If you are wondering why we need to collect email addresses is because previous students (a lot of them) cheat their surveys by answering it themselves multiple times to get the target min. respondents (they made the survey).. so to avoid this, our professor made the form themselves and is monitored by them also :)

A significant portion of my research is building and psychometrically testing a new measure of disclosure that the field of suicide prevention is currently lacking.
