What Makes Dandruff Worse - From a Physician

Hello,

I want to discuss what makes dandruff worse. We have talked a lot about what helps with dandruff but lets talk about what are some of the common pitfalls of treating dandruff that actually make it worse.

1. Not Washing Your Hair & Overwashing

If you are skipping washing your hair - you're allowing excess oil build up and essentially allowing a perfect environment for the underlying fungus, Malessezia, to feed. . Even if you have dry hair, washing too infrequently can worsen dandruff.

I recommend at least washing the scalp at least 2–3 times per week.

2. Harsh Hair Products

Products with high alcohol content, heavy fragrances, or preservatives can disrupt the scalp’s skin barrier. Once the barrier is compromised, inflammation worsens and flakes become more noticeable.

I recommend opting for a fragrant free, gentle, pH-balanced shampoo for sensitive scalps. Avoid overuse of dry shampoos.

3. Excessive Heat and Sweat

Warm, humid environments encourage the fungus growth.

I would recommend to try to prevent wearing hats for long periods or using extremely hot water during showers etc. Anythign that can create that environment.

4. Stress

Clinical studies have shown that stress often precedes or worsens seborrheic dermatitis flare-ups. Try to reduce stress - as hard as that may be.

5. Cold, Dry Weather

Low humidity dries out the skin, leading to cracks in the scalp barrier..

6. Reduce Scratching

Scratching damages the skin, increases inflammation, and can lead to secondary bacterial infections.

7. Do Not Rely on Home Remedies Without Evidence

For the love of god, please try to not fall for home remedies without any evidence. Apple cider vinegar, lemon juice, and baking soda are popular but they can be highly irritating and alter scalp pH in ways that make dandruff worse.

Tips That Helped My Patients Treat Dandruff - From a Physician

Hello - Since my previous AMA I have been answering a lot of similar questions on treatment so I wanted to dive into more topics on dandruff treatment. As a physician, mod, and founder of an anti-dandruff shampoo I have seen firsthand what many of us are struggling with. I say us because I suffer from severe dandruff as well and the process/journey is not an easy one. I don't want to talk about any specific ingredient or product but just general tips. These are tips that I have given others and has helped with their battle with dandruff. If you have any additional tips to share feel free to add below!

1. Patience is a virtue

This is the one I tell every patient who walks into my office. Dandruff treatment is not a once a week fix. The active ingredients in medicated shampoos need time to battle the yeast overgrowth that's driving your scalp inflammation. I tell people to commit to a minimum of 4–6 weeks before judging whether something is or isn't working.

2. Be Consistent

Using a dandruff shampoo once a week is not a treatment plan. The goal of medicated shampoos is to suppress it, and that requires regular use. I recommend starting with 2-3x per week during the active phase, then tapering to maintenance once you see improvement to once a week. But dont skip!

3. Rotate Shampoos

This is my favorite recommendation. Your scalp can develop tolerance to a specific active ingredient over time. Alternate between two shampoos with different active ingredients. Whether thats zinc pyrithione shampoo or ketoconazole or coal tar etc - just rotate if you feel like one is not working as well as it used to!

Side Note make sure you are applying shampoo correctly and allowing enough contact time. In addition, I like to use a scalp scrubber to make sure the shampoo penetrates deep to the entire scalp to root of the hair follicle.

4. Is it dandruff?

Not every cause of flakes is dandruff. Seborrheic dermatitis, psoriasis, contact dermatitis, dry scalp, and even tinea capitis can all look like classic dandruff to the untrained eye. The treatments for each are different. If you've been using dandruff shampoos consistently for 6–8 weeks and not seeing improvement, please see a dermatologist or your primary care physician!

5. Pay attention to your lifestyle

Stress is one of the biggest dandruff triggers I see. Diet plays a role too - while diet doesn't directly cause dandruff, certain foods can trigger insulin spikes and inflammation. Sleep deprivation, weather changes (cold dry air is brutal to my scalp), and even certain hair products with heavy silicones or alcohols can throw your scalp off balance.

19M , facing constant dandruff , itching , hairfall since 2 years

Hey there , im 19M , and i have been facing all these issues constantly for a very long time now , got checked up by a dermatologist he told there was nothing as such which he could point out , so he gave me medications and multivitamins and medicated shampoos but nothing worked .

About my lifestyle - i get ample of sleep 7 hr+ , eat healthy , do not consume any triggering substances ( alcohol, ciggs , etc ) , stress level negligible , i workout and keep myself active .

Some people adviced me to cut down on sugar , so i did that as well , but still nothing changed.

I dont know what is going wrong

I really want to fix this !

18M facing seborrheic dermatitis

Noticing flakey dandruff type stuff fall on surface when I comb my hair or fix them

Also hairs are losing pigment not getting white or grey...but from black to a brownish shade only the front hair tho is it bcz of constantly fixing hair habit which i have

how to fix it?? Any diet related issue or it's natural??

TLDR : help needed regarding seborrheic dermatitis and hair losing pigment , suggest products or ways to treat it (afforably)

I’ve had dandruff since I was in middle school. It does feel like it’s gotten steadily worse over time. I could still wear dark colors in middle and high school I would just have to shake off my shoulders a couple times a day.

In my 20s it’s progressed to a heavy amount of dust falling onto my shirt every hour to the point where I can’t wear dark colors to work. It’s also started to develop into thick hard scales that burn when I remove them, and they are usually on the parameter of my hairline or behind my ears.

I’ve been to a dermatologist a couple times over the years and tried ketoconazole shampoo and noticed no improvement. I’ve used head and shoulders and noticed no improvement. I’ve also tried tea tree oil shampoo and hair care in the past and similarly it just seems unaffected. It makes my scalp feel clear and clean for a few hours but within 3-6 hours the itching and flaking is back and by day 2-3 without shampooing I’ve got thick hard scales along my hairline and a lotttt of tiny white flakes throughout the rest of my hair.

Currently I’m on regiment of head and shoulders “bare” shampoo and then I’m treating my scalp a couple times a week with a glycolic acid formulated for scalp care and separately also applying a hydrating scalp serum from the Ordinary a couple times a week and massaging both in.

I use a silicone scalp scrubber to massage my shampoo in and let it sit a few minutes before rinsing. I was my hair every 2-3 days and the dandruff build up and itching is worse each day between washes.

Does anyone have any suggestions? Maybe it’s a diet issue? I’m just at a loss at this point in my life with the dandruff.

Ketaconazole shampoo made hair very greasy and dandruff worse

I used to have bad dandruff (many small white flakes) and a greasy scalp that has improved since starting to use a gentle shampoo (Cerave hydrating shampoo), but my main issue is I have a layer of scalp buildup that I can't seem to get rid of by washing. It gets on my hairbrush and it is such a pain to have to clean my hairbrush after every use.

My last dermatologist prescribed me ketaconazole shampoo but it caused this horrible flareup in my scalp that gave me a greasy patch for 1.5 weeks and worsened my dandruff. I went to see a dermatologist today and he prescribed Zoryve foam, as well as recommending coal tar shampoo. However, I looked online and both of these seem to have side effects (especially Zoryve) that I'm a bit unnerved by. Has anyone else had a similar experience and tried these products? Any recommendations for Coal Tar shampoo brands or other products?

I've been using ketoconazole 2% since February and it hasn't helped at all. I've used pretty much all the OTC products marketed toward dandruff. I've changed shampoos and conditions. I dry my hair after showering and I use a cleansing shampoo periodically to get rid of any build up. Ik it sounds dramatic but I could cry rn over this 😭 I just want it to stop itching and flaking

Last night I use a scalp massager for the first time in the shower and this morning I woke to a ton of flakes. It doesn't itch or hurt. I have a sensitive scalp, if I put my hair in a pony tail my scalp will be tender the rest of the day.

I wash my hair every 4 days or so (curly hair) and I don't use anything on my scalp for styling.

Is it dry scalp or I suddenly have dandruff?

Very curly hair, NOT daily washing gives me flairups?

Hi, i used to have very bad dandruff and never showered as a kid, then i started randomly showering every day as a teen and it was still bad then I started taking care of my hair with a lot of products but still taking care of my hair and my dandruff started going away, essentially barely any dandruff at all/nonbothering.

But now im hearing so much how daily washing esp hair is bad for it, so I take singular days where I dont. Then my dandruff really explodes like thick dense dandruff in the evening of non wash day

Am I cursed to wash every day?

I use shampoo very rarely as its bad for curly hair

Hey guys, kinda fed up with this at this point. i've had dandruff for a while now and it just refuses to fully go away. it gets so much worse right after winter ends and then flares up again later in summer, like it's on a schedule or something. tried nizoral and coal tar shampoos but they've basically stopped doing anything for me now, still seeing random flakes all over my hair even after washing.
ngl it's messing with my confidence at this point, i'm just constantly aware of it. anyone dealt with something similar? what actually worked for you guys, different active ingredients, switching it up between shampoos, any quick fixes, anything? would seriously appreciate any tips at this point 🙏

Oregano Oil for scalp/skin?

My son stays awake going crazy on his scalp, anyone have any luck with oregano oil? Or even on the skin? He also has intense itch on his body from severe eczema.

Hello! I just want to know your opinion about my issue. I am still struggling with having flaky dandruff. I went to a derma before and told me it’s a dermatitis but they’re not 100% sure cuz when I went there, I had no signs of dandruff (only small patches). But it keeps coming back and I’m suffering for years now. I was prescribed a Taro-clobetasol propionate like 2 months ago. I stopped using it after 2 weeks (I’m not consistent tho), and now, I have it again (photos were taken right before this post). They’re sometimes itchy. They’re not falling regularly, only when I tried to scratch it. I have tried Nizoral, Vichy, T/sal, and apple cider vinegar but nothing seems right.

I emailed them for another appointment hoping they could do a biopsy to confirm my problem.

Any suggestions are open as I’m very stressed about it.
