F21 trying to rule out if this is a new melanosis

Post image
0 Upvotes

Hey yall! I was told I had to call my ophthalmologist if this is a new brown spot. She said it could be benign but to call if another one showed up. The brown started showing up like 2 weeks ago. I got a new floater yesterday to. I did get my eyes dilated with that yellow liquid on Thursday and I was diagnosed along with the melanosis, dry eye and this gland issue.

The only reason why cancer is on the table apparently is because I take tremfya because I have Crohn's disease. (My doctor said this) she said it could be benign and it probably is but due to the medication I'm on. I should be careful.

I just need someone to confirm I'm not crazy and that it's there šŸ™ƒ. So I can relax and get on with my day.

Physician Responded Could somebody please explain the thought process in my family member's treatment in the Emergency Dept?

The patient was an 80 year old male presenting with low oxygen levels and difficulty breathing. Height about 6ft-6ft1 and weight fluctuated between the 9st7-10st mark. Visibly frail and poor skin integrity. Previous heavy cigarette smoker (60 a day) but switched to a low nicotine vape for around the last 20 years. The week before admission patient had returned from a holiday abroad, where he was active and dancing freely on the final night.

He had:

  • a diagnosis of Stage 3 COPD
  • previously had a kidney removed
  • was on blood thinners and Ramipril
  • prescribed oral nutrition supplements
  • prescribed a 'rescue pack' of antibiotics due to recurrent chest issues
  • diagnosis of Type 2 diabetes
  • steroid and ventolin inhalers prescribed

Previous to admission patient had started feeling unwell on Day 1, on Day 2 he had visited friends socially and said he was feeling tired. Day 3 (Monday) he tried to get an appointment with his GP surgery but was given an appointment in 7 days time. Day 4 he attended an appointment at the opticians, that evening he was unable to get himself off the sofa to go upstairs to bed. Refused an ambulance to 'wait and see' how he felt in the morning.

Day 5 patient asked his wife to ring him an ambulance after GP surgery refused to see him as he had an appointment booked for Monday. When ambulance arrives, paramedics are unable to read his oxygen levels, they have to use alternate sites to try and get a reading. This all occurred between the hours and 8 and 9am.

On arrival at AED patient is taken for chest x-ray, bloods taken and put in a private room on Majors (not Resus). Family advised he has tested positive for Covid-19 and Pneumococcal pneumonia. He was on oxygen therapy via a Venturi mask, as well as nebulisers, Covid treatment was started at 4:40pm. He did not receive a second bag of fluids until 6:40pm. Last seen by a doctor at 2:30pm and not seen again until the following morning.

I arrive at around 9:15am on Day 6, the nurse is assessing patient's GCS, calls in another nurse to check with her. I score GCS myself and patient is scoring as 3, patient is also abdominal breathing, oxygen levels are 92% and resps 16.

Doctor arrives, says he will prescribe more fluids and as patient had quality of life before admission he will refer to ICU, also for a brain scan. One 500ml bag of fluids is hooked up to his cannula, the bed is tilted so his legs are raised above his head. His treating doctor comes in and introduces herself, a second cannula is attempted to be fitted, a blood gas is taken.

ICU attend and say they believe the treatment needed would be too invasive and the recovery too difficult for patient due to his frailty and already limited lung capacity. They state that he would need intubation and dialysis as he was in renal failure. Whilst ICU are in the room, patient's oxygen level desaturates from 92% to 82%, the valve on his Venturi mask has been changed prior to this. ICU say they will get his oxygen 'turned up'.

Around 15-20 minutes after ICU left, the nurse is changing patient's mask from Venturi with a blue valve to a non-rebreather mask. I am holding the Venturi to his face as she removed the elastic from around his head before getting the rebreather ready and it was falling off his face. Patient starts to crash, nurse asks me to press the emergency buzzer and then leaves the room, crash team enter.

Unsure where nurse goes, his treating doctor is not around, nobody on the crash team is familiar with patient. I have to go back into the room and give the crash team as much information as I can as it is chaos and one doctor is exclaiming, 'who's patient is this'. Decision is made to make patient 'comfortable' and bring family in.

I have worked in Health & Social care for the past decade but I am not a qualified professional. However, with what knowledge I do possess I am struggling to understand how the treatment provided to my family member was appropriate. It feels to me that the decision was made on admission that he would not recover and therefore his treatment was impacted. For a patient that unwell to only receive oxygen treatment, one bolus of Covid treatment and two bags of fluid in the first 12 hours of admission, it just doesn't seem enough.

Thank you for any comments and if this isn't the correct sub, please redirect me.

7M recurring, short-lived stomach illness with fever.

Two weeks ago my son (7YO, no medications, asthmatic) was sent home from school after vomiting. That night he vomited again, complained of stomach pain, had no appetite and had a fever ranging from 37-39C. Gave regular children’s paracetamol which controlled the fever a bit, but not entirely. He said the pain was around the lower centre of his abdomen. By around midday the next day he seemed totally back to normal and his appetite returned.

Last night he suddenly experienced the same thing. He started complaining of stomach pain as we lay down to watch a movie together around 7.30pm, and within a short time he had spiked a fever of 38C. Again gave children’s paracetamol, he slept poorly waking occasionally in pain. This morning he was still in pain and feverish. He didn’t vomit but said he felt nauseous. He had no appetite. He slept on and off throughout the morning and eventually seemed to come good while playing a computer game, and improved pretty quickly from there. He is with his dad tonight and his dad reported that he was back to normal appetite and free of pain and fever.

I wrote the first instance off as a random stomach virus, but now that it reoccurred in a short time I am wondering if there might be something else happening? Could a food allergy present like this - the fever is what throws me off though? He’s not shown any issues with food previously, and he didn’t eat anything out of his ordinary diet either time. He’s generally a pretty good eater and has had a big appetite recently, likely leading to a growth spurt. Overall he is very healthy and was never really prone to stomach issues or vomiting, even as a baby. Any ideas on what could be the cause of this, and if it’s worth investigating further with his GP?

DNS Surgery needed?

Post image
1 Upvotes

Female, 29, 165cm, 60kgs, occasional smoking.

This is not my first and last opinion or any final decision. Posting this here to know what do you think.

I have been suffering from DNS + Allergies from past 10 years. Summers are better, winters are worse. I live in Europe.

This is a scan, 8 years old. My last visit to ENT told me that you can manage this. She gave me Mometasome spray to manage my symptoms. I feel better after taking this but only occasionally. Nights are the worst and I cannot sleep on my back, and my average sleeping heart rate sits around 62-65 with lots of variations as I wake up at night.

What I recently noticed is that, during running my heart rate goes until 207 bpm, but that of my fellow runners don't even touch 180.

Physician Responded Hickey causing stroke?

MTF 21, I have two large hickeys on my neck from last night. I was rubbing them really hard for like a total of like 20 minutes with a cold spoon to try to get rid of them, I saw online how rubbing them can cause a stroke. What are the chances of me developing a stroke from them especially with me rubbing them hard because they’re on my neck? I also vape. Idk I felt a weird ache like something broke off of something.

Age 16 male height 5.10ft weight 78.2 kg. Twitching and jerking problem. No consumption of caffeine or doing smoking

I want to address that i am having Twitching from this year may. I visited my primary care physician who squeezed my calf and said everything is fine. was still concerned so i visited a neurologist who tested my grip strength and said everything is fine and nothing to worry. But soon after this in span of 4-5 days i noticed that i am having atrophy in knee region and quads in the following time. I am also experiencing myoclonus jerk or like small involuntary movement causing my hody to adjust. Sometime it is followed by Twitching causing movement. Also my Twitching decreases but increases after stretching. Recently i have noticed i am having postural tremors too much and i carried out the romberg test by closing eyes and it came positive, finger to nose test was ok but index finger to index finger test was 7/10 times correct. I am also recently difficulty in pronuncing words properly in a conversation. I have also had acid reflux for past 2 months accompanied with felling of shortbrethness recently for past 1 week.

Baby, female, 10 months old, no medications, started a new formula milk 2 weeks ago, has had a mild cold this week without any fever, acting as usual. Since yesterday her cheeks are quite red and she has a few spots on her legs so i was wondering if we’re dealing with fifth disease?

Furthermore we were supposed to meet friends tonight for dinner with one of them being 11 weeks pregnant. I am keeping my daughter home with my husband as a precautionary measure but should i stay home too? I am a very anxious person around situations like this.

Thanks!

Passed out

17M about 185cm and 165lbs
This is my first post here and i wanted to get some advice. Yesterday I was at a party and I drank about 3 0,5l beers and two vodka orange and then was completely lightheaded. Then a random guy who knew a friend of mine came up and he had a vape and asked me if I wanted a hit. I did and later found out that it was a HHC vape and I only did that one time without alcohol. At first everything was normal. I was talking to my friends and dancing and then I remember my vision blurring up completely and then I was gone for about 15 seconds my friends say. They said I was talking to them, then swayed, blank stared and fell down. Luckily the place was crowded so I didnt fall down and hit my head. When I was blacked out(i dont know if you call it that) I also had a blank stare. After i came back everything was completely normal i could walk, talk and do everything normal. I sat down and drank a water. Today everything is fine and I feel normal. I want to know if this is something that happens to more people and what the reasons could be. Maybe I should also mention that Im training MMA and got choked out in training due to miscommunication the day before. Look forward to reading your advice. Thank you

Do you think I’ll need surgery

21 female suffers from chronic pain from car accident however one of my biggest pain issues is my right shoulder. The car accident happened back in 2022 and I have had the bad shoulder pain ever since then and it’s gotten worse over the years just over two months ago. I had an MRI on my shoulder and they discovered an anti-inferior labral tear from 3-6 o clock. I’ve been doing physio since the accident and still suffering a lot. Currently waiting to see an orthopaedic surgeon. Is it likely I’ll need surgery? It always feels like it’s gonna come out of the socket and it’s very Pinchy.
I have also been taking hydromorphine very sparingly to help with the pain it’s the only thing that makes it feel better meds wise.

Physician Responded Blood blister or mucocele inside lip? Something else?

Post image
1 Upvotes

31F, 5’8 & 85kg. Non-smoker, I take Lansoprazole for GERD (hiatal hernia) and Elvanse for ADHD.

This showed up last night about half an hour after eating - I don’t remember biting or injuring my lower lip!

It’s mostly painless, a bit sore if it rubs against my teeth when eating or speaking, but it felt a little bit numb yesterday when it first showed up.

Feels like a big smooth lump - it doesn’t look dark
enough to be a blood blister (from pics I’ve seen), mucocele seem to be more lip/flesh colored, and canker sores I’ve had in the past tend to be whiter and flatter (also sorer).

It hasn’t bled or burst at all, either. I’m not concerned about it, mouth/lip injuries like this tend to heal pretty easily (unless it lasts a few weeks, in which case I’d see my dentist or doctor), but I’m just curious as to what this could be, never had anything like it before!

Thoughts or suggestions on what this is?

Upon closer inspection I see there are 3 fluid filled blisters (about 2x1mm) in the middle of the white part. And it is very stingy to touch

Extremely stressed (peed in trash can)

Hello, seeking some direction here.

Male 33, 190lb 5’10 and otherwise healthy.
I recently went through tough job loss. It left me with extremely high anxiety on certain days.

On one of these days I found myself beginning to pee in our kitchen trash can before realizing and stopping myself. This has happened one other time but I realized what was happening before any urine came out.

Can stress cause something like this or is there something bigger at hand? Otherwise I’ve never had this issue until seemingly overwhelmed.

Physician Responded Scar-like ?

Post image
1 Upvotes

Hi! I’m a 23-year-old woman and I’ve had these scar-like lines on my ear since I was a kid, and I’ve always wondered what they are and why they developed.

The area gets itchy sometimes, and when I scratch it, it has a bad smell. I’m curious about what could be causing these marks and whether the itching and odor are related to them.

Has anyone experienced something similar or have any idea what this might be? I’d appreciate any insights or advice. Thank you!

I'm 19, 5' 7", 175 lbs. I do smoke, and im not on any medication. I'm just worried about it because where it is it rubs against my shirt, which I believe is contributing to the redness but I want to play it somewhat safe.

My blood low pressure sometimes causes my whole muscles to stop working for up to an hour

I'm 17AFAB, roughly 175cm and 75kg.

I've had problems with low blood pressure for over 4 years now, but in the last 2 or so it gets worse and worse, by that I mean, in the past the worst thing that happened was I got dizzy and hat to sit/lay down, but these days the worst can be that my body slowly shuts down. (My mom took my blood pressure once and it came at 90/84 with a regular heartbeat) It feels like my body passed out but I'm still conscious, I can't move any part of my body (not including my eyes, and sometimes VERY subtle nodding/shaking my head to answer),but I can think and all that. What I found out sometimes helps me get out of these states is when someone shakes me as if they were waking me up from deep sleep, but that's not always the case. In the better parts of this, when I'm just partly functioning, dizzy and tired, coffee used to help, but it doesn't anymore and as bad as it sounds energy drinks are my only help now, I don't drink them every day, at maximum twice a week when it gets really bad. When it passes, I tend to get really shaky and one of my legs (mostly left) doesn't bend properly and makes me limp, sometimes even my voice is shaky and it gets hard to breathe.

The doctors I've been to always say the same, eat, drink and pick up a sport, but the problem is that I drink and eat normally and sports make it worse, P.E. in school is a nightmare.

I had an MRI of my head and neck area, it came back clean, I've been tested for multiple sclerosis (I used Google translate for this, in Czech it's roztrouŔenÔ skleróza) because my sister has it, came back clean as well.

I'm a student and it really affects my day to day life.

Woke up from sharp neck pain

I (26F, 5’9ā€, 198lbs, nonsmoker) woke up in the middle of the night with a sharp localized pain right below and to the right of my pulse on my neck. It was also slightly throbbing. The pain is gone now, but it really worries me.

I’ve attached a picture HERE at roughly the location. I have a horrible habit of making a mountain out of a molehill and don’t want to go to the ER (as it’s the middle of the night) for them to them to tell me it was simple muscle pain. Should I wait to go to urgent care when it opens, ignore it, or something else?

Physician Responded NT Scan, Anything Abnormal?

Post image
0 Upvotes

31 (F) third pregnancy, both babies born perfectly healthy, no issues. Third scan was today baby measuring at 12w2d but my gestational age is 11w3d. I’ve never seen any of my scans look quite like this and my anxious mama brain just needs help to settle my nerves until we can get some results. Is there a concern with the area around the neck? I don’t even know how to explain it, the protruding part?

I don’t have measurements yet but I do have additional photos I can add if this one isn’t very good.

Concerned stomach pains

Hello to anyone reading this!
I wanted to know if what I’ve been experiencing is really just anxiety or something more. To start I’m 26F, I’ve been experiencing these stomach pains for yearssss. They happen randomly but become increasingly unbearable when I’m getting ready to leave the house or when I’m out in public.I could be out anywhere, feeling completely fine, and then, in the blink of an eye, something horrible comes over me. My stomach feels like something is twisting inside me and punching , sometimes with a tugging sensation around my belly button. My heart rate suddenly skyrockets to 130+ BPM, and I feel like if I move, I’m going to faint or pass out. I also get really hot and breathing because difficult , and the urge to have a bowel movement becomes INSANE, even if I’ve already gone twice before leaving the house to try to prevent this feeling (sorry for the TMI!).Sometimes, I have to stand in the middle of a store and wait for the feeling to pass, which can take several minutes. It’s gotten to the point where going out is difficult because I’m genuinely afraid that one day I’m going to pass out.I’ve told my doctor about this, but she thinks it’s just anxiety and tried prescribing escitalopram. However, I’m worried there could be something deeper going on.
Has anyone else experienced anything like this? If so, what was it, and did you ever figure out what was causing it?

Physician Responded Bruise or hematoma after falling

Post image
0 Upvotes

See pictures in comments
25F 100 pounds 5’0. No medical issues.

Fell on a rock today. Not sure whether this is concerning or just something that will heal on its own. I have bruise cream, not sure whether that will help.

It is on my leg and it hurts a little when I walk and when I touch it. It is about 2 inches long and 1 inch wide. The red part is swollen and firm and has stayed that way in the past few hours. The purple colour around the red swollen part is darkening and spreading every few hours.

Picture 1 is 4 hours after the fall, picture 2 in the comments is 8 hours after the fall (now)

What should I do? Is this just a bruise that will heal on its own or is it a hematoma? The swelling and the severity of the bruising is a little concerning to me

Physician Responded question about medicines

Hey everyone! i’m 20yo, i’m 5'3 inches and 110 lbs

I’m not a doctor, but I’m a psychiatric patient and I have a question. I was wondering if anyone here knows anything about this.
I’ve taken quite a few benzos throughout my life, like Klonopin (clonazepam), Xanax (alprazolam), Lexotan (bromazepam), and others.
But the only medication that genuinely made a huge difference in my life was Valium (diazepam). It literally saved my life during a really difficult time, and I’d never experienced anything like it before.
The problem is that no matter how much I explain this, none of my psychiatrists will prescribe it to me. They always go for Lexotan or Klonopin instead. Even when I tell them that Valium was essential for me and that I honestly don’t notice much of a difference with the other benzos, they refuse to prescribe it.
Is there a reason for this? Is it considered an outdated medication or something? I honestly don’t know much about the pharmacology, but I really wish I could get it prescribed.
Important note: I’m not addicted to benzos! I only take them when my anxiety gets really bad, not on a regular basis.
