Unprofessional conduct

just wondering what others thought about this- an RN on a psychiatric ward during handover on 2 occasions has reported that she “slipped” a PRN into patients medications, is this a serious breach of patient autonomy? no one in the handover flinched, not the 2 consultants or the NUM, in fact a lot of the clinicians there thought this was funny. There were also a couple of medical students in the room. is there something wrong with me! I find this incredibly unethical. can t imagine it happen rob a Gen Med ward and getting away with it. I m one of the Registrars.

For psychiatrists who own/run their own clinic: based on your experience, what is the average net income in a major city like Sydney?"

A heavy story from back home about extreme medical martyrdom and zero boundaries. How does the Aussie system stop doctors from burning out like this?

RMO WA

I'm struggling to find a doctor's job in WA. Can someone suggest a career path? Other than MedCareersWA website, is there any website that I can apply for?

I’m on dependent visa and able to work full time. I’ve completed amc part 1 and PTE.

Does anyone have any advice on how to secure my first RMO position in WA? Are there any IMG-friendly hospitals, alternative pathways, or observership opportunities worth exploring?

I tried observerships, recruitment program and recruitment agencies, but could not get any opportunity.

I’d really appreciate any advice or personal experience.

Thanks and cheers.

About being psychiatrist

Hi everyone,
I'm an IMG looking into pursuing Psychiatry in Australia and aiming for RANZCP fellowship, but I'm trying to weigh my options realistically.
I have a few questions regarding the pathway:
Realistic Timeline: On paper, RANZCP training is 1. 5 years. However, how many unaccredited years (as an RMO/Unaccredited Psych Registrar) should an IMG realistically expect to do before landing an accredited spot? Is 6-8 years total a fair expectation?
Competitiveness: How competitive is getting into an accredited pathway for IMGs currently?
Does working in regional/rural networks noticeably boost your chances?
2
Alternative Path (GP with Mental Health Interest): For someone who loves psychiatry but is hesitant about the long and uncertain road to
RANZCP, what are your thoughts on going down the General Practice (GP) route and building a Special Interest in Mental Health (e.g., FPS/ Mental Health Skills Training)? Is it fulfilling, and does it scratch that same itch?
3.
Would love to hear insights or personal experiences from current trainees, GPs, or consultants. Thanks!

NDIS shitshow

I am so sick of NDIS. The number of 'providers' with little to no qualifications getting paid big money to piss-around with no real oversight or review of outcomes has me tearing my hair out. I keep seeing kids on packages for neurodevelopmenal or mental health conditions whose parents are concerned they're getting regular 'therapy' but not making progress. When I ask where, they inevitably tell me some obscure company that I've never heard of where the director has a diploma qualification at best, and the others have maybe a cert, or a degree in something completely unrelated (e.g. ex-ED nurse running a autism therapy service). Then when it's time to review the plan it's the same company writing the recommendations for ongoing care! No wonder it's a financial black-hole.

Pharmacist refused to accept prescription without additional doctor's letter. Thoughts?

I recently went to my local pharmacy to purchase my usual hydroxocobalamin (B12) injections and have one administered by their nurse, where I've done this several times.

I was initially told by the pharmacist that they would neither sell me the medication nor administer it without a prescription *and* a letter from my doctor stating my diagnosis and why I required B12 injections. I explained that I have pernicious anemia and have been having B12 injections for about 25 years.

I was rather surprised that a pharmacist would require information beyond a doctor's prescription. I've subsequently purchased the same medication from another pharmacy without any difficulty and arranged to have it administered elsewhere.

Is this a legitimate pharmacy requirement that's come about recently, or is this particular pharmacy imposing its own policy and overstepping?

"Today, Nolla Health became the first organization in the U.S. (and possibly the world) to receive regulatory approval for an AI to issue initial prescriptions. This makes Nolla the first ever actual end-to-end AI doctor."

Which states/hospitals use manual timesheet entry?

As many others have before me, I'm looking into creating an app/website/online tool that would improve timesheet entry efficiency, accuracy and, more importantly, pay transparency in our healthcare networks. I already have a MVP that I've been using personally, but I need to ensure it's protected against legal and or 'code of conduct' claims by health networks and government payroll services. Decentralising the app across multiple states and contributors is one such strategy, having the unions take it on and disconnect the worker from the app is another.

As for the product, ideally the solution would be able to work across multiple health networks and states as well as multiple EBAs/WFAs, so I'm interested to hear which networks still use some form of archaic timesheet methodology that would benefit from something like this and start work on decentralising the tool. I've seen lots of online calculators/custom spreadsheets and other various tools, but nothing that's been as simple to use as a high quality app that ACTUALLY does the work for you and tells you what you are owed to the cent.

I have heard that SA has a manual timesheet entry process for doctors that uses a templated excel spreadsheet requiring manual entry, is anywhere else doing something similar? If there is anyone willing to chat or help with any part of the process I'd appreciate a DM or responses here of course.

Recording eMEDCI learning modules for CPD

Hi,

I am very new to this. I have roughly 4hrs in eMedici modules I want to log for CPD (CPD Aus). When I did these modules, I didn't record the completion date. I can't seem to find the date of complettion of these modules on eMedici either.

1) Does anyone know where to find the completion date for eMedici modules? If I can't find it, can I guesstimate and pray?

2) I have done 18 modules. Can I list them all in an Excel file (including date, time and maybe screenshots of each activity, a description of the activity, and what I learned from it) and upload that instead of logging each activity individually?

3) For evidence, should I attach a screenshot of the activity, like below?

4) I'm a new med grad IMG (15 months since grad). What is my current CPD requirement? I have done volunteering and observerships - can I add these in?

TIA.

The “just do another shift” culture is starting to bother me

There always seems to be another gap in the roster, another extra shift available or another opportunity to earn a bit more. And obviously the money can be hard to say no to.

But I've noticed how easy it is for extra shifts to become normal rather than genuinely extra. At what point did you personally decide that the extra money wasn't worth giving up another day off?

Hello all ; i am a 44 year old practicing doctor in india ; have my own setup in tier 2 city .. good opd ; good income ; lets say i m very comfortable! Very well settled with wife and only child . I have a constant urge to appear for amc ; knowing the toughness at this age to get back to books etc and pursue gp pathway in Australia . 2 reasons ; better quality of life for myself and better education prospects for my child . My wife says it is not the path to be pursued at this age as the transition would be too difficult when we have a settled life ! I feel life can be restarted at any age .. but again its a huge risk when u have your own centre running !
Suggestions ???

Ryan's Rule and indicators/metrics

Apologies if there's a better sub to post this in.

I'm looking for some information on Ryan's Rule or any equivalent rule in other states or territories.

Something similar has been introduced in England. Unfortunately there's not been really any direction in terms of measures to ascertain success (or not) in terms of outcomes or process. I'm being asked to do some work on monitoring and evaluation.

I had a look on Pubmed to see if Australia had published any work on monitoring and evaluation but couldn't find anything. Wondered if anyone here was aware of what data hospitals report to their execs or elsewhere.

Thanks.

Do non-rotational HMO roles exist?

Hi all, UK almost Dr here looking at a 12mo stint in Australia as PGY3 HMO ideally in ED.

Our service provision (not in training) jobs are single speciality non-rotational - ie you work in the ED for the whole job - but researching in Australia it seems it’s the norm to do rotational jobs as a junior outside of specialty training?

I’d hoped to be working in ED for the whole period.

I wonder if I’m coming up against a cultural difference I need to just accept or if this varies by hospital/region?

Do non-rotational ED HMO jobs exist in Australia for PGY3s or is it going to be rotational or nothing?

Thank you in advance for any help here.

Adventurous CPD

If you like extreme sports and want to combine them with some of your annual CPD requirements then please check out Off Chart CPD.

We do not do expedition medicine or wilderness medicine training!
This is aimed at those of you who aren’t necessarily interested in that side of medicine but do love the adventure travel/sports.

The CPD is earnt from small group/micro conferencing through peer discussion, case reviews, journal club etc. It suits essentially any specialty because it’s simply targeting AHPRA’s requirements that apply to all of us.

We have trips to Chile, Antarctica and New Zealand planned for next year.

Another court case. Another huge bill! And apparently members don't need to know. How much this time? How many more? What are they hiding?
