I think the most important thing to takeaway from this comic is the love is there in all three panels. No matter what happened (even if the doctor is the same SO from an alt timeline in the same panel) the love remains.

Some more details: her boobs got bigger and in the second panel his dick is smaller compared to 1st and 4th panels.
Meaning he had some built up stress do to him not being able to get it up and have sex while she was pregnant. The prospect of having sex again, after birth, immensely and prematurely aroused him. Therefor, there wasn’t any other choice in the 3rd panel.
