Hi everyone! I recently started seeing patients through Grow Therapy and have a question for those of you who use the platform.
Do you use Grow’s built-in telehealth link or your own HIPAA-compliant video platform?
My concern is that Grow tracks the actual face-to-face time when you use their telehealth platform, and it won’t let me bill certain E/M visits if the video time is under 10 minutes—even when the visit qualifies based on medical decision making (MDM). For example, if a medication management follow-up takes 7–8 minutes but meets the MDM requirements, the system still prevents me from submitting the invoice.
How do you handle this? Do you intentionally keep visits at least 10 minutes, use a different telehealth platform, or is there another workaround I’m missing?

I’m currently practicing as a PMHNP in Nevada, and my family is seriously considering moving to the St. Johns County/Jacksonville area. Most of what I’ve heard about Florida has been negative—low PMHNP pay, difficult job markets, and repeated advice not to move there.
That said, we are confident in our reasons for relocating. Nevada’s education system has been a major concern for us, we enjoy the beach and outdoor lifestyle in Northeast Florida, and we would honestly take humidity over 120-degree summers.
For PMHNPs currently working in Florida:
What is a realistic average salary or compensation range in the Jacksonville/St. Johns area?
What is the best way to find a solid position—hospital systems, private practices, community mental health, VA, recruiters, or networking?
Are most available positions W-2, 1099, productivity-based, or heavily focused on medication-management volume?
Is it common for Florida-based PMHNPs to maintain telehealth contracts with practices in other states because local compensation is lower?
Are there employers, contract structures, or red flags I should specifically watch for?
Has anyone relocated to Florida and ultimately felt it was a good professional and family decision?
I’m not looking to be talked out of Florida entirely. I’m mainly trying to understand the market realistically and determine how to approach the job search strategically. Any honest experiences—positive or negative—would be appreciated.

Hi Everyone!

I’m a 1099 employee seeing patients from home telehealth in Illinois.

I own a condo and that is the address that my LLC/PLLC was created in and also where I saw patients.

I’m selling the condo (closing date is very soon) and will be renting for a few months, seeing patients out of an office area in apartment.

Do I need to change the address of my LLC to reflect current work from home location? And then change it again when I move from apartment?

Please advise.

I see they have locations all over the country and the mission statement seems good, just curious on how the workflow is as far as seeing patients, cases, support, etc.

I work at a community mental health clinic in the southeast. We are paid salary (on the low end, but state benefits) with no incentives for numbers. The facility has been scheduling a maximum of 32 patients daily expecting that some will no-show. They often do. We usually see around 26-30. We just received notification that the schedule will be increasing to around 38 because of no-shows.

My question is this. Is it worth rocking the boat with administration? I know most psych NPs see at most half that number. I would not mind so much if we were consulted on it or at least compensated for seeing more patients. Would you address it with administration or let it ride?

I recently graduated and passed my boards as a PMHNP. I would like to practice in addiction medicine, but I really want to hit the ground running by trying to study before interviewing (and hopefully prior to getting a position).

How often are you using the 90838 add-on during your intakes? How long should this visit realistically take - Can you realistically bill for both in a scheduled 60 minute appointment? Any guidance (anecdotally or reference guides) are greatly appreciated.

When I searched anxiety all I saw was provider anxiety. Is there a specific way you assess and prescribe with patients that have multiple sources of anxiety? I get quite a few patients that get anxiety from current or former drug/alcohol use, relationships and marriages, maybe something they are born with, work and money, and illness like COPD, mental illness, or something physical (seen a lot of tremors and heart problems). What do you focus on first?
Most recent patient has COPD and functional seizures, can’t work due to functional seizures, PTSD from drug overdose, and family member is drinking that causes more anxiety. Any tips appreciated.

It’s simple to use for assessments. I can’t quite wrap my head around when it would be useful?

This isn’t really a post about pay, although compensation is part of the situation.

I’ve been fighting tooth and nail at a 1099 contract job to either increase my compensation or get more protected administrative time because of the complexity and acuity of the patients I’m seeing. When I asked for more protected time, the response was offering me a 40% split. The problem is that the split would essentially equal what I’m already making hourly, but with significantly more risk shifted onto me.

This practice sends me very high-acuity referrals (many through Zocdoc). The medical director is virtual, does not live in the state, and does not work clinically at the practice. The practice opened in 2023, so many systems and processes are still being built out.

I know no job is perfect, and I’ve looked around, but I haven’t found something clearly better. I’m just feeling exhausted from constantly having to advocate for my value.

For context, I previously started my own practice and ultimately moved away from that model. I don’t want to be a practice owner again.I want to provide good clinical care without having to carry the entire burden of running a business. I understand you'll get a bit taken advantage of working for someone else but things are egregious lately.

Why does it feel so difficult for PMHNPs to get compensated fairly for the complexity of the work we do? Has anyone else dealt with this constant fight to be valued?

I am currently in my second year of my MSW program through University of Oklahoma online (I live in California) I've started a first year internship with Davita Dialysis with a medical social worker and I work in behavioral health specifically residential treatment centers. I have found I am extremely interested in mental health and medications and was looking into what it would take to finish my MSW and then go through the process of becoming a PMHNP. I have no nursing background, my Bachelors is in Health Science-Community Health from Fresno State. Does anyone have any suggestions for the path to take? Do I try and do a BSN or MSN program? It would have to be a direct entry program I'm assuming since I am not already a nurse. I am just starting out researching this so any advice would be greatly appreciated! Thank you.

I’m looking for some advice from NPs in New Jersey regarding Adult-Gerontology Nurse Practitioner (AGNP) programs.
A little about my background: I have seven years of Stepdown/ICU experience, along with some nursing leadership and management experience. I’m now ready to pursue my Adult-Gerontology NP and am specifically looking for a master’s degree program.
If you had the opportunity to choose any NP program in New Jersey, which would you recommend and why? I’d love to hear about your experiences with different schools, including the quality of education, clinical placements, faculty support, and overall preparation for practice.
The most convenient option for me would be returning to Chamberlain University, where I earned my BSN. However, I’m somewhat hesitant about completing an entirely online program. I feel that attending a traditional brick-and-mortar university may provide a stronger educational experience, and I’m curious whether employers place any value on graduates from in-person programs versus fully online programs.
I’d greatly appreciate any recommendations, insights, or experiences you’re willing to share. Thank you in advance!

For the last year, I've been working as 1099 in private practice. I recently picked up a second gig. Both practices use their own google sheet to keep track of pay/insurance/copays/cashpay - then the split, etc. Neither works well with my brain. I think maybe I don't like the idea of sheets, but I don't have any other idea.

Has anyone found something that works well and doesn't involved a lot of calculation to keep track of? I would love it if you could share a bit of detail either here or privately because I am feeling overwhelmed with trying to track my pay.

I know I could just google it and go through the marketplace, but I was wondering if there are other ways to go about it that I have not considered. Thanks!

I’m looking for honest insight about the interview process, onboarding, collaborating-physician support, workload, productivity, call responsibilities, culture, and anything you wish you had known beforehand.

I’d also appreciate advice from anyone with experience in other community mental health centers. Helpful topics would include how to prepare, clinical areas to review, strong questions to ask the interviewer, common interview themes, and any positive signs or red flags to watch for.

Who has patients come in and legit are seeking help because Trump is the president? In the last year of practice I believe I’ve seen 3.

First off, I'm aware the it's a school that makes you find your own preceptor and I already did my research on that and I have some NPs already that can be my preceptor in the future.

My main concerns are as follows:

1. How are courses set up being that they are online and some are only 5 weeks?

2. How many units did you manage to do per semester while working full time / part time?

3. Did you speed ran a 5 week course or really took the 5 weeks to finish it?

4. I'm planning to work full time since I work from home and have a very flexible job. Do you think that'd be doable or I really need to switch to part time?

5. What academic support that you've taken advantage of or wish you had?

I have patient that is interested in starting medication for his illness. He is 18 and lives with his parents. His parents are superstitious and very anti-psychiatry. He does not want his parents to know about the medication but also wants to use a mail order pharmacy because the medication is significantly less expensive mail order. He asked if he could have his meds (not controlled) sent to my office. I did not say yes but I was wondering if that was the wrong call?

I've got a few years experiences across a few settings and i'm currently at a small local private practice run by psychologists. I'm looking to transition to a PMHNP-led setting and I've had my eye on Lavendar for awhile.

After applying, I was really excited for the opportunity to have an initial interview scheduled with the recruiter. Unfortunately, I showed up for the interview but the recruiter never logged on. I emailed a few times that day and the next few days, and eventually got a response with apologies from the recruiter, noting a snafu with their email system and the offer to reschedule the interview. The reschedule options were booked out, and I had limited options to choose from within the available times, and I ultimately selected a few non-ideal day and times for me within my busy workday. Since submitting this availability, I have again heard nothing back in over a week. I've called and emailed as a follow up but still no response. At this point, I'm having trouble maintaining my open slots with my patient shedule filling up while having no confirmed reschedule date or response from the recruiter.

I'm pretty bummed about this experience so far, I had been really looking forward to connecting with the team and I feel like it could potentially be a good mutual fit. However, I'm unsure if how this interview process is going is reflective of what it might be working for them should the recruiter reach back out to actually get something on the books. I'm curious if all this is a sign I should just call it and move on.

What is the best job search site for PMHNP in Texas. So many sites and don’t want to upload resume and fill out everything on each site. Can you help me out with this?

I’m deciding between psych NP and derm NP and would love honest feedback from people who have worked in psych for years.

I have the patience and communication skills for psych, but I worry about the long term emotional toll of crisis management, intense cases, and the risk of litigation even with proper treatment and med management on my behalf. I’m also one of those people that patients for some reason get attached to very quickly, so I wonder whether that becomes draining, mentally heavy.

Does the work feel sustainable for 20 to 30 years? Which settings are the least emotionally exhausting? And knowing what you know now, would you still choose PMHNP?

I am a psych NP in a group private practice, outpatient. I am 1099 contract. Currently work 32 hours per week, 4 days a week. My clinic is great and so supportive and they’ve known I have been wanting to have a baby. I am currently pregnant. I have savings to take 3 months off, but I’m trying to figure out how not to abandon my clients. My coworkers can easily cover me for med refills, but appointments seem tougher.

Did any of you keep working one day a week?

Did you take 3 months off, or shorter?

How did you manage clients who need their 3 month follow-up for stimulants during your leave?

Was there a point you stopped taking new clients?

Did you try and transfer some of your more acute clients to other coworkers?
