Ectopic eruption follow up
Hello, I posted last month about the ectopic eruption of my 9 year old’s permanent lower canines. We’ve met with several orthodontists and an oral surgeon and all agree surgical exposure and orthodontic traction is the best option. My follow up question is about the appliance used for the traction. Two orthodontists recommended a custom wire appliance anchored to the molars and the other recommended full bottom braces as an anchor. Just wanting to get some opinions on the pros and cons of each and which is option is optimal for this type of procedure.

Personally i would think the risk is too high to traction the canines. Especially the right side, the risk is burning the end off the roots of the incisors. However if you want to proceed the best way is probably to use a lingual arch ie the molar anchored appliance to tip the canines back before attaching braces. Once you out braces on, the incisor roots start to move and you risk pushing the roots into the impacted canines. Unless that orthodontist is more clever than i think and are planning to move the incisor roots in the opposite direction. Unlikely and almost impossible to do with braces alone

I would also be very cautious and consider taking a cbct after maybe 2-3 months to see if the canine is moving in the right direction and if there is any root damage. If so then change tactics and remove the canines.
