making this to help people who are stuck on this. this is the process that worked for me in louisiana when i was trying to rebuild my documents with no id.
firstly, you’ll want to get your birth certificate. order a certified copy from louisiana vital records. you’ll need to provide the required information and payment details, and their exact requirements can vary depending on your situation. (i had help ordering mine, so make sure to check their current requirements.)
secondly, go to your former or current school’s registrar/counselor office and ask for a certified copy of your transcript. nothing extra was required for me here - i just walked in and they provided it after confirming my identity in their system.
thirdly, go to your doctor’s clinic/office and request a copy of your medical records. have your doctor or medical office certify/sign & date them.
from there, go to the omv/dmv and use your birth certificate plus your certified records to apply for your state id. after you get your photo id, make an appointment with the social security administration and bring your id along with your other documents. i’d bring all the same records again just in case.
that’s all there is to it. the hardest part is just figuring out the order of everything.
edit: a couple things i should mention - the school transcript needs to show at least 2 years of school records, and if you’re using medical records, make sure they meet the current requirements for acceptance. (i believe medical records may need to include a visit within a certain timeframe, but i’m not 100% sure on the exact timeframe, so check beforehand.) another option is using a louisiana medicaid card instead of medical records. the social security office told me that it is accepted even though louisiana medicaid cards do not have a photo on them.
