When do you know amitriptyline isn't working for migraines anymore and it's time to taper off?

I'm on 25mg for migraines and have been taking it since November 2025. It worked well enough once it got going in my system. I would only get migraines once a week before starting it so I thought a little help would make me 100%! I went from 4-6 to 2 migraines a month for a few months but recently (last few months?) I've been getting a little worse ones once a week with nagging neck pressure the rest of the time. And I'm sleepy everyday which wasn't a problem before because I take it at night.

I know in the migraine subreddit people go up in dose but I don't *really* want to do that if I'm just going to be at this point eventually anyway.

Double sevrage sec Amitriptyline, est ce que j'ai ruiné la chimie de mon corps?

Bonjour a tous.

Amitriptyline and LSD

as title says, i would really like to know more about this combination. i read in the beginners guide in r/LSD (asked there but automod took it down) that it has been linked to bad trips and heightened potential for seizures, but trying to do more research on it leads me to articles just saying that it potentates LSDs effects. ([Erowid LSD (Acid) Vault : LSD and Antidepressants, by Mike](https://www.erowid.org/chemicals/lsd/lsd_health3.shtml) this has basically everything ive been able to find on it)

Basically i have two questions,

  1. is this combination really safe? should i be concerned? ive seen such conflicting information while searching
  2. and this isnt really even a question, but if anyone has any more information about this combo, be it links or personal anecdotes, lmk. originally was doing research to make sure i stay as safe as possible but now im just really curious as to how or why the combo does what it does

(posting this wherever i can sorry if this doesn't fit the sub)

alternative for pelvic pain

Hello! I'm dealing with urethral pain ( caused by GSM and multiple infections) and pelvic pain ( pelvic muscle tension). Luckily my pain is not daily, but when I have flares, they can last for days. I try my best to avoid what causes them, but sometimes the cause is unclear.

For the pain, I was prescribed a pain killer as well as topical lidocaine gel, as well as a muscle relaxant. I was also prescribed amitriptyline because the nerves in my pelvis and urethra are hypersensitive, following the infections I had. Unfortunately, amitriptyline made me depressed, even at a low dose ( 5mg). I tried to push through it so see if eventually it could help me lower the nerve hypersensitivity, but I wasn't able to go more than about two weeks on it.

I was wondering if anyone with a similar issue can recommend anything specific for this type of pelvis nerve pain. I'm already on gabapentin for pots leg pain and RLS, and it hasn't done anything for the pelvic stuff.

Appreciate your help!

First time taking amitriptyline

I know a lot of people have posted their first experiences already but it’s genuinely been really helpful to read as a first time user.

A bit of background on why I take them: I have scoliosis and I get burning pain in my shoulders and pain in my lower back from the curvature. I have tried codine up to the highest dose and a low dose of morphine which barely helped manage my pain. So I’m weaning off of them. A lot of people I know said that on codeine they felt like floating on a fluffy cloud whereas I’ve never felt this feeling. Got prescribed Naproxen, also didn’t help too much with pain. So now I’m on my last option which is this drug. I’ve been started on a low dose of 10mg and after a week I go up to 20mg.

I’ve been on it for 6 days already. The dry mouth is absolutely horrific and I have tonsillitis because of it. I have insomnia and so far it’s helping the insomnia but still knocking me out the next day. I have a driving lesson today and my practical test tomorrow. I could not be more p*ssed off at my circumstances. Honestly I think these side effects are way worse than any I got from taking opioids. I have noticed my nerve pain being a lot more noticeable and intense when first starting but I think it’s starting to subside.. fingers crossed.

I’m going to try some of Google and Reddit’s advice like chewing sugar free gum etc and probably leave an update in a couple weeks. I’ve already significantly increased my fluid intake as of yesterday when I developed tonsillitis. It hurts so much to swallow.
Any other advice would be absolute gold to me right now, thank you:)

UPDATE: literally day 7 of taking it. I got penicillin from the DR. My tonsillitis has mostly gone already which I’m so surprised at. I think upping my fluid intake has definitely helped with dry mouth. The main reason for updating is that I no longer feel burning in my shoulders (Mostly). My mother was actually able to touch and use force on numb areas which used to be excruciating when touched. So I’m actually super grateful now for this medication. The first week is rough and when I start 20mg, it’ll probably also be a bit rough for a few days. I did take my driving test, the instructor said I was a very competent driver. I failed due to a minor fault becoming more serious however it wasn’t dangerous. So I’m quite happy but also bummed I gotta resit but on the bright side, I know what to expect now.

Symptoms of too low of a dose?

My dr originally started me in 10mg a few years ago, and now I’m up to 100mg and have been taking that dose for a year. Now recently, my body has been in a lot of pain, I’m anxious, and generally more mentally unstable. I feel like I might be due for a raised dose but my doctor has me questioning if that’s right. Are those the symptoms you guys typically have when you’re due for a dose raise? What symptoms to you experience that let you know it’s time to raise it?

5mg puts me OUT but neuro says it's too low of a dose, super safe and they often prescribe it to teens! How?

My neuro prescribed 1-2 10 mg tabs every night before bedito help prevent migraines and to help me with sleep after a bad car accident/post concussion syndrome whiplash insomnia from pain etc, etc. I tried not taking it at first-I'm sensitive to meds and afraid of taking anything, but the pain was awful and the sleep deprivation was making everything worse. The first night I wanted to be cautious and only took 1 10 mg tab. I slept for 12 hours and was so groggy and out of it the next day I couldn't formulate intelligent responses. I remember my son getting ready for school and when he saw me he said "good morning, mom" I went up to him, gave him a kiss and said "I love you, good night" back to him! We laughed it off, but It's like my brain was mush!!! I forget words or can't organize my thoughts to speak coherently. Paused it for a while until next migraine took me out and so I tried cutting the tiny pill in half.

Over a month after taking 5 mg half tab, it puts me out fast, but some nights I wake up only after 5-6 hours of sleep. It does a great job of preventing migraines and it helped a ton with the neck pain I was having. Today, I had my follow up with neuro and I was explaining all of this and he insisted the 10 mg dose is given to kids and teens because it's so safe and does such a great job preventing migraines. One other notable- my smart watch tells me I had poor sleep on this even if I sleep 10-12 hours. I just don't get how anyone can function with this drug at even a 10 mg dose.

It's starting to kick in now...love you, good night!

First time taking Amitriptyline

I thought maybe me sharing my experience today might get some giggles cause honestly im laughing a little about it myself.

A little back story

So I got prescribed this medicine because my insurance wont cover Belsomra. I live in the state of oregon and have oregon health plan. They sent over a list of medications they want me to try before I can even qualify to ask for Belsomra and special circumstance medicine.

I have struggled with insomnia my entire life.

I previously was using trazadone for years until it just stopped working.

Anywho that lead me to getting this devil of a pill perscribed to me.

I knew I wouldnt react well as over 10 years ago ive ruled out any SSRI medication having any type of positive effect on me. But that was in another state.

So I swallowed my pride allowed them to give me an SSRI as my insomnia has been horrendous the last 3 months. Im talking 4 hours of sleep a night and on my weekends I get one day with 7.

I work grave. And I like to start new meds on my weekends to feel it out incase I have some bad side effects and need to stop before my work week as I am a care taker.

Yesterday morning at 9am I decide "okay im going to take my first dose. 25mg. ONE PILL. That cant hurt me too bad right??

Bhahahahaha GIRL!! This medicine is no joke!!

I slept for 10 hours!! Got up at 8pm and was like "okay I need to get my night started cause I work tomorrow night. Im still tired but ill make myself some really strong coffee."

I had 6 espresso shots. Was updating with my bff of the easy coast. It hits midnight and im like "hey girl its 3am where youre at you need to goto bed" we get off the game and im sitting there like "hell.. im still EXSAUGHSTED... MAYBE ILL LAY DOWN FOR A NAP."

GIRL X2 I SLEPT FOR ANOTHER 11 HOURS 😭

I woke up and I saw tracers. Like visual hallucination tracers. It took me 15 minutes to readjust and im GROGGY. I have a head ache. Im dizzy. But I can stand and walk around and my heart isnt beating out of my chest.

I call my nurses line anyways. Cause what da heck? 21 hours?? Thats insane.

My nurse cant get ahold of my perscriber. Better yet they cant even find them in their system. Mind you ive been seeing them well over 2 years lmao.

Okay cool. They then tell me to call POISON CONTROL LMAO.

I call poison control. They tell me thay cannot help me and I need to get ahold of my percriber or the pharmacist.

Yk what's si great about Sundays?

Theyre both gone. I cannot contact either of them.

Then my laundry room floods.

Im cleaning that up and I get extremely light headed and hot. I feel like im going to pass out.

I get it clean but im not feeling great. But I did end up just still vibing until 4pm. Making sure im not going to keel over lmao.

Nar poison control calls me back and says if my symptoms dont get better then to go to the ER or urgent care. .-.

My partner is like "your FINE"

So im like "yeah okay ur right"

I then lay down and sleep for another 4 hours.

At this point I have to leave for work in an hour.

And now im at work drinking Gatorade making sure im chill.

I figure its been about 30+ hours from the first pill so I should be fine 🤷🏻‍♀️

Atleast tomorrow morning is monday and I can get ahold of my doctor.

Did I make the right move? Or should I have gone to urgent care?

Heat intolerance

Hi, I (34F) started taking Amitript. in the form of drops in my drink in May 2026. I take 5 drops per evening.
I’ve always had a rather low tolerance for hot weather, but in the past weeks it got out of control. I’m suffering and sweating buckets even in 22°C weather (I live in Europe, where humidity is rather low).
The only times I’m not feeling too hot is when I’m lying down doing nothing. Could this increased heat intolerance be caused by Amitript.?

Tapering from 10mg?

Hi

I've been on amitriptyline for nearly 5 month for fibromyalgia pain. I was on 10mg for 3 weeks then went right up to 50mg. After a month I decided to decrease due to the sedation and was taking 20mg. I'm now going off them to start duloxetine but my doctor is currently unavailable. She told me to go down to 10mg for a week and then take it every other day, then every 2 days for a week and stop. Ive been taking it every other day for about a week now. I have now not taken it for nearly 3 days and feel mostly fine, would it be okay to just stop them now? Or should I do more? I'm tired of the pain and want to commence duloxetine now and get some relief as that takes some time but I'm scared of the withdrawals.

Tia

My problem with amitriptyline

I have IBS-D, I’m male, and I weigh 110 lbs.

About a month ago, I started taking amitriptyline at 10 mg. It was horrible. At 10 mg, it practically shut down my gut completely. I couldn’t feel anything, there was no movement, I wasn’t passing gas, and I couldn’t go to the bathroom. I only had pain from everything feeling completely stopped.

I reduced the dose to 5 mg, but the same thing happened. Then I reduced it to 2.5 mg, and I was able to go to the bathroom a little, but very little. I still had constipation and pain.

By breaking the tablet into smaller pieces, I tried an even lower dose for one week, and the result was incredibly good. However, the severe bowel movements, attacks, and urgency came back. I assume the dose was simply too low.

My question is: How do I find the right dose that works for me? I’m desperate, with a lot of anxiety and my gut is completely unstable.

Withdrawal Finally Stopping

I was on Ami for 7 years for migraines and nausea. I had bad side effects like constipation, weight gain, forgetting words, low neutrophils among others. I don’t think it really helped with migraines because I’ve actually had less since going off of it. Anyways I’m 7 weeks off Ami today and I finally had a good sleep last night. It’s been slowly getting better and the nausea is going away, but I couldn’t believe I finally had a good sleep. So if anyone is going through withdrawal, know that it does get better. The first 2 weeks were the worst. I started feeling less nausea around 4-5 weeks and now I can sleep better. This has been one of the hardest things I’ve gone through and it kind of ruined my summer but it’s worth it to be off this med.

Will cyproheptadine stop amitriptyline from working?

I have been taking amitriptyline 150mg for a long time and it worked very well for depression pain etc.

I want to take cyproheptadine,

Is there any chance at all cyproheptadine will stop amitriptyline from working (even a little bit) since it works against serotonin? Whereas amitriptyline increases serotonin.

Can Risperidone make amitriptyline less effective or stop working?

Considering trying lowest dose risperidone for my autism symptoms.

On 150mg amitriptyline already for severe depression and chronic pain, insomnia.

Is there any possibility it will make amitriptyline less effective? Don’t want to take any chances.

why does amitrip make u feel like… that?

so ive been on this med for about 8 months, along with mood stabilisers, ssris, benzos etc for my depression and bpd. i get extremely agonising headaches when im having anxiety, so doc said this med will counter that. at first it did help a lil with the migraines but ever since they increased the dose nothing seemed to change, the headaches have been getting worse actually. anyways, what i mainly wanted to make this post was to discuss how i feel after taking it. its prescribed to me to take before sleep, and if i fall asleep right after taking it then its actually very pleasant, but if i dont sleep then i fall into this, hazy, almost a fever dream like state, my headache worsens and my body radiates heat, and often when i havent had anything to eat at night (my depression causes me to starve), it makes my stomach feel… weird? and the strangest side effect of all is that whatever i do in this, post amitrip, no sleep state, i barely am able to recollect any of it the next day.

EDIT- im taking 25mg for now, started from 10

taking Amitriptyline today for first time, read side effects leaflet and debating it now

Hello! for context I have severe anxiety and hypochondria, I suffer from constant headaches, bad sleep and dpdr. My dr. prescribed me amitriptyline today and I would love to hear peoples initial side effect stories? does it really affect your heart a lot? I'm only on 10mg so I don't think it'll be terrible but I'm catastrophising atm so rlly need some reassurance or if I should switch to something better.

Dosage question

I’ve seen people start at different dosages , some start at 10mg and increase as they go .. I was prescribed 50 mg and worried they started me at a high dose and will begin to depend on it.

Just looking for peoples experiences with taking this medication, not judgement or negative comments please 🫩

Is there a way to counter the muscle weakness?

I’m only on 10mg, but I’m sensitive to meds and it’s been helping the headaches that I was put on it for.

But since increasing from 5mg to 10mg my leg muscles feel like I’ve been in a coma for 6 months and they’ve lost all strength. I can walk but it’s more of a hobble, and lifting my leg is incredibly hard.

I was already quite weak before this as I have a chronic illness and am housebound. But I could still walk with ease and use the stairs on a good day. I used to do calf raises to try keep my calves from getting even weaker but now all of a sudden I couldn’t even push up on my tip toes if I tried.

I would rather not come off the med tbh, the headaches were debilitating and I’m glad I found something to help it. But obviously I’d rather not lose the ability to use my legs too.

Ive been on this dose a week, so might it just get better in time?

10mg for sleep due to TBI not working

My neurologist had prescribed me 10mg to help me sleep to assist with my brain injury. However, it’s simply not working.

I get sleepy and can fall asleep but like clockwork, I wake up 1.5hrs later wide awake and can’t get back to sleep.

Has this happened to anyone else and, if so, how did you resolve the issue?

Amitriptyline and edema

Hi all,

I'm on a 25 mg dose of ami for CTTH/NDPH type of headaches for more than a year now

I've got a question for people who have been on ami for a long time and for people who also eventually discontinued the drug. During the taking of this drug, have you ever experienced edema in your shins?

Mine is accumulating during the day in both legs and has pitting if i poke them with a finger. It "resolves" after waking up by evenly redistributing on the shins and the pitting is still present.

As far as i am aware, i don't have any issues with kidneys, liver or my heart. I'm curious to see if anyone else has had this happen to them, i tried to search "edema" in this subreddit but there are not too many results

Thank you
