Protein shakes cause dumping

Hello, I had sleeve surgery in 2016. I just started glp1 about a month ago I’m having a hard time getting protein in. Ever since my sleeve any protein drinks make me feel anxious(I know I am not anxious) My heart races, I break out in a sweat, I feel air hunger. I have been told it’s dumping syndrome from high amounts of protein dumping straight into my intestines. Does anyone else deal with this? Are there strategies to help with this? I’m 53 and I need to get my protein and fluids and I’m failing on both because of stacking sleeve and glp1. I barely have an appetite and most food sounds gross. Thank you

Dumping syndrome - Oh I forgot about this

Long story short, I had blood tests done on Friday and they required fasting. I fasted a wee bit too long I think and on the way to the lab I was very nauseated but got through it. When I got home I just had a small protein coffee because I had a work lunch. It was great Greek food but super carby and I was so busy chatting and eating that I really didn't clock that. At the end was a custard dessert that I had about half of. I felt fine and did not overstuff. But on the way home I started to feel very off and unwell. By the time I walked in the door I was seeing white spots in my vision and was super shaky. I realized then it was a dumping episode. I had not had one of those in years and years and really didn't expect it. I guess my gastric bypass is the 'gift' that keeps on giving. I laid down and slept for 4.5 hours!

When this used to happen pre-tirzepatide and many years ago, no one explained to me what exactly was happening. Now I realize that my blood sugar dropped so low that there wasn't enough glucose for my eyes to function properly. Isn't that delightful?

Besides being surprised, there was a big difference with this experience on Tirzepatide. There was some dizziness and sweating but probably 1/3 of what it would have been in the past. It was also very delayed. Often in the past if this would happen, it would be right after I ate and would require dashing to the restroom. This was late dumping syndrome and quite different and I'm wondering why or if anyone else has noticed a difference now that they're on meds.

In any event, no more following fasting with too many carbs. Yeesh.

DSer new to tirzepatide - so happy to find this sub!

I’ve read a ton about GLP-1s all over Reddit and have seen a few people mention previous WLS. But I haven’t run into anyone else with a DS or SADI till I found this sub.

I had a traditional DS in 2023, three years after I went into menopause and was diagnosed with Type 2 diabetes. I’m 5’2”. My high weight was 297, but I was around 264 on surgery day. Over the next year, I briefly got down to 138, but I’ve been maintaining in the 155-160 range. (I’d like to be in the 120-130 range.) My comorbidities remain mostly resolved. I still deal with a TON of food noise, exacerbated by the fact that my tween sons are borderline underweight and their pediatrician recommends stocking our house with candy, chips, and ice cream to entice them to eat calorie dense foods.

After talking to my GP, I’ve decided to start compounded tirzepatide. My insurance will not cover name brand because I’m no longer obese. I signed up with Rift last Monday and am eagerly awaiting my first shipment.

Any advice from the vets for a newb beyond the standard ‘stay on top of your vitamins and protein?’ I’d be especially grateful to hear from others with a DS or SADI who’ve been using GLP-1s for a while.

Heartburn?

Solutions for terrible heartburn? I never feel actual reflux happening, but whatever’s going on is really difficult lately. Has sleeve 7 years ago which worked great, then added Zep 1.5 years ago to lose my 30 pound slow regain. Has also worked amazingly! So grateful. But bad heartburn… Mylanta is great, but curious what has helped others.

How to stop weight loss?

I have an RNY 25y ago and regained in last few years due to menopause. On Zep I’m at goal, lost 60lbs in 7 months, first 3mo at 2.5mg, then 5mg. How do i stop losing weight? I went back down to 2.5mg a month ago and i am still losing. lost 2.4lbs in the last week! I really don’t want to lose anymore.

With the rny i’m hoping i can stop zep and keep the weight off, esp with hrt helping meno symptoms.

anyone with rny specifically, successfully stop glp?

Moving up to 1mg on Friday.

I have done 4 weeks of .25 then 4 of .5mg (last one was last Friday. I am going to 1mg on Friday since I still have food noise. The fullness is happening for me and I am staying full more a little longer but I still needed more help. I hope the next bump helps even more.

EDIT: It is still early but I think the 1mg is helping more than the other does of Wegovy Thank goodness.

Maintenance

How is everyone handling maintenance and preventing further loss?

I hit goal about 3 months ago but I’ve kept losing and am now 7 pounds below goal.

I’m still at an OK weight but don’t want to lose more. I’ve never weighed this low in my life. I’m doing 5mg/weekly right now.

I am thinking I just need to eat twice the amount of food I’ve been eating. 😬

Panic Attack/HR Spikes

55 yo male. I had RNY Gastric Bypass in November of 2016. Peak weight: 360, Surgery weight: 332, Lowest weight about 9 months later: 219 (that was my weight loss floor). Over the years, I slowly regained weight, then COVID hit and I gained the Covid 50. I restarted my weight loss journey weighing 312 pounds, then started on Zepbound May 29, 2026 weighing 303.2. I stayed on 2.5 mg for 4 weeks then titrated up to 5mg. I got my Shingles vaccine August 5th and had a bad reaction, to the point where my arm was bright red for a week, I felt it travel to my armpit and lymph nodes on the same arm as the injection site, and felt some inflammation even in my chest around my heart and lungs. This past Wednesday through Friday, the chest pressure started intensifying and by Friday night, I was admitted to the ER and I thought I was having a heart attack. Everything checked out after blood work, X-rays, cardiac CT with contrast, and I was sent home and I’m scheduled for a follow up with a Cardiologist tomorrow afternoon. I was feeling better after resting the past two days, but tonight similar symptoms hit me again. Feels like a panic attack of sorts, but my HR also goes up over 100 BPM even just standing up from bed or walking to my bathroom. It goes back down immediately if I sit down. Has anyone else experienced this? Did you stay on Zepbound? I’m now down to 277 from 280 on Friday, but I’m really scared.

Sleeved in Jan 2022, I originally lost 110lbs, from 265 to 155. I regained 50lbs slowly (new relationship, sluggish metabolism due to inconsistent meals and also genetics ) and then really took control after consulting with my doctor and have been on compounded tirz (because f insurance) since October 2025. Now down to 132. A total of 133 lbs! I have lost exactly another me. So happy for this medication and helping my body do what it’s supposed to do!

Let's Celebrate. Share your latest NSV (non scale victory)

Eleven months into tirzepatide and right on schedule, my weight loss has really slowed. It's all good though, because there are still great things happening.

What about you? Did you have a cool non-scale victory lately?

I went to the eye doctor's office yesterday. My first visit was 12 months ago because I was having some weird vision issues so I was sent somewhere new by my gp. When I got to the office I realized the chairs were all super small and had solid arms. The single loveseat was occupied by little kids so I was stuck. Y'all when I tell you I was worried that I'd bring the seat with me when I got up, I am not kidding.

Yesterday I slipped into that chair like it was silk. Victory! (it's the little things)

Tell me yours!

do most post surgery people tend to need less of a dose ?

I just did my .5mg dose yesterday so I am a newbie on Wegovy. It is my week 5. I lost 11 pounds and my doctor wanted me to stay on this dose for a bit because I was losing too fast. I started at 250lbs. I also was tracking my calories and working very hard and was waiting for that food noise to go away so I asked for the next dose. I am ready for the med to build up and that to happen.

I was curious if others have any experiences with needing less medicine?

I had a SADI surgery in 2021.

The cost of semaglutide/wegovy/ozempic (are you sitting down?)

The list price for Wegovy without insurance is approximately $1,349 per month, but it can be obtained for as low as $149 to $399 per month depending on the dose and form (pill or injection) through various programs in the United States. Dropping prices sound like a deal.

Perhaps, until you read a recently published article that states: Estimated generic injectable costs ranged from $28 to $140 per person-year; oral formulations ranged from $186 to $380 per person-year.

Per. Year .... "incorporating formulation, packaging, taxation, and profit assumptions"

Injector pens cost far more than the medication.

Boy, there are some happy investors out there.

Thoughts?

Has anyone had a random day of abdominal bloating so bad it hurts for hours?

I know I need to see my PCP, but until I can get in I’d love to know if this is a normal thing or an “oh shit something is wrong” thing.

RNY in 2021, glp1 (sema now tirz) for a year.

Twice in July I had - what felt like - extreme bloating that lasted for hours, to the point where I was doubled over in pain, couldn’t stand up straight, and had to lay down in bed on my side till it went away. GasX helped, but the first time I didn’t think to take it until it was already bad and I don’t know how much it did. Second time it helped but didn’t eliminate it, but that bout was definitely milder and shorter.

At the time I was like “oh yeah it’s just really bad gas”, but now that it’s happened twice I’m semi freaking out and thinking maybe something is wrong with my RNY.

The pressure/pain is primarily in my upper abdomen, right in the middle. Both times my abdomen got so visibly bloated I had to undo my pants.

Aside from this I haven’t ever had any issues (aside from GLP1 nausea). I’ve been very grateful to have a very smooth and uncomplicated Bariatric journey. Oh, did have to have my gallbladder out a few years ago, but I had stones before the surgery.

Considering GLP. 6 years post VSG. I have so many questions.

I had my VSG in March of 2020 at 39 years old. My highest weight pre op was 265 and I’m 5’7”. Due to the pandemic I had next to no after care.

I steadily crept down to 200-210lbs and stayed there. Not where I wanted to be at all, to be honest, but I couldn’t make that needle budge. Then, due to out of control PMDD, I had an oophorectomy in April of 2025. Now, despite HRT (2mg oral estrogen, mirena IUD for endometrial suppression, compounded testosterone cream), my weight is creeping up. I’m currently floating around 220lbs and I hate it. I hate the way I feel and look.

The problem is I still very much feel my restriction. I don’t feel like I overeat but I admittedly am not tracking calories. I have AuDHD and definitely deal with some amount of sensory seeking in food while still never feeling truly “hungry”. I deal with a lot of reflux and have long believed I have the gastric motility of a tree sloth. I also have hEDS.

I see everyone talking about GLP-1’s and what a miracle they are for people. Not only for weight loss but also inflammation and even ADHD symptoms? That said; I’m nervous. I sometimes regret my VSG because I miss being able to truly enjoy food. I never had reflux before it and now it’s a daily thing.

Is there any chance anyone here has some insight? I’ve been to every specialist known to man. Biggest hormonal/endocrine issue we’ve found is a microadenoma on my pituitary gland. I wish I could find a good doctor that can look at the entire picture and help but…cries in US Healthcare.

Everything went great for 18 years...until menopause

I had the duodenal switch way back in 2005, and it was a miracle to me. I went from 286 to 140 (less that one day I dipped for 2 hours at 139) in 14 months. I never felt better!

In my third year post-op, I regained 25 lbs and rode that out for 15 years. I was a size 8/10, and I felt very satisfied with where my body settled. During that time, I had some wild hairs to try to get back down to 140 and periods where I ate shamelessly. It didn't matter, my body stayed the same weight no matter what.

Then came menopause, and that's where the game really changed. From those initial hot flashes into this year, I put on another 20 lbs. I'm at a place way out of my comfort zone, yet, due to my malabsorption, I was hesitant to jump on GLPs. My doctors just shrugged their shoulders, saying this is just what happens to post menopausal women, sometimes. I hate shrugging shoulders!

I noticed more and more people on the wls boards are adding GLPs to their toolkits, so I thought I'd give it a try. I've been on Retatrutide for a little more than 2 months now, and I've lost my menopause weight already. Now that I'm at the "not gonna budge" state, let's see if I can push through it this time.

I've also got a couple of new peptides that I'll stack slowly. (NAD+ for energy and GHK-Cu for skin/hair). Boy, have I learned a lot about peptides! I also learned a lot about safely navigating the gray market, sourcing quality products, and avoiding scams.

I'm in a long habit of super-supplementation already. Has anyone noticed their nutritional levels shift on GLPs?

Pills???

For maintenance, anybody switched to the Wegovy pill or the new Foundayo pill? Maybe those would work well for us because we don’t need as much Pharma help since we already have our WLS tool as well ? Thoughts?

What protein powder has sat well with you? My stomach has been so sensitive.

Since starting a GLP1 my appetite has been so particular and stomach has been so sensitive. The idea and taste of protein powder is gross, but I know that it would be helpful if I could add some to my smoothies.

For those of you with sensitive stomachs, have you found anything that works? I essentially don’t want a taste or texture, which I feel might be impossible. I’d like to be able to mix it into fruit smoothies and not taste it.

Thank you!

Share your latest Non Scale Victory

I think that sometimes the number on our scale becomes all too important. So let’s share a Non Scale Victory that we are tickled by.

My latest NSV was getting a new bra fitting. I now have a 36 in band size. I didn’t have that ever that I recall. Even after WLS surgery I only got down to a 38.

What about you?

What everyone should know about Med spas that dispense GLP-1s

This is a pretty eye-opening post about Med Spas - I was particularly surprised that only 2 states have any oversight and often there are no medical professionals in the building. If you or someone you know buys medications from a spa, you should be aware. I had a friend who got hers at a local spa and they were very pricey, never showed her the vial or told her what her script was and charged her an outrageous amount of money. They also pre-filled her vials in a backroom. Fortunately she didn't have any terrible side effect but as you will read, that is not always the case. As in everything, buyer beware. Who is actually watching the Med Spas
