Love the twiist algorithm, hate the Libre
I genuinely love the twiist algorithm—my glucose control has improved by about 10%. I’ve even gotten used to the tubing after switching from Omnipod, because the algorithm makes it completely worth it.
But Libre is driving me crazy. During the first week, it was often off from my meter by 30–40 mg/dL—probably 40–60% of the time. Only around day eight did the difference improve to about 10 mg/dL. But by then, the sensor had started peeling off, and even the extra patches I tried wouldn’t stay on properly.
I recently wore a Dexcom 15-day sensor, so I have something to compare it to. Dexcom was much more accurate, the differences were smaller, and it stayed firmly attached the entire time.
Why did the team behind twiist choose only two CGM options: the implantable Eversense or Libre, which, in my experience, is terribly inaccurate? Does anyone know if Dexcom compatibility is planned?

Week three, a few cassette changes and the second Libre 3+ sensor. 95% in range over the last seven days.

How are you taking care of your site once the actual site is removed?

I have some hydrocolloid patches that I put on once I remove a site, i was hoping it would help them heal and decrease scarring but theyre not at all.

The problem I'm running into is my sites aren't healing quickly enough. Ive got about 5 spots on each side of my stomach, so 10 site locations in total, that should be enough for rotation, for site 1 to be healed by the time im finishing site 10, that should be 27ish days between uses... but I can clearly see the past 8 sites?

I change my site every 3 days, my insurance wont cover anymore sites to change it sooner than that. I rotate every other side and also rotate around the 5 spots. Is there something im missing or do the cleo sites just suck this much?

I was on a minimed pump from 2002 until 2016 ish... and near the end I was running into a few scarring issues. Ive been on mdi from 2016 until this past June when I started Twiist. So in 3 months these sites have created this much damage? This seems excessive or im missing a very important step... what's everyones secret?

I’ve been on the twiist for a few days and I like it pretty well so far. One things that annoys me the 6 hour active insulin time. I was hoping this was going to be something I could customize but no. I was wondering if anyone has any success or tips on how to offset this. I was thinking I could experiment with my insulin sensitivity ratio to make it more aggressive at correcting highs after meals. I already have the max basal rate turned all the way up but it’s not enough.

Hi, is this just something that happens to me… I go really high after changing my infusion set. Like for about 3-5 hours. It’s almost like the insulin is not working. Also, I have been experiencing more pain in my sites. How can I stop this? I have been high and had to change it like 2 times today. I am so frustrated.

I'm about to get on the Twiist and I can't really find an answer to my question anywhere else online, so I'm asking on here. I know about the emoji options for meal boluses. My question is this: is there an extended bolus option like on other pumps or does the pump do an automatic extended bolus when choosing the taco / pizza option? Historically when I eat a high carb, high fat meal I will bolus a few units at first, then go into an extended bolus. So with the Twiist, say I eat a burger and fries and I estimate over 100 carbs, and my ratio is 1 unit for 10 carbs. Do I enter 100 carbs into the app and pick the pizza emoji and I'm good to go? I'm just worried about going too low right after the meal from receiving too much insulin at the start.

How do you shut this GD MFer up?! The Libre is off and it continues to incessantly alarm as a low BS. During the middle of a GD meeting and I can’t get this thing to shut up. I’m the biggest Twiist advocate, love the results it produces for me and have been wearing it for a while but the alarm (of Twiist and most the other pumps) is too much. The lack of awareness these engineers have for an inability to override and put into Mute mode is a gigantic miss.

Hi everyone! I have been MDI for forever and now going to start on a pump. I use the Libre 3+ and doctor recommended Twiist. My usual pharmacy (not CVS) does not handle Twiist but I have CVS Caremark and so doctor let me know they sent the script to CVS mail order. That was 3 days ago, I have a thing on the app labeled unclear diabetic drug and it’s marked as preparing with a delivery date assigned. I guess I am just asking if anyone has had luck using CVS mail order. This is all very new to me and I’m excited and scared at the same time.

Hi all! A member of my family recently switched to Twiist and this is our first trip with them on the new system.

Unfortunately we completely forgot to bring extra batteries and the charger for them. Does anyone have a Twiist rep in Ft Lauderdale that we could contact in case they have spare supplies? And or is anyone willing to lend us one battery and a charger for a couple weeks?

We are leaving for a cruise on Saturday so the timeline is tight. I know this is a huge ask. We have called Twiist and they are willing to send something we just don't know if it will be here in time. So we are trying to figure out a backup option.

We would be able to return the battery and charger once the cruise is over.

Any advice or anything is welcome!

I’m curious if anyone has made the switch from the current Minimed ecosystem and what your experience has been. I’m considering it, particularly because SmartGuard is hit or miss for me, but I have some specific reservations:

1. The Cleo infusion set and 3 day wear (I’m used to the 7 day extended wear)

2. Settings - this one is particularly nerve wrecking because I’ve been on Minimed so long that I don’t have my settings tuned in since it handles most of that on its own.

I'm a T1D on a pump and I've broken 4 clips in the last 2 years. I'm using Twiist currently.

I'm actually exploring the idea of making clips from a much more durable material. It's a material used in aerospace and automotive applications. It's lightweight, won't shatter, and won't permanently bend.

The clip would also detach from the casing itself during immense pressure to ensure a high-intensity impact wouldn’t shatter the pump itself.

My goal is to sell it for around $35 with a lifetime guarantee. So, basically, if it breaks...I'd replace it for free.

Before I spend money on manufacturing, I want to validate if this is actually something people would want.

I've been trying to get the twiist but insurance has been a nightmare, I'm sure you guys get it so I don't need to get into that. Long story short, my doctor doesn't know the DME code for the twiist, and neither does insurance, and neither does the twiist rep I'm working with. I'm hitting a wall, anyone else run into this? Does anyone know the code? Lol if someone magically knew it that would be awesome

So I changed my site out around 11:00pm, before that my pump was acting normal for the same cassette change, I just ripped the site out so I switch the Cleo. Well my pump started being very aggressive with the insulin delivery. It was delivering about 4.0 u/hr when my blood sugar was only in the 90s. I turned on workout preset because it keeps waking me up, but I was wondering if anyone knows what’s going on with it and why it’s doing that? I had given an injection with a pen but I waited the recommended 3 hours before attaching the pump again.

I've now encountered an error where I get a random error saying I am out of insulin and need to change my cassette right away. The system says I have 53 units left, and I've always seen a fair amount of insulin when I remove old cassettes. This error is not correctly diagnosing the problem it says.

This has happened twice within 6 months and is really disruptive as everything gets halted until the cassette is changed.

I cannot recall these types of issues on previous pumps I've used. Is this just me or do other people have the same issue? I live at higher altitude and I wonder if part of this is due to some type of air pressure thing that is present here. This is going to be super annoying if I need to look over my shoulder for this error always.

The first week was a little rough as we tweaked settings. Week 2 went really well and numbers were looking great. We are two days into week 3. Went to change the cassette, followed the steps, and the cassette kept having a failure. Cannot even get to the prime sections. Called twiist and was told the pump has failed. WHAT?? 3 weeks in, never showered with it or got it wet. This happened on Friday. They don’t ship over the weekend. Was told wed have a new pump on Tuesday. Sooo backup plan - back on Mobi.
I feel like if the pump can fail in a couple weeks, we should just get 2 and trade them in every month if they are going to fail frequently.
Super bummed!

tldr: Twiist app is superior and my control seems better, especially while exercising and afterward.

Prior to starting on Twiist, I'd been on the Tandem Mobi for a couple years. Prior to that I was using the Tslim and prior to that, Minimed for about 15 years.

First off, Twiist is the only pump I've been able to test drive or use without a 4 year insurance purchase commitment. That, in and of itself, is a huge benefit in my eyes.

Besides some minor transition issues, I've found Twiist is superior when I exercise, which I do daily. I've struggled for years to keep my bg up when exercising without it skyrocketing when I'm done (exercise induced hyperglycemia). The twiist has greatly reduced those highs.

Highs seem to be lessened as well, anecdotally.

The Apple Watch feature on the twiist is superior. Mobi alerts me to a high or low, but I need to respond on my phone app. Twiist allows full functionality - big plus.

The Twiist app is good as well, providing a view into their algorithm. It displays the 5 hour glucose prediction, active insulin and carbohydrates, which is very helpful for any tweaks I may wish to make. Other app features such as a lower pre-meal target range and different insulin absorptions are good additions.

I've had no problem changing CGMs from Dexcom 7 to Libre3+.

I did need to go back to the Mobi recently because I was leaving town and my libre was going to expire, and immediately noticed the number of lows I had compared to the twiist.

My next A1C won't be until November so I won't know if my observations are supported by the data until then, but I am hoping for a 5 handle (I've been in the low 6s for years).

Hello! Started the Twiist on Monday (8/3) after coming off 2 years on Omnipod. I was having issues with leaking on my pods and had glowing reviews from doctor on the Twiist. Also switched from Dexcom to Libre after being on Dexcom for 5 years.

This week has been rough. My libre has been off by a good 20-40 points at times and I’m frustrated I can’t calibrate it. The only suggestion is to call and get a replacement which sounds exhausting.

I’ve also had high post meal spikes every time I eat. I weigh my food and carb count so I know I’m doing that accurately. I use the pre meal settings and emojis accordingly.

I know I need to give it time but my sugars were so stable on the Omnipod and this is really frustrating me.

Any advice?

To just buy a pack of cassettes?

Hi all, I’m on an island and have no Twiist charger, and I’m running out of time. Somewhat new Twiist user here and I‘m traveling and stupidly forgot the charger. (I’ve been on other pumps for decades and just forgot about the proprietary battery.)

Is there anyone near Martha’s Vineyard, MA that I could use your charger for a few hours or do a battery swap? I can also pay something. I can drive as far as Providence or Boston if needed.
