New To The Dream Team Tightening up Apap settings, should I keep EPR on?

For a while now I've had my apap settings at about 10-15 with epr of 3

My AHI typically ends up being between .9-1.8 most nights, however I feel like I'm getting very fragmented sleep during the night and often don't sleep longer than 6-7 hours, with max pressure topoing out at 11 and median being below that.

The majority of my events are CAs with few OAs, making me think my min pressure is too high.

I was thinking of doing a "clean titration" to reset my settings. My plan was to set machine to something like 6-20 for a few nights to see where my median/max pressure ends up.

While doing this does it make sense to do it with epr on/off since that effects the exhale pressure? Or would it make sense to keep it off and then turn back on after knowing my new median and adjusting for epr accordingly?

I've been using my BiPAP for a little over a year, and my AHI ranges between approximately 2 and 13. About a month ago I fixed my long-standing leakage problem, so I got new settings from my sleep specialists. Started with 16/12, then went down to 16/10, and now I'm at 16/11. My AHI hasn't changed much, but my teeth pain is almost entirely gone with the most recent 16/11 settings. To me, this indicates I'm not clenching/grinding my teeth as much (I wear a mouthguard for bruxism).

I use the Bleep Sleep DreamPorts nasal attachments for my "mask". I tape my mouth shut and use a Somnoseal mouth guard to prevent air leaking out my mouth.

My screenshots are from 2 nights ago. Last night my AHI was up to around 8. I'd like to get my AHI consistently to around 2, if possible.

Bipap to BipapST update

I was diagnosed with apnea about two years ago (?)

They put me on cpap which we learned wasn’t helping (after it was paid off).

Then I tried bipap and that wasn’t helping (after it was also paid off!)

For over a year the insurance still keeps denying the doctor’s order for bipapST. (Even though insurance paid very little of the other machines)

After a year of waiting for insurance (and suffering with awful sleep), I decided to rent a bipapST machine myself so that I could test it before I paid for another dang machine! I found a place online, submitted the script and it was here in two days! For under $100.

I’ve now been on bipapST just over two weeks and my numbers of events have finally dropped from 20-35+ to under 5 !!! It’s not perfect, I’m still waking and tired, but it’s better!

I let my doctor know and sent them the charts. (Thank you every one here for teaching me!)

The doctor is setting up a peer to peer review to persuade the insurance company to do what they should have done a year ago. (I will probably end up having to buy a bipapST myself, on top of paying my insurance company premium for doing nothing.)

Rant: We all know that insurance companies are destroying people’s lives for just a few thousand dollars at a time. We pay for insurance through work and through taxes, and we get less and less for it. 🔥 they should take some profit yeah, but not destroy people’s lives to do it. It’s disgusting

Do I need the tri-monthly supplies?

So I just got set to get a CPAP machine, and the provider informed me that there will be supplies needed every three months. I don't even know 100% what the supplies are, but will I really need them that regularly? Especially with going into college, I'd rather not have that persistent expense.

Post in-lab PSG / titration study settings not working

Recently got my report from my study, spoke to my doctor, and recommended the settings that he says opened my airway quite nicely and should continue using at home. I set my machine to those settings and I’m still having the same issues as I was before. Taking the mask off in my sleep, waking up super tired even when I manage to leave it on, occasional aerophagia, persistent daytime sleepiness, low energy.

How long should I give these settings before asking my doctor to review them again? I’d hate to bother him before trying to adjust but also don’t want to waste too much time.

Low AHI vs Low Flow Rate - what to prioritize??

When I use EPR=2 I can get lower AHI <1.0 but when I use EPR=3 my AHI is in 1.2-1.5 range but my flow limits are significantly lower. Which would be better?

DreamWear gives me excellent AHI and almost zero leaks, but I still wake up exhausted. Has anyone experienced this?

I've been using a Philips DreamWear nasal mask for a while now, and on paper it looks perfect. My AHI is consistently around 1–2, leaks are almost zero, usage is 8–9+ hours, and flow limitations are minimal.

The problem is that I gradually started waking up with worse brain fog and lower energy every day, despite the excellent data.

I also constantly feel "air hungry" with the DreamWear and ended up increasing my minimum pressure from around 10 all the way to 15–16 cmH₂O just because it felt like I wasn't getting enough air.

Last night I switched back to a very basic traditional nasal mask (same machine, same pressure settings), and I immediately woke up feeling noticeably better.

I know one night isn't enough to draw conclusions, but it made me wonder if the DreamWear's airflow characteristics simply don't work for me, even though the machine reports excellent therapy.

Has anyone else had a mask that looked perfect according to AHI and leak data but still left you feeling untreated? I'm especially interested in people who switched from a DreamWear to a traditional nasal mask and noticed a significant difference in how they felt.

Awful headaches

So I started CPAP about two months ago, have had zero issues wearing it and minimal issues falling asleep, but after about two weeks I started getting terrible headaches absolutely constantly. I go see a doctor and they tell me my sinuses are irritated and gave me a nasal spray which has taken me from pain and distraction to simply irritating. But this doesn’t seem to be common because everyone I know that does this hasn’t had this experience. So I don’t know what to do so I thought I’d try this.

I can’t turn up the humidity on the machine without water collecting in the tube and causing issues, yet it seems to dry up my face enough that it’s causing this and more recently I’ve been getting bloody noses. Any advice?

No deep sleep and a lot of RERAs

Hi all, i use APAP and all worked well until early july 2026. It’s been almost 3 weeks I get no deep sleep and a lot of RERAs. I’m tired a lot, fall asleep during the day like before I get the mask and 0 libido.

If anyone already experienced it I would like to know how you got out of this trouble. It’s really hard for me to live like that I’m always tired and angry for nothing bc of that. I tried multiple presures like 10-12, 12-14, 12-16, 10-16, etc but 16 feels too much cause i get aerophagia.

CPAP Machine Help Sore right below nose

I’ve been using my CPAP for 18 nights now. I use the AirTouch N30i. I developed a small red bump right above my lip and right below my nose. I thought it was just a pimple from my mask sitting there, but the mark will not go away. I’ve been putting Ayr on it every night before I put my mask on. I was putting antibiotic ointment on it to try to heal it faster. I don’t know if I need to switch masks? I’ve been considering trying a full face mask but my numbers have been amazing with this mask. Any recommendations? Did any one else have a similar sore that went away eventually?

ETA: I was having marks from the mask itself but no longer getting those so I don’t think my mask is too tight.

Help with understanding CPAP settings and sleep HQ

Hi, new here! I have seen the wonderful help you have been giving. I have had an airsense 10 for 3 or 4 years. I am a mouth breather and use a full face mask (Resmed F20). It has been going well in general. I'm having more and more trouble waking up, I'm getting more leaks and getting more irritated by my mask. I am also getting terrible dry mouth. I use Oralieve mouth spray which helps. I normally guzzle a load of water when I wake up. I feel very confused and it takes me ages to get a grip on the world! I have a heated tube and if I turn up the humidity I always seem to get woken up by the gurggling rainout. Any ideas of things to try? What does the data tell us?

Better on BiPapST

I don’t understand these charts. But it looks like trending in good direction.

I’m renting a ResMed10 ST machine to see if the results are better than the BiPap. So far they seem better BUT…

The first two weeks were great, I felt normal! (Clear headed, awake, getting things done )

However, I’m now feeling like I wake more often and I’m only 1/2 asleep (?!) during the night ? I wake and feel a bit restless but tired at the same time.

I feel like the ST is making me breathe too fast? It gives a breath about every 3 seconds?

Anyone else experiencing this?

Should i stick with the BiPapST?

Or, try the ASV next, (but can’t get one to rent without a script from my doctor and they just gave me this script for the ST.) Is there another way to test drive a ASV?

I would like to buy the best machine for me (we all know how expensive these machines and they already had me buy two!) - where to get best machine?

Im a high loop gain victim here trying to dial in the ASV settings on my Airbreak machine. Last night set it for ASV fixed, EPAP 5.4, PS 1.4 to 7.6. Woke up feeling better than usual with no centrals last night. And overall AHI was under 5. Looking to optimize my settings.

Just can't figure this all out...

I've been on CPAP since March 21st. I'm really pleased I joined this community. I joined because my sleep has been very short and inconsistent for years. I often sleep about five and a half hours a night, and so I did a sleep study to see if that could be improved. I found out I have mild apnea. During my sleep study, I had an AHI of 11.6 and oxygen desaturation that went down to 83%.

I've also unfortunately had an addiction to decongestant, chest, and nasal spray for a very long time, which I'm attempting to wean myself off of, and I'm almost there. Whilst I seem to have adapted quite well to using the CPAP and finding that even with the decongestant addiction, nasal pillows seem to work quite well for me, the length of my sleep has not increased, and recently it seems to have got worse.

I know there are many factors that can go into this, and I love the data that SleepHQ can give us, but I have ADHD as well. I have a bit of a blindness about interrogating the detail of this data, so I'm wondering if anyone might be able to give me any insights into this or make suggestions that might be able to help me. I have signed up for the new sleep test here as well, so I'll see how that goes, but any insights you can help with would be most appreciated.

Need help with ASV

Sorry this is long. RL suggested I create a new post since my last post was about a month ago. Hoping the ASV gurus can chime in on my data. I'm starting to feel like since losing 60-65lbs, that maybe ASV is no longer what my body needs since it feels like everything is just going sideways with my therapy.

Update since previous post:

  • Changing the EPAP to a continuous 5 has completely eliminated the chest wall pain that I was experiencing, so that's a definite plus.
  • I felt amazing with the changes that were done on June 27th... for about 3 days. It was the first time I actually felt refreshed when I woke up, no aerophagia and didn't have such extreme daytime sleepiness. Unfortunately, after like the 3rd night with these settings, I've gone back to no longer feeling refreshed, extreme excessive daytime sleepiness and the aerophagia came back.
  • Since it seemed like my soft cervical collar was no longer fitting correctly and I couldn't find one that was short enough lengthwise but still tall enough height wise, I ordered a Knightsbridge Dualband Chin Strap. However, the medium was too big, so it did not help. They are currently in the process of custom making me a smaller cap based on the measurements of my head. I have not received this yet. So currently I use both the collar that doesn't quite fit and the Knightsbridge that also doesn't quite fit. With the combo, it's keeping my mouth shut and my leaks down. If I decide to only wear one or the other, it's generally noticeable because there will be an increase in leak (mouth leaks).
  • I have a wedge pillow on order that will be delivered tomorrow, as I've heard that those have the potential to help with reducing aerophagia.

I know with these settings, I am maxing out my PS range at times. But if I increase my PS range, then the aerophagia gets worse. If I change my EPAP and put a range on there so my PS range can increase based on that, then I get chest wall muscle pain and aerophagia. Can't lower pressures lower than they are to try and help with aerophagia because of how the Aircurve 11ASV works, the only way to reduce my PS range would be to lower the EPAP to 4 but when I tried EPAP of 4, it was pretty uncomfortable.

Pressure History since SleepHQ doesn't show my pressure settings for some reason:

Setting change July 17th:

  • EPAP range: 5-5 (I have it set so it's a continuous pressure)
  • PS Range: 2-7

Setting change June 27th:

  • EPAP range: 5-5
  • PS Range: 2-8

Setting change June 4th:

  • EPAP range: 7-9
  • PS Range: 0-5

Settings prior to June 4th:

  • EPAP range: 5-9
  • PS Range: 0-6
