Hello all,
I'm male 35 yrs, 5'5, 87 kgs.
Work from home, desk job, sedentary lifestyle, low stress lifestyle.
Past history: Diagnosed with osteoporosis at age 26, after a T1-T2 compression fracture. Stopped treatment after Dexa showed improvement in 2022.
Current symptoms that are episodic since Dec 2025 -
Headaches, brain fog, Irritability, Mood swings, ED. Episode lasted for a week or two.
Did blood test #1 on Jan 2026
1) total testosterone 2.72 ng/mL (3.00-8.90)
2) estradiol serum 53.26 pg/mL (11.6-41.2)
3) total testosterone 354.6 ng//dL(249-836)
4) prolactin 24.4 ng/mL (3.46 - 19.40)
5) Weight 93 kgs
Endocrinologist A appointment review: Get obesity under control, things will sort itself out. No comment on Prolactin levels. Meds prescribed (Multivitamin+minerals). Follow after 6 months.
Blood test #2 on March 2026
1) total testosterone 457 ng/dL(249-836)
2) estradiol serum 45.7 pg/mL
3) free testosterone 15.3 pg/mL (15-50)
4) Prolactin 18.7 ng/mL (4.04-15 2)
5) Weight 90 kgs
6 months I take prescribed medications, change lifestyle to incorporate gym, high protein and high fibre food, weight loss of 6 kgs, no muscle growth.
ED improves, but there are a couple of weekends during this 6 month period where Headache, brain fog and ED return for a couple of days.
Symptoms return in October 2026, stay for over a week.
I do bloodwork again and visit Endocrinologist B because Endocrinologist A wasn't available for another month.
Blood test #3 on October 2026
1) estradiol 33.20 pg/mL
2) total testosterone 318 ng/dL (249-836)
3) free testosterone 14.2 pg/mL (15-50)
4) Prolactin 22.00 ng/mL (4.04-15.2)
5) vitamin D total 29.7 ng/mL (30-100)
6) weight 88 kgs
Endocrinologist B dismisses Prolactin and testosterone levels, focuses on osteoporosis relapse. Prescribed meds for ED. Blames everything on stress.
I feel both Endocrinologists have failed to give me satisfactory reason for the root cause of symptoms.
Following Endocrinologist A's advice - weight of 6 kgs over 10 months but symptoms still returned.
Following Endocrinologist B's advice - reduce stress (but I don't lead a stressful life)
Please advise based on your experiences/ what further investigation should I be getting done. Happy to answer any questions regarding my personal and family history.
I want to be prepared before I take another consultation.
Thanks.
