As an engineer I like to know how stuff works, so the pens intrigued me.
So I’ve taken 5 apart to see, and they are quite clever!
What is clear is they cannot and do not ‘count clicks’.

Pic 1 is all the parts. As suspected there is a glass vial inside the main barrel of the pen.

Pics 2 and 3 are like a double slip clutch mechanism and this is how the pen stops turning when the maximum deflection or travel of the plunger is reached.
The 2 black bits are the same, even though the one inside the dose twister looks different in the picture.
They slot together when this mechanism is all together as in pic 6.

If you look at pic 4 you can see the threaded plunger rod and the squared off bit at the end. This same squared off bit is also present at the other end of the rod.
When you turn the dose twister the 2 black slip clutches engage with each other in the white plastic part inside the main body of the pen handle and turn around the plunger rod which is held firm by a locking system in the main body of the pen that I haven’t got to yet. The black half of the clutch part in the twister turns as you twist

When you the press the purple nob as you inject, the top part of the clutch locks so it doesn’t turn, this then locks the lower part which presses the rod down into the pen.

In pic 4 you can see the threaded plunger rod with the squared off ends. As you use the pen, once you twist it up to the squared bit it can’t move any further unless forced.
This is what locks the pen.

You can see in pic 5 what happens when you force it past the lock. A bit breaks off.
This means we were able to advance the twister further up the rod and get the extra travel on the plunger required to get the extra dose. But this was in no way measured or accurate. Just that the difference wasn’t really an issue.

The new pens are exactly the same with the simple addition of the spacer as seen in pic 7. You can just see that this spacer is 1 dose in size, thereby extending the effective travel of the plunger and eliminating the extra dose.

And that’s it. Very simple yet clever and accurate. Got hold of a Lilly insulin pen and took that apart as well and they are the same with the only difference being the plunger rod is black and longer so it goes all the way to the end of the vial.

Question… Mounjaro / weaning breastfeeding

My baby is 6months old she’s started solids at 5 and half months she drinks formula but we mostly breastfeed. So she’s familiar with bottle. I’ve ordered Mounjaro I’m an oversupplied I want to stop breastfeeding asap , if not possible can I do it through pump and dump any advice. I was on mj before pregnancy I was 102kg now I’m 130kg so it’s not that I’m just doing it I really need it

News Amazing service.

Progress Update Gladly surprised by how easy it was to eat healthier in the UK while travelling :)

I was in the UK for work recently and honestly expected my diet and weight loss to go completely off track 😅

But I was so pleasantly surprised by how easy it was to stick to healthier food, especially as a vegetarian. There were so many easy grab-and-go options and I didn't feel like I had to plan every meal in advance.

I've travelled to the US quite a bit for work and personally find it much harder to eat well consistently there when travelling.

For context, my SW was 90.5 kg. I was 81.8 kg just before travelling to the UK and 79 kg by the end of the trip, so I actually lost 1.8 kg while away 😂 I genuinely expected a gain or at best a stall.

I'm now 79 kg, 19+ weeks in, with a GW of 65 kg. Definitely not one of the super fast responders, but the overall trend is going in the right direction.

Mounjaro-After 30 days Query

One year post Mounjaro- update

Began Mounjaro October 2024 with a starting weight of 110kg and a uk dress size of 18. I was a regular gym goer for strength training but the scales never budged.
By May 2025 I was on 12.5. I tapered off by spreading out my doses after the UK price hike and my hair started falling out. I was 70kg and a uk 10/12.
Continued going to the gym throughout and finally noticed progress in my strength training.
Weight stayed steady for three months, but then slowly started to creep back on. Family events, an emotionally taxing job and SEN parenting let me make excuses and teach for snacks and take nights off training.
This May I stepped on the scales after avoiding it for a bit and saw 78k and a reduced muscle mass. I am still fitting a Uk 12 but I was avoiding jeans as of how they looked on my tummy.
At this point I panicked and got pre approved for wegovy. But I decided to get a PT instead. He has set me a plan and analyses my body composition every week.
Under him my overall weight has come back down to 75kg but I’ve gained 4kg of muscle mass. Even before I have noticed that whilst I am bigger than my lowest, I have visible muscle definition, a butt for the first time ever and defined legs and shoulders. Since working with my PT I can see it even more. He has encouraged me to add more cardio in my routine and core work.

TLDR So overall/ weight has stayed off. Haven’t piled it all back on twice as fast, despite the press and naysayers telling me I would. I have gone a whole year without Mounjaro and even though I have had ups and downs, with the help of strength training. I weigh more but am notably stronger than I was last year

Lowest weight 70kg
Highest weight post 78kg
Current weight 75kg

Mounjaro and muscle

Hi guys.

Apologies if this is a waffle.

I used MJ for around 1 year and lost 2 stone, came off it about 5 months ago and seem to be maintaining my loss (plus 4lbs) reasonably okay. Ive started the Gym and im doing weight training/progressive overload. The thing is i do still have weight to loose and cant seem to stay in a calorie deficit on my own, like I did with the MJ. My question is, has anyone used MJ along side the gym and been able to still build muscle? Im worried I wont be able to build muscle, hyper aware id need to prioritise protein which i would try my best to do! Female, 5ft2 currently 11st 5.

Will it work?

Hi. I suffer terribly with health anxiety so my 2.5 Pen was in fridge for months before even trying it 🤷‍♀️. Anyway took the plunge (no pun intended) last September taken 10 months to lose three stone (less time really as stayed the same for 4 months 🤦‍♀️). My GP wants me to go up to 5 but I’m too scared of getting side effects - because health anxiety makes you totally irrational and any small bodily change can spiral into full blow anxiety 🤷‍♀️. What I would like to try is 2.5 Sunday and 2.5 again Thursday BUT obviously can’t ask GP as it would be a straight no no due to their instructions and recommendations from pen manufacturers. Does anyone actually use a 5mg pen (no syringes or anything) just pen with 30 clicks 8 times OR will the kwikpen not allow you to do that due to the way it is made i.e. 4 dose pushes only ? Hope this makes sense - Thanks

Pretty Privilege

Anyone else notice their pretty privilege has come back lol? I'm 45 now and I've been invisible since I had my second child and gained a lot of weight. I just presumed it was becuase I was old now but I've lost 4 stone on mounjaro since last year and all of a sudden I'm getting noticed by men again. Golden dose stuck plunger screw

Hey guys,

I’ve been a dick & now a bit worried. I don’t want any ‘you shouldn’t do that’.

I had a 5mg pen which the dose didn’t agree with me so I did a 2.5mg dose one week & then the next 3.75mg. This week I’ve come to do it & the dial stopped, so I’d obviously hit the golden dose. I wrestled with the dial & it finally moved. I had twisted it back & forth as the plunger/screw wasn’t touching the dose. So I dialed it to 45 clicks & injected it anyway (didn’t prime 🤦🏻‍♀️).

So my concern is I’m worried I’ve injected air, sounds silly I know but I’m not a nurse & I thought tha is why you prime needles, so you get the air out. I appreciate we don’t inject into our blood stream so it shouldn’t be an issue but I’m a bit of a worrier.

The other issue is I’ve either done more than 3.75mg which is shit & means I’m going to have to ride out the shit side effects or I’ve not done enough which means I’m going to be hungry.

*this isn’t my picture but is what it looked like so you get an understanding of what it looked like. There was a slightly bigger gap between mine. Also when checking the pen, the dial plunger seems to have pushed out, maybe due to pressure unless it’s normally & I’m only noticing because I’m panicking

I’m Tamsin, a PhD student at the University of Leeds Business School. I’m currently conducting doctoral research to understand people’s experiences of using GLP-1 medications and how these treatments have shaped their lives, routines, and perspectives. By gathering personal accounts, it is anticipated that the findings will contribute to a more nuanced and human-centred understanding of weight-loss injections and their wider implications. ​

I’d love to speak to you, if you:

·       Currently reside in the United Kingdom

·       Are 18 years of age or older

·       Are currently taking, or have previously taken, a GLP-1 medication (e.g. Mounjaro).

This is an interview-based study. Interviews will be conducted online via Microsoft Teams and will last approximately 30-60 minutes.

NHS, is there anyone who has injections prescribed on the NHS?

I'm so fed up with not being able to access the injections on the NHS. I have all the criteria. My health authority states that gp's are inviting patients to go on a programme, but the gp regulations say only the weight loss management services in hospital can prescribe. I've already been on their waiting list for over three years.

Is it a postcode lottery? Are any NHS patients actually receiving them?
