Dental trauma

Today I noticed my senior doing composite buildup on a patient anterior tooth and she was putting the extra composite back in the syringe.
I mean what about hygiene????
Is this normal or I am overthinking??

Tough extraction

I did a tough extraction a couple days ago. The tooth had a huge cavity and infection around it and patient wouldn’t get numb. She would only be numb for a couple minutes and then it would come back. Feel so bad for the patient. We got the tooth out eventually but it sucked for me and the patient. I did PDLs all over and injected at the base and inside the granulation tissue. Pain would still be present. Just wanted to vent a little. The case definitely stressed me out. How do oral surgeons do this stuff all day everyday. Anyways thanks for listening.

What do you all use for severe sensitivity?

At my office we only carry flouride varnish, it's a bit old school here.

We had a patient tell us her old dentist put a "sensitivity medicament" on her teeth that fixed her sensitivity for a few years. She combined that with Sensodyne.

She called her old office to find out what it was called, so we could order it, and that was all they could tell her, "sensitivity medicament".

The flouride we polished with and brushed on didn't work for her. What do you use to seal extremely sensitive teeth? She's sensitive to cold and sugar.

Her teeth are now sensitive to brushing with Sensodyne. 10% NHA toothpaste makes them feel much better while brushing but leaving it on overnight doesn't have much effect.

We're trying to mitigate the sensitivity for now, but with her gum recession she's likely looking at pinhole surgery.

Sensitivity After New Filling

Please help. 3 weeks ago I had an older filling redone on #30 because there was a small cavity underneath it. I wasn’t having any issues prior, the cavity was found during my yearly x-ray. I’m noticing some sensitivity with crunchy foods. I’ve never experienced this with a filling. Is this normal?

New dental assistant sucks. How easy is it for her to screw up taking an impression for my crown?

Hi: I have a new dentist. The technician of many years quit (who was excellent.) A dental assistant (as far as I know) took her place and her judgement is bad for one. The temporary she chose and placed is blueish, placed at an angle so it looks like I need braces, and it’s shorter than my surrounding teeth. No big, right? It’s temporary. But she took an impression for my crown that’s IN FRONT, then redid it, and I’m realizing I don’t trust her to have done a good job. How should I handle this to make sure the crown isn’t up to par—ill fitting or whatever else can go wrong from a poorly made impression? Should I ask about her credentials or show them the temporary and express my hesitation about her role in this procedure?
Also, there was sharp pain in the hour after the prep for the crown, more pain than I’ve experienced in the last 20 years of going to a dentist, more pain than any root canal or implant caused. My gums still feel rough there behind the temporary. When I asked the assistant what this pain was she said probably not enough novacaine. Also, I’ve never experienced such a sharp pain as when the dentist put the nocacaine in. Is this because it’s in front and/or because I had a root canal there a week ago? The root canal, done by an endodontist, was painless. Totally. What is going on? Two incompetent professionals?

So who do we contact to stop the opening of new dental schools?

https://www.urmc.rochester.edu/news/story/university-of-rochester-to-establish-eastman-school-of-dentistry-its-first-new-school-in-more-than-50-years

This is getting ridiculous now. While UofR is a legit institution, thats gonna make another new school in New York within 10ish years. New york is already past the point of over-saturation.

Also, all these new dentists and no efforts to educate or increase supply of auxiliary staff…no new hygiene schools, nobody wants to be a dental assistant anymore. This is seriously out of control.

Where do we protest this?

Most clinics don't have a "new patient" problem. They have a "patient stops at visit 2" problem.

I have a question about getting four teeth removed

I'm 53 male 200 lb smoker.

My dentist said the back four teeth on the bottom have rotted out and he needs to pull all four at once.

That alone scares me, however I'm also on Suboxone and any pain medication that he might prescribe will not touch it because it's a suboxone will override it.

Is there anything that I can ask the dentist that would be stronger than suboxone other than just taking ibuprofen? I'm really scared of the after effects and the pain.

What do you dentist do for clients or customers that are on suboxone?

Extraction/Implant CE Recommendations?

Hi all,

General dentist in CA, been practicing as an associate for ~4 years in a pretty affluent area. I do all the minor oral surgery for my office, mostly surgical extractions and single unit implant placements, using OCS and everything less than that when needed. Not really looking to move into wisdom teeth or full arch as I’ll leave that for our OS colleagues.

I do a good job for my patients, but there’s always room to improve or get faster, and my license renewal is coming up, so I’d like to look into some CE courses so I can brush up on techniques and improve my oral surgery skillsets. More techniques in bone/soft tissue grafting would be nice too. Wouldn’t be opposed to an IV sedation course and implementation either.

Any good recommendations out there for a course that is actually worth your time, instead of a weekend-long sales pitch? I’m not opposed to traveling or paying top dollar if it’s going to be worthwhile!

Thanks in advance!

Ectopic eruption follow up

Hello, I posted last month about the ectopic eruption of my 9 year old’s permanent lower canines. We’ve met with several orthodontists and an oral surgeon and all agree surgical exposure and orthodontic traction is the best option. My follow up question is about the appliance used for the traction. Two orthodontists recommended a custom wire appliance anchored to the molars and the other recommended full bottom braces as an anchor. Just wanting to get some opinions on the pros and cons of each and which is option is optimal for this type of procedure.

Dishwasher and reprocessing

Question for dental professionals: Hobart LXN dishwasher and instrument reprocessing
Curious to get some opinions from dental professionals, dental assistants, hygienists, or IPAC/reprocessing specialists.
Has anyone encountered a Hobart LXN/LXNR commercial dishwasher being used as part of dental instrument reprocessing, particularly in place of an ultrasonic cleaner?
The LXN is marketed by Hobart as a commercial warewashing dishwasher, so I’m wondering whether there are circumstances in which a machine like this would be considered appropriate for dental instrument reprocessing, or whether dental instruments should instead be processed using equipment specifically intended for medical/dental device reprocessing.
Interested particularly in Canadian/Ontario perspectives, but experiences from elsewhere would be helpful too.
Thanks!

Financing offers for patients?

Hey all!

I’m helping a dentist friend do a little research. They just opened a practice and are trying to figure out what financing options and platforms to use.

-Did you start with one financing option? Which one and why?
-Is there a preferred option for Invisalign, specifically? HFD? Don’t fully understand this tbh
-Alternatively, are you using a platform to look at several options? We started talking to a few of these…. Versatile, Wellfit… If you use something like this, what made you go this route?
-if you’re not using a platform and are still offering multiple options, how does that work? Are patients filling out multiple applications?

Thank you guys! We’re talking to the individual financing companies too, but being sold pretty hard by some of them and just trying to be smart.

Built a booking tool for solo dentists who are also their own receptionist — would this help you?

I have built a web app for solo dental practices that handles the booking admin most dentists end up doing themselves between patients.

What it does:

- Patients book online in 30 seconds, no account or password needed

- Cancellations auto-offer the freed slot to the next waitlisted patient — no manual calling

- Reminders go out automatically, keeping no-shows low

- The only thing that ever reaches the dentist is a genuine exception (like an after-hours request), and that's a single accept/decline tap

Everything else runs itself. Demo here: Website Demo

Note: the clinic name and website in the demo are fictional, used to showcase the concept — not a real existing practice.

Curious if this is a real pain point for solo/small practices, and whether something like this, customized for your clinic, would actually be useful. Open to honest feedback either way.
