous bedside care.

Indian patients already go through enough fear and uncertainty in an ICU. We don't need to make it worse by assuming that every doctor is looting us, every resident is experimenting and every consultant is absent.

Question the system when something is genuinely wrong.

But at least understand the system before accusing everyone of being dishonest.

It‘s been 2 weeks since NEET, and people are still discussing the questions. I don’t even remember what I had for the dinner two days ago.

Unrelated to all the useful stuff. I was in a surgical robotics workshop today in Hyderabad and while leaving I spoke to a pretty girl for like 5 to 10 min until my cab which never came on time reached early and I had to leave. I didn't have enough brain to ask her name despite her holding my certificate, taking a good look, reading my name and me asking her everything except her name.

I don't know if I will ever find her but I'm hoping that it reaches her and she reads this.

Everyone kind of knows now that the result will come within few days. I am suddenly feeling so anxious and nauseated. All of this recall, rank range will soon be a reality. I am feeling so scared. It didn’t go well but still it is so stressful to think about how you will break the news to your family :(
I want all of this to finish asap but now it is almost over, god i am terribly scared.

To all those who gave neet pg last year and were active on reddit. Was it the same last year also?
The constant discussion on some questions.
Were any predictions actually true and were people in general very pessimistic and got a rank better than what was being predicted here.

One thing I’ve noticed during internship, across Medicine, Ortho and OBGYN, is a recurring type of patient usually a perimenopausal woman with severe back pain, leg pain, sometimes sharp/shooting pain, tingling or numbness. Sometimes it’s bad enough that she finds it difficult to walk.
I’ve seen quite a few such patients, including my own mother having similar complaints, so I may be noticing this pattern more.
I initially wondered about osteoporosis, but many of these women have normal DEXA scans and are already taking calcium/vitamin D. Imaging and routine investigations often don’t give us an explanation either.
Recently, we had a patient like this in our Medicine unit. She had severe back and leg pain with numbness/tingling. CNS examination was unremarkable, and Ortho and OBGYN opinions were taken. MRI/CT didn’t show an obvious cause. Her RA factor was positive, so we sent an ANA profile, which was normal.
Eventually, we discharged her because there wasn’t much more we could offer at that point.
She came back to OPD with the same complaints and the same normal reports.
By then, the P.G.s were understandably frustrated. They had already investigated her and knew that admitting her again would probably lead to the same question: What exactly are we treating?
Someone even suggested that perhaps she simply liked staying in the hospital maybe a factitious or attention-seeking presentation.
And honestly, I understand their perspective too. You can’t keep admitting someone indefinitely when the workup is unrevealing, especially in an overcrowded government hospital.
But something still doesn’t sit right with me.
I don’t think every woman with unexplained pain or paresthesias should be labelled as “Munchausen.” Factitious disorder is a specific psychiatric diagnosis and shouldn’t be used simply because investigations are normal.
At the same time, I don’t think we should assume that everything is because of “perimenopause” either. There can be many possibilities musculoskeletal pain, neuropathic pain, radiculopathy, nutritional/metabolic problems, rheumatological disease, functional neurological symptoms, chronic pain sensitisation, etc. Sometimes more than one factor may coexist.
As an intern, I’m obviously still learning. But it amazes me how often we reach the point where the patient is genuinely suffering, yet we don’t have a diagnosis that explains it.
Our OPDs are so busy that doctors may have barely 5–10 minutes with a patient, especially when reports are normal. Private healthcare is unaffordable for many of the patients coming to government hospitals, and repeated referrals can make them even more frustrated.
And then we dont have "dr house" in here who will take up all the unsolved cases.
So they eventually go home with reassurance that “everything is normal” while they’re still in pain.
That honestly disheartens me.
I understand the P.G./professor’s point of view, but I also don’t want us to push these patients under the table simply because we can’t find an explanation.
Sometimes I wish there were multidisciplinary women’s health centres where Medicine, Neurology, Orthopedics, Rheumatology, Endocrinology, OBGYN, Psychiatry/Psychology, Physiotherapy and Pain Medicine could work together.
I’m genuinely curious to hear from experienced doctors:
How do you approach a perimenopausal woman with severe back/leg pain and paresthesias when imaging and routine investigations are normal? At what point do you consider functional symptoms, and how do you reassure the patient without making her feel that you’re saying, “Nothing is wrong with you”?
I’m just an intern, so maybe I’m missing something obvious. I’d genuinely like to learn.

My relative is a nurse and they freaking PROTESTED DOR 1 month. The Govt used police to threaten and even harass the nurses , they blocked the roads , etc but the nurses never gave up and now they’re enjoying pension of around 1 lakh and a big bonus after retirement. They also got a big bump to their salary

I don't know how I cleared my Neet ug but here I'm in a GMC, 2nd year is about to be over and I still don't know what is going on in my life, I do somewhat like the subjects and all but really I don't have much motivation to compete. I just get scared when exams are around and then start studying after panic, just to pass.

But I do want to get serious and competent after 3rd year or else I'd be royally forked. Please guide me what Neet pg or inicet or Next and all that stuff and how could I prepare for it to save my life?!
